A Case Report Of Severe Adverse Reactions Following Diethylcarbamazine Treatment Of Lymphatic Filariasis, S Nuchprayoon, V Sanprasert, P Jaratsing, S. Jaijakul

Diethylcarbamazine (DEC) treatment for lymphatic filariasis can be accompanied by adverse drug reactions. Severe adverse reactions are usually uncommon but once happen, they could be acute and sometimes life-threatening. We report here a case of 15-year-old patient who was treated with DEC and developed severe adverse reactions including fever, headache, dizziness, chest pain, sore throat, cough, dyspnea, nausea and vomiting. A complete profile of clinical findings and blood parameters, including parasitological, hematological, biochemical, and immunological parameters was reported. The physical examination showed rhonchi on chest examination. Laboratory investigations revealed leukocytosis with the increased levels of neutrophils and monocytes. The bimodal …

Wolbachia is a group of obligatory intracellular gram-negative bacteria. This bacterium is responsible for manipulation of host reproduction in many arthropod species and involved in embryonic and larval development, adult female fertility, and survival in filarial nematode. The applied biology of Wolbachia and modification of pest and vector species using Wolbachia have been studied in order to suppress or modify natural populations. In addition, Wolbachia has been used as a novel chemotherapeutic target for filariasis control program. The estimation of the postmortem interval (PMI) is necessary for supporting justice to the dead. During the early stage of death, estimation of PMI of the corpses depends on the changing of muscle contraction and physical conditions, but this method can be used effectively in case of death within 24 hours. Flies are the first organisms which swarm on corpses. Consequently, accuracy of the estimation of PMI using fly larvae collected from corpses depends on identification of the species of flies. Some kinds of eye drop such as Latanoprost, Chloramphenicol, Cefazolin, Ceftazidime, Vancomycin and Amphotericin B need to be kept at the temperature of 2-8 degree celsius to maintain their efficiency. Patients are usually recommended to keep these drugs in the refrigerator while dispensing. However, the temperature in the refrigerator is not consistent especially the refrigerator's door has higher temperature than other parts.

ภาวะอ้วนในเด็กเป็นปัญหาที่พบมากขึ้นในปัจจุบัน ทำให้เกิดภาวะแทรกซ้อนในหลายๆ ระบบของร่างกายรวมทั้งระบบหายใจ ความผิดปกติทางระบบหายใจเป็นผลมาจากการสะสมของเนื้อเยื่อไขมันตามอวัยวะต่างๆ ที่เกี่ยวข้องกับการหายใจ เช่น ผนังทรวงอก ผนังหน้าท้อง ทางเดินหายใจ และกะบังลม ทำให้ความยืดหยุ่นของผนังทรวงอกและปอดลดลง ปริมาตรความจุปอดและความสามารถในการออกกำลังกายลดลง นอกจากนี้ไขมันที่สะสมอยู่บริเวณทางเดินหายใจส่วนต้นและลำคอยังทำให้ทางเดินหายใจตีบแคบโดยเฉพาะอย่างยิ่งในขณะหลับ เกิดภาวะ obstructive sleep apneaและ obesity hypoventilation syndrome ทั้งหมดนี้ทำให้เด็กอ้วนเสี่ยงต่อการเกิดภาวะออกซิเจนในเลือดต่ำหรือมีคาร์บอนไดออกไซด์ในเลือดสูงและเกิดภาวะหายใจล้มเหลวตามมาได้ โดยเฉพาะอย่างยิ่งหากมีการติดเชื้อในระบบหายใจหรือภายหลังได้รับยานอนหลับหรือยาสลบเพื่อทำการผ่าตัดนอกจากนี้ ยังพบว่า การที่มีระดับออกซิเจนในเลือดต่ำหรือคาร์บอนไดออกไซด์ในเลือดสูงแบบเรื้อรังร่วมกับภาวะ leptin resistance ซึ่งพบได้ในเด็กอ้วนจะทำให้เด็กเหล่านี้มีการตอบสนองทางระบบหายใจน้อยกว่าปกติต่อภาวะขาดออกซิเจนหรือคาร์บอนไดออกไซด์ในเลือดคั่ง ทำให้ภาวะดังกล่าวรุนแรงมากยิ่งขึ้น ซึ่งหากไม่ได้รับการดูแลรักษาก็จะนำไปสู่ภาวะการหายใจล้มเหลวหรือเกิดภาวะแทรกซ้อนทางระบบหัวใจและหลอดเลือดตามมาได้ นอกจากนี้ สารเคมีที่หลั่งจากเนื้อเยื่อไขมันยังมีผลทำให้เกิดการอักเสบในระบบหายใจ และอาจเกี่ยวข้องกับการเกิดโรคหืดและภาวะหลอดลมไวเกินในเด็กเหล่านี้ การดูแลทางด้านโภชนาการและส่งเสริมการออกกำลังกายอาจช่วยให้สมรรถภาพปอดดีขึ้นและลดการเกิดภาวะแทรกซ้อนต่างๆ เหล่านี้ได้.

Objectives : To study the effectiveness of antibiotic drug use for the treatment of sepsis after transrectal ultrasound guide core biopsy of prostate (TRUS-Bx). Methods : This was a retrospective study from January 2004 to December 2008 of all men who received TRUS-Bx at the Department of Urology, King Chulalongkorn Memorial Hospital, for the elevation of prostatic specific antigen (PSA > 4.0 ng/ml) or abnormal digital rectal examination (DRE). A total of 1,246 patients underwent TRUS-Bx developed fever that required hospitalization for antibiotic treatment; 27 patients (2.17%). Mid aortic syndrome (MAS) is characterized by an extensive narrowing of the abdominal aorta, with involvement of the renal and visceral arterial branches. Angiography is the gold standard for the diagnosis of MAS. However, computed tomography angiography (CTA) can effectively determine the location and the extent of the stenosis. Medical management of MAS generally means a lifelong, combination of multiple classes of antihypertensive medications. Surgical correction such as aortoaortic bypass with renal revascularization remains the definitive treatment of the idiopathic MAS. Miller Fisher syndrome is a clinical variant of Guillain-Barre syndrome with classic triad of ophthalmoplegia, ataxia and areflexia which there were some reports of multiple cranial nerves enhancement on MRI. We report a case of 66-year-old male presenting with headache and numbness of hands and feet, then gradual developed bilateral diplopia, ptosis, decreased deep tendon reflex and impaired tandem gait. Post contrast MRI showed multiple cranial nerves enhancement. Miller Fisher syndrome was diagnosed based on clinical triad and electrophysiologic evidence of polyneuropathy involved axon and myelin. Idiopathic granulomatous mastitis is an uncommon benign breast disease diagnosed from exclusion. It mimics malignant breast tumor in terms of clinical and imaging features. We hereby report a case of painful lump in the left breast. Mammograms and ultrasound findings could not exclude malignant mass. Wide excision was performed and the final histopathological diagnosis of granulomatous mastitis was concluded. In conclusion, the clinical and imaging findings of granulomatous mastitis are similar to carcinoma. Tissue diagnosis was very important to distinguish these two entities before starting management and avoiding unnecessary mastectomy.

Iodinated contrast media ถูกใช้ในปริมาณที่มากขึ้นตามการเติบโต และขยายตัวอย่าง รวดเร็วของการตรวจทางรังสีวิทยาวินิจฉัย โดยเฉพาะการตรวจด้วยเครื่องเอกซเรย์คอมพิวเตอร์ ในขณะเดียวกัน เราก็ได้เรียนรู้ผลข้างเคียงจากการใช้ iodinated contrast media มากขึ้น ผลข้างเคียง โดยรวมพบได้ 5 - 8 เปอร์เซ็นต์ และมีน้อยกว่า 0.1 เปอร์เซ็นต์ ที่มีอันตรายถึงชีวิต ผลข้างเคียงแบ่ง ได้ว่า เป็นผลข้างเคียงต่อไตหรือต่ออวัยวะอื่น ๆ หรืออาจจะแบ่งเป็นแบบเฉียบพลัน หรือแบบล่าช้า การประเมินผู้ป่วยที่มีความเสี่ยงรวมถึงการจัดการและการรักษาผู้ป่วยเหล่านี้อย่างถูกต้อง และทันท่วง ทีเป็นสิ่งที่สำคัญและเป็นหน้าที่สำหรับรังสีแพทย์และทีม ซึ่งรวมถึงแพทย์เจ้าของไข้ แพทย์ประจำบ้าน พยาบาล นักรังสีการแพทย์ และเจ้าหน้าที่ธุรการที่ทำหน้าที่นัดผู้ป่วย บทความพิเศษเกี่ยวกับแนวทาง ปฏิบัติทางด้านคลินิคสำหรับการฉีด contrast media ฉบับนี้ เกิดจากการรวบรวมทบทวนบทความ ต่าง ๆ ที่เกี่ยวข้องและได้สรุปแนวทางที่เหมาะสมในการจัดการและป้องกันผลข้างเคียงต่าง ๆ ที่จะเกิด จากการใช้ iodinated contrast media.

วัตถุประสงค์ : เพื่อศึกษาภาวะซึมเศร้าและพฤติกรรมการเล่นเกมออนไลน์ของนักเรียนมัธยมศึกษาตอนปลายในเขตกรุงเทพมหานครรูปแบบการวิจัย : การวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งกลุ่มตัวอย่าง : นักเรียนชั้นมัธยมศึกษาตอนปลาย ในสังกัดสำนักงานการศึกษาขั้นพื้นฐานกระทรวงศึกษาธิการในเขตกรุงเทพมหานครวิธีการศึกษา : นักเรียนชั้นมัธยมศึกษาตอนปลายปีการศึกษา 2552 โดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบทดสอบการติดเกมสำหรับเด็กและวัยรุ่น และแบบสอบถามเกี่ยวกับความรู้สึก CES-D วิเคราะห์ข้อมูลโดยโปรแกรม SPSS สถิตที่ใช้ได้แก่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และวิเคราะห์ความแปรปรวนทางเดียวผลการศึกษา : นักเรียนชั้นมัธยมศึกษาปีที่ 4 จำนวน 257 คน (ร้อยละ 33.3) นักเรียนชั้นมัธยมศึกษาปีที่ 5 จำนวน 255 คน (ร้อยละ 33.1) นักเรียนขั้นมัธยมศึกษาปีที่ 6จำนวน 259 คน (ร้อยละ 33.6) ซึ่งเป็นเพศชายจำนวน 396 คน (ร้อยละ 51.4)และเพศหญิง จำนวน 375 คน (ร้อยละ 48.6) จากการวิจัยพบว่ากลุ่มไม่มีปัญหาใน การเล่นเกม ร้อยละ 86.5 คลั่งไคล้ เริ่มเกิดปัญหาในการเล่นเกม ร้อยละ 9.9กลุ่มติดเกม มีปัญหาในการเล่นเกมมาก ร้อยละ 3.6 ส่วนด้านความรู้สึกพบว่ากลุ่มที่ไม่อยู่ในข่ายภาวะซึมเศร้า ร้อยละ 67.7 ส่วนกลุ่มที่อยู่ในข่ายภาวะซึมเศร้า32.2 และปัจจัยที่เกี่ยวข้องพบว่ามีปัจจัยที่มีผลต่อพฤติกรรมการเล่นเกม ได้แก่เพศชาย ระดับการศึกษาชั้นมัธยมศึกษาปีที่ 5 เกรดเฉลี่ยต่ำ รายได้ส่วนตัวต่ำและสูง จำนวนเพื่อนสนิทเพศเดียวกันน้อย และปัจจัยทางจิตสังคม มีภาวะซึมเศร้า ร้อยละ 32.3สรุปผล : นักเรียนมัธยมศึกษาตอนปลายในโรงเรียนที่ทำวิจัย ส่วนใหญ่ยังไม่มีปัญหาในการเล่นเกม แต่ก็พบว่าปัจจัยที่เกี่ยวข้อกับพฤติกรรมการเล่นเกม ซึ่งควรที่จะได้รับการประเมินและติดตามต่อไปในกลุ่มที่มีปัญหาพฤติกรรมการเล่นเกมและมีภาวะซึมเศร้าร่วมอยู่ด้วย รวมทั้งวางแนวทางความร่วมมือทั้งโรงเรียนและผู้ปกครอง เพื่อที่จะช่วยให้เด็กลดพฤติกรรมการเล่นเกมลงและปฏิกิริยาทางอารมณ์ที่เกิดขึ้น.

Channelopathies are defined as inherited diseases, mostly located in an autosomal dominant gene. This review describes the significant number of new gene associated with genetic epilepsy syndromes that have emerged during the last decade, together with additional mutations relating to previously known gene associations. Problem : The incidence of complications in cervical disc arthroplasty was 6.2% per treat level.
