A 79-year-old farmer (the victim) was killed while driving his tractor. He had been a full-time farmer who lived alone and farmed on the 136-acre land nearly all of his life; the main commodity was beef cattle. As was his usual practice, he was working alone on the day of the incident. Every evening he had dinner with his two sisters who lived in a house on the same property about 50 yards from his home. When he did not show up for the evening meal, they went out to look for him. They found that he had taken the …

In this issue of American Behavioral Scientist, we describe the state-of-the-science of drug abuse prevention, including program evaluation and content suggestions, and major methodological and substantive challenges as we understand them at this late date in the twentieth century. These articles also suggest important areas for prevention research as we enter into the next century.

This article summarizes current issues in drug abuse prevention research through integration of other articles in this journal and by heeding historical trends in prevention science. Recommendations are made for future research directions. For prevention to advance, iterative processes are needed involving both quasi-experimental and experimental designs and involving both small, simple units and large, complex, interactive units. Accuracy of measurement and replication are of paramount importance.

The most popular school programs for tobacco and other drug abuse prevention address a variety of substances from tobacco to alcohol to other drugs. It is not uncommon for the same school program to address an even broader range of health and social objectives including exercise and nutrition promotion, pregnancy and HIV prevention, and violence prevention. In this article, the authors address the question of whether it is more effective to target a narrow range of objectives (e.g., smoking prevention) or a wider range of related behaviors (e.g., tobacco, alcohol, and other drug abuse). Negotiating The Healthy Self: A Theory Of Health Behavior In Older Adults, Barbara Schwitz White

Researchers have identified discrete behaviors of older persons to maintain or improve health and prevent or treat illness. Few have developed theories of health behavior specific to this population. The purpose of this study was to develop a substantive theory to explain how health and illness are integrated into and influence behaviors in the daily lives of community-residing older persons. Using grounded theory design and analysis, 26 participant observations involving 186 individual encounters, and seven semistructured [sic] one-on-one interviews were conducted at two senior centers in urban southern California, between 1992 and 1994.

Endothelial dysfunction has been found prior to any signs of atherosclerotic plaque formation in patients with a variety of risk factors for atherosclerosis. Elevated serum cholesterol has repeatedly been associated with endothelial dysfunction as demonstrated by impaired vasoreactivity to vasodilating stimuli.
The aim of this research was to determine whether short term lowering of serum cholesterol levels by life style changes or by cholesterol lowering medication could reverse or reduce the endothelial dysfunction. The brachial artery diameter was measured by high-resolution ultrasound at rest and during reactive hyperemia pre and postintervention. Measuring Hospital Use Without Claims: A Comparison Of Patient And Provider Reports., R E. Clark, S K. Ricketts, G J. Mchugo

We compared the validity of hospital admission and length of stay reports from patients, outpatient providers, and hospitals, and we examined possible sources of error. Data were collected from people enrolled in a randomized trial of treatment for severe mental illness and substance use disorders, from community mental health centers (CMHCs), and from hospitals. Reports for each of the 74 study participants covered two-year time periods beginning and ending at various times between 1989 and 1993. We compared reports from the various sources and constructed a hybrid with data from all three sources. Using parametric and non-parametric statistics, we compared …

Ki, Japanese for living energy (Chi in Chinese), is at the absolute core of the Oriental model of medicine and wellness. Complementary to this model, many Oriental martial art exercises have evolved to cultivate Ki for optimal health. However, the concept of Ki remains poorly understood and relatively unexplored in traditional Western health and medical literature. Further, the health focus of martial art research has remained primarily concerned with the arts' contribution to psychological health. This has left a significant gap in our understanding of the other health benefits that these arts and Ki have to offer. To begin closing …

A 42-year-old logger was killed after being struck on the head by a falling limb from a snag (dead standing tree). The victim had been working with one co-worker, who had temporarily left the scene to remove some logs. They were selectively cutting dead or dying trees which were approximately 20 inches in diameter. While his co-worker was away, the victim cut a tree which broke a snag approximately 30-35 feet up. The snag fell directly on the victim, striking him in the forehead. He was not wearing any type of head protection at the time of the incident. The …

A 44-year-old farmer was killed when his jacket became entangled on the protruding metal shaft of a feed bin auger. The victim was atop the feed bin, trying to unclog feed from the auger by beating on the tin cylinder through which the feed passed into the bin. (See Figure 1.) The feed had become clogged at this point several times in the past, and the victim had performed this task on those occasions without injury. There were no witnesses to this incident. Studies show that numerous and complex variables influence pregnancy outcomes and infant mortality rates. These variables include demographic, medical, physical, environmental, behavioral, and attitudinal factors and prenatal care. Improving the health of mothers and infants is a national challenge. In 1987, more than 3.8 million infants were born in the United States. Of these, 38,408 died before their first birthday. Although the infant mortality rate is at an all-time low, the pace of progress has slowed. The purpose of this study was to address any changes in select organizational and director or CEO attributes between 1989 and 1994, as well as, to determine whether or not college student health services are systematically evaluating their programs and incorporating efficiency measures used in other health care settings. The population of study was student health services at four-year institutions of post-secondary education in the United States. This researcher resurveyed the sample used by Kevin E. In just a little over two years, at least 16 Kentucky loggers have been killed as a result of being struck by falling trees or branches (12), or by logs falling from trucks they were unloading (4).
Described below are two representative fatal incidents, followed by our recommendations for prevention of similar incidents.

Lifestyle changes from traditional diet and activities to modem diets high in fat and sugar, and sedentary habits have increased inherited health risk for diabetes and obesity among Native American youth. Nutrition education and physical activity programs have been recommended to help reduce health-risk factors. This study evaluated a summer nutrition education and fitness program for effectiveness in improving the dietary habits, nutrition knowledge, fitness level, and health status of American Indian children ages 9-14 living on or near the Uintah-Ouray Reservation in Uintah and Duchesne Counties of Eastern Utah.

The purpose of this study was to determine if the analytical results (when determining cadmium, lead, and zinc concentrations) for air sample filters prepared and analyzed according to N:OSH Method 7300, differ significantly, from those obtained when the instrument is calibrated using EPA Method 200.7 acid matrix standards. The null hypothesis was that no statistically significant difference, measured at the 95% confidence level, would exist between results obtained from the two calibration standards. Careful standard and sample preparation, the use of an internal standard, and the application of working standard and working internal standard correction factors served to isolate the …

This historical-descriptive study examined Southern Appalachian settlement schools as early initiators of integrated health and social services with education from the 1900s through the 1970s. Three schools were studied: Hindman Settlement School (KY), Pine Mountain Settlement School (KY) and Crossnore School, Inc. (NC). The purpose of the study was to determine the type and extent of services provided, the relationship of the settlement schools with their respective county public school system, and the transfer, if any, of integrated services from the settlement schools to public schools as the public schools took over educational responsibilities once offered by the settlement schools. …

The purpose of this study was to perform a qualitative analysis of an elementary school-based health clinic (SBHC). This situational analysis was performed to further understand the impact of accessible primary health care on an elementary school population. Data were collected through 30 participant interviews of parents, educators and medical personnel. Additional data were collected through the clinic files, and a t-test was used to compare the attendance and achievement data of clinic consumers for the clinic year against records from the previous year. A 50-year-old male was killed when he fell from the top of a 12-foot high oil tank. The victim was an oil gauger for a company that transports crude oil. His job was to obtain samples of the oil to check its quality. On the day of the incident he was to check the oil at a site located about 100 miles from his office.
