Objectives : To determine whether there are valproic acid (VPA) pharmacokinetic (PK) parameters differences between the VPA-responsive and VPA-resistant patient groups. Methods : Thirty-three subjects (14 patients were in VPA-resistant group and 19 patients in VPA-responsive group) who were epileptic patients were recruited from the Neurology Clinic, Neurology Institute, Bangkok. Their ages were 15 - 65 years old. They were either treated with VPA (Depakine Chrono) monotherapy or polytherapy in which the combined drug did not have the same metabolic pathways as VPA. สตรีที่มีภาวะ Polycystic ovary syndrome (PCOS) มีลักษณะอาการทางคลินิกที่หลากหลายและเกี่ยวข้องกับการทำงานของอวัยวะหลายระบบ แต่ระบบที่มีความเกี่ยวข้องหลัก คือระบบสืบพันธุ์สตรีและระบบเมตาบอลิสม ซึ่งปัจจุบันเป็นที่ทราบกันว่าพยาธิกำเนิดของ PCOS คือภาวะฮอร์โมนเพศชายในเลือดสูง และภาวะดื้อต่ออินซูลิน ทำให้เกิดอาการทั้งระยะสั้น ได้แก่ ความผิดปกติของรอบระดู ภาวะมีบุตรยาก มีสิว ขนดก ศีรษะล้านแบบเพศชาย และผลระยะยาว ได้แก่ เบาหวานชนิดที่ 2และมะเร็งเยื่อบุโพรงมดลูก นอกจากนี้ยังอาจเพิ่มความเสี่ยงต่อโรคหัวใจและหลอดเลือด ในบทความนี้จะรวบรวมเกี่ยวกับผลของ PCOS ที่มีต่อภาวะเจริญพันธุ์ของสตรีทั้งในระยะสั้นและระยะยาวรวมทั้งการรักษา นอกจากนี้ ยังรวบรวมเกี่ยวกับ PCOS ในสตรีวัยรุ่นที่พบมากขึ้น และมีความสำคัญทั้งในแง่การวินิจฉัย และการรักษา เนื่องจากภาวะนี้จะส่งผลต่อสุขภาพในระยะยาว จึงเพิ่มความเสี่ยงหากเริ่มเป็นตั้งแต่อายุน้อย.

วัตถุประสงค์ : เพื่อศึกษาภาวะสุขภาพจิต วิธีจัดการกับปัญหา และปัจจัยที่เกี่ยวข้องในบุตรวัยรุ่นของผู้ป่วยโรคซึมเศร้ากลุ่มตัวอย่าง : บุตรวัยรุ่นของผู้ป่วยโรคซึมเศร้าที่มารับการรักษาที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์และโรงพยาบาลศรีธัญญา จำนวน 61 คนระเบียบวิธีการวิจัย : โดยใช้แบบสอบถามข้อมูลทั่วไป แบบสอบถามดัชนีชี้วัดสุขภาพจิต คนไทยฉบับสั้น (TMHI-15 ) และแบบสอบถามวิธีจัดการกับปัญหา วิเคราะห์ข้อมูลโดยใช้โปรแกรมทางสถิติ SPSS ที่ระดับนัยสำคัญ P<.05ผลการศึกษา : 1) ผลการศึกษาพบว่ากลุ่มตัวอย่างส่วนใหญ่มีภาวะสุขภาพจิตอยู่ในระดับดีเท่ากับค่าเฉลี่ยของคนทั่วไป (ร้อยละ 39.3) 2) วิธีจัดการกับปัญหาที่กลุ่มตัวอย่างส่วนใหญ่เลือกใช้บ่อยมากที่สุด ได้แก่ การนอนหลับ (ร้อยละ 80.3)วิธีที่เลือกใช้บ่อยน้อยที่สุด ได้แก่ ไปหาผู้เชี่ยวชาญในการให้คำปรึกษาอื่น ๆเช่น จิตแพทย์ นักจิตวิทยา (ร้อยละ 93.4) 3) ส่วนวิธีจัดการกับปัญหาที่เกี่ยวข้องกับคะแนนสุขภาพจิตอย่างมีนัยสำคัญทางสถิติ เช่น การวาดฝันถึงสิ่งอยากจะให้มี อยากจะให้เป็น และการพยายามใช้เหตุผลและพูดคุยประนีประนอมกับบิดามารดา เป็นต้น 4) ปัจจัยอื่น ๆ ที่เกี่ยวข้องกับคะแนนสุขภาพจิตอย่างมีนัยสำคัญทางสถิติ เช่น คะแนนเฉลี่ยสะสม ระดับการศึกษาของบิดา บุคคลที่ปรึกษาเมื่อมีเรื่องที่ไม่สบายใจ ความสัมพันธ์กับบิดาความสัมพันธ์กับมารดา การทำกิจกรรมร่วมกันในครอบครัว จำนวนเพื่อนสนิทและความสัมพันธ์โดยรวมกับเพื่อน ๆผลสรุป : บุตรวัยรุ่นของผู้ป่วยโรคซึมเศร้าส่วนใหญ่มีมีภาวะสุขภาพจิตเท่ากับค่าเฉลี่ยของคนทั่วไป จึงควรมีการสนับสนุน ปัจจัยและวิธีจัดการกับปัญหาที่เกี่ยวข้องกับการมีสุขภาพจิตดีให้มีประสิทธิภาพมากขึ้น เพื่อช่วยส่งเสริมให้วัยรุ่นที่มีบิดามารดาป่วยเป็นโรคซึมเศร้าประสบความสำเร็จ.

Background : A large number of diabetic patients wear ill-fitting shoes which may either have incorrect width or length. Custom-made shoes are the choice for diabetic patients because they are uniquely made for them. โรคพาร์กินสัน เป็นโรคที่สมองมีการเสื่อม โดยมีการเสื่อมของเซลประสาทส่วนที่สร้างสารโดปามีนใน substantia nigra ของสมองส่วน basal ganglia จึงส่งผลให้ผู้ป่วยพาร์กินสันไม่สามารถควบคุมการเคลื่อนไหวของร่างกายได้ ผู้ป่วยจะมีอาการเคลื่อนไหวช้าหรือเคลื่อนไหวได้น้อย ร่วมกับอาการหนึ่งในสามอย่างดังนี้ ได้แก่ (1) การสั่นของมือ แขน ขา ขากรรไกร ใบหน้า (2) อาการแข็งเกร็งของแขนขาและลำตัว (3) มีการทรงตัวไม่มั่นคง นอกจากนี้ยังมีอาการของระบบประสาทอัตโนมัติการรับความรู้สึกเจ็บปวด การกลืน ลำไส้และกระเพาะปัสสาวะ ความรู้สึกทางเพศ อารมณ์ ที่ผิดปกติอีกด้วย ในปัจจุบันมีหลักฐานทางงานวิจัยและทางคลินิกชัดเจนแล้วว่าการรักษาทางเวชศาสตร์ฟื้นฟูมีส่วนช่วยให้ผู้ป่วยเดินและเคลื่อนไหวดีขึ้น การทรงตัวดีขึ้น ลดความเสี่ยงในการล้ม ประกอบกิจวัตรประจำวันได้มากขึ้น ทำให้คุณภาพชีวิตดีขึ้น.

Background : Although magnetic resonance imaging (MRI) has been the most sensitive and useful investigation in detecting and delineating spinal lesion, spinal tumor and spinal infection may be difficult to be distinguished. Objective : To find reliable findings to differentiate spinal tumor from spinal infection. Methods : Four hundred and seventy-nine cases of spinal MRI in 12 months were retrospectively reviewed. We report a case of 34 years old healthy pregnant female scheduled for elective cesarean hysterectomy due to previous cesarean section with placenta previa totalis and suspected placenta accrete with transverse lie. We anesthesized this patient with regional anesthesia in combination with general anesthesia due to regional anesthesia wore off. Massive bleeding during the operation was resuscitated. Co-ordination within the teamwork is necessary. At end of the operation, the mother and her newborn were safe without significant complication.

Objective : To survey the clinical memorable experiences, which were suggested to intraoperative malignant hyperthermia (MH) among Thai anesthesiologists. Material and Method : Questionnaires for a national survey of dantrolene requirement were sent by the Royal College of Thai Anesthesiologists (RCAT) to all Thai anesthesiologists (855 members) in 2007. One hundred and two (11.93%) questionnaires were returned. There were 25 anesthesiologists (24.51%), who had experiences of suspected MH events. วัตถุประสงค์ : เพื่อศึกษาประสิทธิผลของโปรแกรมการปรับเปลี่ยนการดำเนินชีวิตที่มีต่อปัจจัยเสี่ยงโรคหลอดเลือดหัวใจตีบ และคะแนนคุณภาพชีวิต และศึกษาผลของซีรั่มต่อเซลล์บุผนังหลอดเลือดแดงของหัวใจ (human coronary artery endothelialcell, HCAEC)วิธีการศึกษา : ผู้ป่วยโรคหลอดเลือดหัวใจตีบ 30 คน อายุเฉลี่ย 63.28 ปี แบ่งเป็นกลุ่มที่ได้รับการรักษาแบบปกติ (กลุ่ม UC) และกลุ่มที่เข้าโปรแกรมฯ (กลุ่ม LM) วิเคราะห์ปัจจัยเสี่ยงของโรคหลอดเลือดหัวใจตีบ เป็นค่าไขมันรวม oxLDL และ proteincarbonyl ในซีรั่ม ทิ่ baseline 6 เดือน และ 12 เดือน และทดสอบผลของซีรั่มดังกล่าวในเซลล์ HCAEC ตรวจวัดเซลล์มีชีวิตด้วยวิธี MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide] assay และวัดระดับของอนุมูลอิสระที่เกิดขึ้นภายในเซลล์โดยวัดปริมาณแสงฟลูออเรสเซนต์ของสาร2,7-dichlorofluorescein (DCF) ทำแบบสอบถามความสามารถในการดูแลตนเองและแบบสอบถามคุณภาพชีวิต เพื่อประเมินประสิทธิภาพของโปรแกรมฯ และสมรรถนะการทำงานรวมทั้งคุณภาพชีวิตของผู้ป่วยทั้งสองกลุ่ม โดยเปรียบเทียบข้อมูลที่ได้ก่อนและหลังการศึกษา เมื่อติดตามครบ 1 ปีผลการศึกษา : จากผลการศึกษาพบว่าผู้ป่วยกลุ่ม LM มีระดับไขมันในเลือด ระดับของ oxLDLและ protein carbonyl ลดลงหลังจากเข้าโปรแกรมฯ 6 เดือน ในขณะที่เพิ่มสูงขึ้นในผู้ป่วยกลุ่ม UC แต่ที่ 12 เดือนไม่พบความแตกต่างทางสถิติเมื่อเทียบกับ baselineทั้งสองกลุ่มยกเว้นระดับ protein carbonyl ในผู้ป่วยกลุ่ม UC ที่เพิ่มสูงขึ้น และระดับ HDL-C ลดลงอย่างมีนัยสำคัญทางสถิติ และจากการทดสอบผลของซีรั่มที่ได้จากผู้ป่วยต่อเซลล์ HCAEC พบว่าร้อยละของเซลล์มีชีวิตเพิ่มขึ้นเมื่อincubated ด้วยซีรั่มจากผู้ป่วยกลุ่ม LM ที่ 6 เดือน แต่ไม่พบความแตกต่างทางสถิติที่ 12 เดือน การวัดระดับของอนุมูลอิสระที่เกิดขึ้นภายในเซลล์ พบว่าเมื่อincubated ซีรั่มที่ 6 เดือนของผู้ป่วยกลุ่ม LM ระดับของอนุมูลอิสระภายในเซลล์ลดลงจาก baseline แต่เพิ่มขึ้นในผู้ป่วยกลุ่ม UC เมื่อ …

ปัญหาผู้ติดสุรา เป็นปัญหาสุขภาพที่พบบ่อยในประเทศไทย ผู้ติดสุราอาจจะหยุดดื่มสุราด้วยสาเหตุหลาย ๆ อย่าง อาจจะตั้งใจหยุดเอง หรือเกิดจากความจำเป็น เนื่องจากเจ็บป่วยด้วยอุบัติเหตุหรือ โรคทางอายุรกรรม เฉียบพลัน ซึ่งผลการหยุดสุราทันทีหลังทีจากดื่มมาเป็นเวลานาน ก็จะเกิดกลุ่มอาการถอนสุราขึ้นกลุ่มอาการถอนสุรา แบ่งเป็นระดับความรุนแรง เป็นแบบไม่มีโรคแทรกซ้อน กับมีโรคแทรกซ้อนซึ่งระดับรุนแรงไม่มาก มักจะไม่มีอันตรายและหายได้เอง ส่วนระดับรุนแรงมาก มักจะประกอบด้วยกลุ่มอาการ 3 ชนิด ได้แก่ โรคลมชักเนื่องจากการถอนสุรา โรคจิตที่เกิดจากการใช้สุรา และโรค Deliriumที่เกิดจากการถอนสุราเนื่องจากผู้ดื่มสุราเป็นเวลานาน มักจะมีโรคประจำตัวทางอายุรกรรมอื่นร่วมด้วย โดยเฉพาะอย่างยิ่งโรคตับเรื้อรัง นอกจากนี้ผู้ป่วยอาจจะป่วยด้วยโรคทางอายุรกรรมขณะที่มีอาการถอนสุราดังนั้น การประเมินผู้ป่วย การวางแผนรักษา การเฝ้าระวังติดตามอาการอย่างใกล้ชิดจึงมีความสำคัญมากบทความนี้ได้ทบทวน สาเหตุของการเกิด Alchol tolerance และการถอนสุรา อธิบายการดำเนินโรคของกลุ่มอาการถอนสุรา การวินิจฉัยและการวินิจฉัยแยกโรค การตรวจทางห้องปฏิบัติการแนวทางในการรักษาผู้ป่วยถอนพิษสุรา ข้อบ่งชี้ในการรับรักษาในโรงพยาบาล วิธีประเมินความรุนแรงของภาวะถอนสุรา ขั้นตอนการให้ยารักษา และการเฝ้าระวังความเสี่ยงต่าง ๆ.
