Ultrasound Examination Of Shoulders In Wheelchair Users With Paraplegia, Jiann-Perng Chen, Yi-Pin Chiang, Gwo-Chi Hu, Wan-Yi Shih, Kuan-Wen Chen

Rehabilitation Practice and Science

Due to the prolonged use of upper extremities for weight-bearing, the shoulder joints of paraplegic wheelchair-users are prone to injury. Once shoulder pain occurs, performance of activities required for daily living would thus be disturbed. In past studies, the shoulders of the spinal cord injured patients were evaluated with arthrography or magnetic resonance images and the rotator cuff injuries were found. In the current study, we used ultrasonography to detect rotator cuff injuries of paraplegic wheelchair-users. The factors associated with ultrasonographic findings were also investigated.Thirty paraplegic wheelchair-users, 26 males and 4 females, were recruited for this study. Both shoulder joints …

Femoral Nerve Injury Following Transfemoral Angiography: A Casereport, Chien-Ching Yu, Ying-Ju Shih, Su-Ju Tsai

Rehabilitation Practice and Science

Femoral nerve injury is a rare complication after femoral angiography. Symptoms including quadriceps weakness and atrophy, absent or reduced knee jerk and sensory impairment along the anterior thigh and medial lower leg have been found. This leads to both muscular weakness and sensory deficit, which impairs walking and interferes with quality of life. Early identification and management of femoral nerve injury could result in better prognosis. We report on the history, diagnosis and rehabilitative outcome of a 54-year-old woman with a medical history of peripheral arterial occlusion disease. She developed significant femoral neuropathy one month after angiography. Severe neurological deficits were …

The Cost-Effectiveness Analysis Between Outreach And Hospital Rehabilitation Models, Wei-Ni Chou, Shu-Yen Huang, Shu-Chuan Jennifer Yeh, Hing-Man Wu, Ming-Yang Koe, Yi-Ru Chen

Rehabilitation Practice and Science

Hospitals and clinics are usually the places where patients receive medicare. However, it is extremely inconvenient for the elderly to frequently go between the homes and hospitals/clinics for long-term medicare. With doctor-served home care, the accessibility of medical treatment would increase; the risk of trauma or infection from traveling in-between would be avoided; more prompt and timely treatment-offered effectiveness would rise. However, there has been no study on the difference between the cost effectiveness of outreach versus hospital rehabilitation. Thence, the peculiarity of the outreach rehabilitation regarding the physiatrists providing medical services and the physical therapists and occupational therapists offering …

Investigation Of Flatfoot Screening In Developmentally Delayed Children, Li-Chen Tung, Pei-Chi Hsiao, Willy Chou, Cheng-Cheng Hsu, Wen-Chih Lin, Isbel Tou

Rehabilitation Practice and Science

Flatfoot disease is caused by disappearance or collapse of the medial longitudinal arch of the foot. Most of patients with flatfoot disease have what is known as ”flexible flatfoot”. Flatfoot in developmentally delayed children, however, is due to muscle weakness of lower limbs, which leads to disproportionate muscle growth or joint contracture. A study conducted by Chii-Jeng Lin in 2001 indicated that flat-footedness may influence gait, single-foot standing, and jumping ability. The author considered that flat-footedness was not only a problem in skeletal structure but that it may also be a consequence of dynamic foot function. Children with developmental delays …

Rehabilitation Practice and Science

Although many studies are available in focus on prognostic factors for acute cerebral vascular disease, there are few reports to discuss the impact of the initial glucose level on the functional outcome after stroke. The initial glucose level in the emergency department is more representative of acute glucose response after stroke and is not influenced by the treatment in the emergency department. The aim of our study is to evaluate the prognostic role of the initial glucose level on the functional outcome after stroke. We recruited individuals who sustained first-ever stroke and were admitted to an inpatient stroke rehabilitation program. Patients …

Rehabilitation And Prognosis Of Childhood Arterial Ischemic Stroke: A Casereport, Chia-Huang Dai, Wei-Jang Yen, Ta-Shen Kuan, Chang-Zern Hong Dec 2008

Rehabilitation And Prognosis Of Childhood Arterial Ischemic Stroke: A Casereport, Chia-Huang Dai, Wei-Jang Yen, Ta-Shen Kuan, Chang-Zern Hong

Rehabilitation Practice and Science

The incidence of ischemic stroke in childhood is two to 13 per 100,000 children per year. Although the incidence is much lower than adults, strokes in children not only cause a large number of deaths, but also induce significant disabilities. Because childhood ischemic strokes are rare, multi center, controlled trials should be performed to provide evidence based data related to effectiveness of medical treatment and rehabilitation. Current studies on the management of stroke in children are mainly based on casereports, uncontrolled trials, or are adapted from adult literature. An eleven-year-old, obese boy presented to the Emergency Department with an initial presentation …

Combining Botulinum Toxin Type A Injection And Modified Constraint-Induced Movement Therapy In A Taiwanese Patient With Spastic Hemiplegic Cerebral Palsy: A Casereport, Yi-Chen Tseng, Jui-Yun Su, Yu-Ching Lin

Rehabilitation Practice and Science

Hypertonia interferes with selective motor control and performance of activities in children with spastic cerebral palsy. At present, most therapists use traditional therapeutic techniques such as neurodevelopmental treatment or other approaches to handle the problems posed by cerebral palsy but the results are not very good. Many studies had shown that botulinum toxin type A (BTX-A) injection can control abnormal muscle tone effectively without major side effects. Constraint-induced movement therapy (CIMT) includes constraint of the sound side upper limb and intensive therapy of the affected limb. In order to reduce spasticity and enhance the use of the affected limb simultaneously, …

Levamisole-Induced Multifocal Inflammatory Leukoencephalopathy: A Casereport, Chueh-Hung Wu, Lu Lu

Rehabilitation Practice and Science

Levamisole, which is the L-isomer of tetramisole, has been used as an antihelminthic agent. Due to its immunomodulating property, it has been used to treat recurrent aphthous ulcers, pemphigus vulgaris, and malignant melanoma. It also has been used along with 5-fluorouracil to treat colon cancer. The side effects are mild and include nausea, vomiting, diarrhea and metal taste. However, existing evidence suggests that levamisole can induce multifocal inflammatory leukoencephalopathy. The course of levamisole-induced multifocal inflammatory leukoencephalopathy is monophasic and progressive. Clinical symptoms include gait ataxia, dysarthria, dysphasia, conscious disturbance and weakness of the extremities (from hemiparesis to tetraplegia). The exact …

Rehabilitation Practice and Science

The nasogastric tube syndrome (NG tube syndrome) is a rare but a potentially life-threatening complication of a patient with indwelling nasogastric tube feeding. The cause of the syndrome is due to the NG tube irritation, resulting in ulceration and infection of the posterior cricoid region with subsequent dysfunction of vocal cord abduction. This dysfunction may result in complete loss of vocal cord abduction and cause serious airway obstruction. Reports of this syndrome are rare in the literature. A 64-year-old female patient with long-term NG tube feeding, suffered from serious upper airway obstruction. The patient had obvious desaturation while the tracheostomy …

Spontaneous Spinal Epidural Hemorrhage With Shoulder And Neck Pain As Initial Presentation: A Casereport, Yu-Min Kuo, Wai-Keung Lee, Cheng-Chih Liao, Li-Rong Chen

Rehabilitation Practice and Science

Spontaneous spinal epidural hemorrhage is a very rare disease, with an annual incidence estimated at one per million. Emergency surgery is indicated for a good prognosis, without which the death rate would be higher. Although the initial symptoms of cervical, thoracic, and back pain make it difficult to differentiate spontaneous spinal epidural hemorrhage from other common disease with similar symptoms, magnetic resonance imaging (MRI) is a highly sensitive diagnostic method. We report the case of a 22-year-old male patient who initially complained of pain over the left shoulder and left cervical area. After nine days, numbness was noted in his left …

Rehabilitation In A Patient With Delayed Post-Hypoxic Encephalopathy: A Casereport, Yao-Jen Chen, Su-Ju Tsai, Yueh-Lin Tsai

Rehabilitation Practice and Science

Carbon monoxide poisoning from accidents or suicide attempts often result in delayed post-hypoxic encephalopathy. Patients with delayed post-hypoxic encephalopathy typically present with recurrent apathy, confusion, agitation, and/or progressive neurological deficits, despite apparent transient recovery. Autopsy reports often reveal diffuse demyelination of subcortical white matter, but the exact pathogenesis is not clear. While comprehensive physical examination and history taking is necessary in diagnosing delayed post-hypoxic encephalopathy, magnetic resonance imaging can be very helpful in the diagnostic work-up. Rehabilitation for severe post-hypoxic encephalopathy can prevent complications such as immobilization and can improve patients' functional status. We report the history, diagnosis, and rehabilitative outcome …

Ultrasonographic Features Of Schwannoma Of The Lower Limb: Two Casereports, Hsiao-Wei Lin, Jeng-Yi Shieh, Tyng-Guey Wang, Wen-Shiang Chen, Chung-Li Wang, Yen-Ho Wang

Rehabilitation Practice and Science

Schwannoma is a benign encapsulated nerve sheath tumor. It typically develops in middle age with unknown cause and usually involves spinal root and peripheral nerves. We report two cases of schwannoma presenting as palpable mass over lower limbs. Both masses had heterogeneous hypoechogenicity, faint posterior enhancement, and were located eccentric to a nerve fascicle. Preoperative sonography revealed both patients had nerve tumors derived from nerve fascicles. These cases illustrate how sonographic information about the relationship between mass and surrounding structures can facilitate surgical planning.
