Objective: To assess whether perioperative management guided by near-infrared spectroscopy to determine tissue oxygen saturation and haemodynamic monitoring reduces postoperative complications after off-pump coronary artery bypass grafting.

Design: Assessor blinded, single centre, randomised controlled trial (Bottomline-CS trial).

Setting: A tertiary teaching hospital in China.

Participants: 1960 patients aged 60 years or older who were scheduled for elective off-pump coronary artery bypass grafting.

Interventions: All patients had multisite monitoring of tissue oxygen saturation (bilateral forehead and unilateral forearm brachioradialis) and haemodynamic monitoring. If a patient is rebellious or undisciplined in following his treatment regimen, the consequences can include decreased production, increased morbidity, and even death. Objectives

Infectious diseases are often associated with decline in quality of life. The aim of this study is to analyze the relationship between personal history of communicable, i.e., infectious and parasitic diseases and self-rated health.

Study design

Secondary analysis of a large dataset multi-country observational study.

Methods

We used a four-pronged analysis approach to investigate whether personal history of infectious and parasitic diseases is related to self-reported health, measured with a single item.

Results

Three of the four analyses found a small positive effect on self-reported health among those reporting a history of pathogen exposure. The meta-analysis found no support …

This study explores how behavioral health clinicians perceive Value-Based Healthcare (VBHC), a model designed by Porter and Teisberg (2006) to improve outcomes relative to costs. While widely promoted in healthcare reform, VBHC poses unique challenges when applied to behavioral health settings. Using an explanatory mixed-methods design, this study first assessed clinicians’ awareness of VBHC through a survey of 23 licensed clinicians at a Community Mental Health Center (CMHC) in Colorado. Quantitative findings revealed that one-third of participants were aware of VBHC with awareness differing by role prompting further exploration in a qualitative phase. BACKGROUND: Previous work has shown a role of CCL2, a key chemokine governing monocyte trafficking, in atherosclerosis. However, it remains unknown whether targeting CCR2, the cognate receptor of CCL2, provides protection against human atherosclerotic cardiovascular disease.

METHODS: Computationally predicted damaging or loss-of-function (REVEL > 0.5) variants within CCR2 were detected in whole-exome-sequencing data from 454,775 UK Biobank participants and tested for association with cardiovascular endpoints in gene-burden tests. Given the key role of CCR2 in monocyte mobilization, variants associated with lower monocyte count were prioritized for experimental validation. Background: Anxiety disorders are among the most pervasive mental health challenges worldwide, affecting individuals across all demographics and socioeconomic backgrounds. As a leading contributor to disability and diminished quality of life, anxiety continues to rise in prevalence, exacerbated by global crises and societal stressors. This study explored how Zero Balancing (ZB) supports individuals with anxiety. Large-scale events such as the College World Series (CWS) pose unique challenges for medical services due to high crowd density, prolonged event durations, and variable environmental conditions. This study aimed to assess the medical usage rate (MUR) and injury prevalence at the 2024 CWS to inform future event planning and emergency medical preparedness. A retrospective analysis was conducted using patient encounter data collected between June 14 and June 24, 2024. Background: This article examines the problematic phenomenon of faculty assigning graduate health science students systematic reviews as semester-based assignments while expecting a health science librarian to be a willing support system for those students. Despite published conduct and reporting guidelines establishing that systematic reviews require a team, time, and methodological expertise, some faculty still turn to full systematic review assignments in the classroom. The authors propose applying cognitive load theory and chunking the systematic review process into manageable steps, allowing both faculty and students to better understand the required methodologies and to enhance educational outcomes.

Hypertension is a significant risk factor for cardiovascular and kidney diseases but is often under-diagnosed and poorly managed. Electronic Health Records (EHR) can help by providing automated reminders to recheck elevated blood pressure (BP) readings, ensuring accurate measurements, and guiding clinical interventions. This study aims to determine if EHR-based reminders improve BP control in adult patients aged 21 and older, focusing on hypertension diagnosis accuracy and timely management.

Loneliness damages health and quality of life. The COVID-19 pandemic increased social isolation and loneliness among some groups in the U.S. This data slice uses data from the 2020 Health and Retirement Study to examine gender and age differences in loneliness among older adults (50+) in the U.S. during the first year of the pandemic. The author finds that loneliness was more prevalent among women than among men, with the highest rate among women ages 80 and older.

Clostridioides difficile is a gram-positive spore-forming pathogen that commonly causes diarrheal infections in the developed world. Although C. difficile is a genetically diverse species, certain ribotypes are overrepresented in human infections, and it is unclear if metabolic adaptations are essential for the emergence of these epidemic ribotypes. To identify ribotype-specific metabolic differences, we therefore tested carbon substrate utilization by 88 C. difficile isolates and looked for differences in growth between 22 ribotypes. As expected, C. difficile was capable of growing on a variety of carbon substrates. In addition to embryo quality and endometrial function, proper oviduct function is also essential for successful pregnancy establishment. Importance: The effectiveness of different approaches to dementia care is unknown.

Objective: To determine the effectiveness of health system-based, community-based dementia care, and usual care for persons with dementia and for caregiver outcomes.

Design, setting, and participants: Randomized clinical trial of community-dwelling persons living with dementia and their caregivers conducted at 4 sites in the US (enrollment June 2019-January 2023; final follow-up, August 2023).

Perinatal depression (PND) and perinatal anxiety (PNA) are increasing among pregnant and parenting adolescents (PPAs), with PNA often underrecognized. This leads to significant socioeconomic costs and adverse maternal consequences if untreated. Hence, this dissertation explores the experiences of PND and PNA among PPAs, focusing on their knowledge gaps, perception, mental health needs, care preferences, and access to PMH care. The research employs a qualitative design (free-listing and in-depth interviews) to gather participant insights. Key findings reveal that many PPAs possess limited knowledge about PNA when compared to PND. Lack of support from partners, family, and peers emerged as the topmost …
