We go into medicine to help others, cure disease, end suffering. We want to be caretakers, but to do so, there has to be disease. Does that make us thankful for the processes that cause this pain and suffering because it gives us purpose, it allows us to fulfill our role as caretakers? Who would we be without it? How can we help, be of use, if nothing is wrong, if no one needs us?
Are we subconsciously endorsing disease? Are we thankful for it? We say we’re thankful when we end it, cure it, thwart it. But, to be …

We in the College of Osteopathic Medicine welcome each one of our classes with a White Coat Ceremony. It has been my honor to take part in this event for the last two years, the first time receiving my white coat and the second time as a companion. This event in it’s entirety is as much a celebration of an individual student as it is of their friends and family members. We never truly appreciate the sacrifices our families make, a debt of their love that can never be repaid!
Our missed dates,
Promises wiped from life’s slates,

This list of resources was designed to complement a project funded by the National Network of Libraries of Medicine (NN/LM) New England Region (NER) entitled “Empathy Learned Through an Extended Medical Education Virtual Reality Project." The project used a virtual reality (VR) experience for 1st year medical students developed by Embodied Labs. The interactive “Alfred Lab” immerses users in the story of a 74-year-old patient who has macular degeneration and hearing loss, allowing users to experience these conditions from the patient’s perspective as he interacts with his family and doctor.

A hallmark of smooth muscle cells is their ability to adapt their functions to meet temporal and chronic fluctuations in their demands. These functions include force development and growth. Understanding the mechanisms underlying the functional plasticity of smooth muscles, the major constituent of organ walls, is fundamental to elucidating pathophysiological rationales of failures of organ functions. Also, the knowledge is expected to facilitate devising innovative strategies that more precisely monitor and normalize organ functions by targeting individual smooth muscles. Evidence has established a current paradigm that the myosin light chain phosphatase (MLCP) is a master regulator of smooth muscle responsiveness …

This current study identifies distinct parent prevention communication profiles and examines whether youth with different parental communication profiles have varying substance use trajectories over time. Eleven schools in two rural school districts in the Midwestern United States were selected, and 784 students were surveyed at three time points from the beginning of 7th grade to the end of 8th grade. A series of latent profile analyses were performed to identify discrete profiles/subgroups of substance-specific prevention communication (SSPC). The results revealed a 4-profile model of SSPC: Active-Open, Passive-Open, Active-Silent, and Passive-Silent. A growth curve model revealed different rates of lifetime substance …

Cyclin dependent kinases are proline-directed serine/threonine protein kinases that are traditionally activated upon association with a regulatory subunit. For most CDKs, activation by a cyclin occurs through association and phosphorylation of the CDK's T-loop. CDK5 is unusual because it is not typically activated upon binding with a cyclin and does not require T-loop phosphorylation for activation, even though it has high amino acid sequence homology with other CDKs. While it was previously thought that CDK5 only interacted with p35 or p39 and their cleaved counterparts, Recent evidence suggests that CDK5 can interact with certain cylins, amongst other proteins, which modulate …

Spinal cord injury (SCI) triggers a heterogeneous macrophage response that when experimentally polarized toward alternative forms of activation (M2 macrophages) promotes tissue and functional recovery. There are limited pharmacological therapies that can drive this reparative inflammatory state. In the current study, we used in vitrosystems to comprehensively defined markers of macrophages with known pathological (M1) and reparative (M2) properties in SCI. We then used these markers to objectively define the macrophage activation states after SCI in response to delayed azithromycin treatment. Mice were subjected to moderate-severe thoracic contusion SCI. Azithromycin or vehicle was administered beginning 30 minutes post-SCI and …

Background: Food allergy is an important public health problem because it affects children and adults, can be severe and even life-threatening, and may be increasing in prevalence. Beginning in 2008, the National Institute of Allergy and Infectious Diseases, working with other organizations and advocacy groups, led the development of the first clinical guidelines for the diagnosis and management of food allergy. A recent landmark clinical trial and other emerging data suggest that peanut allergy can be prevented through introduction of peanut-containing foods beginning in infancy.
Objectives: Prompted by these findings, along with 25 professional organizations, federal agencies, and patient advocacy …

Health Policy and Management Issue Briefs
Donald Trump and Congressional leaders have stated their intent to repeal the Patient Protection and Affordable Care Act (ACA or Obamacare). This report examines the consequences of repealing two key elements: (1) federal premium tax credits that help low and middle income Americans afford insurance policies bought through the Health Insurance Marketplaces (exchanges) and (2) federal payments to states for expansions of Medicaid eligibility for low-income adults. Congress passed similar legislation (H.R. 3762) in late 2015, which President Obama vetoed.
This report analyzes how the repeal of these policies could affect state-level employment, economies and fiscal conditions. If tax credits …

Pain in sickle cell disease (SCD) is associated with increased morbidity, mortality, and high health care costs. Although episodic acute pain is the hallmark of this disorder, there is an increasing awareness that chronic pain is part of the pain experience of many older adolescents and adults. A common set of criteria for classifying chronic pain associated with SCD would enhance SCD pain research efforts in epidemiology, pain mechanisms, and clinical trials of pain management interventions, and ultimately improve clinical assessment and management. As part of the collaborative effort between the Analgesic, Anesthetic, and Addiction Clinical Trial Translations Innovations Opportunities …

Promoting student collaboration has often been viewed as one of the many challenges instructors face in the educational setting. Collaboration conjures up notions of students who resist working in groups, students who struggle in creating equitable team effort and the difficulty of evaluating and ultimately grading the productivity produced by students and student groups [1]. Collaborative learning is certainly not a novel teaching strategy with research dating back to the 1970s [2]. In the online environment, student collaboration has come in the form of a computer-mediated version of its in-class traditional counterpart. It has come in forms of discussion boards, …

Repealing The Aca Without A Replacement — The Risks To American Health Care, Barack Obama
United States Public Health Resources
Health care policy often shifts when the country’s leadership changes. That was true when I took office, and it will likely be true with President-elect Donald Trump. I am proud that my administration’s work, through the Affordable Care Act (ACA) and other policies, helped millions more Americans know the security of health care in a system that is more effective and efficient. At the same time, there is more work to do to ensure that all Americans have access to high-quality, affordable health care. What the past 8 years have taught us is that health care reform requires an evidence-based, …

Comprehensive school physical activity programs (CSPAPs) have been highlighted by numerous public health and education agencies for their potential to improve the health and academic achievement of American youth. A CSPAP integrates physical activity throughout the school environment before, during and after school by engaging educators, children, families and community organizations. As the design, implementation and evaluation of effective CSPAP programs has inched forward, so has the call for physical education teacher education (PETE) programs to prepare the next generation of physical educators with the skills and knowledge needed to one day lead CSPAP efforts in K-12 schools. The purpose …

Background
More than 6,800 rare diseases and conditions have been identified in the US, which affect 25–30 million Americans. In 1983, the US Congress enacted the Orphan Drug Act (ODA) to encourage the development and marketing of drugs to treat rare diseases and conditions. This study analyzed all orphan designations and FDA approvals since 1983 through 2015, discussed the effectiveness of incentives for the development of treatments for rare diseases, and reflected on the ethical imperatives for timely access to orphan drugs.
Methods
Study data were derived from the Food and Drug Administration (FDA) Orange Book and the Office of …

Background Taenia solium inflicts substantial neurologic disease and economic losses on rural communities in many developing nations. Ring-strategy is a control intervention that targets treatment of humans and pigs among clusters of households (rings) that surround pigs heavily infected with cysticerci. These pigs are typically identified by examining the animal's tongue for cysts. However, as prevalence decreases in intervened communities, more sensitive methods may be needed to identify these animals and to maintain control pressure. The purpose of this study was to evaluate ultrasonography as an alternative method to detect pigs heavily infected with T. solium cysts. Methodology/Principal Findings We …

Background:
The growing prevalence of obesity is paralleling a rise in the older adult population creating an increased risk of functional impairment, nursing home placement and early mortality. The Centers for Medicare and Medicaid recognized the importance of treating obesity and instituted a benefit in primary care settings to encourage intensive behavioral therapy in beneficiaries by primary care clinicians. This benefit covers frequent, brief, clinic visits designed to address older adult obesity.
Discussion:
We describe the challenges in the implementation and delivery into real-world settings. The challenges in rural settings that have the fastest growing elderly population, high obesity rates, …
