Ketamine Use For Prehospital Status Seizures That Are Refractory To Benzodiazepines, Kyle B. Safko, Justin Gambini

Doctor of Medical Science (DMSc)

In North America, epilepsy affects between 16 and 51 people 100,000, with the most affected being those with lower socioeconomic status1. Most ambulance services carry benzodiazepines, but what happens if administration of these drugs do not terminate seizure activity? The American Epilepsy Society concluded that 50% of patients do not respond to benzodiazepines if they have been stable and compliant on typical antiepileptic drugs and they experience a seizure7. A literature review was done looking at alternative treatment options for status seizures, specifically ketamine administration. Multiple case reviews, literature reviews and retrospective studies demonstrate the effectiveness …

Zero Balancing With The Dizzy Client, Mary L. Behrens, P.T., Czb Aug 2022

Zero Balancing With The Dizzy Client, Mary L. Behrens, P.T., Czb

Journal of Transformative Touch

“Dizziness” is a colloquial term for the general experience individuals associate with a variety of problems with the vestibular system. Because “dizziness” is a general description of a symptom that may have a range of causes, seeking care from various medical specialists, the root problem is often missed, potentially leaving clients suffering for years. Over the last 10 years of clinical practice, I have found Zero Balancing to be an effective treatment for many clients referred for vestibular disorders.

The goal of this paper is to shed light on the reasons for my clinical decision making and understand the patterns …

Characterization And Comparison Of Human Glioblastoma Models, Julia A. Schulz, Louis T. Rogers, Richard J. Kryscio, Anika M. S. Hartz, Björn Bauer

Sanders-Brown Center on Aging Faculty Publications

Abstract
Glioblastoma (GBM) is one of the deadliest cancers. Treatment options are limited, and median patient survival is only several months. Translation of new therapies is hindered by a lack of GBM models that fully recapitulate disease heterogeneity. Here, we characterize two human GBM models (U87-luc2, U251-RedFLuc). In vitro, both cell lines express similar levels of luciferase and show comparable sensitivity to temozolomide and lapatinib exposure. In vivo, however, the two GBM models recapitulate diferent aspects of the disease. U87-luc2 cells quickly grow into large, well-demarcated tumors; U251-RedFLuc cells form small, highly invasive tumors. Using a new method to assess …

Publications and Research

Breast cancer (BC) is the most common cancer in Korean American (KA) women. In view of its high prevalence in these women, their low screening rates, and the cultural influence of BC risk factors in their lifestyles, we developed a community-based culturally tailored BC prevention program, the Korean Breast Cancer Risk Reduction Program (KBCRRP). Guided by the PRECEDE-PROCEDE and health belief models, the KBCRRP was developed to achieve four goals: (1) healthy weight, (2) physically active lifestyle, (3) healthy diet, and (4) BC screening and adherence. KBCRRP combines effective multicomponent strategies for BC screening and a group-based lifestyle intervention incorporating …

Active Consideration Of Health And Nutritional Quality Of Food Choices, Christopher R. Gustafson Aug 2022

Active Consideration Of Health And Nutritional Quality Of Food Choices, Christopher R. Gustafson

Cornhusker Economics

Everyone can be positively influenced by effective interventions promoting healthier food choices. A handful of recent papers have documented positive impacts on dietary quality resulting from interventions that remind people to consider health through prompt messages or subtle priming in supermarkets and in controlled, online experiments. These studies have focused on the impact of reminders on nutritional quality rather than explaining how reminders work. An exception, Arslain, Gustafson, and Rose (2021), collected data on multiple elements of individuals' choice processes to trace the impact of reminders on decisions, showing that reminder messages led people to consider a healthier set of …

Department of Medical Oncology Faculty Papers

Abiraterone acetate (AA) and enzalutamide (ENZ) are commonly used for metastatic prostate cancer. It is unclear how their outcomes and toxicities vary with patient-specific factors because clinical trials typically exclude patients with significant comorbidities. This study aims to fill this knowledge gap and facilitate informed treatment decision making. A registered protocol utilizing PRISMA scoping review methodology was utilized to identify real-world studies. Of 433 non-duplicated publications, 23 were selected by three independent reviewers. ENZ offered a faster and more frequent biochemical response (30-50% vs. 70-75%), slowed progression (HR 0.66; 95% CI 0.50-0.88), and improved overall survival versus AA. ENZ was …

Melanoma-derived brain metastases (MBM) represent an unmet clinical need because central nervous system progression is frequently an end stage of the disease. Immune checkpoint inhibitors (ICI) provide a clinical opportunity against MBM; however, the MBM tumor microenvironment (TME) has not been fully elucidated in the context of ICI. To dissect unique elements of the MBM TME and correlates of MBM response to ICI, we collected 32 fresh MBM and performed single-cell RNA sequencing of the MBM TME and T-cell receptor clonotyping on T cells from MBM and matched blood and extracranial lesions. We observed myeloid phenotypic heterogeneity in the MBM …

Faculty, Staff and Student Publications

The median-effect equation has been widely used to describe the dose-response relationship and identify compounds that activate or inhibit specific disease targets in contemporary drug discovery. However, the experimental data often contain extreme responses, which may significantly impair the estimation accuracy and impede valid quantitative assessment in the standard estimation procedure. To improve the quantitative estimation of the dose-response relationship, we introduce a novel approach based on robust beta regression. Substantive simulation studies under various scenarios demonstrate solid evidence that the proposed approach consistently provides robust estimation for the median-effect equation, particularly when there are extreme outcome observations. Moreover, simulation …

Dual Leucine Zipper Kinase Regulates Dscam Expression Through A Noncanonical Function Of The Cytoplasmic Poly(A)-Binding Protein, Monika Singh, Bing Ye, Jung Hwan Kim

Faculty, Staff and Student Publications

Dual leucine zipper kinase (DLK) plays a pivotal role in the development, degeneration, and regeneration of neurons. DLK can regulate gene expression post-transcriptionally, but the underlying mechanism remains poorly understood. The Drosophila DLK, Wallenda (Wnd), regulates the expression of Down syndrome cell adhesion molecule (Dscam) to control presynaptic arbor growth. This regulation is mediated by the 3′ untranslated region (3′UTR) of Dscam mRNA, which suggests that RNA binding proteins (RBPs) mediate DLK function. We performed a genome-wide cell-based RNAi screen of RBPs and identified the cytoplasmic poly(A)-binding protein, pAbp, as an RBP that mediates Wnd-induced increase in Dscam expression. Genetic …

Gender Differences In Parental Leave Before And After The Introduction Of A Paid Parental Leave Policy: A Sequence Analysis Of Administrative Time-Keeping Records, Julia M. Goodman, Lisset Dumet Poma, David Hurtado, Anna Steeves-Reece, Dawn M. Richardson

OHSU-PSU School of Public Health Faculty Publications and Presentations

Paid leave confers health benefits to new parents and their children, but the absence of a national paid family leave policy in the United States has left workers to navigate a patchwork of paid and unpaid parental leave benefits accessed through their employers. As public and private paid leave policies expand across the US, it is imperative to determine how these benefits impact leave taking behaviors among new parents. We use sequence and cluster analyses of administrative time-keeping records to detail parental leave-taking during the first 180 days after adding a child among employees of a large public-sector organization with …

2020-Current year OA Pubs

Children, adolescents, and adults with autism spectrum disorder and intellectual disability experience high rates of co-occurring psychiatric conditions throughout their lifetime. However, there is a shortage of psychiatrists to treat these populations. We evaluated how much education psychiatrists-in-training receive on how to care for individuals with autism spectrum disorder/intellectual disability. We found that in many psychiatry programs, residents receive limited training experiences in autism spectrum disorder/intellectual disability involving lectures and patient contact and that psychiatry program directors would benefit from more resources to strengthen education in autism spectrum disorder/intellectual disability.

Training The Next Generation Of Practitioners In Early Intervention And Telepractice: Three University Models, K. Todd Houston, Lauri Nelson, Kristina Blaiser

Journal of Early Hearing Detection and Intervention

The COVID-19 pandemic continues to shape the provision of family-centered early intervention services for children who are Deaf or Hard-of-Hearing and their families. In programs, schools, and centers, direct in-person contact with families have been significantly curtailed as a means to limit the exposure to and spead of the virus. Emergency remote learning has lead to an increase in telepractice, also referred to as teleintervention, as the designated model of service provision. Most early interventionists, speech-language pathologists, and teachers of the Deaf were not sufficiently trained to suddenly implement emergency remote teaching or telepractice services, but service providers had no …

Telepractice-Based Assessment Of Children Who Are Deaf/Hard-Of-Hearing: Focus On Family-Centered Practice, Kristina M. Blaiser, Lauri Nelson, K. Todd Houston

Journal of Early Hearing Detection and Intervention

Ongoing assessment and progress monitoring is considered best practice to serve children who are Deaf/Hard-of-Hearing (DHH) yet logistics related to provider shortages, distances between families, and illness make regular assessment difficult if not impossible. In the last ten years, telepractice has become a more commonly used service delivery model for serving children who are DHH and their families, however, many providers lack the training needed to adequately assess this population (Behl & Kahn, 2015). With explicit planning of the assessments and tools needed on both sides of the camera, providers can create a shared framework to collect the information needed …

The genus Glycyrrhiza, comprising approximately 36 spp., possesses complex structural diversity and is documented to possess a wide spectrum of biological activities. Understanding and finding the mechanisms of efficacy or safety for a plant-based therapy is very challenging, yet it is crucial and necessary to understand the polypharmacology of traditional medicines. Licorice extract was shown to modulate the xenobiotic receptors, which might manifest as a potential route for natural product-induced drug interactions. However, different mechanisms could be involved in this phenomenon. Since the induced herb-drug interaction of licorice supplements via Pregnane X receptor (PXR) is understudied, we ventured out to …
