บทคัดย่อ

การวิจัยนี้มีวัตถุประสงค์เพื่อการศึกษาสถานการณ์โรคไม่ติดต่อและปัจจัยที่สัมพันธ์กับการใช้บริการรักษาพยาบาลแบบผู้ป่วยนอกและผู้ป่วยใน ในประชากรไทยอายุตั้งแต่ 45 ปีขึ้นไป โดยการใช้ข้อมูลจากโครงการสำรวจด้านสุขภาพ การสูงอายุ และการเกษียณในประเทศไทย (Health, Aging, and Retirement in Thailand: HART) รอบที่ 3 ปี พ.ศ. 2564 เป็นการวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง (cross-sectional analytical study) ผลการวิจัยพบว่า ปัจจัยด้านอายุ เพศ สถานภาพสมรส การศึกษา การสูบบุหรี่ สถานะสุขภาพกาย สถานะสุขภาพจิต ภูมิภาค และสิทธิหลักประกันสุขภาพ สัมพันธ์กับการเจ็บป่วยด้วยโรคไม่ติดต่อ ยกเว้นการออกกำลังกาย สำหรับการใช้บริการรักษาพยาบาล พบว่า ผู้ที่ป่วยด้วยโรคไม่ติดต่อตั้งแต่ 2 โรคขึ้นไป มีโอกาสในการใช้บริการรักษาพยาบาล จำนวนครั้ง (กรณีผู้ป่วยนอกและผู้ป่วยใน) และจำนวนวันนอนรักษาในโรงพยาบาล (กรณีผู้ป่วยใน) สูงกว่าผู้ที่ป่วยด้วยโรคไม่ติดต่อเพียง 1 โรค ปัจจัยที่สัมพันธ์กับการใช้บริการรักษาพยาบาลผู้ป่วยนอก ได้แก่ ปัจจัยด้านการออกกำลังกาย สถานะสุขภาพจิต และภูมิภาค ในขณะที่บริการรักษาพยาบาลผู้ใน พบว่า การเจ็บป่วยด้วยโรคไม่ติดต่อตั้งแต่ 2 โรคขึ้นไป อายุ และสถานะสุขภาพกาย สัมพันธ์กับการใช้บริการรักษาพยาบาลผู้ป่วยใน นอกจากนี้แล้ว การวิจัยนี้ไม่พบว่า สิทธิหลักประกันสุขภาพที่แตกต่างกันสัมพันธ์กับการใช้บริการรักษาพยาบาลทั้งผู้ป่วยนอกและผู้ป่วยในที่แตกต่างกัน ข้อค้นพบจากการวิจัยนี้สามารถนำไปใช้ประโยชน์ในการกำหนดนโยบายสาธารณสุขด้านโรคไม่ติดต่อและเพื่อพัฒนาระบบสาธารณสุขของประเทศไทยต่อไปได้

Factors Associated With Early Onset Anal Cancer And Facilitators And Barriers To Screening Among Men Living With Hiv And Their Providers In The Deep South: A Mixed Methods Investigation, Anna Junkins

For my dissertation work, I aimed to 1) determine the occurrence of early onset (diagnosis at <50 years) anal cancer and the impact of HIV infection and other risk factors, and 2) utilize a mixed methods research approach to identify and explain factors associated with screening among men living with HIV (MLHIV) and their providers. In Aim 1, I used logistic regression to estimate the odds of early onset cancer by sociodemographic and clinical characteristics among anal cancer patients in the Alabama cancer registry from 2012-2018. Among 519 patients, 17.7% were diagnosed at <50 years. Male sex, Black race, and HIV infection were associated with younger age at diagnosis. Black patients had a 4-fold increased odds of early onset anal cancer compared to White patients and disproportionately lived in areas with higher social vulnerability. In Aim 2, I used logistic regression to estimate the odds of anal cancer screening by sociodemographic and sexual risk characteristics among MLHIV using retrospective survey data from 1917 Clinic in 2018. Among 1,114 men, 52% had received an anal pap within one year of completing the survey. Men who were screened were more likely to have multiple sexual partners. Heterosexual men had lower odds and men who reported unprotected anal sex had higher odds of screening. Older Black men were less likely to be screened. In Aim 3, I used thematic analyses to identify themes related to health beliefs, barriers and facilitators, and perceived and evaluated need of screening using in-depth interviews among MLHIV and their providers. Benefits of screening among MLHIV were early detection and awareness about one’s body. Suggestions for overcoming barriers to screening among providers included screening guidelines, patient education, inclusion of screening into routine care, and self-collection. Among men that were aware of HPV-associated cancers, there was perceived risk and desire to receive HPV vaccination. Individuals who are male, Black, or living with HIV may be at increased risk of developing anal cancer younger. Among MLHIV, improving education about HPV-associated cancer, and offering self-collected screening as part of routine care could facilitate screening in the South.

Historically, tobacco companies have used flavored tobacco products to enhance the appeal of tobacco consumption, encourage initiation and experimentation of tobacco use, and contribute to sustained tobacco use. While flavored tobacco products are regulated in several countries, there is no existing regulation on flavored tobacco products in the Philippines, specifically for cigarettes and cigars. This study aims to update evidence on the flavored tobacco product landscape in the Philippines by assessing both the flavor descriptors and flavor imagery featured on cigarette and cigar packaging.

BACKGROUND: A wealth of clinically relevant information is only obtainable within unstructured clinical narratives, leading to great interest in clinical natural language processing (NLP). While a multitude of approaches to NLP exist, current algorithm development approaches have limitations that can slow the development process. The spatial arrangement of immune cells within the tumor microenvironment (TME) and their interactions play critical roles in the initiation and development of cancer. Several advanced technologies such as imaging mass cytometry (IMC) providing the immunological landscape of the TME with single-cell resolution. In this study, we develop a new method to quantify the spatial proximity between different cell types based on single-cell spatial data. Consensus statements can be very influential in medicine and public health. Some of these statements use systematic evidence synthesis but others fail on this front. Many consensus statements use panels of experts to deduce perceived consensus through Delphi processes. We argue that stacking of panel members toward one particular position or narrative is a major threat, especially in absence of systematic evidence review. Stacking may involve financial conflicts of interest, but nonfinancial conflicts of strong advocacy can also cause major bias. The human sex ratio at birth (SRB) undergoes temporary changes around a mean proportion of 0.51 male births. SRB has been well studied for historical, geographical, and secular trends, but until now not linked to health outcomes in the total population, e.g. for cardiovascular disease (CVD) or mortality during follow-up of birth cohorts. We used linkage analysis based on national registers in Sweden that cover all births from 1900 to 2016. BACKGROUND: Targeting kinases presents a potential strategy for treating solid tumors; however, the therapeutic potential of vaccines targeting kinases remains uncertain.

METHODS: Adenovirus (Ad) vaccines encoding Aurora kinase A (AURKA) or cyclin-dependent kinase 7 (CDK7) were developed, and their therapeutic potentials were investigated by various methods including western blot, flow cytometry, cytotoxic T lymphocyte assay, and enzyme-linked immunospot (ELISpot), in mouse and humanized solid tumor models.

RESULTS: Co-immunization with Ad-AURKA/CDK7 effectively prevented subcutaneous tumor growth in the Renca, RM-1, MC38, and Hepa1-6 tumor models. In therapeutic tumor models, Ad-AURKA/CDK7 treatment impeded tumor growth and increased immune cell infiltration. Administration of …

This study aimed to identify the challenges faced by informal caregivers in managing the oral health of stroke patients. This study conducted semi-structured individual interviews with eight home-based caregivers of stroke patients recruited from a community-based clinic facility of a public university in Malaysia. BACKGROUND: Emerging evidence suggests a central role for inflammation in cardiac conduction disorder (CCD). It is unknown whether habitual physical activity could modulate the inflammation-associated risks of incident CCD in the general population. METHODS AND RESULTS: This population-based cohort was derived from the China Kailuan study, including a total of 97 192 participants without prior CCD. The end points included incident CCD and its subcategories (atrioventricular block and bundlebranch block). Systemic inflammation was indicated by the monocyte-to-lymphocyte ratio (MLR). Introduction: Men with African ancestry have the highest incidence and mortality rates of prostate cancer (PCa) worldwide. Methods: This study aimed to identify differentially methylated genes between tumor vs. adjacent normal and aggressive vs. indolent PCa in 121 African American patients. Epigenome-wide DNA methylation patterns in tumor DNA were assessed using the human Illumina Methylation EPIC V1 array. Results: Around 5,139 differentially methylated CpG-sites (q < 0.01, lΔβl > 0.2) were identified when comparing normal vs. tumor, with an overall trend of hypermethylation in prostate tumors. Since SSRI medication is the most common treatment for depression, it is important to determine if SSRIs are associated with reduced dementia and better cognitive outcomes. Using longitudinal data from the 2004-2014 waves of the Health and Retirement Study merged with the 2005 Prescription Drug Study, we examined potential associations between SSRI and TCA medication and cognitive impairment and cognitive decline. BACKGROUND: Understanding the distinct immunologic responses to SARS-CoV-2 infection among pediatric populations is pivotal in navigating the COVID-19 pandemic and informing future public health strategies. This study aimed to identify factors associated with heightened antibody responses in children and adolescents to identify potential unique immune dynamics in this population.

METHODS: Data collected between July and December 2023 from the Texas Coronavirus Antibody REsponse Survey (Texas CARES), a statewide prospective population-based antibody survey among 1-to-19-year-old participants, were analyzed. Single nucleotide polymorphism (SNP) interactions are the key to improving polygenic risk scores. Previous studies reported several significant SNP-SNP interaction pairs that shared a common SNP to form a cluster, but some identified pairs might be false positives. This study aims to identify factors associated with the cluster effect of false positivity and develop strategies to enhance the accuracy of SNP-SNP interactions. The results showed the cluster effect is a major cause of false-positive findings of SNP-SNP interactions. Introduction. Chronic obstructive pulmonary disease (COPD) is the sixth leading cause of death in the US and approximately 10% of people 65 years and older have COPD. This study examines the association of geographic-level risk factors with chronic obstructive pulmonary disease (COPD) prevalence using spatial-based analyses with the goal of identifying risk factors associated with county-level COPD and to identify communities at risk.

Human exposure to environmental contaminants is associated with various pediatric health outcomes. Agrichemicals are a significant environmental risk factor for childhood cancers and congenital anomalies, and reduced exposure could improve pediatric health outcomes. Previous studies in Nebraska have explored the association between single pollutants and pediatric cancers and birth defects. Background: Ensuring the proper fit of N95 respirators used by healthcare workers in long-term care facilities (LTCF) is essential for their safety. The Nebraska Department of Health and Human Services launched a LTCF strike team project (STP) in the summer of 2022, focused on increasing the number of staff trained in infection control and building internal capacity for LTCFs to fit test their employees. This study describes the N95 fit-testing initiative for Nebraska LTCFs.

This paper proposes a phase IV study to estimate the effectiveness of Abryvso and Arexvy in older adults against severe RSV disease. Additionally, the study aims to assess factors involved with low vaccine uptake for these RSV vaccines.

Methods

For the background literature review of this proposal, I conducted a search of the PubMed and CINAHL Ultimate databases for articles on older adults and RSV. Protein tyrosine kinases (PTKs) function as key molecules in the signaling pathways in addition to their impact as a therapeutic target for the treatment of many human diseases, including cancer. PTKs are characterized by their ability to phosphorylate serine, threonine, or tyrosine residues and can thereby rapidly and reversibly alter the function of their protein substrates in the form of significant changes in protein confirmation and affinity for their interaction with protein partners to drive cellular functions under normal and pathological conditions.
