The primary motivating factor for seeking orthodontic treatment in adults is facial and dental esthetics, especially the alignment of their anterior teeth. The aims of the study were to compare the degree of demineralization and the size alteration of white spot lesions after orthodontic treatment using fluoride dentifrice or tooth mousse therapy. Twelve upper incisors with demineralized white spot lesions of eight participants were identified after fixed appliances were removed. These participants were randomly and double blindly assigned to use one of the two dentifrices (TM and FD). Objective: To retrospectively study the dentition of southern Taiwan children with unilateral complete cleft lip and alveolus for the prevalence of hypodontia. Patients: 680 Taiwan children with unilateral complete cleft lip and alveolus aged between 10 and 12 years from July, 1983 to June, 2008 in Department of Orthodontics, Kaohsiung Chang Gung Memorial Hospital. Purpose: To investigate the relation between the change of mandibular plane and maxillary molar intrusion with mini-implants. Materials and Methods: Twenty-eight consecutive adult patients with skeletal Class II, retrognathic mandible and high mandibular plane were included (mean age: years). They were treated with 4 premolar extractions and mini-implants for maxillary molar intrusion. The surgery-first approach in surgical orthodontic treatment is re-emerging as the preferred technique when compared to the traditional orthodontic first-approach. Despite its growing popularity, no planning protocol that offers comprehensive and accurate predictions has been described for the surgery-first approach. An adult female patient aged 34 years and 8 months with Wilson’s disease, a rare autosomal recessive genetic disorder, demonstrated poor chewing ability and facial asymmetry which were worsened gradually since the disease attacked her 10 years ago. She was treated by surgical orthodontics with minimal presurgical orthodontic decompensation. We designed an atypical maxillary anterior subapical osteotomy to close the maxillary incisor pontic space, retract anterior segment of the maxilla, and maintain the dental midline simultaneously. Class III malocclusion with mandibular protrusion and lateral displacement are commonly observed at orthodontic clinic. If patients don’t accept orthognathic surgery and belong to mild to moderate malocclusion, non-surgery treatment will be available. This was a 25-year-old female; her chief complaint was anterior crossbite with anterior spacing and mandibular protrusion. The diagnosis was dental and skeletal Class III malocclusion with mandibular prognathism. A 26-year-old female presented with Möbius syndrome. She came with chief complaint of excessive exposure her front teeth. She had Class III malocclusion with anterior cross bite. However she had a hyperdivergent face and backward rotation of the mandible without effective chin. After evaluation, she underwent orthognathic surgery and orthodontic treatment. The surgical techniques for correction her skeletal problems included maxillary Le Fort I osteotomy impaction and setback; mandibular bilateral sagittal split osteotomies setback; advancement and reduction genioplasty. The two-year follow up after treatment indicated that the surgical outcome and occlusion were stable.

The double-jaw surgery is frequently desirable to correct skeletal discrepancies and obtain better esthetic results for patients with severe skeletal Class III deformities. Instead of standard surgical principles, an alternative rotation of the maxillo-mandibular complex (MMC) is frequently required for patients with extreme degrees of skeletal discrepancies to achieve optimal facial esthetics. Using intraoral molar distalization appliance is one of the treatments of patients with crowding dentition besides extraction for crowding relief and achievement of normal occlusion. This 16-year-old female presents with Class II Division 1 subdivision right malocclusion, complicated by gummy smile, large mandibular plane angle and the mandibular right first molar missing. Treatment consisted of 4 temporary anchorage devices (TADs) application and extraction of bilateral maxillary first premolars and the mandibular left first premolar. Bimaxillary anterior and posterior teeth were intruded by miniscrews for counterclockwise rotation of the mandible. A very favorable result was achieved due to reduction of gingival display and the mandibular plane angle was decreased successfully.

The purpose of this research is to compare the arch changes after non-extraction orthodontic treatment on the mandibular crowding dentition between self-ligating and conventional bracket systems. Seventy-one samples that satisfied the criteria were selected from the patient database, including forty samples in the conventional bracket group and thirty-one in the self-ligating bracket group. This case report presents a 39 years old female adult patient which diagnosis was skeletal Class II and dental Class III malocclusion with severe bimaxillary protrusion, chin deficiency and two maxillary supernumerary teeth over premolars area. This is a case report about an adult female patient with bilateral idiopathic condylar resorption. Extraoral check revealed obvious convex profile with retruded chin, lip incompetence; intraoral check revealed class I malocclusion with moderate crowding. Owing to the bilateral condylar resorption, the condyle position may not be easily hold during operation; besides, trying to maintain the integrity of whole mandible to avoid post treatment relapse. Conventional surgical treatment planning uses dental casts and two-dimensional radiography to predict the amount of movement needed. The iatrogenic injuries, which often lead to medical tangles, have been widely discussed in recent years.The areas of intraoral iatrogenic injuries in orthodontics include both hard tissues and soft tissues: tooth, alveolar bone, gingiva, oral mucosa, nerve and so on. Almost every procedure during orthodontic therapy could result in unpredictable injuries. To avoid negative effects, the clinicians should understand all causes of various iatrogenic injuries, and take measures during orthodontic treatment. A case was presented with an adult male patient diagnosed as skeletal class III and dental class I malocclusion, and complaining of his dental crowding. Undergone orthodontic related examinations and periodontal evaluations, the treatment plan was to combine atypical extraction with insertion of mini-implants into mandible, in order to facilitate control of anchorage. After the treatment, lateral profile and occlusion were both apparently improved. Discussions were made about usage of mini-implants in this case and other possible ways to treat the patient.

A case of a 14 y/o male patient with a lingually impacted mandibular right 2nd molar was presented. After the third molar was extracted, a closed coil spring was used to connect the impaction to a miniscrew inserted in the nearby buccal shelf. Throughout the treatment process, all orthodontic appliances were hidden underneath the flap. The advantage of this closing flap technique is that it improves patient comfort while minimizing irritation to the surround muscle and soft tissue. Treatment options for uprighting lingually impacted mandibular 2nd molar, clinical considerations of orthodontic miniscrew placement, and design of biomechanics were also discussed.

Maxillary canines are essential for function and esthetics. Patients with impacted maxillary canine are considered to be more difficult to treat than the average orthodontic patients because successful treatment requires proper surgical technique and orthodontic mechanisms. This case report describes the treatment of a 10-year-old boy with a palatally impacted canine and rotated premolar. Using closed exposure with placement of an attachment, following by distal orthodontic force to bring the canine crown away from the root of lateral incisor, then buccal traction into normal occlusion. Purpose: The purpose of this study was to evaluate the possible factors that affected the duration of forced eruption of impacted upper incisors. Methods: 24 patients (mean age eight years and nine months old ), 25 upper impacted incisors were included in this study. Factors analysed included the age, the distance from impacted incisor edge to the occlusal plane, presence of odontoma or supernumerary tooth, the crown orientation,eruption space loss and root dilaceration. The purpose of this study was to investigate the differences in morphological characteristics of Class III malocclusion patient who can properly treated by orthodontic treatment only or combined orthognathic surgery.
