วัตถุประสงค์ : 1) เพื่อศึกษาความชุกของความมั่นคงทางอาหารและติดตามในระยะ 6 เดือน และ 2) เพื่อศึกษาปัจจัยทำนายความมั่นคงทางอาหารของสตรีสูงอายุที่อาศัยอยู่ในชุมชนแออัดเขตเมือง\n\nแบบแผนการวิจัย : แบบบรรยายเชิงทำนาย\n\nวิธีดำเนินการวิจัย : กลุ่มตัวอย่าง คือ สตรีสูงอายุที่อาศัยในชุมชนแออัด เขตกรุงเทพมหานคร จำนวน 315 คน เลือกแบบสะดวกตามเกณฑ์คัดเข้า เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามแรงสนับสนุนทางสังคม และแบบสอบถามความมั่นคงทางอาหาร ที่ผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ 3 คน มีค่าดัชนีความตรงเท่ากัน คือ .94 มีค่าสัมประสิทธิ์อัลฟ่าของครอนบาค เท่ากับ .87 และค่าความเที่ยงของการวัดซํ้า เท่ากับ .99 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติถดถอยลอจิสติค\n\nผลการวิจัย : 1) ระยะเริ่มต้นการศึกษา พบว่า กลุ่มตัวอย่างร้อยละ 83.8 มีความมั่นคงทางอาหารในครัวเรือน ในขณะที่ร้อยละ 14.0 มีปัญหาความมั่นคงทางอาหารในระดับตํ่า และร้อยละ 2.2 มีปัญหาความมั่นคงทางอาหารในระดับปานกลางถึงสูง ผลการติดตามในระยะเวลา 6 เดือน พบว่า สถานการณ์ปัญหาความมั่นคงทางอาหารในระดับตํ่าลดลงเล็กน้อยเป็นร้อยละ 13.7 และปัญหาความมั่นคงทางอาหารในระดับปานกลางถึงสูง เพิ่มขึ้น เป็น 7.3 2) ปัจจัยทำนายปัญ หาความมั่นคงทางอาหารในครัวเรือนของสตรีสูงอายุ ได้แก่ รายได้ครอบครัว (OR = 1.223, 95% CI .095-.522, p = .001) จำ.นวนสมาชิกในครอบครัว(OR = 48.282, 95% CI 11.646-200.166, p < .001) สถานภาพสมรส (OR = 1.604, 95% CI 1.514-3.293, p = .031) แรงสนับสนุนทางสังคม (OR = 1.163, 95% CI .086-.306, p < .001) ลักษณะสิ่งแวดล้อมของที่อยู่อาศัย (OR = 2.383, 95% CI 1.570-5.711, p = .033) และจำ.นวนโรคร่วม (OR = 41.725, 95% CI 14.813-117.533, p < .001)\n\nสรุป : บุคลากรทีมสุขภาพควรประเมิน ติดตาม และสนับสนุนการจัดการปัญหาความมั่นคงทางอาหารในครัวเรือนของสตรีผู้สูงอายุ โดยพิจารณาปัจจัยด้านรายได้ครอบครัว จำ.นวนสมาชิกในครอบครัว สถานภาพสมรส แรงสนับสนุนทางสังคม ลักษณะสิ่งแวดล้อมของที่อยู่อาศัย และจำ.นวนโรคร่วม รวมทั้งการพัฒนาโปรแกรมสร้างเสริมความมั่นคงทางอาหารในผู้สูงอายุที่อาศัยอยู่ในชุมชนแออัดเขตเมือง

วัตถุประสงค์: เพื่อศึกษาอำนาจการทำ นายของอายุ การรับรู้สมรรถนะในตนเองในการเป็นมารดาความเครียดในการเลี้ยงดูทารก การสนับสนุนทางสังคม และพื้นอารมณ์ทารก ต่อการตอบสนองความต้องการของทารกในมารดาที่มีบุตรคนแรก\n\nแบบแผนการวิจัย: การวิจัยแบบความสัมพันธ์เชิงทำนาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ มารดาหลังคลอดบุตรคนแรกที่พักรักษาในหอผู้ป่วยหลังคลอดสามัญโรงพยาบาลศิริราช และมาตรวจหลังคลอดที่หน่วยอนามัยเจริญพันธุ์และวางแผนครอบครัว จำนวน 95 ราย เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความเชื่อในความสามารถในการเป็นมารดา แบบสอบถามการตอบสนองต่อความต้องการของทารก แบบสอบถามความเครียดในการเลี้ยงดูทารก แบบสอบถามการสนบั สนนุ ทางสงั คมในการเลยี้ งดบู ตุ ร และแบบประเมนิ พนื้ อารมณข์ องทารก โดยแบบสอบถามการตอบสนองต่อความต้องการของทารกได้ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 คน มีดัชนีความตรงเชิงเนื้อหาเท่ากับ .86 ค่าสัมประสิทธิ์อัลฟ่าของครอนบาคของแบบสอบถามในการศึกษาครั้งนี้ เท่ากับ .80, .82, .92 และ .92 ตามลำดับ สำหรับแบบประเมินพื้นอารมณ์ของทารกหาความเที่ยงโดยการทดสอบซํ้า มีค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สันเท่ากับ .79 วเิ คราะหข์ อ้ มลู โดยการคำนวณร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน และสถิติถดถอยพหุ\n\nผลการวิจัย: 1) ปัจจัยทำนายการตอบสนองความต้องการของทารกในมารดาที่มีบุตรคนแรก อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ได้แก่ การรับรู้สมรรถนะในตนเองในการเป็นมารดา (β= .530) ความเครียดในการเลี้ยงดูทารก (β= -.102) และพื้นอารมณ์ทารก (β= -3.223) 2) ปัจจัยทั้ง 5 ปัจจัย ได้แก่ อายุ การรับรู้สมรรถนะในตนเองในการเป็นมารดา ความเครียดในการเลี้ยงดูทารก การสนับสนุนทางสังคม และพื้นอารมณ์ทารก สามารถร่วมกันอธิบายความแปรปรวนของการตอบสนองความต้องการของทารกในมารดาที่มีบุตรคนแรกได้ร้อยละ 34.1 (Adjusted R2 = 0.341)ที่ระดับนัยสำคัญทางสถิติ .05\n\nสรุป: พยาบาลควรส่งเสริมให้มารดาตอบสนองความต้องการของทารก โดยการสร้างเสริมให้เกิดความเชื่อมั่นในความสามารถของตนเองในการเลี้ยงดูทารก ประเมินความเครียดและแนะนำแนวทางในการจัดการความเครียด โดยคำนึงถึงพื้นอารมณ์ของทารก

ความตรงเชิงโครงสร้างและความสอดคล้องภายใน ของแบบสอบถามคุณภาพชีวิตในขณะตั้งครรภ์ ฉบับภาษาไทย, ตติรัตน์ เตชะศักดิ์ศรี, วรรณทนา ศุภสีมานนท์, นารีรัตน์ บุญเนตร

วัตถุประสงค์: เพื่อตรวจสอบความตรงเชิงโครงสร้างและความสอดคล้องภายในของแบบสอบถามคุณภาพชีวิตในขณะตั้งครรภ์ ฉบับภาษาไทย\n\nแบบแผนการวิจัย: การพัฒนาเครื่องมือวิจัย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ หญิงตั้งครรภ์ในไตรมาสที่ 3 จำนวน 180 คน ที่คัดเลือกตามสะดวก เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามตอบด้วยตนเองเกี่ยวกับคุณภาพชีวิตในขณะตั้งครรภ์ที่พัฒนาเป็นภาษาอังกฤษโดย Vachkova, Jezek, Mares และ Moravcova (2013) และแปลเป็นภาษาไทยตามหลักการแปลแบบย้อนกลับ ได้ค่าดัชนีความตรงเชิงเนื้อหา (CVI) เท่ากับ 1 วิเคราะห์ข้อมูลโดยการวิเคราะห์ องค์ประกอบเชิงยืนยัน (Confirmatory factor analysis) และวิเคราะห์ความสอดคล้องภายในด้วยการคำนวณค่าสัมประสิทธิ์ครอนบาค (Cronbach's alpha coefficient)\n\nผลการศึกษา: ผลการตรวจสอบความตรงเชิงโครงสร้าง พบว่า แบบสอบถามคุณภาพชีวิตในขณะตั้งครรภ์ ประกอบด้วยองค์ประกอบ 3 ด้าน คือ ด้านร่างกาย (5 ข้อ) ด้านจิตใจ (2 ข้อ) และด้านความสัมพันธ์ทางสังคม (2 ข้อ) ได้ค่าความสอดคล้องอยู่ในเกณฑ์ที่ยอมรับได้ ( = 29.17, df = 24, p = .21; /df = 1.22; GFI = .96; AGFI = .93; CFI = .98; RMSEA = .03) โดยรายด้านมีนํ้าหนักองค์ประกอบระหว่าง .46 ถึง .96 และรายข้อมีนํ้าหนักองค์ประกอบอยู่ระหว่าง .47 ถึง .82 ส่วนผลการตรวจสอบความสอดคล้องภายใน พบว่า ค่าสัมประสิทธิ์ครอนบาค (Cronbach's alpha coefficient) ของแบบสอบถามทั้งฉบับ เท่ากับ .75 โดยอยู่ในเกณฑ์ที่ยอมรับได้ \n\nสรุป: พยาบาลและบุคลากรสาธารณสุขควรนำ.แบบสอบถามนี้ไปใช้ประเมินคุณภาพชีวิตของหญิงตั้งครรภ์ในบริบทของสังคมไทย

Background

The objective of this study is to evaluate the safety and quality of computed tomographic angiography of the thoracic aorta (CTA-TA) exams performed using intraosseous needle intravenous access (ION-IVA) for contrast media injection (CMI).

Methods

All CTA-TA exams at the study institution performed between 1/1/2013 and 8/14/2015 were reviewed retrospectively to identify those exams which had been performed using ION-IVA (ION-exams). ION-exams were then analyzed to determine aortic attenuation and contrast-to-noise ratio (CNR). Previous research has shown that minorities hold negative attitudes towards members of law enforcement. Contact with law enforcement has been shown as a major predictor of attitudes towards law enforcement among minorities. Research on this subject has primarily focused on Blacks’ attitudes towards police. Hispanics have been the focus of only a small amount of research on police contact and attitudes towards law enforcement. This study examined contact and attitudes towards several law enforcement entities: local police, state troopers, and border patrol agents. Purpose: The purpose of this project was to implement a quality improvement project to improve blood specimen acceptance rates at a specific Emergency Department (ED) using strategies to enhance knowledge, improve motivation, and reinforce skill. Background: Specimen acceptance rates continue to fall outside of the recommended target rate of 98% or higher at a specific ED. In this particular ED, the responsibility for specimen collection has shifted away from phlebotomists and towards nurses, many of whom have received little to no phlebotomy training. The investigator examined reports of vocal health among singers in two different choral groups on a university campus, a population consisting of both music majors and non-music majors. These student singers consented to filling out a 36-question survey that asked about their overall vocal health and daily habits that might affect that health. A section of this survey involved rating answers from 0-4, while the other section required write-in answers. Personal sound amplifiers (PSAPs) provide accessible and affordable healthcare to hearing impaired individuals that would otherwise not seek treatment. Like a self-fitting hearing aid, PSAPs do not involve an audiology service. The current pilot study investigated the impact different levels of instruction have on the effectiveness of using a personal sound amplifier product for four participants with mild to moderate sensorineural hearing loss. Basic written instruction and premium audiology services were utilized to teach participants how to use the device. The primary purpose of this qualitative study was to describe the experience of compassion fatigue among nurse professionals in order to examine natural grieving reactions and how they potentially change nurses’ lives. The focus of this study included nurse professionals working in rural community hospitals in Upstate New York. Background: Orphaned and vulnerable adolescents (OVA) in sub-Saharan Africa are at greater risk for adverse psychological outcomes compared with their non-OVA counterparts. Social interventions that provide cash transfers (CTs) have been shown to improve health outcomes among young people, but little is known about their impact on the psychological wellbeing of OVA.

Objective: Among OVA in western Kenya, we assessed the association between living in a household that received monthly unconditional government CTs and psychological wellbeing.

With recent approvals for therapeutic antibodies that block CTLA4, PD-1 and PD-L1, immune checkpoints have emerged as new targets in cancer therapy. In addition, there is accumulating evidence highlighting the role of cancer-associated immunity in patient response to cytotoxic anticancer agents. Inhibitors of poly (ADP-ribose) polymerase (PARP) have shown substantial cytotoxic effects against tumors with defects in DNA damage responses. However, whether a crosstalk between PARP inhibition and immune checkpoints exists remains unclear. Here, it has been shown that PARP inhibitors (PARPis) upregulate PD-L1 expression in multiple cancer cell lines, human xenograft tumors, and syngeneic mouse tumors. The purpose of this study was to investigate the effect of the HDAC inhibitor entinostat on the efficacy of NK cell therapy for OS lung metastasis. The Lung is the most common site of OS metastatic spread in OS and pulmonary metastasis is the main cause of mortality. We have previously demonstrated that NK cell therapy has minimal efficacy against OS metastasis. Objectives: Catecholamine-mediated effects driven by elevated adrenergic signaling are known to increase tumor growth and metastasis by direct effects on tumor cells. However, knowledge of effects of adrenergic signaling on other cells such as cancer-associated fibroblasts (CAFs) within the tumor microenvironment is limited. DIRAS3 is a maternally imprinted tumor suppressor gene that is downregulated by multiple mechanisms across several tumor types. When re-expressed, DIRAS3 decreases proliferation, inhibits motility, and induces autophagy and tumor dormancy. DIRAS3 encodes a 26 kDa small GTPase with 60% homology to Ras and Rap, differing from oncogenic Ras family members by a 34-amino acid N-terminal extension that is required for its tumor suppressive function in ovarian cancer. By assessing the structure-function relationship, I found that DIRAS3 inhibits Ras-induced transformation and is a natural antagonist of Ras/MAPK signaling. Chemotherapeutic agents impair memory in humans as well as in animal models. Such memory impairments can be persistent, lasting years after exposure to chemotherapy. Doxorubicin (DOX), a common chemotherapeutic agent, has been associated with memory impairments in humans and induces memory deficits in rodent models. AtlA is the major peptidoglycan hydrolase of E. faecalis involved in cell separation of dividing cells. SalB is a secreted stress-induced protein regulated by the CroRS system. In addition, these two proteins also appear to be affected by the virulence factor, gelatinase (GelE). Genetic counselors (GCs) have recently reported an increase in the discussion of insurance-related, or “genesurance,” topics during genetic counseling sessions. Despite increasing frequency, little knowledge exists about genesurance conversations and patient expectations. This study aimed to assess patient expectations of GCs in genesurance discussions and evaluate if health insurance literacy impacted these expectations. A 38-item survey, including a validated tool to assess health insurance literacy (HIL) was administered prior to patients receiving prenatal or cancer genetic counseling at three participating institutions. In response to DNA damage, how cells activate checkpoints have been well studied, but little is known about checkpoint deactivation (recovery), which directly impacts on cell fate. In tumor cells, the signaling network has been rewired due to epigenetic and genetic alterations, which result in resistance to the cell cycle control, and thus resistance to chemotherapy or radiation therapy. Estimation of perceptual variables is imprecise and prone to errors. Although the properties of these perceptual errors are well characterized, the physiological basis for these errors is unknown. One previously proposed explanation for these errors is the trial-by-trial variability of the responses of sensory neurons that encode the percept. Initially, it would seem that a complicated electrophysiological experiment would need to be performed to test this hypothesis. The rate of twinning is rising and is associated with delayed age at childbirth and increased infertility treatments. Since the introduction of non-invasive prenatal testing, interest in and uptake of genetic screening and testing in twin pregnancies has not been investigated. Therefore, this study aimed to describe the attitudes toward and uptake of current prenatal genetic screening and testing options in twin pregnancies.
