Otitis media continues to be a common and disabling problem which has become, in recent years, a fertile area for research. Based on studies in animals and humans, otitis media is classified into four groups: acute purulent otitis media (POM), serous otitis media (SOM), and mucoid or secretory otitis media (MOM). Together, these are referred to as otitis media with effusion (OME). The types can overlap and evolve into another type to become chronic otitis media and mastoiditis, characterized by the presence of granulation tissue or cholesteatoma. How Useful Is Peritoneal Lavage Amylase?, Eugene Meyer, James C. Gruenberg

Prospectively for 23 months, 289 patients with penetrating and 317 patients with blunt trauma to the lower chest and/or abdomen had amylase determinations in the peritoneal lavage as part of their initial evaluation. In nine (1.5%) of the 606 patients, the amylase in the lavage was higher than normal serum values and, when elevated, it identified intra-abdominal injury. Intra-abdominal injuries usually involved a hollow viscus injury—either isolated or combined with solid organ injuries—rather than pancreatic injury. Although elevation of the lavage amylase was a reliable indication of intra-abdominal injury in this series, we feel that routine use of this test …

To identify risk factors for the occurrence of Kaposi's sarcoma and Pneumocystis carinii pneumonia in homosexual men, we conducted a case-control study in New York City, San Francisco, Los Angeles, and Atlanta. Fifty patients (cases) (39 with Kaposi's sarcoma, 8 with pneumocystis pneumonia, and 3 with both) and 120 matched homosexual male controls (from sexually transmitted disease clinics and private medical practices) participated in the study. The variable most strongly associated with illness was a larger number of male sex partners per year (median, 61 for patients; 27 and 25 for clinic and private practice controls, respectively). The composition of phosphate metabolites and the intracellular pH in erythrocytes from a patient with hereditary pyrimidine-5'-nucleotidase deficiency were examined using ³¹P NMR spectroscopy. Several resonances were identified in spectra from intact cells and from extracts. The 2,3-bisphosphoglycerate line intensities were normal but the NTP resonances were about twice normal due to the presence of millimolar quantities of pyrimidine phosphates. Several intense resonances were also observed in the diphosphodiester region of the spectrum. One compound contributing to these lines has been identified as cytidine diphosphocholine. Quand les vingt membres originaux s'associèrent en 1975, leur but était de gagner un salaire et d'apprendre des techniques reconnues et negociable sur le marché de l'emploi. En 1981, elles gèrent avec succès une cooperative qui produit et vend des pièces d'étoffe teintes et du savon de lessive. This issue of SEEDs tells the story of how the Working Women's Forum came into being in Madras, India and how it has brought together more than 13,000 poor, urban women around the issue of credit. It also describes how the Forum has not only provided its members with access to funds, but has expanded to include support services such as child care, education, health, and family planning and how the sense of strength and purpose that has grown up among the members is helping them to tackle the political and social problems that affect their lives.

A hyperviscosity syndrome developed in two patients with rheumatoid arthritis. Analytic ultracentrifugation of the sera from one patient demonstrated 22S complexes. Intermediate, 22S, and 31S complexes were found in the second case. Platelet aggregometry and electron microscopy surveys of the platelet reactivity in one patient demonstrated inhibition of platelet reactivity caused by the presence of the high molecular weight complexes. These abnormalities, as well as the clinical bleeding, whole blood, and plasma viscosity, became normal after treatment. In vivo leukocytic function was studied with the Rebuck skin window technique, and phagosomal inclusions were observed in both the polymorphonuclear neutrophils and …

/Among many measurable hormones, human chorionic gonadotropin (FICG) is commonly used for pregnancy testing because it is very sensitive and relatively specific. Pregnancy can be identified shortly after implantation. Because some reagents cross-react with luteinizing hormone (LH), the sensitivity of urine tests has been adjusted to maintain specificity. Radioreceptor assay s on serum are more sensitive than urine tests but similarly lack specificity. The problems of LH cross-reactivity are eliminated by antisera specific for the beta subunit. Quantitation of HCG provides additional information useful in diagnosing ectopic pregnancy, providing a prognosis in threatened abortion, and following neoplasms. Considerations of cost, …

Assessing the microbiological water quality of impoundments and the potential for waterborne disease outbreaks is a difficult task when using traditional sampling programs. Problems associated with using fecal coliform bacteria as indicators of human pathogen presence complicates assessments of future water quality in preimpoundment areas. Reliable determination of future and present microbiological water quality requires knowledge of how the chemical, physical, and biological characteristics of the watershed and impoundment interrelate to influence microbial indicator and pathogen densities. Accurate estimates of microbial indicator and pathogen densities, obtainable by using the enumeration methods and their modifications suggested in this report, will allow …

Continuing problems in the management and surgical treatment of primary hyperparathyroidism include the localization of the elusive single tumor, the recognition and management of multiple gland involvement, prompt confirmation and operative treatment for hypercalcemic crisis, delineation of a liberal but selective policy relative to surgery for apparent uncomplicated hyperparathyroidism in the elderly, and recognition of the surgeon's responsibility to determine the extent of the operation on the basis of operative findings, realizing that histologic study may not confirm apparent gross abnormalities despite subsequent clinical recovery. When all four glands are involved in primary hyperparathyroidism, subtotal parathyroidectomy is recommended except for …
