With the liberalization of abortion laws by the Supreme Court in 1973, American women have been able to obtain therapeutic abortions in hospitals and out-patient clinics in ever increasing numbers. With the increased number of women seeking abortions the medical field has responded with abortion procedures that are safe and relatively quick. With the speed doctors use to perform the procedure, many women may wish to take advantage of the medically safer method of abortion under local anesthesia. The white-faced ibis (Plegadis chihi) which nested in northern Utah was assessed as a species with a relatively low reproductive potential. Ibises normally laid either three or four eggs in a clutch and did not renest persistently, nor with good success, if initial nesting attempts failed. Evidence indicated that the birds did not breed until at least 2 years old. Competition for food, exposure to severe weather, and predation were conspicuous sources of nestling mortality. White-faced ibises usually fed in irrigated agricultural areas, where they came into direct contact with insecticides, such as DDT which was used routinely …

Intraoperative transanal fiberoptic colonoscopy is presented as an alternative to transcolonic procedures to remove colonic polyps, or to excise or biopsy other intraluminal lesions. This method should be considered when the standard transanal colonoscopy has failed. The results with this procedure have been excellent. The method also can be applied to patients who have colonic lesions and are to undergo elective procedures such as hysterectomy and cholecystectomy.

Over a one-year period beginning in April, 1946, nine transthoracic vagotomies were done at the Henry Ford Hospital. Three of the patients had had a partial gastrectomy previously and the operation was done for recurrent bleeding from marginal ulcers. Thirty years later, an attempt was made to ascertain the present condition of these patients. Long term follow-ups were possible on six patients and follow-ups of one, six and 14 years on the others. All nine patients had immediate relief of ulcer pain and cessation of bleeding if present. No subsequent drainage operation was required for the six patients with primary …

Hereditary spastic paraplegia was diagnosed in 19 patients belonging to four families. All had spastic gait and upper motor neuron signs in lower limbs. In family A, the disease was expressed in this manner, although one member had muscle wasting in an upper limb. There was additional involvement of the posterior columns in six of the eight affected members of family B and cerebellar ataxia was noted in upper limbs of three of them. Technetium 99m pyrophosphate (99mTCPyP) accumulates in recently infarcted myocardium and can be detected by external imaging techniques. This study was performed to evaluate the ability of this isotope to identify the presence of acute myocardial infarction. In 82 patients admitted to a coronary care unit with chest pain of varying etiology, scan was positive in all 13 patients with acute transmural myocardial infarction and in 23 of 27 patients with nontransmural myocardial infarction. The scan was negative in 37 of 42 patients without evidence of recent infarction. Four of the remaining five patients in this group had unstable angina pectoris. …

In a review of 68 patients with dissecting thoracic aorta aneurysm (DTAA), eight patients (11.7%) were found to have painless dissection. Three of the eight patients presented with gross aortic insufficiency (Al). Angiography on a fourth patient, who had severe hypertension, showed Type III DTAA. Three other patients had either dilatation of the ascending aorta or a possible mediastinal mass, according to chest roentgenograms. One patient received treatment for suspected myocardial infarction with cardiogenic shock. The complexity of the issues involved with providing appropriate health care and social services from the appropriate setting to people over age 65 can hardly be overstated. One of the present debates in the field focuses on the value of institutions as the customary setting for providing health care; the arguments are based on considerations of economic efficiency and the recipient's quality of life. Some of the debators suggest deinstitutionalizing as many of the health care recipients as possible, while simultaneously upgrading the quality and quantity of home based support services. The purpose of this study was to determine what sociocultural, sociodemographic, and sociopsychological factors the Sisters in four Wards in the Taipei Taiwan Stake of the Church of Jesus Christ of Latter-day Saints perceived as keeping them from using the health services during the period from June 1975 to May 1976.

Forty-five Sisters, age twenty-one and older, were surveyed by a written questionnaire during Relief Society meetings during the last week in June and the first week in July 1966.

Technedum 99m stannous pyrophosphate (TcPyP) myocardial scans were obtained in 42 selected nonconsecutive patients within 12 days following open heart operations for both valve replacement (19 patients) and coronary bypass (23 patients) procedures. The scans were correlated with elevation of serum lactic dehydrogenase, glutamic oxalacetic transaminase and the electrocardiograms obtained serially in the postoperative period. TcPyP scan was more specific than enzyme studies and it was more sensitive than the electrocardiogram. All new Q waves were associated with a positive scan but there were five positive scans which were not associated with new Q waves. Myotomy and direct surgical trauma …

A logical method of repair of esophageal hiatus hernia based on anatomic principles was proposed nearly 25 years ago by the late Phillip Allison. There was general agreement that his operation had advantages, but because of a disappointing number of recurrences and problems with reflux, more complicated operations were devised. Some of these involved plication of the stomach around the lower esophagus (Nissen, Belsey). In a series of 562 operations for hiatus hernia at the Henry Ford Hospital, there has been no significant deviation from the Allison technic. Susceptibility At High Inoculum Of Penicillin-Resistant Staphylococcus Aureus To Six Cephalosporins, Andres Suarez, Ramon Del Busto, Edward L. Quinn, Donald Pohlod

Six cephalosporins (currently available for clinical use or undergoing clinical trials) were tested by tube dilution method against high inoculum (10⁷ colony forming units) of 37 strains of penicillin resistant Staphylococcus aureus. 3.1 μg/ml of cephalothin inhibited 100% of strains while higher concentrations of cefoxitin, cephapirin, cefamandole, cephalexin and cephradine were required. The percent of strains with minimal inhibitory concentration higher than 3.1 μg/ml ranged between 10.8% (cefoxitin) and 100% (cephradine). These findings support the observation that cephalothin is less susceptible to inactivation than the other cephalosporins. A case of miliary tuberculosis complicated by respiratory insufficiency and disseminated intravascular coagulation was treated successfully with antituberculosis drugs, ventilatory support, and heparin, packed red cells, platelet transfusions and plasma. Tuberculosis is a rare cause of disseminated intravascular coagulation.

A technique for treating the para-aortic nodes to a high dose of irradiation in continuity with the pelvis has been developed at Henry Ford Hospital and recently described by one of the authors. The "nine-field technique" was initially used in late 1973 to treat two patients with advanced prostatic carcinoma who had grossly involved para-aortic nodes. Since that time, 16 additional patients with carcinoma of the prostate, one patient with carcinoma of the urinary bladder, and one patient with carcinoma of the cervix have been treated by this technique. This study presents the clinical features, roentgenographic manifestations, pathological features, abnormal laboratory studies, treatment and prognosis of 157 cases of osteogenic sarcoma. The series is the fifth largest ever reported. The authors favor prompt surgical ablation followed by controlled intensive chemotherapy as a prophylactic or combined chemotherapy and radiation therapy for metastatic osteogenic sarcoma. All case reports but one present rare or unusual situations.
