The hematological disorder sickle cell disease (SCD) is a prevalent human genetic disease. It causes severe tissue damage that can result in strokes, splenic infarction, kidney failure, liver and lung disorders, painful crises, and other complications. Although palliative therapies and allogenic stem cell transplantation therapies have been developed for this disorder, there is still no optimal treatment and SCD patients continue to suffer significant morbidity. This dissertation had three central goals. The first was to review the existing literature for reports of virologic failure among adults initiating non-nucleoside reverse transcriptase inhibitor-based antiretroviral therapy in resource-limited settings. The second was to describe the epidemiology of virologic failure over the first 6 months of therapy in Lusaka, Zambia. The third was to examine the performance of different approaches for diagnosing virologic failure. We performed a meta-analysis using data from previously published reports to create summary estimates for the prevalence of virologic failure and found that the lowest estimate of the 95% confidence interval was 10% or higher …

This study was conducted in conjunction with the Birmingham Water Works Board to evaluate Kinetico ceramic filter media against GAC (Granular Activated Carbon) and Sand/Anthracite filters. The Birmingham Water Works Board Water Treatment Pilot Plant was located and operated on the Shades Mountain Filter Plant (SMFP) premises. The evaluation was conducted in three phases: plant matching, plant optimization, and pre-filter testing. The initial phase called for determining the best combination of Kinetico Macrolite Ceramic media to match or outperform the existing Sand/Anthracite dual media filters located at SMFP. Phase III trials often have both efficacy and safety outcomes. Sometimes the safety outcomes are closely associated with the efficacy outcomes. We sought to find a statistical method to treat both efficacy and safety outcomes as bivariate primary outcomes and set up stopping rules at interim stages based on the following criteria: (1) greater safety; (2) greater efficacy; (3) futility. In the nervous system, PI 3-kinase has been implicated in neuronal differentiation. My studies have focused on a candidate neuronal PI 3-kinase target centaurin alpha-1, which binds PtdIns(3,4,5)P3, and is an Arf6 GAP. Centaurin alpha-1 is localized in dendrites, dendritic spines and synapses, and is required for neuronal differentiation and spine morphogenesis. In dissociated neuronal cultures, expression of centaurin alpha-1 enhances dendritic branching, and increases dendritic filopodia, lamellipodia and spine-like protrusions. Expression of centaurin alpha-1 GAP inactive mutant or knocking down centaurin alpha-1 levels using siRNA leads to inhibition of dendritic outgrowth and branching. Isolated cerebral folate deficiency was detected in a 13-year-old girl with cognitive and motor difficulties and juvenile rheumatoid arthritis. Her serum contains autoantibodies that block membrane-bound folate receptors that are on the choroid plexus and diminish the uptake of folate into the spinal fluid. Whereas her serum folate exceeded 21 ng/mL, her spinal fluid contained 3.2 ng/mL of 5-methyltetrahydrofolate as a consequence of the autoantibodies diminishing the uptake of this folate.

Animal studies using the self-administration paradigm suggest that duration of drug access (Ahmed, 2005) and individual phenotype (Piazza, Deminiere, Le Moal and Simon, 1989) are important factors in the development of drug addiction. Wheel running in rats has been proposed as a model to study non-drug addictions (Eikelboom & Lattanzio, 2003). When first given ad lib wheel access, young male rats initially run at low levels (about 1000 wheel turns per night) and gradually increase this distance to high levels (about 5000 wheel turns per night) (Afonso and Eikelboom, 2003). Catecholaminergic neurotransmission is regionally altered following injury, and drugs aimed at these systems offer promising avenues for post-TBI pharmacotherapies. Atomoxetine is a selective norepinephrine transporter (NET) inhibitor currently indicated for treatment of attention-deficit hyperactivity disorder (ADHD). The studies in this dissertation were designed to test the efficacy of atomoxetine for treating cognitive deficits following experimental TBI and the potential mechanism for any beneficial effect. The first part of the study focused on behavioral recovery following atomoxetine treatment. The purpose of this study was to compare the shear bond strength of orthodontic brackets to enamel in four bonding protocols: SEP without prior pumicing (None), Pre-etch and SEP without prior pumicing (Pre-etch), control: SEP with prior pumicing (Pumice), and Pre-etch and SEP with prior pumicing (Both). 80 extracted bovine incisors were randomly divided into 4 groups of 20, and brackets were bonded under the different experimental conditions. Debonding force was measured with an Instron universal testing machine. A two-way ANOVA comparing the four groups indicated that there was a significant difference in debonding force (P = 0.001). Despite the use of highly aggressive treatment regimens (surgery, chemotherapy and radiation), almost all patients succumb to metastatic disease within 6-10 months of diagnosis. The hedgehog (HH) signaling pathway was originally discov-ered to play a critical role in mammalian embryological development. The study purpose was to examine the effects of a 15-min live-music therapy intervention on heart rate, oxygen saturation, level of motor activity, behavioral distress, and behavioral state levels in premature infants in the neonatal intensive care unit. Eating disorders are among the most frequently seen chronic illnesses found in adolescent females. In this paper, we discuss school-based prevention and intervention efforts that seek to reduce the impact of this serious illness. School counselors play a key role in the prevention of eating disorders and can provide support even when not directly involved in psychological or medical treatment. Because of their ability to play a leadership role in school-based prevention of eating disorders, school counselors are essential in facilitating a collaborative approach to the prevention of and intervention in eating disorders and their associated risk factors.

Objective: The authors investigated the use and perceived use of anabolic androgenic steroids (AAS) among nonathlete college students. Participants: The authors surveyed a sample of 485 nonathlete college students at a major metropolitan university. Methods: They administered a survey on use and perceived use of AAS to the students. Results: Forty-two participants (9%) reported using AAS (37 men, 5 women). Seniors were the most likely to use AAS (36%), and freshman the least likely (7%). Thirty-four percent of nonusers and 41% of users indicated they knew between 1 and 5 AAS users. Of the total sample, 36% perceived that 5% …

BACKGROUND: Variants in the complement cascade genes and the LOC387715/HTRA1, have been widely reported to associate with age-related macular degeneration (AMD), the most common cause of visual impairment in industrialized countries. Human papillomavirus are small DNA tumor viruses. The viral genome is a small circular double stranded DNA that replicates autonomously as an extrachromosomal plasmid. Occasionally infections by the high risk HPV viruses can lead to DNA integration and progression to cancer. How the HPV DNA is maintained and becomes established in the dividing host cells is not well understood. Western Australian medical graduates will double by 2012 and treble shortly afterward. Insufficient training positions in hospitals will likely require additional training opportunities to be provided by general practices.

Many different tumors have been documented to have elevated levels of the enzyme ST6Gal I, a Golgi glycosyltransferase that adds α2-6 sialic acids to glycoproteins. Concurrently, upregulated ST6Gal I is associated with metastasis and poor patient prognosis. We initially showed that HD3 colonocytes made to express oncogenic-ras, a common mutation in colon carcinoma, have increased expression of ST6Gal I and elevated α2-6 sialylated integrins. Having established that ras regulates ST6Gal I expression in colon epithelial cells, we found that colonocytes carrying hyper α2-6 sialylated β1 integrins exhibit increased adhesion to and migration toward collagen I. Synaptic transmission mediates the processing of incoming information, and recent studies have demonstrated that long-term changes in synaptic strength at CA1 synapses are important for learning and memory. Lipoteichoic acid (LTA) is an essential bacterial membrane polysaccharide (cell wall component) that is linked to a glycerol backbone with two acyl chains. Since an antibody to LTA has been shown to protect from Gr+ bacterial infections or colonization, and the mutant Gr+ bacteria which produce less amount of LTA have less pathogenic than WT, LTA may be an important role in Gr+ bacterial infection and inflammation. Childhood immunization coverage in Pakistan remains sub-optimal despite universal and free of cost availability of vaccines. Structural impediments to progress may include the immunization card that is hard to read and interpret. A randomized controlled trial was conducted to assess the effect of redesigned immunization card and center-based education to mothers on immunization completion at rural immunization centers in Karachi, Pakistan. Dental enamel forms through a protein-controlled mineralization and enzymatic degradation with a nanoscale precision that new engineering technologies may be able to mimic. Recombinant fulllength human amelogenin (rH174) and a matrix-metalloprotease (MMP-20) were employed in a pHstat titration system that enabled a continuous supply of calcium and phosphate ions over several days, mimicking the initial stages of matrix processing and crystallization in enamel in-vitro.
