Despite decades of compelling evidence regarding patient and family interest to be with loved ones or the lack of detrimental effects from being together, some nurses are not convinced of the merits of family presence (FP). Implementation of family presence in adult ICUs remains controversial for nurses. The feelings of many nurses, who are gate keepers related to patient visitation, continue to influence restrictions and/or inconsistent visiting practices for patients and families. African American patients have been reported as having a greater number of aggressive cancer treatments compared to Caucasians (McWayne & Heiney, 2005; Meeske et al., 2009; Ridner & Dietrich, 2008), and have higher incidence of BCRLE due axillary lymph node dissection (ALND) and radiation intervention (Thomas-MacLean, Miedema, Tateemichi, 2005). Research regarding BCRLE has used BCRLE samples comprised almost exclusively of married and well-educated Caucasian women. Few studies (Bowman, Deimling, Smerglia, Sage, & Kahana, 2003; Eversley et al., 2005; Joslyn, 2002; McWayne & Heiney, 2005) have included sizeable numbers of African American breast cancer survivors. Acid sphingomyelinase (ASM) has been implicated in the development of hyperhomocysteinemia (hHcys)-induced glomerular oxidative stress and injury. However, it remains unknown whether genetically engineering of ASM gene produces beneficial or detrimental action on hHcys-induced glomerular injury. The present study generated and characterized the mice lacking cystathionine β-synthase (Cbs) and Asm mouse gene by cross breeding Cbs+/− and Asm+/− mice. Given that the homozygotes of Cbs−/−/Asm−/− mice could not survive for 3 weeks. Cbs+/−/Asm+/+, Cbs+/−/Asm+/− and Cbs+/−/Asm−/− as well as their Cbs wild type littermates were used to study the role of Asm−/− under a background of Cbs+/− with hHcys. HPLC …

My mother has always referred to smoking cigarettes as “the lesser of the evils.” Hearing that statement from someone who has been in and out of drug and alcohol rehabilitation programs her whole life, I suppose you could sympathize with it. In fact, most rehabilitation centers dedicate certain times of the day just to allow their patients to smoke. Although my mother considers smoking not to be an “evil” worth sweating over, it will most likely ultimately result in her demise.

International maritime health has largely developed within the sphere of occupational health services and international health problems. We reviewed publications in the journal International Maritime Health from 2000 to 2010 to establish the coverage of the journal and the scope of research in maritime health. We identified six thematic categories: healthcare access, delivery and integration; telehealth; non-communicable diseases and physical health problems; communicable diseases; psychological functioning and health; and safety-related issues. The Patient Centered Medical Home (PCMH) model of care incorporates many public health concepts. Some supporters of PCMH regard the Affordable Care Act (ACA) as legislation that will invigorate PCMH progress across the country. This policy analysis intends to understand the influence of the ACA on the development of PCMH. The policy analysis compares relevant sections of the ACA with core PCMH concepts agreed upon by major medical professional organizations. Background: Type 1 diabetes (T1DM) is a chronic illness that requires intensive treatment to improve glycemic control and limit the risk of diabetes-related complications. The management of T1DM is challenging for adolescents as the diabetes self-care activities invade all aspects of their life. Purpose: The purpose of this study was to determine if vital tooth whitening affects oral health-related quality of life (OHRQOL) in adults age 50 years and older, and if tooth whitening causes increased participation in social activities.

Methods: Using a 2 group, single blind, randomized, pre-test, multiple post-test design, 62 participants were enrolled. The experimental group used a whitening product twice daily for 3 weeks. The control group used no whitening products. The Oral Health Impact Profile (OHIP) served as the pre- and post-test measure. Affecting a large percentage of human population herpes simplex virus (HSV) types -1 and -2 mainly cause oral, ocular, and genital diseases. Infection begins with viral entry into a host cell, which may be preceded by viral “surfing” along filopodia. Viral glycoproteins then bind to one or more of several cell surface receptors, such as herpesvirus entry mediator (HVEM), nectin-1, 3-O sulfated heparan sulfate (3-OS HS), paired immunoglobulin-like receptor, and non-muscle myosin-IIA. At least five viral envelope glycoproteins participate in entry and these include gB, gC, gD and gH-gL. Post-entry, these glycoproteins may also facilitate cell-to-cell spread of the virus, …

Objective: Medical schools have always strived to develop high quality assessment questions that have the capacity to test the high order thinking of clinical year medical students. While clinicians are enthusiastic in their contribution to writing these questions, they often lack confidence in their ability to write good quality items. The benefits of running targeted question development workshops have not been adequately investigated.

The Glasgow Coma Scale (GCS) was designed for use in a wide range of clinical environments with staff that had no special training (Teasdale and Jennett 1974). Evidence (Waterhouse 2007 and 2008) suggests however that there are ongoing problems with the use of the GCS assessment, and the impact that this might have on patient outcome is unknown. The association between the use of tumor necrosis factor-α inhibitors and the increased risk of granulomatous infections, especially tuberculosis, has been well documented. Given the rapidly expanding list of inflammatory conditions for which tumor necrosis factor-α inhibitors are receiving FDA approval, the incidence of tuberculosis in this patient population has increased. Despite heightened awareness by physicians, the diagnosis of tuberculosis can remain challenging, given that extrapulmonary sites of infection are more frequently involved. We present a case of pulmonary and peritoneal tuberculosis in a gentleman being treated with a tumor necrosis factor-α inhibitor and discuss the diagnostic challenges of establishing …
