Role Of Thymectomy In Myasthenia Gravis, Sohaila Mohsin Ali, Aziz Sonawalla, Irfan Altafullah, Hizbullah Sheikh

Thirteen patients with myasthenia gravis underwent total thymectomy between January, 1988 and December, 1991. The duration of symptoms prior to surgery varied from 2 months to 20 years. In a follow-up ranging from 2 months to 4 years, 11 patients showed a significant improvement with either complete discontinuation of medication of a marked reduction in doses. One patient with a small benign thymoma showed some improvement but subsequently required stepping up of anticholinesterase medication and addition of steroids and immune suppressants; another patient with atrophic thymic tissue had complete remission after thymectomy but developed myasthenic symptoms six months later requiring …

Sertoli Cell Only Syndrome (Secos): Lessons From Case Studies, Jamsheer J Talati, Hizbullah Sheikh

Between June 85 and December 87, 69 testicular biopsies were submitted for histopathological examination during investigation of infertility; ten (14%) patients had a Sertoli cell only syndrome. The history, clinical features, and hormonal profiles were analyzed in an attempt to categorize these patients on aetiological basis. Two followed treatment of malignancy--one by radiation for testicular cancer and one by cyclophosphamide for a lymphoma. One had unilateral cryptorchidism. Mumps was etiological factor in one patient. FSH levels determined in 6 patients were elevated in all suggesting a possible dependence of (sick) Sertoli cells on spermatogenic cells for production of inhibin. Alternative …

Arginine vasopressin (AVP) and atrial natriuretic peptide (ANP) have important influences on water and electrolyte metabolism, and studies on the interactions between these hormones may have important implications. We have investigated the effects of sodium intake, furosemide, and infusion of ANP on the urinary and metabolic (nonurinary) clearances of AVP in hydrated normal subjects. On a high sodium diet there was an increase in urine volume, sodium excretion, osmolal clearance, plasma ANP concentration, and urinary clearance and fractional excretion of AVP, with a decrease in PRA. After circumcision the first operations done on man were for bladder stone. Done by charlatans the operation often resulted in loss of life and caused the early lithotomists to maintain a peri­patetic existence, vanishing from the town the night after surgery was performed. About two centuries ago stones became a common disease amongst kings: surgeons took over from the charlatans and commenced blind skilful mechanical lithotripsy where a curved instrument was introduced per urethram, its jaws opened to engage the stone which was then crushed by a mechanical (screw driven) closure of the jaws. It has been assumed until recently that chronic renal failure is more prevalent among men than among women, but data now indicate that at least half of all renal patients are women. The literature continues to focus on adjustment problems of male patients, especially sexual adjustment and job-loss problems, and to assume that women can adjust more easily because of their ability to maintain the homemaker role. However, women patients whose work status is that of homemaker are found to have the highest depression scores, and job loss results in low satisfaction for those who have held meaningful outside jobs. …

This study was concerned with the problem of depression among elderly patients following surgery. The purpose of the study was to find out if older men following transurethral prostatectomy experienced a higher level of depression than normal elderly people in the general population. An attempt was made in the study to suggest factors in the patients' illness or environment which may have influenced the amount of depression they experienced.

Thirty patients, sixty-five years or older, were included in the study sample. Depression was measured by a self-rating depression scale (Zung) three weeks following their operations. Other facts about the patients …

This exploratory retrospective study was designed to evaluate selected factors, including the ability to regain bladder function related to post indwelling urethral catheterization in an acute general hospital population.

The records of 107 patients who met the study criteria became the study population. These patients had three-way urethral catheters, continuous antibacterial irrigation and closed drainage system. Twenty patients died (19 percent) before the catheter was removed. Of the 87 patients who were alive, 37 percent (32 patients) had in-adequate bladder function. Of these 32 patients, no attempt was made to remove the catheter from 15 prior to their hospital discharge. …

The problem with which this study was concerned was the development of criteria that could be used to determine when urinary catheter changes were necessary. By determining when encrustation occurred, it was felt that some complications could be eliminated by changing catheters before obstruction occurred due to encrustation.

It was the purpose of this study to: 1) explore some indications for catheter change, 2) determine if encrustation of urinary catheters necessitates catheter change, and 3) estimate potential criteria for a routine time for catheter changes.

An experimental study was conducted in a university and a convalescent hospital using eight female CVA patients with urinary incontinence to test the hypothesis that the experimental technique of icing and brushing LI dermatome skin area would increase the probability that the CVA patient would void within a prescribed time and place. A modified one group method was used in which the patient served as her own control by participating in two types of bladder training programs over an eight to twelve hour period for four consecutive days. The purpose of this research was to find out what effects the TURP has on ejaculatory ability, frequency of coitus and the emission of seminal fluid. The sample consisted of 50 men, age 46-65, who were married, and had no cancer or other possible physical causes for impotence. Each man was personally interviewed.

Forty-six patients (92 percent) were able to ejaculate. A decrease in the frequency of coitus was reported by 40 per cent of the men. A normal ejaculation of seminal fluid was experienced by thirteen (26 percent) of the men. Considering all the changes in the sexual function …

The purposes of this descriptive survey were to (1) find out if the incidence of bladder infection could be reduced by decreasing the number of postoperative catheterizations, (2) find out what the incidence of current methods of postoperative bladder evacuation were, (3) find out if and what changes had occurred in the incidence of the methods of bladder evacuation since the publication of previous studies, and (4) discover the current relationships of the methods of bladder evacuation to various factors such as age, sex, marital status, admission status, type of operation, and type and length of anesthesia.
