Pandemi coronavirus disease 2019 (COVID-19) tidak hanya berdampak pada ekonomi dan kesehatan, tetapi juga aspek pendidikan termasuk praktek klinik mahasiswa profesi kesehatan di rumah sakit. Berbagai perubahan dan penyesuaian jumlah mahasiswa yang melaksanakan praktik klinik, peralatan praktik dan juga metode bimbingan dapat berpengaruh terhadap kepuasan mahasiswa. Prevalensi global stroke dan sindrom metabolik, sebagai kumpulan faktor risiko utama yang mempengaruhi stroke terus mengalami peningkatan. Penelitian ini bertujuan melihat hubungan antara sindrom metabolik dan kejadian stroke di Indonesia. Desain studi ini adalah cross-sectional dan menggunakan data survei skala nasional. Responden berusia >15 tahun yang mengalami stroke dengan data lengkap terkait pengukuran lingkar perut, tekanan darah, kadar glukosa darah, trigliserida, dan kadar high-density lipoprotein akan masuk dalam studi ini. Model regresi logistik digunakan dalam analisis kami. Dari 24.451 responden, studi kami menunjukkan mayoritas berusia <55 tahun (73,9%), perempuan (62,2%), dengan proporsi penderita stroke serta proporsi sindrom metabolik adalah 1,2%, dan 24,4%. Studi ini memperlihatkan adanya hubungan antara sindrom metabolik dan kejadian stroke (adj POR: 2,4; 95% CI: 1,9-3,1). Analisis Survival: Hubungan Konversi Sputum Dengan Keberhasilan Pengobatan Pasien Tuberkulosis Resistan Obat Di Indonesia, Shena Masyita Deviernur, Nurhayati Adnan

Angka keberhasilan pengobatan Tuberkulosis Resistan Obat (TB RO) di Indonesia masih rendah. Konversi sputum dapat menjadi monitoring awal untuk memantau keberhasilan pengobatan. Penelitian ini bertujuan untuk melihat hubungan antara konversi sputum <3 bulan dengan keberhasilan pengobatan. Penelitian ini menggunakan desain studi kohort retrospektif dengan data yang bersumber dari sistem informasi tuberkulosis Kementerian Kesehatan RI. Populasi pada penelitian ini adalah pasien TB RO yang memulai pengobatan tahun 2020-2021 di Indonesia dengan jumlah sampel sebanyak 8.025 kasus dan participation rate sebesar 88,6%. Kurva Kaplan Meier dengan uji log-rank dibuat untuk analisis deskriptif, sedangkan analisis regresi Cox digunakan untuk melihat hubungan antara konversi sputum dengan keberhasilan pengobatan. Hasil analisis menunjukkan bahwa 45,9% pasien konversi sputum < 3 bulan pengobatan dengan 74,6% memiliki keberhasilan pengobatan. Secara keseluruhan median time untuk keberhasilan pengobatan berdasarkan konversi sputum < 3 bulan adalah bulan ke-18. Hasil analisis multivariat menunjukkan konversi sputum < 3 bulan 1,25 (95% CI 1,18-1,34) lebih tinggi peluangnya untuk mencapai keberhasilan pengobatan dibandingkan pasien yang tidak konversi < 3 bulan setelah dikontrol oleh status HIV dan paduan pengobatan. Berdasarkan hasil penelitian ini dapat disimpulkan bahwa monitoring konversi sputum dapat menjadi cara untuk melakukan monitoring keberhasilan pengobatan sedini mungkin. Konversi sputum merupakan standar yang dilakukan untuk pemantauan pengobatan pasien TB RO. Namun jumlah laboratorium yang dapat melakukan pemeriksaan untuk konversi pada pasien TB RO masih terbatas. Perlu adanya perluasan layanan dan peningkatan mutu layanan untuk melakukan pemeriksaan konversi sputum pada pasien TB RO.

This qualitative interpretive study aimed to describe the experiences of 13 American Indians living in urban settings and their use of traditional healing and biomedical health services for type 2 diabetes. Urban American Indian adults living in the United States who used traditional healing and biomedical health services for type 2 diabetes were recruited for the study through purposive sampling, including snowball sampling. Thirteen participants completed semi-structured interviews. Participants reported positive experiences and barriers to traditional healing and Western biomedical services. We hypothesize distress in AA males during treatment is higher than in other races and sexes. INTRODUCTION: Middle meningeal artery embolization (MMAE) has emerged as a promising new treatment for patients with chronic subdural hematomas (cSDH). Its efficacy, however, upon the subtype with a high rate of recurrence-septated cSDH-remains undetermined.

METHODS: From our prospective registry of patients with cSDH treated with MMAE, we classified patients based on the presence or absence of septations. The primary outcome was the rate of recurrence of cSDH. Secondary outcomes included a reduction in cSDH thickness, midline shift, and rate of reoperation.

BACKGROUND: The pathophysiological causes of kidney disease are not fully understood. Here we show that the integration of genome-wide genetic, transcriptomic, and proteomic association studies can nominate causal determinants of kidney function and damage.

RESULTS: Through transcriptome-wide association studies (TWAS) in kidney cortex, kidney tubule, liver, and whole blood and proteome-wide association studies (PWAS) in plasma, we assess for effects of 12,893 genes and 1342 proteins on kidney filtration (glomerular filtration rate (GFR) estimated by creatinine; GFR estimated by cystatin C; and blood urea nitrogen) and kidney damage (albuminuria). Our objective was to examine serum ferritin trends after conversion to permanent vascular access (PVA) among children who started hemodialysis (HD) using tunneled cuffed catheters (TCC). Retrospective chart reviews were completed on 98 subjects from 20 pediatric HD centers. Serum ferritin levels were collected at the creation of PVA and for two years thereafter. There were 11 (11%) arteriovenous grafts (AVG) and 87 (89%) arteriovenous fistulae (AVF). Their mean TCC use was 10.4 ± 17.3 months. Serum ferritin at PVA creation was elevated at 562.64 ± 492.34 ng/mL, increased to 753.84 ± 561.54 ng/mL (p = < 0.001) in the first year and remained at 759.60 ± 528.11 ng/mL in the second year (p = 0.004). …

Due to their high-risk status, individuals with chronic diseases may be at increased risk and disproportionately adversely affected by the COVID-19 pandemic. The investigators examined associations between having a high-risk chronic condition, social connectedness, and general distress and COVID-19-specific distress among U.S. adults during the COVID-19 lockdown. Background: Public safety telecommunications officers (PSTCOs), aka emergency “dispatchers,” are exposed to workplace traumatic stress and can experience situations characterized by uncertainty, communication difficulties, and a lack of resources. Importance SARS-CoV-2 infection can result in ongoing, relapsing, or new symptoms or other health effects after the acute phase of infection; termed post-acute sequelae of SARS-CoV-2 infection (PASC), or long COVID. The characteristics, prevalence, trajectory and mechanisms of PASC are ill-defined. Parasomnias, such as sleepwalking and nightmare disorder, are abnormal behaviors and emotions emanating from sleep or wake to sleep transitions. Parasomnias can lead to significant adverse outcomes – sleepwalking can result in injuries and hospitalizations, frequent nightmares are associated with self-harm and suicidal thoughts, and rapid-eye movement (REM) sleep behavior disorder (RBD) is linked to neurodegeneration. Despite a history of fascination with abnormal sleep behaviors in the general and medical communities, epidemiologic understanding of parasomnias remains limited, in part due to their amnestic nature and presumption that they are benign childhood problems. IMPORTANCE: SARS-CoV-2 infection can result in ongoing, relapsing, or new symptoms or other health effects after the acute phase of infection; termed post-acute sequelae of SARS-CoV-2 infection (PASC), or long COVID. The characteristics, prevalence, trajectory and mechanisms of PASC are ill-defined. The objectives of the Researching COVID to Enhance Recovery (RECOVER) Multi-site Observational Study of PASC in Adults (RECOVER-Adult) are to: (1) characterize PASC prevalence; (2) characterize the symptoms, organ dysfunction, natural history, and distinct phenotypes of PASC; (3) identify demographic, social and clinical risk factors for PASC onset and recovery; and (4) define the biological mechanisms underlying PASC pathogenesis. …

Childhood obesity is one of the most serious public health challenges of the 21st century. The effects of childhood overweight and obesity are devastating to the health and well-being of children now and throughout adulthood. Introduction Lebanon has battled the COVID-19 pandemic in the midst of an economic crisis. Thermal equilibrium is primarily maintained by the body’s ability to dissipate internal heat generated during muscle contraction. A number of methods have been used to quantify this thermal burden such as charts and observation of workplace activities and behavior, however these have varying levels of validity. Evidence shows that older patients with advance directives such as a living will, or durable power of attorney for healthcare, are more likely to receive care consistent with their preferences at the end of life. Less is known about the use of advance directives between veteran and non-veteran older Americans.
