A 62-year-old male was killed when the tractor he was operating punctured an aboveground gas line. He had recently moved to the area and was in the process of acquiring some land for farming. The land was overgrown with weeds and brush about 5 feet high. On a warm, clear afternoon he went out to clear the land with a tractor and rotary mower. Many nurses are not familiar with the highly technical language of research and the types of graphics that researchers use to communicate findings, creating a major barrier to research utilization in nursing. In an effort to find alternative approaches to displaying research results, W.G. Cole introduced "metaphor graphics" a decade ago as a way of graphically representing knowledge. He proposed that data be summarized using visual metaphors - i.e., abstract signs and symbols - to show patterns and convey meaning. Research into the relationship between housing and health has frequently been narrowly focused, fragmented, and of marginal practical relevance to either housing or health studies. Population health research, in its reference to the importance of the social and physical environment, rarely mentions housing. While it has been recognized for some time that there is a need for the development of a co-ordinated, integrated and cumulative body of housing and health research, there is still very little formal co-ordination between housing policy and population health policy. Alertness-Supporting Activities For Control Room Operators In Automated Industrial Plants, Roger C. Jensen

A survey of control room operators in a nuclear power plant identified practical activities for maintaining alertness during the early morning hours. Nine on-duty operators rated 16 activities they were allowed to do on shift. Activities most effective for alertness were splashing cool water on the face, drinking coffee, and doing things that used muscles. The least effective activities were passive monitoring and studying for exams. The large range of ratings indicates that activity has a major role in operator alertness during the early morning hours.

Carboxy-Terminal Conversion Of Profibrillin To Fibrillin At A Basic Site By Pace/Furin-Like Activity Required For Incorporation In The Matrix, Michael Raghunath, Elizabeth A. Putnam, Timothy Ritty, Daniel Hamstra, Eun-Sook Park, Mathias Tschödrich-Rotter, Reiner Peters, Alnawaz Rehemtulla, Dianna M. Milewicz

Fibrillin-1, the main component of 10-12 nm microfibrils of the extracellular matrix, is synthesized as profibrillin and proteolytically processed to fibrillin. The putative cleavage site has been mapped to the carboxy-terminal domain of profibrillin-1, between amino acids arginine 2731 and serine 2732, by a spontaneous mutation in this recognition site that prevents profibrillin conversion. This site contains a basic amino acid recognition sequence (R-G-R-K-R-R) for proprotein convertases of the furin/PACE family. In this study, we use a mini-profibrillin protein to confirm the cleavage in the carboxy-terminal domain by both fibroblasts and recombinantly expressed furin/PACE, PACE4, PC1/3 and PC2. This report represents a compilation and new analysis of data on the effects and consequences of violent crime among American Indians. The report uses data from a wide variety of sources, including statistical series maintained by the Bureau of Justice Statistics (BJS), the FBI, and the Bureau of the Census. Data are reported from American Indian crime victims on how they were affected by the victimization and about who victimized them. The report also includes the first BJS estimates of the total number of American Indians under the custody or supervision of the justice system.

Endocytosis of the Flaviviridae viruses, hepatitis C virus, GB virus Cyhepatitis G virus, and bovine viral diarrheal virus (BVDV) was shown to be mediated by low density lipoprotein (LDL) receptors on cultured cells by several lines of evidence: by the demonstration that endocytosis of these virus correlated with LDL receptor activity, by complete inhibition of detectable endocytosis by anti-LDL receptor antibody, by inhibition with anti-apolipoprotein E and -apolipoprotein B antibodies, by chemical methods abrogating lipoproteinyLDL receptor interactions, and by inhibition with the endocytosis inhibitor phenylarsine oxide. In this issue of HEPATOLOGY, Mathurin et al. report the results of a 10-year retrospective study of kidney transplant recipients that compared rates of patient and graft survival among patients with and without chronic hepatitis B virus (HBV) or hepatitis C virus (HCV) infection.1 Following this cohort of patients at intervals of 5 and 10 years, the investigators found a statistically significant decrease in survival in the HBV-infected group at both 5 and 10 years and a similar decrease at 10 years in the HCV-infected group. Literature Review: Challenging Hiv-Related Stigma And Discrimination In Southeast Asia: Past Successes And Future Priorities, Joanna Busza

HIV and AIDS

Pervasive stigma has surrounded HIV/AIDS since the beginning of the pandemic. In Southeast Asia, as elsewhere, it has been accompanied by discrimination, affecting transmission patterns and access to care and support. This paper describes the regional experience of stigma and discrimination and offers a review of community-based interventions that have attempted to reduce them. The evidence presented comes primarily from unpublished literature and anecdotal evidence gained through interviews with project staff throughout the region.

According to a 1998 UNAIDS/WHO report, HIV infection among women is the fastest-growing segment of the AIDS pandemic, especially in developing countries. Unfortunately, the HIV-prevention strategies put forward by the public health community—monogamy, condom use, and treatment of sexually transmitted infections—are not feasible for many women. Women urgently need a means within their personal control to protect themselves against HIV. Recognizing this need, a number of international and nongovernmental organizations and government agencies have called for the development of microbicides, products that women could use vaginally to prevent infection with sexually transmitted infections, particularly HIV. As the world grapples with the HIV pandemic, the implementation of the agenda determined by the International Conference on Population and Development (1994) at Cairo, and the matter of providing health services of adequate quality in an ethical, gender-sensitive manner, new questions are arising about how to attend to reproductive tract infections, including sexually transmitted infections (STIs), among women. This paper reviews the results of validation studies of syndromic algorithms, other nonlaboratory clinically based tools, and risk scoring for finding women infected with chlamydia and gonorrhea, particularly among those attending family planning and antenatal clinics in developing countries. The Female Condom In Zimbabwe: The Interplay Of Research, Advocacy, And Government Action, Population Council

During the past decade, a significant increase in reported cases of HIV/AIDS in Zimbabwe prompted the government, local organizations, and international donors to intensify prevention efforts. As part of this response, in November 1996 Zimbabwe’s National AIDS Coordinating Programme invited Population Services International to launch a social marketing program to promote the female condom, making this protective device widely available for the first time in Africa. After an acceptability study showed that Zimbabwean men and women liked using the female condom, leaders of these organizations encouraged public discussion and media attention, which helped pave the way to approval and introduction …

In an effort to address the global crisis of HIV/AIDS and to reduce the spread of other sexually transmitted infections (STIs), maternal and child health/family planning (MCH/FP) programs have attempted to integrate the management of STIs into their services. This integration was endorsed at the 1994 International Conference on Population and Development in Cairo. However, as noted in this report, programs have encountered a number of difficulties as they try to effectively manage STIs in an MCH/FP setting. In particular, the effective detection and treatment of STIs has proven difficult among MCH/FP populations. This current study was developed to help …

In 1991, the Indian Health Service funded a nationwide initiative with the goal of determining the need for culturally appropriate mental health services among American Indian people residing in urban areas. In the author's opinion, the assessment method used in this initiative is a valuable, fresh approach that is a significant change from flawed attempts to make these assessments in the past.

Objective: Tractor rollovers are a major cause of farm injuries and fatalities. The authors used data from a statewide surveillance study to estimate the prevalence of safety features such as rollover protective structures, seat belts, and power take-off shields on farm tractors in Kentucky.

An increasing number of high schools in the United States have begun to use the "Baby Think It Over" doll as part of their curriculum in certain classes. This study examined the effectiveness of this doll on influencing 46 male and female adolescents' attitudes and behaviors regarding pregnancy, sexual activity, and contraceptives. Five specific subscales were developed to measure the teens' contraceptive attitudes and behaviors before and after using the doll regarding their feelings about becoming pregnant, understanding/comprehension of the responsibilities of child-rearing, appreciation of the impact of child-bearing on their lives/future, and attitudes toward premarital sex. Family conceptions of the nature of their relative's illness are part of the coping process and reveal the cultural construction of the illness experience. As part of a larger qualitative study conducted at the Schizophrenia Program of the Department of Psychiatry, Escola Paulista de Medicina - Unifesp, 14 relatives of 8 out-patients diagnosed with schizophrenia were interviewed and invited to talk freely about their ideas and feelings concerning their relative's problem. Poverty, Gender, and Youth

Despite ongoing declines in fertility in many countries, the population of the world is experiencing a period of rapid expansion, and its size is expected to exceed 10 billion by the end of the demographic transition. Three causes of this growth are identified and quantified: 1) fertility above the replacement level of two surviving children per woman, 2) continuing declines in mortality, and 3) population momentum resulting from a young age structure. A set of simple analytic expressions is proposed for estimating these factors from standard demographic indicators. Population momentum is shown to be the main cause of future growth …
