เหตุผลของการวิจัย : พยาบาลเป็นอาชีพที่เสี่ยงต่อการเกิดอาการปวดหลังส่วนล่าง(Low back pain) โดยอาการปวดหลังส่วนล่างนั้นมีผลกระทบต่อระดับคุณภาพชีวิต ทำให้สมรรถภาพของร่างกายและความสามารถในการทำงานลดลง คุณภาพในการดูแลผู้ป่วยลดลง และส่งผลต่อการขาดงาน รวมถึงส่งผลกระทบทางด้านเศรษฐกิจและการบริหารบุคลากรวัตถุประสงค์ : ศึกษาความชุกและปัจจัยเสี่ยงที่มีความสัมพันธ์กับอาการปวดหลังส่วนล่างในพยาบาลวิชาชีพ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทยตัวอย่างและวิธีการศึกษา : เก็บรวบรวมข้อมูลจากพยาบาลวิชาชีพที่ปฎิบัติงานในหอผู้ป่วยในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ผ่านเกณฑ์การคัดเข้าและยินยอมเข้าร่วมการวิจัย จำนวน 311 ราย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลเป็นแบบสอบถามชนิดตอบด้วยตนเอง ประกอบด้วย4 ส่วน ดังนี้ ส่วนที่ 1 ข้อมูลส่วนบุคคล ส่วนที่ 2 ข้อมูลเกี่ยวกับลักษณะงานประจำที่ทำ ส่วนที่ 3 ข้อมูลเกี่ยวกับอาการปวดหลังที่จำกัดความสามารถในการทำกิจวัตรประจำวัน ประเมินโดยใช้แบบสอบถามออสเวสทรีฉบับภาษาไทย ส่วนที่ 4 ข้อมูลสภาวะความเครียด ประเมินโดยแบบวัดความเครียด กรมสุขภาพจิต ข้อมูลที่ได้วิเคราะห์ด้วยสถิติเชิงพรรณนาในรูปของความถี่ ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน ใช้ไค-สแควร์และสถิติการถดถอยโลจิสติก เพื่อวิเคราะห์ความสัมพันธ์ระหว่างปัจจัยเสี่ยงกับอาการปวดหลังส่วนล่างผลการศึกษา : พบว่าความชุกของอาการปวดหลังส่วนล่างในรอบ 12 เดือนที่ผ่านมาของพยาบาลวิชาชีพ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย เท่ากับร้อยละ 39.2 ซึ่งอาการปวดหลังจำกัดความสามารถในการทำกิจวัตรประจำวันเล็กน้อย และพบว่ากลุ่มตัวอย่างมีความเครียดอยู่ในระดับปานกลาง ผลการวิเคราะห์ปัจจัยเสี่ยงที่มีความสัมพันธ์กับอาการปวดหลังส่วนล่าง ด้วยสถิติการถดถอยโลจิสติกพบว่าปัจจัยที่มีผลต่ออาการปวดหลังส่วนล่าง อย่างมีนัยสำคัญทางสถิติที่ระดับ P <0.05 ได้แก่ การยกของหนัก (OR = 5.597, 95%CI = 3.275-9.565) การอยู่ในท่าทางที่ไม่ดีหรือไม่ถูกต้องขณะทำงาน(OR = 2.447, 95%CI = 1.185-5.053) และความเครียดระดับสูงถึงรุนแรง (OR = 2.312, 95%CI = 1.341-3.985)สรุป : ความชุกของอาการปวดหลังส่วนล่างในพยาบาลวิชาชีพ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ค่อนข้างสูง ซึ่งปัจจัยเสี่ยงของอาการปวดหลังส่วนล่าง คือ การยกของหนัก รวมถึงการอยู่ในท่าทางที่ไม่ดีหรือไม่ถูกต้องขณะทำงาน และระดับความเครียด จากผลการศึกษาปัจจัยเสี่ยงของอาการปวดหลังส่วนล่าง ทำให้ได้แนวทางในการป้องกันการเกิดอาการปวดหลังส่วนล่างในพยาบาลวิชาชีพ ตามสาเหตุที่เกี่ยวข้องได้.

เหตุผลของการทำวิจัย : จากการศึกษาค้นคว้าเกี่ยวกับการเตรียมตัวก่อนเข้าสู่วัยสูงอายุของพยาบาลวิชาชีพที่ผ่านมา ส่วนใหญ่เป็นการศึกษาจากกลุ่มตัวอย่างที่เป็นข้าราชการหรือปฏิบัติงานในกลุ่มสังกัดกระทรวงสาธารณสุขผลการศึกษาจึงอยู่ในบริบทที่อยู่ในวงจำกัดและอาจแตกต่างจากภาคเอกชน ผู้วิจัยจึงสนใจประเด็นคุณภาพชีวิต และการเตรียมตัวเพื่อเข้าสู่วัยสูงอายุของพยาบาลวิชาชีพที่ปฏิบัติงานในภาคเอกชน ซึ่งการศึกษานี้น่าจะเป็นประโยชน์ต่อหน่วยงานที่เกี่ยวข้อง และเป็นข้อมูลพื้นฐานในการศึกษาค้นคว้าแก่ผู้ที่สนใจต่อไปวัตถุประสงค์ : เพื่อศึกษาคุณภาพชีวิตและการเตรียมความพร้อมเข้าสู่วัยสูงอายุของพยาบาลวิชาชีพในโรงพยาบาลเอกชน เขตกรุงเทพมหานครตัวอย่างและวิธีการศึกษา : ศึกษาจากกลุ่มตัวอย่างจำนวน 165 คน โดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการสนับสนุนทางสังคม แบบสอบถามคุณภาพชีวิตและแบบสอบถามการเตรียมพร้อมเข้าสู่วัยสูงอายุวิเคราะห์ข้อมูลด้วยโปรแกรม SPSS ใช้สถิติเชิงพรรณนา ได้แก่ ร้อยละค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ใช้สถิติเชิงอนุมาน ได้แก่ t-test,F-test (One-way ANOVA), Pearson product moment correlationcoefficient และทำนายปัจจัยโดยใช้การวิเคราะห์ถดถอยพหุแบบขั้นตอน (Stepwise multiple regression analysis)ผลการศึกษา : พยาบาลวิชาชีพส่วนใหญ่ (ร้อยละ 74.5) มีคุณภาพชีวิตอยู่ในระดับปานกลางการเตรียมความพร้อมเข้าสู่วัยสูงอายุ พบคะแนนเฉลี่ยอยู่ในระดับปานกลางทั้ง 6 ด้าน ได้แก่ ด้านจิตใจ ด้านการเงิน ด้านบทบาทในครอบครัว ด้านสุขภาพกาย ด้านการใช้เวลา และด้านที่อยู่อาศัยปัจจัยที่เกี่ยวข้องกับคุณภาพชีวิต อย่างมีนัยสำคัญทางสถิติที่ระดับ0.05 ได้แก่ ระดับการศึกษา ตำแหน่งงาน ปัจจัยการสนับสนุนทางสังคม และการเตรียมความพร้อมเข้าสู่วัยสูงอายุมีความสัมพันธ์ทางบวกกับคุณภาพชีวิต อย่างมีนัยสำคัญ ที่ระดับ P <0.01 ปัจจัยที่สามารถทำนายคุณภาพชีวิตของพยาบาลวิชาชีพ ได้แก่ การสนับสนุนทางสังคมจากผู้บังคับบัญชา เพื่อนร่วมงาน ครอบครัว ร่วมกับการเตรียมความพร้อมเข้าสู่วัยสูงอายุ (r2 = 0.593, P <0.01)สรุปผลการวิจัย : พยาบาลวิชาชีพส่วนใหญ่มีคุณภาพชีวิตอยู่ในระดับปานกลางการตระหนักถึงปัจจัยการสนับสนุนทางสังคมจากผู้บังคับบัญชาเพื่อนร่วมงาน ครอบครัวและการเตรียมความพร้อมเข้าสู่วัยสูงอายุน่าจะเป็นประโยชน์ในการส่งเสริมคุณภาพชีวิตของพยาบาลวิชาชีพให้ดียิ่งขึ้นได้.

กิจกรรมสร้างงานศิลปะและความคิดสร้างสรรค์ของเด็กนักเรียนในโรงเรียนประถมศึกษากรุงเทพมหานคร, ภรินธร วินิจฉายะจินดา, วัลย์ฐิภา วิทยาศัย

เหตุผลของการทำวิจัย : จากผลการสำรวจพบว่าทักษะการคิดวิเคราะห์สร้างสรรค์ของเด็กไทยอยู่ในเกณฑ์ต่ำ แต่ทักษะการคิดสร้างสรรค์นี้เป็นสิ่งที่สามารถฝึกให้พัฒนาขึ้นได้ด้วยการจัดกิจกรรมที่กระตุ้นให้เกิดการพัฒนาการคิดที่เหมาะสมวัตถุประสงค์ : เพื่อศึกษาผลของกิจกรรมสร้างงานศิลปะผ่านการเล่นที่มีต่อความคิดสร้างสรรค์ของเด็กนักเรียนชั้นประถมศึกษาตัวอย่างและวิธีการศึกษา : เด็กนักเรียนระดับชั้นประถมศึกษาปีที่ 2 และ 3 ในโรงเรียนประถมศึกษา แบ่งกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 21 คนวัดผลก่อนและหลังการทดลองด้วยแบบทดสอบความคิดสร้างสรรค์จากการวาดภาพ (TCT-DP) ให้กลุ่มทดลองทำกิจกรรม 10 ครั้งก่อนจากนั้นจึงให้กลุ่มควบคุมได้ทำ สถิติที่ใช้ ได้แก่ descriptive statisticanalysis, Non – parametric Mann – Whitney Test และ Non –parametric Wilcoxon Signed Ranks Testผลการศึกษา : หลังการทดลอง กลุ่มทดลองมีผลคะแนนเฉลี่ยความคิดสร้างสรรค์เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 (P = 0.002) และกลุ่มควบคุมที่ไม่ได้รับกิจกรรมใด ๆ มีคะแนนความคิดสร้างสรรค์เฉลี่ยสูงขึ้น แต่ไม่มีนัยสำคัญทางสถิติ (P = 0.08) หลังจากให้กลุ่มควบคุมได้ทำกิจกรรม พบว่ากลุ่มควบคุมมีคะแนนความคิดสร้างสรรค์เฉลี่ยเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 (P < 0.001) เช่นเดียวกับในกลุ่มทดลองสรุป : เด็กนักเรียนระดับชั้นประถมศึกษาทั้งกลุ่มทดลอง และกลุ่มควบคุมมีคะแนนความคิดสร้างสรรค์เฉลี่ยเพิ่มขึ้นอย่างต่อเนื่อง และมีการเพิ่มขึ้นรวดเร็วอย่างเห็นได้ชัดหลังได้ทำกิจกรรมสร้างงานศิลปะ แสดงว่ากิจกรรมสร้างงานศิลปะ มีผลต่อความคิดสร้างสรรค์ของเด็กนักเรียนในโรงเรียนประถมศึกษา.

การพัฒนาเครื่องมือวัดประสบการณ์การเผชิญเหตุการณ์รบกวนใจสำหรับนักเรียนชั้นมัธยมศึกษา, นวพรรษ เชื้อต่าย, ปริชวัน จันทร์ศิริ

เหตุผลของการทำวิจัย : บาดแผลทางใจมีผลกระทบต่อสุขภาพจิตเป็นอย่างมาก โดยเฉพาะบาดแผลทางใจที่เกิดจากเหตุการณ์รบกวนใจ ที่สามารถเกิดขึ้นได้บ่อยในชีวิตประจำวัน การสร้างเครื่องมือสนใจศึกษาดังกล่าวขึ้นเพื่อใช้สำรวจทำความเข้าใจเกี่ยวกับเหตุการณ์รบกวนใจในนักเรียนชั้นมัธยมศึกษา จะสามารถนำผลการวิจัยไปใช้เป็นแนวทางในการป้องกันแก้ไข ปัญหาที่เกิดจากเหตุการณ์รบกวนใจวัตถุประสงค์ : เพื่อสร้างเครื่องมือที่สามารถแบ่งนักเรียนที่มีประสบการณ์การเผชิญเหตุการณ์รบกวนใจเป็นกลุ่มที่มีประสบการณ์มาก ปานกลาง น้อยและเพื่อศึกษาเหตุการณ์รบกวนใจของนักเรียนชั้นมัธยมศึกษาตัวอย่างและวิธีการศึกษา : กลุ่มตัวอย่างที่ใช้ในการศึกษาวิจัยในครั้งนี้ คือ นักเรียนชั้นมัธยมศึกษาเพศหญิง จำนวน 660 คน ซึ่งได้จากการคำนวณขนาดของกลุ่มตัวอย่างตามจำนวนของข้อคำถามที่ใช้วิเคราะห์องค์ประกอบ (Factor analysis)โดยใช้เครื่องมือแบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามประสบการณ์การเผชิญเหตุการณ์รบกวนใจ และวิเคราะห์คุณภาพเครื่องมือ โดยใช้ค่าความเที่ยงตรงเชิงเนื้อหา ค่าความเที่ยงตรงเชิงโครงสร้าง ค่าความเชื่อมั่น ค่าอำนาจจำแนก และวิเคราะห์ค่าตำแหน่ง percentile เพื่อกำหนดจุดตัดค่าคะแนนผลการศึกษา : พบว่านักเรียนส่วนใหญ่มีประสบการณ์การเผชิญเหตุการณ์รบกวนใจในระดับน้อย ( = 105.46) โดยปัจจัยที่สัมพันธ์กับระดับประสบการณ์การเผชิญเหตุการณ์รบกวนใจมีเพียงเพศ กับสถานภาพครอบครัว(Chi-square; P = 0.006 และ 0.019) ในส่วนของคุณภาพเครื่องมือพบว่าแบบสอบถามมีค่าความเที่ยงตรงเชิงเนื้อหา ค่าความเที่ยงตรงเชิงโครงสร้าง ค่าความเชื่อมั่น (Cronbach’s Alpha = 0.936) ค่าอำนาจจำแนกอยู่ในเกณฑ์ดีสรุป : นักเรียนส่วนใหญ่มีประสบการณ์การเผชิญเหตุการณ์รบกวนใจในระดับน้อยโดยปัจจัยที่สัมพันธ์กับระดับประสบการณ์การเผชิญเหตุการณ์รบกวนใจ มีเพียงเพศกับสถานภาพครอบครัว ส่วนของคุณภาพเครื่องมือพบว่าแบบสอบถามมีคุณภาพอยู่ในเกณฑ์ดี.

Lymphangiomas are congenital malformations of lymphatic tissue. This benign condition is generally found in the neck region and is discovered by 2 years of age. Here we report the case of a 12 year old child who presented with a cervical and mediastinal cystic hygroma that had progressed to such a space occupying nature as to cause the shifting of the trachea. The entire mass was successfully resected with a lower neck incision. Our case suggests the necessity of considering cystic lymphangioma in the differential diagnosis of lateral neck masses and use of adequate imaging to substantiate the preoperative diagnosis.

Elbow Injuries In The Throwing Athlete: A Review, Drew K. Newhoff

Overhand-throwing motions can create supraphysiologic stresses on the elbow. Repeated stresses often result in overuse injuries affecting the elbow, which are commonly seen in throwing athletes. Noted problems include medial epicondylitis, valgus extension overload, olecranon stress fractures, ulnar neuritis, and damage to the ulnar collateral ligament. Symptoms of pain or numbness at presentation vary in location depending on the injury. Successful diagnosis is typically made using results of physical examinations of elbow, wrist, and arm extensions; results of tests for valgus instability; and findings radiographs can all help indicate the problem. Anterior shoulder instability can be problematic in the young, active population, particularly in athletes and military personnel. The shoulder joint is the most frequently dislocated joint, and there is a high rate of dislocation recurrence in younger patients. The stability of the glenohumeral joint is conferred through the bony anatomy and the static and dynamic stabilizers. Necrotizing fasciitis, an infection of the skin and subcutaneous tissue, can spread rapidly. Studies have emphasized the importance of prompt surgical intervention with aggressive debridement of diseased tissue; however, mortality rates are high despite immediate treatment. We describe two men, aged 44 years (case 1) and 74 years (case 2), presenting with necrotizing fasciitis who underwent aggressive surgical intervention, multiple debridements, and eventual transhumeral amputation of the upper extremity. In each case, the patients were discharged from the hospital to their homes on amoxicillin-clavulanate at postoperative days 15 (case 1) and 32 (case 2). Treatment of subtrochanteric femur fractures can be difficult owing to high risk of complications. No cases of sciatic nerve palsy after hematoma following open reduction and intramedullary nailing of a subtrochanteric femur fracture have been reported. We describe a 28-yearold man who presented with a subtrochanteric fracture in the left femur after a motor-vehicle collision, in whom open reduction and intramedullary nailing led to an immediate postoperative hematoma and sciatic nerve palsy. We describe a 29-year-old former collegiate women’s basketball player in whom MS was diagnosed during a visit for recurrent evaluation of Bell palsy. Objectives: The purpose of this study was to determine the best predictor of training and/or match load using session RPE. Design and Methods: 20 NCAA DI male soccer players participated in the study during the 2014 and 2015 competitive seasons. Players completed 15.20 ± 1.05 matches for a total of 304 individual data points and 29.90 ± 1.89. training sessions for a total of 598 individual data points. GPS variables (total distance, High-intensity running distance, and Player load) were analyzed with session RPE using Pearson product-moment correlations. Objectives: The purpose of this study was to examine the relationship of velocities in the back squat between one repetition maximum (1RM) and submaximally loaded repetition maximum (RM) conditions, specifically in regard to what has been described as the minimal velocity threshold (MVT). The MVT describes a minimum concentric velocity that an individual must reach or surpass in order to successfully complete a repetition. The purpose of the current study was (1) to examine the differences between standing and lying measures of vastus lateralis (VL), muscle thickness (MT), pennation angle (PA), and cross-sectional area (CSA) using ultrasonography; and (2) to explore the relationships between lying and standing measures with isometric and dynamic assessments of force production—specifically peak force, rate of force development (RFD), impulse, and one-repetition maximum back squat. Fourteen resistance-trained subjects (age = 26.8 ± 4.0 years, height = 181.4 ± 6.0 cm, body mass = 89.8 ± 10.7 kg, back squat to body mass ratio = 1.84 ± 0.34) agreed to participate. …

Purpose To assess radiography faculty perceptions of the effectiveness of online courses.

Methods An original survey instrument was created by selecting items from 3 instruments used in prior research and adding unique questions designed to elicit demographic data from faculty. The sample included a national dataset of radiography faculty members employed in Joint Review Committee on Education in Radiologic Technology–accredited programs in the United States.

Results Findings showed that faculty perceptions of online course effectiveness are not affected significantly by faculty position, type of institution, faculty age, or years of teaching experience. Positive perceptions of the effectiveness of online courses …

Within the past 10 years, mobile devices have been widely adopted by adults and are now present in the lives of almost all U.S. children. While phones are common, our understanding of what effect this technology has upon children's development is lagging. Bioecological theory and attachment theory suggest that this new technology may be disruptive, especially to the degree to which it interferes with the parent-child relationship. This article reflects a National Organization for Human Services conference presentation and shares preliminary results from semi-structured interviews conducted with 18 youth, ages 7 through 11. This article is a demonstration of how an implementation-science (IS) framework can be coupled with the measurement of implementation outcomes to effectively integrate evidence-based family interventions in primary care. The primary care environment presents a number of challenges for successfully integrating such interventions. However, IS methods can improve the prospect of successfully implementing a new intervention while simultaneously and rigorously evaluating the impact on salient outcomes. A systems theoretical analysis to capture the evolution and transition of the network systems supporting Ebola survivors and their affected communities, during the 2014-15 Ebola outbreak and recovery phases. The qualitative analysis includes a literature review, archival review, and interviews with representatives of key actors operating in strategic action fields. Epstein-Barr virus (EBV), an oncogenic virus that ubiquitously establishes life-long persistence in humans, encodes viral miRNAs in two clusters, BHRF1 and BART. EBV also regulates expression of a large pool of cellular miRNAs, including miR-155, miR-146a, miR-21, miR-29, and miR-34a. These miRNAs targets both viral and cellular genes involved in the entire viral lifetime from lytic infection to oncogenesis, including viral replication, immune responses, cell cycle regulation, apoptosis, and cell proliferation, and are indispensable for persistent infection, latency establishment and maintenance, and cancer development.
