Dissertations and Theses

The current study was designed: (1) to develop an educational tool on female masturbation capable of reaching a wide audience of women with accurate and complete information on female masturbation; (2) to provide initial validation for the further educational usefulness of this tool and (3) to assess its potential ability for improving overall sexual functioning in women by possibly altering negative attitudes toward masturbation and/or by dis-inhibiting masturbatory behavior.

Intractable Hiccups And A Posterior Fossa Arteriovenous Malformation: A Case Report, Timothy J. Laing, Mircea A. Morariu, Ghaus M. Malik, Suresh C. Patel

A 29-year-old man is presented who developed intractable hiccups following a subarachnoid hemorrhage from a posterior fossa dural arteriovenous malformation (AVM). The hiccups persisted even after various treatments had been attempted, and the AVM had been excised surgically. The hiccups stopped after a bilateral phrenic nerve section was performed. This report reviews the central causes and mechanisms of hiccups and emphasizes the importance of selective vertebral angiography in demonstrating posterior fossa AVM.

Traumatic injury to the upper third of the neck, cephalad to the angle of the mandible, distorts the complexity of normal anatomy and underscores the importance of preoperative arteriography when possible. This report describes the case of a young man who was brought to the Emergency Room of our hospital suffering from a severe gunshot wound to the cervical spine, vertebral artery, and maxillofacial skeleton. His injury was successfully managed by combining a standard arterolateral incision with a procedure that has been described for exposing retro maxillary tumors. The Polytetrafluoroethylene (Ptfe) Graft For Portacaval Interposition: An Experimental Observation, Alexander G. Yap, George Isshak, Ronald Lam, Roger F. Smith

In an attempt to determine the efficacy of the polytetrafluoroethylene (PTFE) graft as a portacaval interposition shunt, 26 mongrel dogs were randomly selected to receive a side-to-side shunt with a 6 mm, 8 mm, or 10 mm PTFE graft after portal hypertension had been created by hepatic vein ligation. The dogs were clinically observed, and if they showed signs of graft occlusion, they were reexplored surgically. The 6 mm graft had a 0% patency rate; the 8 mm graft had a 28.6% patency rate; and the 10 mm graft had an 87.5% patency rate. Histological examinations revealed adequate incorporation of …

Atlanto-axial subluxation in patients with rheumatoid arthritis presents a difficult therapeutic problem. Because of their underlying disease, these patients do not tolerate the heavy orthotic devices or prolonged periods of immobilization associated with traditional methods of surgical fusion. This paper describes a surgical technique which allows early mobilization and minimal external fixation. The results from a series of ten patients in which this technique was used are also discussed.

A Role For Dentistry In Managing The Oro-Nasopharyngeal Irradiated Cancer Patient, Michael R. Britt

Radiation therapy for oral and nasopharyngeal carcinoma produces direct and indirect changes in the oral cavity. Before such therapy is begun, the cancer patient should be examined by a trained dental specialist so that associated problems such as oral mucositis, xerostomia, radiation dental caries, osteoradionecrosis, and ageusia can be addressed. During and after irradiation therapy as well, the dental specialist should be involved in the treatment program. In this way, it Is possible to maintain the patient's dental system through restorative measures, mandatory use of topical fluorides, dental rinses, and preventive prophylaxis.

Behcet's syndrome is a disease commonly found in Japan and in countries around the Mediterranean Sea. In the U.S., it is relatively uncommon. This report describes a 24-year-old Lebanese-American man with Behcet's syndrome who presented with symptoms four months after he developed arthritis and who had been initially diagnosed as having incomplete Reiter's syndrome. A literature review is Included.

The Epidural Defect On Myelography: Some Unusual Causes, Keenan L. Carroll

This report describes four cases of epidural defect found on myelography that illustrate some unusual causes of this defect. These include a type I chondrosarcoma, primary hemangioendothelioma, hematoma, and an epidural abscess. Surgery was performed, and all four patients have recovered with little or no remaining neurological deficit. If there is anything unusual about the epidural defect or if the clinical findings do not agree with the radiographic films, the radiologist should be alert for other, less common causes of the defect.

Second-Year Follow-Up Study On Systematic Treatment Of Chronic Pain With Antidepressants, Dietrich Blumer, Mary Heilbronn

Systematic therapy with antidepressant drugs can be an effective treatment for chronic pain patients with the pain-prone disorder. This survey follows the pain status, medications, depressive symptoms, work status, and treatment of 104 patients treated for chronic pain at Henry Ford Hospital's Pain Clinic during a two-year period for chronic pain in the absence of a significant somatic disorder. No patients were excluded from treatment and the dropout rate of 64% after two years was not surprising. The pain of those who complied with therapy improved significantly (62%), compared to that of the patients who had dropped out of the …

Combining Provocative Agents Of Calcitonin To Detect Medullary Carcinoma Of The Thyroid, J. C. Sisson, M. D. Gross, J. E. Frietas, C. E. Jackson, B. G. England

The presence of clinically undetectable medullary thyroid cancer can be revealed by characterisdc patterns of calcitonin in serum after the injection of a provocative agent, pentagastrin or calcium ion. However, in some reports, medullary thyroid cancer was uncovered in a test with pentagastrin but not in one with calcium ion, and vice versa. To reduce the number of erroneous responses, each provocative agent has been employed in separate tests. Combining pentagastrin and calcium into one stimulus should give the advantage of each agent while requiring less time and fewer analyses in a single test.
