Background — This paper presents results from a public engagement effort in Nebraska, USA, which measured public opinions about governmental involvement in encouraging the use of electronic health records (EHRs).
Objective — We examine the role of trust in government in contributing to public support for government involvement in the development of EHR technologies. We hypothesize that trust in government will lead to support for federal and state governmental encouragement of the use of EHRs among doctors and insurance companies. Further, because individual experiences with health-care professionals will reduce perceptions of risk, we expect that support for governmental involvement will …

Snake venoms are rich sources of biologically active proteins and polypeptides. Three-finger toxins are non-enzymatic proteins present in elapid (cobras, kraits, mambas and sea snakes) and colubrid venoms. These proteins contain four conserved disulfide bonds in the core to maintain the three-finger folds. Although all three-finger toxins have similar fold, their biological activities are different. A new three-finger toxin (hemachatoxin) was isolated from Hemachatus haemachatus (Ringhals cobra) venom. Its amino acid sequence was elucidated, and crystal structure was determined at 2.43 Å resolution. The overall fold is similar to other three-finger toxins. Animals have to cope with starvation. The molecular mechanisms by which animals survive long-term starvation, however, are not clearly understood. When they hatch without food, C. elegans arrests development at the first larval stage (L1) and survives more than two weeks. Here we show that the survival span of arrested L1s, which we call L1 longevity, is a starvation response regulated by metabolic rate during starvation. A high rate of metabolism shortens the L1 survival span, whereas a low rate of metabolism lengthens it. The longer worms are starved, the slower they grow once they are fed, suggesting that L1 …

Numerous investigations of cortical crossmodal plasticity, most often in congenital or early-deaf subjects, have indicated that secondary auditory cortical areas reorganize to exhibit visual responsiveness while the core auditory regions are largely spared. However, a recent study of adult-deafened ferrets demonstrated that core auditory cortex was reorganized by the somatosensory modality. Because adult animals have matured beyond their critical period of sensory development and plasticity, it was not known if adult-deafening and early-deafening would generate the same crossmodal results. The dorsal lateral geniculate nucleus (dLGN) serves as the primary conduit of retinal information to visual cortex. In addition to retinal input, dLGN receives a large feedback projection from layer VI of visual cortex. Such input modulates thalamic signal transmission in different ways that range from gain control to synchronizing network activity in a stimulus-specific manner. However, the mechanisms underlying such modulation have been difficult to study, in part because of the complex circuitry and diverse cell types this pathway innervates. The purpose of this paper is to determine the exact dangers of leisure music to society, as peoples’ hearing can be negatively impacted by excessive exposure to music, in terms of both duration and sound (dB) level. Two types of studies are analyzed. One study analyzes the effects of concert and disco style music on musicians and party guests, primarily through experiments which test pure-tone audiometry, distortion product otoacoustic emissions (DPOAE), and general sound levels of people and places before, during, and after exposure. This study assessed whether and how self-explanation reading training, provided by iSTART (Interactive Strategy Training for Active Reading and Thinking), improves the effectiveness of comprehension processes. iSTART teaches students how to self-explain and which strategies will most effectively aid comprehension from moment-to-moment. We used RSAT (Reading Strategy Assessment Tool) to assess how iSTART changes the relation between important selfexplanation reading strategies—bridging and elaboration—and online comprehension, and how often they are produced. College and high school students received iSTART and were administered RSAT prior to and post-training. Results from three experiments showed that iSTART primarily benefits bridging inferences when self explaining. …

Mindfulness refers to an enhanced attention to and awareness of present moment experience. This study examined how trait mindfulness, as measured with six items from Mindfulness Attention Awareness Scale, might influence adolescent cigarette smoking frequency through its impact on depressive affect, anger affect and perceived stress mediators. Self-reported data from Chinese adolescents (N = 5287, mean age = 16.2 years, SD = 0.7; 48.8% females) were collected within 24 schools. The product of coefficients test was used to determine significant mediation paths. This study evaluated effects of a key session from a nationally recognized drug abuse prevention program on basic memory processes in 211 high-risk youth in Southern California. In a randomized, between-subject design, the authors manipulated assignment to a Myth and Denial program session and the time of assessment (immediate vs. 1-week delay). The authors examined program decay effects on memory accessibility and judgment errors. Those participants exposed to the program session generated more myths and facts from the program than those in the control group, suggesting that even a single program session influenced students’ memory for program information and this …

Objective This study investigated patterns of tooth deformation in response to immersion in hot and cold water, and attempted to correlate the measured dimensional change with calculated values based on coefficients of thermal expansion. Materials and methods The extent of cuspal deformation and strains within dentine were assessed in extracted human molars, in response to immersion in hot and cold water. Cuspal displacement was measured using an extensometer attached to the buccal and lingual cusps, and strains on the pulpal dentine surface were measured using strain gauges attached to the occlusal pulpal surface. วัสดุเรซินคอมโพสิตถูกแนะนํามาใช้เป็นวัสดุบูรณะฟันครั้งแรกในปี ค.ศ. 1956 บริษัทผู้ผลิตได้มีการ พัฒนาวัสดุชนิดนี้มาอย่างต่อเนื่องทั้งในส่วนของวัสดุอัดแทรกและเมทริกซ์อินทรีย์ อย่างไรก็ตาม แม้ว่าวัสดุเรซิน คอมโพสิตในปัจจุบันจะแสดงคุณสมบัติที่ดีเหมาะกับการนําไปใช้ในทางคลินิก แต่ยังคงพบค่าการหดตัวจากการ เกิดปฏิกิริยาพอลิเมอไรเซชันจากตัววัสดุเองในระดับหนึ่งซึ่งทําให้เกิดปัญหาต่าง ๆ ตามมา ปัจจุบันมีแนวคิดในการ ใช้เรซินคอมโพสิตที่ไม่ใช้เมทาคริยเลทเป็นส่วนประกอบในเมทริกซ์อินทรีย์เพื่อลดปัญหาดังกล่าว (ว ทันต จุฬาฯ 2555;35:65-78)

Objective The purpose of this study was to investigate factors associated with turnover intention of Thai dentists. Materials and methods A mailed survey was conducted on a systematic random sample of 2,723 Thai dentists, 733 of which returned completed questionnaires. Turnover intention was assessed by dentistsû intenion to change job or have a career break within 5 years. Demographic, economic, practice characteristic and psychosocial work environment factors were used to build three logistic models for the public and private sectors, as well as for the overall sample. วัตถุประสงค์ เพื่อสํารวจความชุกของอาการของเท็มโพโรแมนดิบิวลาร์ดิสออเดอร์ (ทีเอ็มดี) ในกลุ่มนักดําน้ํา สกูบาไทย และหาปัจจัยเกี่ยวข้องที่สัมพันธ์กับการเกิดอาการเหล่านั้นหลังดําน้ํา วัสดุและวิธีการ กลุ่มตัวอย่างเป็นนักดําน้ําไทยที่ได้จากการคัดเลือกแบบเจาะจง 280 ราย โดยให้ตอบ แบบสอบถามที่เกี่ยวกับอาการที่เอ็มดีทั้งในชีวิตประจําวันก่อนดําน้ํา ขณะดําน้ํา และหลังดําน้ํา รวมถึงคําถาม เกี่ยวกับปัจจัยต่าง ๆ ที่อาจสัมพันธ์กับการเกิดอาการที่เอ็มดี วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณาและการ วิเคราะห์ถดถอยพหุแบบลอจิสติก ผลการศึกษา ความชุกของอาการที่เอ็มดี (อย่างน้อย 2 อย่างขึ้นไป) หลังดําน้ําคิดเป็นร้อยละ 22.9 โดย อาการเมื่อยขากรรไกรพบได้มากที่สุด (ร้อยละ 26.4) รองลงมา คือ ปวดข้อต่อขากรรไกร (ร้อยละ 21.8) และ ปวดกล้ามเนื้อขากรรไกร (ร้อยละ 15.0) แต่มีเพียงร้อยละ 1.9 ที่รายงานว่า อาการผิดปกตินี้รุนแรงถึงกับต้องหยุด ดําน้ํา ในขณะที่ร้อยละ 96.3 อาการผิดปกติค่อย ๆ ดีขึ้นเองโดยไม่ต้องรับการรักษา ปัจจัยที่สัมพันธ์กับการเกิด อาการหลังดําน้ําอย่างมีนัยสําคัญ (p < 0.05) ได้แก่ การมีชีวิตที่เคร่งเครียด (OR-2.27; 95% CI: 1.10-4.69) และมีอาการที่เอ็มดีอย่างน้อย 1 อย่างก่อนการดําน้ํา (OR-4.06, 95% CI: 2.13-7.76) ส่วนอายุ การนอนกัดฟัน การกัดเน้นฟัน ประวัติการจัดฟัน ผ่าฟันคุด การบาดเจ็บที่ขากรรไกร-ใบหน้า และความกังวลในขณะดําน้ํา ไม่มี ความสัมพันธ์อย่างมีนัยสําคัญกับการรายงานอาการที่เอ็มดีหลังดําน้ํา สรุป อาการที่เอ็มดีหลังดําน้ําพบได้ในกลุ่มนักดําน้ําไทย แต่อาการที่เกิดขึ้นมักไม่รุนแรงและหายได้เอง ปัจจัยที่ สําคัญในการเกิดอาการผิดปกติเหล่านั้น คือ มีชีวิตที่เคร่งเครียด และปรากฏอาการที่เอ็มดีอย่างน้อยหนึ่งอาการก่อนการดำนํ้า (ว ทันต จุฬาฯ 2555;35:15-26)

วัตถุประสงค์ เพื่อสังเคราะห์และศึกษาลักษณะของโครงค้ํายันคอลลาเจนที่สกัดจากหนังสุกรผสมไฮดรอกซีอะพาไทต์สําหรับการนําไปใช้เป็นโครงค้ํายันในการชักนําให้เกิดการซ่อมแซมกระดูก วัสดุและวิธีการ คอลลาเจนที่ใช้ในการเตรียมโครงค้ํายันในการศึกษานี้ สกัดได้จากการละลายหนังสุกรในกรด ร่วมกับเอนไซม์ และตกตะกอนด้วยเกลือ นําคอลลาเจนที่สกัดได้มาทําเป็นสารละลายที่มีความเข้มข้น ร้อยละ 1.5 ร้อยละ 2 และร้อยละ 2.5 โดยน้ําหนักตามลําดับ แล้วผสมเข้ากับผลึกไฮดรอกซีอะพาไทต์ ในอัตราส่วนไฮดรอกซี อะพาไทต์ต่อสารละลายคอลลาเจน 1:10 (น้ําหนักต่อปริมาตร) สารผสมของคอลลาเจนและไฮดรอกซีอะพาไทต์ ถูกทําให้เป็นโครงค้ํายันที่มีลักษณะคล้ายฟองน้ําด้วยการแช่แข็งและอบแห้งเยือกแข็งเป็นเวลา 24 ชั่วโมง โดยใช้ อุณหภูมิในการแช่แข็งแตกต่างกัน 2 กลุ่ม คือ กลุ่มที่ทําให้อุณหภูมิแช่แข็งต่ําลงอย่างช้า ๆ และกลุ่มที่ทําให้ อุณหภูมิแช่แข็งต่ําลงอย่างรวดเร็ว จากนั้นทําให้โครงค้ํายันคอลลาเจนคงตัวด้วยการทําปฏิกิริยาเชื่อมขวางศึกษาลักษณะโครงสร้างโดยละเอียดของโครงค้ํายันคอลลาเจนที่สังเคราะห์ภายใต้สภาวะแตกต่างกันด้วยกล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราด และศึกษาขนาดของรูพรุนด้วยกล้องจุลทรรศน์ชนิดแสงขาว วิเคราะห์ ความแตกต่างของขนาดรูพรุนด้วยวิธีการทางสถิติ ผลการศึกษา โครงค้ํายันคอลลาเจนที่สังเคราะห์ขึ้นด้วยวิธีการดังกล่าว มีลักษณะคล้ายฟองน้ํา กดนิ่ม สีขาว ไม่มีกลิ่นและละลายน้ําได้อย่างรวดเร็ว และเมื่อผ่านการทําปฏิกิริยาเชื่อมขวางแล้ว โครงค้ํายันมีความคงตัว มากขึ้น จากการศึกษาด้วยกล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราดพบว่าโครงค้ํายันมีลักษณะเป็นรูพรุน ประกอบด้วยแผ่นที่เกิดจากการอัดแน่นของเส้นใยคอลลาเจนจํานวนมากสานกันไปมา โดยมีผลึกของไฮดรอกซีอะพาไทต์ติดอยู่ที่ผนังของรูพรุนเหล่านี้ จากการศึกษาเปรียบเทียบลักษณะของโครงค้ํายันที่เตรียมภายใต้สภาวะแตกต่างกัน (P>0.05) ในขณะที่โครงค้ํายันที่ผลิตจากคอลลาเจนที่มีความเข้มข้นเดียวกัน แต่เตรียมภายใต้สภาวะ แช่แข็งต่างกัน มีขนาดของรูพรุนแตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p < 0.05) กล่าวคือ โครงค้ํายันที่ใช้ อุณหภูมิแช่แข็งต่ําลงอย่างช้า ๆ ขนาดเฉลี่ยของรูพรุนใหญ่กว่าโครงค้ํายันที่ใช้อุณหภูมิแช่แข็งต่ําลงอย่างรวดเร็ว สรุป คอลลาเจนที่สกัดได้จากหนังสุกร เมื่อนําไปผสมกับไฮดรอกซีอะพาไทต์ สามารถนําไปผลิตเป็นโครงค้ํายันที่ คงตัวได้ด้วยวิธีอบแห้งเยือกแข็งร่วมกับการทําปฏิกิริยาเชื่อมขวาง โดยลักษณะและขนาดของรูพรุนโครงค้ํายันคอลลาเจนผสมไฮดรอกซีอะพาไทต์ มีความสัมพันธ์กับความเร็วในการลดต่ําลงของอุณหภูมิในการแช่แข็ง แต่ไม่สัมพันธ์กับความเข้มข้นของสารละลายคอลลาเจนที่ใช้ในการสังเคราะห์ (ว ทันต จุฬาฯ 2555;35:1-14)

Postural control was evaluated in cochlear implant participants with and without amplification under several auditory paradigms. Speed of sway was recorded in each condition by means of Computerized Dynamic Posturography. Results indicate that an external sound source significantly improves balance in patients with cochlear implants.

The focus of this study was to review existing literature and analyze a survey of professional opinion regarding how children with hearing loss caused by congenital cytomegalovirus (CMV) function audiologically and educationally. Insert and circumaural earphones were used during visual reinforcement audiometry with children 12-to 24-months of age. Acceptance of earphones was determined by the number of ear specific thresholds obtained and by audiologist subjective ratings. Results indicate that children in this age range accept both types of earphones; however, significantly more ear specific thresholds were obtained using insert earphones compared to circumaural.

Inconsistencies exist between traditional objective measures such as speech recognition and localization, and subjective reports of bimodal benefit. The purpose of this study was to expand the set of objective measures of bimodal benefit to include non-traditional listening tests, and to examine possible correlations between objective measures of auditory perception and subjective satisfaction reports.
