ปัญหา : การตรวจ hand elevation test นำมาใช้ในการวินิจฉัยโรคเส้นประสาทมีเดียนถูกกดรัดบริเวณอุโมงค์ข้อมือ (carpal tunnel syndrome, CTS) ได้ดีหรือไม่วัตถุประสงค์ : เพื่อศึกษาความไวและความจำเพาะของการตรวจ hand elevation test ในการวินิจฉัยโรค CTS และเปรียบเทียบกับการตรวจอื่นวิธีการศึกษา : ผู้ป่วยที่มีอาการเข้าได้กับโรค CTS มารับการตรวจที่ห้องตรวจไฟฟ้าวินิจฉัยโรงพยาบาลจุฬาลงกรณ์ ผู้ป่วยที่มีอาการเข้าได้กับโรค CTS 48 ราย ได้รับการตรวจ hand elevation test, Tinel’s test, modified Phalen’s test และcarpal compression test ร่วมกับตรวจไฟฟ้าวินิจฉัยเพื่อยืนยันการวินิจฉัยโรคCTS แล้ววิเคราะห์ค่าความไว และความจำเพาะของการตรวจแต่ละประเภทผลการศึกษา hand elevation test, Tinel’s test, modified Phalen’s test และ carpalcompression test ความไว คิดเป็นร้อยละ 74, 24.7, 46.8 และ 64.9ความจำเพาะ คิดเป็นร้อยละ 52.6, 89.5, 57.9 และ 73.7 ตามลำดับ การแปลผลแบบร่วมของ hand elevation test และ carpal compression test มีความไวสูงสุด ร้อยละ 80.5สรุป : การตรวจ hand elevation test มีความไวสูงสุด เหมาะในการใช้วินิจฉัยโรคเส้นประสาทมีเดียนถูกกดรัดบริเวณอุโมงค์ข้อมือ และการแปลผลร่วมกันของ hand elevation test และ carpal compression test ความไวสูงสุด ช่วยเพิ่มความเชื่อมั่นในการวินิจฉัยได้.

เหตุผลของการวิจัย : โรคหลอดเลือดสมอง (Stroke หรือ Cerebrovascular disease: CVD)เป็นภาวะความเจ็บป่วยเฉียบพลันที่พบได้มากในผู้ป่วยกลุ่มอาการทางระบบประสาทและยังเป็นปัญหาสาธารณสุขที่สำคัญของโลกผู้ป่วยต้องตกอยู่ในสภาวะความพิการ บกพร่องหรือสูญเสียการทำหน้าที่ของแขนและขา ซึ่งต้องใช้ระยะเวลาพอสมควรในการฟื้นฟูสภาพร่างกายความพิการส่งผลต่อสุขภาพจิตของผู้ป่วยในหลาย ๆ ด้าน โดยเฉพาะอาการวิตกกังวลและอาการซึมเศร้าวัตถุประสงค์ : เพื่อศึกษาความชุกของอาการวิตกกังวลและอาการซึมเศร้าในผู้ป่วยโรคหลอดเลือดสมองที่ได้รับการเยี่ยมบ้าน และเพื่อศึกษาความสัมพันธ์ของอาการวิตกกังวลและอาการซึมเศร้าต่อการฟื้นตัวของระบบสั่งงานกล้ามเนื้อ และความสามารถในการทำกิจวัตรประจำวันของผู้ป่วยโรคหลอดเลือดสมองตัวอย่างและวิธีการศึกษา : เก็บรวบรวมข้อมูลจากผู้ป่วยโรคหลอดเลือดสมองที่เข้าร่วมโครงการเยี่ยมผู้ป่วยที่บ้าน ผ่านเกณฑ์การคัดเข้า และยินยอมที่จะเข้าร่วมการวิจัยจำนวน 52 ราย จากนั้นจะทำการเก็บข้อมูลส่วนบุคคล ข้อมูลทางการแพทย์ แบบวัดอาการวิตกกังวลและอาการซึมเศร้าของผู้ป่วยฉบับภาษาไทย แบบประเมินความสามารถในการเคลื่อนไหวแบบประเมินดัชนี บาร์เธล เอ ดี แอล และพิสัยข้อไหล่ เมื่อผ่านไป 1 เดือนผู้วิจัยได้เข้าเยี่ยมผู้ป่วยที่บ้านอีกครั้งหนึ่งและทำการประเมินครั้งที่ 2ด้วยแบบประเมินชุดเดียวกับที่ใช้ในครั้งแรก ข้อมูลที่ได้วิเคราะห์ด้วยสถิติเชิงพรรณนาในรูปของร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐานใช้ไค-สแควร์ และสถิติการถดถอยโลจิสติกเพื่อวิเคราะห์ความสัมพันธ์ระหว่างตัวแปรต้นกับตัวแปรตามผลการศึกษา : พบว่าร้อยละ 55.8 มีอาการวิตกกังวลและอาการซึมเศร้าจากการเยี่ยมในครั้งแรก (คิดเป็นอาการวิตกกังวลร้อยละ 19.2 และอาการซึมเศร้าร้อยละ 51.9) เมื่อผู้ป่วยเข้าร่วมโครงการเยี่ยมผู้ป่วยที่บ้านเป็นเวลา1 เดือน พบว่ามีอาการวิตกกังวลและอาการซึมเศร้าเพียงร้อยละ 25.0(คิดเป็นอาการวิตกกังวลร้อยละ 9.6 และอาการซึมเศร้าร้อยละ 21.2)นอกจากนี้ยังพบว่าอาการวิตกกังวลและอาการซึมเศร้ามีความสัมพันธ์กับการฟื้นตัวของระบบสั่งงานกล้ามเนื้อ ความสามารถในการทำกิจวัตรประจำวันและพิสัยข้อไหล่อย่างมีนัยสำคัญทางสถิติที่ระดับP <0.05 และเมื่อวิเคราะห์ด้วยสถิติการถดถอยโลจิสติก พบว่าปัจจัยทำนายการฟื้นตัวของระบบสั่งงานกล้ามเนื้อและการเคลื่อนไหว คือนักกายภาพบำบัดส่วนตัวที่จ้างเพิ่มเติมเพื่อมาทำให้ที่บ้าน, ตำแหน่งพยาธิสภาพของสมองซีกซ้าย, อาการวิตกกังวลและอาการซึมเศร้า(P <0.05) ปัจจัยทำนายความสามารถในการทำกิจวัตรประจำวันคืออายุ, รายได้ อาการวิตกกังวลและอาการซึมเศร้า(P <0.05) และปัจจัยทำนายพิสัยข้อไหล่ข้างอ่อนแรงคือ รายได้, อาการปวดไหล่, และนักกายภาพบำบัดส่วนตัว (P <0.05)สรุป : ความชุกของอาการวิตกกังวลและอาการซึมเศร้าในผู้ป่วยโรคหลอดเลือดสมองค่อนข้างสูง แต่ความชุกของอาการวิตกกังวลและอาการซึมเศร้าลดลงเมื่อผู้ป่วยโรคหลอดเลือดสมองได้เข้าร่วมโครงการเยี่ยมผู้ป่วยที่บ้าน นอกจากนี้อาการวิตกกังวลและอาการซึมเศร้ายังมีความสัมพันธ์กับการฟื้นตัวของระบบสั่งงานกล้ามเนื้อ การทำกิจวัตรประจำวัน และพิสัยข้อไหล่ด้วยเช่นกัน.

ที่มาของงานวิจัย : ผลกระทบจากการใช้สมาร์ทโฟนเพิ่มสูงขึ้น โดยเฉพาะภาวะ nomophobiaคือ ภาวะวิตกกังวลกลัวการขาดสมาร์ทโฟน ซึ่งพบมากขึ้นในกลุ่มเยาวชนในประเทศไทย การศึกษาวิจัยที่เกี่ยวข้องยังมีจำกัดขณะที่ในหลายประเทศศึกษาวิจัยในประเด็นนี้กันแพร่หลายวัตถุประสงค์ : เพื่อศึกษาความชุกของภาวะ nomophobia ในกลุ่มนิสิตนักศึกษาไทยระดับปริญญาตรีทั่วประเทศวิธีวิจัย : รวบรวมข้อมูลในกลุ่มนิสิตนักศึกษาไทยระดับปริญญาตรีจำนวน 3,045 คนที่ถูกสุ่มเลือกด้วยเทคนิคการสุ่มแบบหลายขั้นตอนจากมหาวิทยาลัยภาครัฐทั้งหมด 9 แห่งทั่วประเทศ เก็บข้อมูลโดยใช้แบบสอบถามแบบตอบด้วยตนเอง วิเคราะห์ข้อมูลโดยใช้สถิติการทดสอบไคว์สแควร์ผลการศึกษา : ร้อยละ 99.5 ของกลุ่มตัวอย่างมีภาวะ nomophobia ส่วนใหญ่อยู่ในระดับความรุนแรงปานกลาง (ร้อยละ 60.6) ลักษณะอาการที่แสดงออกมาอย่างเด่นชัดมากที่สุด คือ กลุ่มตัวอย่างรู้สึกวิตกกังวลใจเมื่อสมาชิกในครอบครัวหรือเพื่อนไม่สามารถติดต่อกับตนเองได้อีกทั้งพบว่าเกมส์เป็นโปรแกรมที่กลุ่มตัวอย่างดาวน์โหลดมาใช้เป็นประจำมากที่สุด (ร้อยละ 98.5) และเป็นกิจกรรมที่กลุ่มตัวอย่างทำมากที่สุดเมื่อรู้สึกเบื่อหน่ายและเมื่ออยู่คนเดียว(P < 0.05)สรุป : การศึกษาการใช้สมาร์ทโฟนควรต้องมีการดำเนินการอย่างจริงจังเพื่อป้องกันภาวะ nomophobia ซึ่งหากอาการรุนแรงมากอาจนำไปสู่ภาวะทางจิตเวชต่อไปได้.

เหตุผลของการทำวิจัย : การศึกษาในระดับบัณฑิตศึกษาเป็นการศึกษาเพื่อมุ่งเน้นให้ผู้เรียนได้พัฒนาองค์ความรู้และความเชี่ยวชาญเฉพาะทางทำให้ผู้เรียนต้องศึกษาค้นคว้าหาความรู้ จากแหล่งสารสนเทศต่าง ๆ ด้วยตนเองเพื่อนำมาใช้ในศึกษาค้นคว้า วิจัย และสร้างองค์ความรู้ใหม่ จึงจำเป็นต้องแสวงหาสารสนเทศจากแหล่งต่าง ๆ เพื่อให้ได้สารสนเทศตรงตามความต้องการ ผู้วิจัยจึงศึกษาพฤติกรรมการแสวงหาสารสนเทศของนิสิตในระดับบัณฑิตศึกษาเพื่อนำมาเป็นแนวทางในการจัดบริการการสร้างเสริมพฤติกรรมการแสวงหาสารสนเทศ หรือการศึกษาค้นคว้าด้วยตนเองของนิสิตวัตถุประสงค์ : เพื่อศึกษาพฤติกรรมการแสวงหาสารสนเทศของนิสิตระดับบัณฑิตศึกษา คณะแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัยและเพื่อเปรียบเทียบพฤติกรรมการแสวงหาสารสนเทศของนิสิตระดับบัณฑิตศึกษาคณะแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัยจำแนกตามสาขาวิชาและระดับการศึกษากลุ่มตัวอย่างและวิธีการศึกษา : นิสิตระดับบัณฑิตศึกษา คณะแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ภาคการศึกษาต้นปีการศึกษา 2558 จำนวน187 ราย ได้รับแบบสอบถามกลับคืน 169 ชุด เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามที่ผ่านการประเมินความเที่ยงตรงจากผู้เชี่ยวชาญ และวิเคราะห์ค่าความเชื่อมั่นโดยวิธีของคอรนบาค สถิติที่ใช้ในการวิจัยได้แก่ค่าร้อยละ ค่าความถี่ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน t – test และ one way ANOVAผลการศึกษา : นิสิตส่วนใหญ่มีวัตถุประสงค์/จุดมุ่งหมายในการแสวงหาสารสนเทศเพื่อทำวิทยานิพนธ์/ดุษฎีนิพนธ์ มีพฤติกรรมการแสวงหาสารสนเทศ ดังนี้ 1. เริ่มต้นจากวรรณกรรมที่เกี่ยวข้อง/บรรณานุกรม 2. สำรวจเลือกดูสารสนเทศจากฐานข้อมูลออนไลน์ในห้องสมุด 3. เชื่อมโยงสารสนเทศจากรายการอ้างอิงหรือบรรณานุกรม 4. แยกแยะสารสนเทศจากความน่าเชื่อถือของแหล่งที่มาความเกี่ยวข้องกันของเนื้อหาสารสนเทศ5. ตรวจตราสารสนเทศจากสื่อสังคมออนไลน์ 6. ดึงสารสนเทศออกมาใช้จากบทคัดย่อ/ผลการศึกษา/ผลการวิจัย/ระเบียบวิธีวิจัยเอกสาร หนังสือ วารสาร อ้างอิงแหล่งที่มา 7. ตรวจสอบสารสนเทศจากแหล่งที่มา/การอ้างอิงของข้อมูล 8. ขั้นตอนสุดท้ายนำสารสนเทศที่ได้ไปใช้งานตามวัตถุประสงค์ที่แสวงหาผลเปรียบเทียบพฤติกรรมการแสวงหาสารสนเทศพบว่านิสิตที่มีสาขาวิชาและระดับการศึกษาต่างกันมีพฤติกรรมการแสวงหาสารสนเทศไม่แตกต่างกันสรุป : นิสิตระดับบัณฑิตศึกษา คณะแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัยส่วนใหญ่มีวัตถุประสงค์มีการแสวงหาสารสนเทศเพื่อทำวิทยานิพนธ์/ดุษฎีนิพนธ์ มีพฤติกรรมการแสวงหาสารสนเทศประกอบด้วย การเริ่มต้น การสำรวจเลือกดู การเชื่อมโยงสารสนเทศ การแยกแยะ การตรวจตรา การดึงสารสนเทศออกมาใช้ การตรวจสอบการจบ นิสิตที่มีสาขาวิชาและระดับการศึกษาต่างกันมีพฤติกรรมการแสวงหาสารสนเทศโดยรวมที่ไม่แตกต่างกัน.

Abstract
OBJECTIVE:
To describe the frequency of color vision deficiency among Pakistani adults presenting for pre-employment health screening in a tertiary care hospital.
METHODS:
The cross-sectional study was carried out at the Aga Khan University Hospital, Karachi, and the data was collected for color vision deficiency, age, gender, and job applied for from pre-employment examination during 2013-2014. IBM SPSS 20 was used for statistical analysis.
RESULTS:

Purpose of Review
Non-invasive neuroimaging methods have been developed as powerful tools for identifying in vivo brain functions for studies in humans and animals. Here, we review the imaging biomarkers that are being used to determine the changes within brain metabolic and vascular functions induced by caloric restriction (CR) and their potential usefulness for future studies with dietary interventions in humans.

Brachytherapy consists of placing radioactive sources into or adjacent to tumors, to deliver conformal radiation treatment. The technique is used for treatment of primary malignancies and for salvage in recurrent disease. Permanent prostate brachytherapy seeds are small metal implants containing radioactive sources of I-125, Pd-103, or Cs-131 encased in a titanium shell. They can embolize through the venous system to the lungs or heart and subsequently be detected by cardiovascular computed tomography. Cardiovascular imagers should be aware of the appearance of migrated seeds, as their presence in the chest is generally benign, so that unnecessary worry and testing are avoided. …

Cognitive dysfunction post-surgery has a significant impact on patients’ quality of life during recovery and afterward. Several studies have been completed on post-operative cognitive dysfunction (POCD), but since studies are varied in their methodologies and designs a metaanalysis is helpful to synthesize the current available research. The present study took a metaanalysis approach to examine neuropsychological tests most sensitive to POCD in adult surgery patients, and determine implications this would have for developing a battery of tests to evaluate for POCD pre and post-surgery. Endothelial cell-specific molecule-1 (ESM-1), also known as endocan, is a soluble proteoglycan expressed by the vascular endothelium, which also circulates in the bloodstream. Inflammatory cytokines and proangiogenic growth factors increase its expression, and increased serum levels have been reported in several cancer types and immunocompetent patients with sepsis. The aim of this study was to analyze the expression profile of CXC-chemokines and the effects of ESM-1 gene knockdown in proliferation, migration and CXC-chemokine expression in highly metastatic human prostate PC-3 cells. Dialysis-related amyloidosis predominantly occurs in osteo-articular structures and dialysis-related amyloid (DRA) substances also deposit in extra-articular tissues. Clinical manifestations of DRA include odynophagia, gastrointestinal hemorrhage, intestinal obstruction, kidney stones, myocardial dysfunction, and subcutaneous tumors. The pathological characteristics of DRA in the heart of hemodialysis patients have rarely been reported. We report the case of a 73-year-old female with a history of cerebral palsy and end-stage renal disease status post two failed renal transplants who had been on hemodialysis for 30 years. BACKGROUND AND OBJECTIVES: There are currently limited data available regarding the safety of endoscopic ultrasound (EUS)-guided drainage of pancreatic fluid collections (PFCs) using the lumen-apposing metal stent without fluoroscopic guidance. This study aims to evaluate clinical outcomes and safety of EUS-guided drainage of PFC using the electrocautery-enhanced lumen-apposing metal stents (EC-LAMSs) without fluoroscopic guidance.
METHODS: We conducted a retrospective study on patients with symptomatic PFC who underwent EUS-guided drainage using EC-LAMS without fluoroscopy. Context: The role of the rotator cuff is to provide dynamic stability to the glenohumeral joint. Human and animal studies have identified sarcomerogenesis as an outcome of eccentric training indicated by more torque generation with the muscle in a lengthened position. Objective: The authors hypothesized that a home-based eccentric-exercise program could increase the shoulder external rotators’ eccentric strength at terminal internal rotation (IR). Design: Prospective case series. Setting: Clinical laboratory and home exercising. Participants: 10 healthy subjects (age 30 ± 10 y). BACKGROUND: Necrotizing fasciitis (NF) is a devastating soft tissue infection associated with potentially poor outcomes. The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score has been introduced as a diagnostic tool for NF. We aimed to evaluate the prognostic value of LRINEC scoring in NF patients. METHODS: A retrospective analysis was conducted for patients who were admitted with NF between 2000 and 2013. Based on LRINEC points, patients were classified into (Group 1: LRINEC /= 6). Vector-borne diseases are a significant public health problem in Western Australia. Mosquitoes are responsible for the transmission of a number of pathogens and may pose a serious nuisance problem. Prevention efforts in the State are multi-faceted and include physical, chemical, and cultural control methods for restricting mosquito breeding. This is less complex where breeding areas are located within public open spaces. In Australia’s developed urban areas, breeding sites are, however, frequently located within private residential landholdings, where the scope of public health officials to act is constrained by law and practicality. Overview—Substance use and substance use disorders are highly prevalent among youth under juvenile justice (JJ) supervision, and related to delinquency, psychopathology, social problems, risky sex and sexually transmitted infections, and health problems. However, numerous gaps exist in the identification of behavioral health (BH) problems and in the subsequent referral, initiation and retention in treatment for youth in community justice settings. This reflects both organizational and systems factors, including coordination between justice and BH agencies.

Written Feedback (WFB) plays a significant role in student’s learning in terms of improvement of their academic writing skills. However, the quality of WFB may be affected by various personal and contextual factors. This study aimed to identify nurse teachers’ conceptions and practices of WFB on written assignments in nursing degree offering institutions in Karachi, Pakistan.

The prevalence of peanut allergies (PAs) continues to rise through recent decades, despite the best attempts to reverse that trend. PAs are unpredictable and can be life-threatening. Therefore, it is imperative that nurse practitioners (NPs) are fully aware of the most recent guidelines and evidence regarding diagnosis, treatment, and prevention of PAs. This article presents information on current research in diagnosis and treatment of PA, as well as the latest guidelines established to prevent PA development. NPs should understand this information, allowing them to provide the best care possible for their patients.

During 5 months in 2014, three Amish children in Missouri, USA, were diagnosed with invasive Haemophilus influenzae type b infection. Two were rural neighbors infected with a genetically similar rare strain, sequence type 45. One child had recently traveled, raising the possibility of maintenance of this strain among unvaccinated carriers in Amish communities.
