Healthcare facilities are high-risk areas for legionellosis infections, as they have large, complex potable water systems ideal for Legionella proliferation, and they host susceptible populations. New York State regulation 10 NYCRR § 4-2: Protection Against Legionella in Healthcare Facilities was implemented in July 2016 to reduce healthcare-acquired legionellosis. This regulation mandated environmental surveillance of healthcare facilities’ potable water systems via culture. Data from these environmental reports were paired with data for cases of healthcare-acquired legionellosis infections for the years 2017-2023. Using these data, geographic Legionella species and serogroup distributions were described, and logistic regressions evaluated facility attributes associated with potable …

Maternal alcohol consumption during pregnancy is known to be associated with poor pregnancy outcomes, including fetal alcohol spectrum disorders. However, evidence for associations between alcohol consumption during pregnancy and some major structural birth defects among offspring is conflicting or inconclusive. This dissertation used data from the National Birth Defects Prevention Study (NBDPS), a multi-site, population-based case-control study, to examine associations between maternal alcohol consumption and risk of three major musculoskeletal birth defects: gastroschisis, omphalocele, and craniosynostosis. Background: Drug overdose deaths in the U.S. have been increasing and are primarily related to opioid use. In 2019, an estimated 7.2% of US high school students reported misusing prescription opioids in the past 30 days. Prescription opioid misuse in adolescents may lead to development of a substance use disorder, overdose, or other serious harms. Some researchers have observed sex and race differences in prescription opioid use. This dissertation examines the validity of maternal self-reported medication exposures and then explores the associations between self-reported sleep aid use, including zolpidem specifically, and birth defects using data from three population-based case-control studies conducted within the United States. The first aim of this dissertation was to explore agreement between two sources of prescription medication use in early pregnancy: medical records and self-reported interview data from the Birth Defects Study To Evaluate Pregnancy exposureS (BD-STEPS). Background: Prenatal exposure to perfluoroalkyl substances (PFASs), a group of persistent environmental chemicals with endocrine disrupting abilities, has been associated with immunomodulation and may contribute to the etiology of asthma in young children. Background: Annually, 3-5% of infants are born with birth defects in the United States. In New York State (NYS), air pollution, specifically ozone (O3) and fine particulate matter (PM2.5), is detectable at levels that affect human health. The body of public health literature related to air pollution epidemiology is extensive. Similarly, the body of literature related to social epidemiology has also expanded greatly in recent years. While studies in these sub-disciplines of epidemiology sometimes seek to control for variables more commonly studied in one or the other, fewer studies report on their combined effects. ACEs stands for Adverse Childhood Experiences, which are traumatic or stressful events that occur in childhood before the age of 18 (Wisner, 2022). These experiences can include physical, emotional, or sexual abuse; neglect; household dysfunction, such as parental separation, domestic violence, substance abuse, or incarceration; or exposure to community violence, racism, or discrimination. Background: Chronic diseases impose heavy burdens on individuals and the healthcare system in the US. Many factors were found to be associated with chronic diseases, including demographics, family history, social environmental factors, and individual behavioral factors such as diet and physical activity. Among those factors, fruit and vegetable intake can have substantial health impacts via a variety of causal pathways. The Akwesasne reservation along the St. Lawrence River has experienced significant exposure to several persistent organic pollutants due to rapid industrialization. The Aluminum Company of America, General Motors, and Reynolds Metals operated aluminum foundries that used polychlorinated biphenyls (PCBs) as hydraulic fluids, which contaminated the community environment, the soil, air and water, and community food sources with PCBs. We administered the World Health Organization Disability Assessment Schedule II (WHODAS II) to 119 Akwesasne Mohawk adults residing along the St. Lawrence River in New York. Fungal infections, especially vulvovaginal candidiasis (VVC), are common among pregnant women. Therefore, it is important to study the effect that antifungal medications may have on the developing fetus. Recent studies have suggested there are positive associations between birth defects and specific oral antifungals. In this dissertation, maternal antifungal use during pregnancy was examined using data from the National Birth Defects Prevention Study (NBDPS). The NBDPS was multi-site, population-based, case-control study and included women with estimated delivery dates during October 1997 through December 2011. Antimicrobial resistance in Salmonella enterica remains a challenging public health threat. We aim to characterize the population structure and evolutionary history of 394 genomes of S. enterica isolates recovered from human clinical cases in New Hampshire, USA from 2017 to 2020. The population is phylogenetically diverse, consisting of 78 STs and 67 serotypes. We detected 61 genes associated with resistance to14 different antimicrobial classes. Rather than a single multidrug resistant clone expanding in the state, we found multiple lineages carrying different combinations of independently acquired resistance determinants. New or emerging lineages can therefore rapidly spread over relatively short timeframes. In …

Human granulocytic anaplasmosis (HGA) and human babesiosis are tick-borne diseases spread by Ixodes scapularis (the blacklegged or deer tick) and are the result of infection with Anaplasma phagocytophilum and Babesia microti, respectively. In New York State (NYS), incidence rates of these diseases increased concordantly until around 2013, when rates of HGA began to increase more rapidly than human babesiosis, and the spatial extent of the diseases diverged. Background: Ticks, Lyme disease, and other tickborne diseases are a concern of public health entities in Maryland and New York State. Children are at an increased risk of contracting a tickborne disease. Healthcare professionals working in schools (HPWS) are often front-line providers and health educators for school-aged children.
