Pain is the main symptom in cancer patients that needs to be addressed immediately because it can reduce the quality of life. Concern or the ineffectiveness of using one type of opioid makes it necessary for clinicians to understand how to rotate opioids. Therefore, this evidence-based case report was conducted to identify the effectiveness of opioid rotation and combined opioids for the cancer pain management through a literature search on three databases. Literature searching was performed by using Pubmed, Cochrane library, and EBSCO according to clinical inquiries. Of the 24 studies obtained, only one study met the criteria and was …

Jurnal Penyakit Dalam Indonesia
Urinary tract infection (UTI) is an infection caused by the entry of pathogens into the urinary tract. Symptoms of a UTI are not always obvious, and some are asymptomatic or atypical. Urinary tract infections must be distinguished from other diseases that have a similar clinical appearance. Many UTIs resolve spontaneously, but often cause significant morbidity and mortality. With the unpredictable nature of the disease, laboratory tests are required for accurate diagnosis and identification of the cause of infection. Thus, it is necessary to understand the interpretation and limitations of urinalysis and urine culture results. To assist in the diagnosis of …

Background: Heart failure with preserved ejection fraction continues to pose multiple challenges in terms of accurate diagnosis, treatment, and associated morbidity. Accurate left ventricular (LV) mass calculation yields essential prognostic information relating to structural heart disease. Two-dimensional (2D) echocardiography-based calculations are solely limited to LV geometric assumptions of symmetry, whereas three-dimensional (3D) echocardiography could overcome these limitations. This study aims to compare the performance of 2D and 3D LV mass calculations. Methods: A prospective review of echocardiography findings at the University of Louisville, Kentucky, was conducted and assessed. Normal ejection fraction (EF) was defined as >=52% in males and >=54% …

Micropeptides, encoded from small open reading frames of 300 nucleotides or less, are hidden throughout mammalian genomes, though few functional studies of micropeptides in the brain are published. Here, we describe a micropeptide known as the Plasticity–Associated Neural Transcript Short (Pants), located in the 22q11.2 region of the human genome, the microdeletion of which conveys a high risk for schizophrenia. Our data show that Pants is upregulated in early adulthood in the mossy fiber circuit of the hippocampus, where it exerts a powerful negative effect on long-term potentiation (LTP). Further, we find that Pants is secreted from neurons, where it …

Lawyers As Caregivers, Paula Schaefer
St. Mary's Journal on Legal Malpractice & Ethics
This Article argues that clients—much like patients in a healthcare setting—need their lawyers to be caregivers. The Article opens by developing a definition of caregiving in medicine and law. It then turns to five key components of caregiving in medicine, explaining the substantial research that this care is crucial for patient satisfaction, trust, and healing. Medical educators have drawn on this research to better prepare medical professionals to be excellent caregivers. The Article then explores the evidence that an attorney’s clients have the same needs and suffer similar harm when attorneys fail to meet these needs. Next, the Article turns …

Patients, Corporate Attorneys, And Moral Obligations, Ioan-Radu Motoarcă
St. Mary's Journal on Legal Malpractice & Ethics
There are two main questions that any account of corporate lawyers’ moral obligations needs to answer: (1) Do corporate lawyers have moral obligations to third parties? and (2) In cases of conflict between obligations to the corporation and obligations to third parties, which should prevail? This Article offers answers to these questions in the context of lawyers working in medical corporations. I argue that lawyers do have moral obligations to third parties, and that in cases where patients’ rights are being violated by a medical company, patients’ rights should prevail. Consequently, attorney–client confidentiality rules should be relaxed to allow for …
