Homelessness has emerged as a major social problem. In an attempt to understand this problem, attention has been focused on postulating its causes, describing the individuals who hold this status, and estimating its magnitude. This study assesses the outcome of one social service program for long-term mentally disabled homeless individuals. It includes a synopsis of the state of the art in serving homeless individuals with severe mental health problems; a description of a program created to meet their needs; and an analysis of the outcome of this program.

Ethnographic methods were used to examine the sociolinguistic dimensions of female nurse practitioner interactions with female patients. The purpose was to provide a contextual account of the discursive practices used by the nurse practitioners during routine office visits with female patients. Direction for this research was provided by my concern about power inherent in professional-client dyadic relationships. Data collection methods included formal and informal interviews, participant observation and audio taping nurse-patient encounters. Nine nurse practitioners and 26 patients participated in the study in both private and public ambulatory care settings. Data analysis yielded two concurrent story lines which I labeled …

The present study was designed to examine possible neuropsychological deficits in migraine and tension headache subjects. Past research has been inconclusive, with some studies indicating that chronic migraine headache sufferers do exhibit some neuropsychological deficits such as short-term memory difficulties, gross motor slowing, and verbal memory deficits, while other studies have indicated that no deficits are seen in this group Also examined were headache precipitant and headache-related behaviors that these groups partake in when experiencing headache pain. Past research has suggested that migraine headache sufferers tend to deal with their headache pain differently than tension headache sufferers.

This study examined the relationship between chronic low back pain (CLBP) patients' perceived (stated) levels of function and their measured (observed) performance in squatting and stair climbing activities as compared to healthy volunteers. Twenty patients with CLBP and 20 healthy subjects were asked through an interview to self-assess their ability to comfortably perform stair climbing and squatting as well as other tolerances. The subjects were then asked to perform the activities and their performance levels were recorded. Occupational therapists treating older people with Alzheimer disease know that they must also consider the others who are affected by the disease, the informal caregivers. Intervention is most effective when it enables both the impaired person and the primary caregiver to manage the secondary symptoms of dementia. Unfortunately, little is understood about how caregivers approach and carry out their tasks and about why male and female caregivers respond differently to their caregiving role in terms of depression, burden, stress, and substance abuse. Field tests of upper body muscular strength and endurance (UBMSE) are often administered to children, but little is known about the determinants of performance on these tests. Therefore the purpose of this investigation was to examine potential determinants of performance on several common field tests of UBMSE including pull-ups, flexed-arm hang, push-ups, and two types of modified pull-ups. Subjects were 56 girls, and 38 boys, ages 9 to 11 years. Potential determinants assessed were age, height, weight, gender, % fat, physical activity, and laboratory measures of muscular strength and endurance. This paper examines the effect of the racial composition of labor markets on wage rates and the racial wage gap. The wage rates of white as well as black workers are significantly lower in industry- occupation-regional groups with high densities of black workers, while the racial wage gap does not vary systematically with respect to racial density. Interpretation of racial gap estimates can be sensitive to inclusion of a racial density variable, particularly in sparse specifications. An explanation for the wagedensity relationship cannot be established, but results are most consistent with a quality sorting explanation and, to a lesser extent, …

According to Ward, this may be the "ultimate question, faced by psychiatrists who are asked to deal with execution competency. This article is not intended to offer an answer to this question. Dementia Management: An Occupational Therapy Home-Based Intervention For Caregivers., Mary A. Corcoran, Laura N. Gitlin

This paper describes an occupational therapy intervention designed for family caregivers of persons with dementia. The intervention, based on the framework of a competence-environmental press model and the principle of collaboration, was implemented during 5 home visits. Each visit was designed to build caregiving skills through collaboration in identifying problem areas, developing and implementing environmental strategies, and modifying management approaches. A case vignette illustrates the therapeutic process and outcomes. The theoretical rationale and structure of the intervention and innovative documentation for evaluation of the theoretic process are also presented.

The hereditary conditions of primary cutaneous lichen amyloidosis and multiple endocrine neoplasia type 2 (MEN 2) are rare clinical entities. The initial reports of two families in which the two conditions coincided have led to the identification of at least eight additional families with this clinical syndrome. In this report we describe the clinical features in five of these eight families. The salient feature in these five families is the presence of unilateral (46%) or bilateral (64%) pruritic and lichenoid skin lesions located over the upper portion of the back. The study of moderators and higher-order effects of social influences on drug use has many implications for theories of health behavior. In the present study, we investigated the longitudinal predictive effects of some of the prominent moderator variables that represent forms of susceptibility toward social influence in teenage drug use. We also studied the possibility that social influence may predict drug use in nonlinear (quadratic) forms, consistent with theories proposing that threshold or decelerating effects may occur in social influences on normatively sanctioned behaviors. This study examined sibling conflict and relationship qualities in early adolescence. One hundred four 10- to 15-year-olds (mean age 11.7 years) completed questionnaires assessing the quality of their relationship with their closest sibling, and were interviewed about a recent, specific conflict with that sibling. Analysis suggests that sibling conflict fulfills several functions in early adolescence, including reinforcing family and relationship rules and delineating interpersonal boundaries. Few differences attributable to age or gender constellation of the sibling dyad emerged, and rivalry did not appear to be a primary impetus to conflict during this age period. The purpose of this study was to evaluate the level of satisfaction with care as experienced by the HIV-infected client in an outpatient setting who has received care from both nurse practitioners and physicians. This study assessed differences between female aerobic exercisers and nonexercisers in sexual functioning and arousability. Fourteen exercisers and 6 nonexercisers between the ages of 19 and 52 participated in the study. All subjects were involved in an ongoing sexual relationship with a duration of at least 3 months. Sexual functioning, satisfaction, and arousability were assessed through two self-report measures, the Sexual Interaction Inventory (LoPiccolo & Steger, 1978) and the Sexual Arousability Inventory (Hoon, Hoon, & Wincze, 1976); sexual arousability also was assessed through two extragenital physiological measures during exposure to erotic stimuli under laboratory conditions.

The purpose of this study was to investigate the relationship of six selected volleyball skill tests to actual volleyball game performance. The six skill test items were administered to 64 subjects, who were all members of a college level introductory volleyball course. Based on the ratings of a panel of experts, subjects were rated during game play as good, average, or poor performers.

The stepwise discriminant analysis was used to analyze the relationship between skill tests and game performance. Four of the six skill tests were identified as significantly contributing to group membership. These tests, in order of the magnitude …

From a sample of convenience, 83 college students were randomly selected to complete a questionnaire. The questionnaire contained 71 questions devised to reflect various aspects of the Patient Self-Determination Act. This study compared multiple sequential performances on a computerized version of the Wisconsin Card Sorting Test (Grant & Berg, 1948; Harris, 1986) by adult men receiving institutional inpatient treatment for chronic schizophrenia and related disorders. Participants were exposed to differential test conditions of minimal post-session non-contingent token reinforcement (baseline phases) and enhanced intra-session performance-contingent token reinforcement (experimental phase). Each major participant resided at the same continuing treatment unit at Kalamazoo Regional Psychiatric Hospital receiving similar social therapies but idiosyncratic psychotropic medication regimens, changes in which were tracked in relation to test performances. Second, I offer a brief narrative of the social and cultural variables that foster rootlessness and social disintegration in much of urban industrial life. Qualitative methodology was employed to conduct a phenomenological inquiry describing the structure of the experience of being pregnant and using drugs. The purpose of the study was to explore the nature of women's experience and perception of the interaction, relationship, and intersection of contextual phenomena of lifestyle, pregnancy, and substance abuse. Data gathering and analysis was accomplished using guidelines provided by Spiegelberg and Van Manen. While the unraveling of the kinship bond has long been suspected to play a role in the epidemiology of homelessness, the connection between kinship and homelessness has been little studied. Based on a normative analysis of the role of family structure in response to adversity, this article explores the impact of the amount and quality of kinship ties on episodes of homelessness experienced by discharged psychiatric patients in Ohio. In 1987, in response to reports of large numbers of veterans among America's homeless, the Department of Veterans Affairs established two new national health care initiatives, which have seen over 40,000 homeless veterans since their inception. We present here evaluation and treatment data on a sample of 14,000 of them. Because of differences in their design, the two programs vary in the degree to which they emphasize community outreach, homelessness prevention, and the provision of aftercare services to patients discharged from other VA programs. Homeless teenagers who have babies pose a significant population of concern for those in health and human services. This article explores demographic, structural, and economic changes for homeless young and single-parent families. It proposes that their homelessness is due to these barriers and the problems that result. Case studies illustrate the process of troubled teens becoming homeless women with babies. Policy recommendations for assisting these youngsters are offered.
