Background-—Sodium-glucose cotransporter 2 (SGLT2) inhibitors are a novel class of antihyperglycemic agents that improve glycemic control by increasing glycosuria. Additional benefits beyond glucose lowering include significant improvements in seated clinic blood pressure (BP), partly attributed to their diuretic-like actions. Less known are the effects of this class on 24-hour ambulatory BP, which is a better predictor of cardiovascular risk than seated clinic BP.
Methods and Results-—We performed a meta-analysis of randomized, double-blind, placebo-controlled trials to investigate the effects of SGLT2 inhibitors on 24-hour ambulatory BP. We searched all studies published before August 17, 2016, which reported 24-hour ambulatory …

The Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA)-Outcome Measures in Rheumatology (OMERACT) Psoriatic Arthritis (PsA) Core Set working group recently published the updated 2016 psoriatic arthritis (PsA) core domain set, a set of disease features that should be measured in all clinical trials. BACKGROUND AND PURPOSE: ADC as a marker of tumor cellularity has been promising for evaluating the response to therapy in patients with glioblastoma but does not successfully stratify patients according to outcomes, especially in the upfront setting. Here we investigate whether restriction spectrum imaging, an advanced diffusion imaging model, performed after an operation but before radiation therapy, could improve risk stratification in patients with newly diagnosed glioblastoma relative to ADC.

Breast cancer metastasis is the main cause for mortality in breast cancer patients. However, knowledge of metastatic recurrence is limited, and there is a need to understand metastatic recurrence in order to treat breast cancer patients more effectively. Highly invasive metastatic breast cancer has shown to exhibit metabolic adaptability, transitioning from glycolysis to oxidative phosphorylation in the presence of microenvironmental stress. NADH and FAD are naturally occurring cofactor products during glycolysis and oxidative phosphorylation, respectively, and they are of particular importance during these metabolic processes due to their endogenous fluorescence. Individuals from all segments of society are affected by homelessness – the old and young, men and women, single individuals and families. The majority of individuals will experience homelessness only on a short-termed basis. However, about 10-20% will experience homelessness for years. Although, the chronically homeless represent only a fraction of the total homeless population, they utilize the majority of resources available. Current literature has focused primarily on other groups within the homeless community, leaving little to be known about the chronic homeless. A widely acknowledged transition to practice gap exists for nursing students who have recently graduated and are starting their first job as a registered nurse (RN). In an attempt to bridge this gap, new graduate nurses (NGNs) working in acute care hospitals are usually offered additional training in unit-specific knowledge and skills, along with time working with a unit-based preceptor. Hospital-based training programs are typically between two and six months in length, but the transition to becoming a competent RN with experience is estimated to take between 12 to 18 months (Duchscher, 2008). Low infant birth weight is a persistent problem in the United States. Psychosocial protective factors may confer benefits to vulnerable women, resulting in increased likelihood of healthy infant birth weight (HIBW). Few studies have examined predictors or correlates of healthy birth outcomes. The purpose of this study was to increase knowledge of protective factors and predictors of HIBW among childbearing women. A secondary analysis of the Fragile Families and Child Wellbeing Study public use baseline maternal surveys (1998 – 2000) was conducted (N = 4,759). The purpose of this study was to evaluate trends in the incidence of serious infectious events (SIE), defined as organ space infection, sepsis, and Cesarean Section (CS) wound disruption, during the birth admission or resulting in between January 1, 2009 and December 31, 2014 in the State of Texas. A retrospective analysis of the Public Use Data File, maintained by the Texas Health Care Information Collection was conducted. Background: Diabetes mellitus is a metabolic disorder defined by high blood glucose. Glucagon-like peptide-1 receptor agonists (GLP-1-RAs) are a newer class of medications that offer potential 2 to 3 kg weight loss and a 1% to 1.5% decrease in A1c. At the Indianapolis Veterans Affairs Medical Center (VAMC), GLP-1-RAs are non-formulary.
Objectives: Primary outcomes included mean change in weight, body mass index (BMI) and A1c in VAMC patients. Secondary outcomes included the comparison to published values, analysis of insulin needs, and analysis of GLP-1-RA discontinuation reasons.

This paper is a part of a larger research study exploring the STEM learning experiences of Black girls who participated in an informal STEM program – I AM STEM. Through the process of reflection and co-construction of counterstories, Black girls reclaimed authorship of their lives. They identified three major attributes of teachers who promoted their academic success and engagement in STEM learning as ones who (a) responded to their needs and built a community of learners, (b) interacted with their parents in a professional manner, and (c) encouraged them to think critically and creatively during the lessons. Numerous studies over the years have documented an effect of human chronotypes on physiological and psychological processes. Studies evaluating the impact of an individual’s chronotype on his/her academic achievement have indicated that morning chronotypes have an academic advantage over evening chronotypes. However, these studies did not account for the time of day in which the participants were being evaluated. The goal of the present study was to examine whether morning chronotypes do have an academic advantage over evening chronotypes when the time-of-day of classes and exams is taken into consideration. INTRODUCTION: Previous investigations have examined nutrient timing with untrained subject and less is known about effects in trained subjects.
OBJECTIVES: The purpose of this study was to examine the effects of a carbohydrate and protein supplement (CHO+PROT) on performance measures in trained weightlifters.
METHODS: 10 trained weightlifters (30.8±5.1 years, 177.4 ±4.0 cm, 94.3 ± 12.4 kg, training age = 5.3 ± 2.9 years) completed a 12 week training program utilizing block periodization. Mental and substance use disorders are predicted to exceed all physical disease processes causing major disability by 2020. Misuse and overdose of opioid pain medications is a significant public health concern in the United States. Approximately 1 in 4 patients receiving prescription opioids in primary care settings struggle with misuse. Half of all opioid prescriptions are written by primary care providers, including nurse practitioners. The current study is undertaken in relation to the recommendations of both Article 30 of the United Nations Convention on the Rights of People with Disabilities-UNCRPD, (UN, 2006) and the new Charter on Physical Education, Physical Activity and Sport (UNESCO, 2015) to serve as a building block in the improvement of the health of people with disabilities (PWDs).

A leukocyte differential count can be used to diagnosis a myriad blood disorders, such as infections, allergies, and efficacy of disease treatments. In recent years, attention has been focused on developing point-of-care (POC) systems to provide this test in global health settings. Acridine orange (AO) is an amphipathic, vital dye that intercalates leukocyte nucleic acids and acidic vesicles. It has been utilized by POC systems to identify the three main leukocyte subtypes: granulocytes, monocytes, and lymphocytes. There are a limited number of validated occupation-based cognitive assessments that are feasible in clinical settings. For individuals with acquired brain injury (ABI), ecologically valid cognitive assessments are needed to understand how cognition influences functional performance. This study implemented a quantitative exploratory correlational design using a battery of gold standard tabletop cognitive assessments as criterion measurements against the Medication Box Task assessment, an occupation-based cognitive assessment. Eight participants completed the test battery. The student researchers used Pearson correlations to analyze each participant’s scores on the Medication Box Task assessment and the scores on the battery of gold standard assessments. Results …

As the population and longevity of older adults’ increases, the prevalence of falls is becoming an ever-growing issue. Falls in older adults may lead to sedentary behavior, decreased independence, and lower quality of life. Evidence has shown that traditional exercise programs emphasizing strength and balance can decrease the fall risk in older adults, but may be difficult to sustain over time. The posttranslational modification of macromolecules caused by highly reactive and toxic α-dicarbonyls, such as methylglyoxal (MGO) derived from glycolysis, is associated with various age-related diseases including diabetic complications and neurodegenerative diseases. Aging and long-term hyperglycemia causes enhanced production of this highly reactive α-dicarbonyl, which reacts with macromolecules causing irreversible damage of DNA and protein promoting the formation of advanced glycation end products (AGEs). The formation and accumulation of AGEs in different cell types affect intracellular and extracellular structure and function. A 56-year hypertensive, multiparous woman presented to the cardiology unit with Canadian Cardiovascular Society (CCS) class-III angina and worsening dyspnea for the past few weeks. Her clinical examination showed high blood pressure and mid-systolic crescendo-decrescendo murmur radiating to carotids. However, there was no radio-femoral delay or significant blood pressure difference between her arms. Her transthoracic echocardiography (TTE) revealed moderate aortic stenosis (AS) and mid cavity left ventricular outflow (LVO) obstruction. Background: Teenage mothers are less likely to obtain postpartum examinations and contraception, increasing risks for negative social and health outcomes. The CenteringPregnancy® prenatal care model has been found to improve outcomes. However, little is known about factors influencing CenteringPregnancy® attendance or how this attendance impacts future pregnancy rates among teenagers.

Gating of ion channels is based on structural transitions between open and closed states. To uncover the chemical basis of individual gates, we performed a comparative experimental and computational analysis between two K+ channels, KcvS and KcvNTS. These small viral encoded K+ channel proteins, with a monomer size of only 82 amino acids, resemble the pore module of all complex K+ channels in terms of structure and function. Even though both proteins share about 90% amino acid sequence identity, they exhibit different open probabilities with ca. 90% in KcvNTS and 40% in KcvS. …

The fluoroquinolone resistance score (FQRS) predicts the probability of fluoroquinolone resistance with good discrimination. The score has been derived from patients with bloodstream infections caused by Gram-negative bacteria and is based on fluoroquinolone use within the past 6 months, among other clinical and health care exposure criteria. This study aims to examine the utility of the FQRS in patients with complicated urinary tract infections (cUTI) and determine whether extension of prior fluoroquinolone use to 12 months improves model discrimination.
