This research paper delves into pediatric autoimmune neuropsychiatric disorder associated with streptococcus (PANDAS). PANDAS causes rapid psychiatric changes in children ages 3 to puberty. These changes are most often obsessive-compulsive disorder (OCD) and/or tics, however, anxiety, agitation, mood disorder, depression, oppositional defiance disorder, attention-deficit hyperactivity disorder, hallucinations, memory/cognitive deficits, enuresis, and sleep disturbances have also been noted. Present-day estimations of PANDAS cases would likely prove to be understated if awareness of PANDAS increased (Centner, 2021). The purpose of this paper is to disseminate awareness and data of a disease that has gone relatively unnoticed in American medicine since being defined …

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is presumed to have originated from wildlife and shares homology with other bat coronaviruses. Determining the susceptibility of North American bat species to SARS-CoV-2 is of utmost importance for making decisions regarding wildlife management, public health, and conservation. In this study, Brazilian free-tailed bats (Tadarida brasiliensis) were experimentally infected with two strains of SARS-CoV-2 (parental WA01 and Delta variant), evaluated for clinical disease, sampled for viral shedding and antibody production, and analyzed for pathology. None of the bats (n = 18) developed clinical disease associated with infection, shed infectious virus, or …

BACKGROUND: Human adenoviruses typically cause self-limited respiratory, gastrointestinal, and conjunctival infections in healthy children. In late 2021 and early 2022, several previously healthy children were identified with acute hepatitis and human adenovirus viremia.
METHODS: We used International Classification of Diseases, 10th Revision, codes to identify all children (<18 years of age) with hepatitis who were admitted to Children’s of Alabama hospital between October 1, 2021, and February 28, 2022; those with acute hepatitis who also tested positive for human adenovirus by whole-blood quantitative polymerase chain reaction (PCR) were included in our case series. Demographic, clinical, laboratory, and treatment data were obtained from medical records. Residual blood specimens were sent for diagnostic confirmation and human adenovirus typing.
RESULTS: A total of 15 children were identified with acute hepatitis - 6 (40%) who had hepatitis with an identified cause and 9 (60%) who had hepatitis without a known cause. Eight (89%) of the patients with hepatitis of unknown cause tested positive for human adenovirus. These 8 patients plus 1 …

Standardizing Perinatal Mental Health Interventions: A Guide For The U.S., Maile Lauren Robles
Master's Projects and Capstones
The term perinatal individual refers to women and other individuals who are pregnant or who have been pregnant within the last 12 months. Every year, in the United States there are 700 pregnancy-related deaths and 10% are due to mental health causes including death by suicide. Mental health conditions including but not limited to anxiety, depression, substance use disorders, and post-traumatic stress disorder affect one in five perinatal individuals throughout the United States. This increases perinatal morbidity, hospitalization costs, and economic burden nationwide.
After 43 days postpartum, mental health conditions are the second leading cause of death. The United States …

Master's Projects and Capstones
With over one million confirmed deaths from COVID-19, the United States (U.S.) continues to battle the public health crisis arising from the spread of the SARS-CoV-2 virus. The COVID-19 pandemic has had a devastating impact on our economic, social, and health systems. Social distancing efforts and other precautionary measures such as mask-wearing have not sufficiently reduced morbidity and mortality from COVID-19; thus, COVID-19 vaccinations are an important tool for substantially alleviating the effects of the pandemic. Although COVID-19 vaccines are available to all Americans aged six months and older, many individuals are hesitant to receive vaccines. The two most common …
The Impact Of Increased Interdisciplinary Communication On Hospital Admission Rates In The Geriatric Population: An Integrative Review, Molly K. Langston
The Impact Of Increased Interdisciplinary Communication On Hospital Admission Rates In The Geriatric Population: An Integrative Review, Molly K. Langston
Doctoral Dissertations and Projects
The medical complexity of geriatric residents puts them at an increased risk of hospital readmissions related to coronary artery disease, myocardial infarction, chronic obstructive pulmonary disease, and diabetes, along with other chronic diseases. Geriatric residents of assisted living, skilled nursing, and memory care facilities should hold a particular interest to stakeholders, as the simple intervention of increasing early communication and correspondence regarding chronic diseases can have a great impact on quality improvement outcomes as well as financial burden of the institution. Additionally, interdisciplinary communication can be individualized to meet the needs of each geriatric resident in order to reduce the …

Doctoral Dissertations and Projects
Aim: To use an ICU-specific pressure injury (PI) risk assessment scale to promptly identify the risk for PIs in critically ill patients and promptly implement the appropriate PI prevention interventions to decrease the incidence rate of pressure injuries over 11 weeks.
Background: The risk of developing PIs is 3.8 times higher in the ICU patients than in other patient populations. The state of illness and or treatments increase their odds of developing hospital-acquired pressure injuries (HAPIs). The Braden scale currently in use may not identify the risk early enough to provide preventive interventions. An ICU-specific tool is needed to screen …

The Overlooked Mission-Transitional Care Programs For Veterans In Civilian Settings: An Integrative Review, Cynthia Louise Gonzales
Doctoral Dissertations and Projects
The number of veterans within the United States continues to increase. With the addition of healthcare laws, veterans can seek care within the Veterans Health Administration (VHA) and civilian settings. The collaboration among multiple healthcare providers must ensure veterans are provided safe and efficient care when transitioning from provider to provider. Transitional care programs (TCPs) are effective strategies to ensure that veterans receive safe and efficient healthcare. The VHA employs interventions across their healthcare settings to facilitate optimal care. A variety of TCPs were discovered during the review. Most TCPs were used in VHA settings, which revealed an opportunity to …

Aedes aegypti mosquitoes are a persistent human foe, transmitting arboviruses including dengue when they feed on human blood. Mosquitoes are intensely attracted to body odor and carbon dioxide, which they detect using ionotropic chemosensory receptors encoded by three large multi-gene families. Genetic mutations that disrupt the olfactory system have modest effects on human attraction, suggesting redundancy in odor coding. The canonical view is that olfactory sensory neurons each express a single chemosensory receptor that defines its ligand selectivity. We discovered that Ae. aegypti uses a different organizational principle, with many neurons co-expressing multiple chemosensory receptor genes. In vivo electrophysiology demonstrates …

Primary open-angle glaucoma (POAG) is a leading cause of irreversible blindness worldwide. Although deep learning methods have been proposed to diagnose POAG, it remains challenging to develop a robust and explainable algorithm to automatically facilitate the downstream diagnostic tasks. In this study, we present an automated classification algorithm, GlaucomaNet, to identify POAG using variable fundus photographs from different populations and settings. GlaucomaNet consists of two convolutional neural networks to simulate the human grading process: learning the discriminative features and fusing the features for grading. We evaluated GlaucomaNet on two datasets: Ocular Hypertension Treatment Study (OHTS) participants and the Large-scale Attention-based …

Eukaryotes use histone variants and post-translation modifications (PTMs), as well as DNA base modifications, to regulate DNA replication/repair, chromosome condensation, and gene expression. Despite the unusual organization of their protein-coding genes into large polycistronic transcription units (PTUs), trypanosomatid parasites also employ a "histone code" to control these processes, but the details of this epigenetic code are poorly understood. Here, we present the results of experiments designed to elucidate the distribution of histone variants and PTMs over the chromatin landscape of

When confronted with a public health emergency, significant innovative treatment protocols can sometimes be discovered by medical doctors at the front lines based on repurposed medications. We propose a statistical framework for analyzing the case series of patients treated with such new protocols, that enables a comparison with our prior knowledge of expected outcomes, in the absence of treatment. The goal of the proposed methodology is not to provide a precise measurement of treatment efficacy, but to establish the existence of treatment efficacy, in order to facilitate the binary decision of whether the treatment protocol should be adopted on an …

Intrusion Of Supraerupted Maxillary Molar Using Posterior Bite Plate And Mini-Screw, Chih-Kai Huang, Chun-Liang Kuo, Yun-I Wang, I-Hua Liu
Taiwanese Journal of Orthodontics
Supraerupted maxillary molars usually result from early loss of the opposing mandible molars. This may induce insufficient intermaxillary space and compromised prosthesis. The best way to deal with supraerupted maxillary molars is no doubt to intrude them. For the minor supraeruption of maxillary molars, simply wearing a posterior bite plate could apply vertical force to intrude them. But the larger supraeruption, combination with temporary anchorage device usage will supply more stable anchorage force to intrude maxillary molars back to the normal position. This article reported three cases, one using only posterior bite plate and the others in combination with mini-screw, …

To manage moderate mandibular prognathism combined with anterior open bite in adult cases, the dento-alveolar growth has to be guided and carefully utilized during orthodontic treatment, especially when the patients have acceptable profile before treatment and tend to deny the needs of orthognathic surgery. Two cases with similar diagnosis and treatment principle were reported. Extraction of mandibular molars provided spaces for lingual tipping movement of lower incisors, the lower occlusal plane showed a counter-clockwise rotation, the open-bite was accordingly corrected and the molar relationship was also normalized. To be able to command the occlusal plane changes is the key to archive …

Overextrusion of maxillary molars usually results from early loss of antagonistic teeth due to caries or periodontitis. Conventional techniques for molar intrusion was used multiple teeth and extraoral devices, that depended highly on patient’s cooperation and usually result in extrusion of other teeth rather than posterior intrusion. This article reviews treatment consideration, force mechanics and side effects about miniscrews for intrusion of upper molars. We also present a clinical case to confirm miniscrew treat supra-erupted upper molars successfully.

Total Arch Distalization To Reduce Bimaxillary Protrusion With Temporary Anchorage Devices, Shu-Chun Tsai, Hau-Hueng Chang, Chen-Ying Wang, Chung-Chen Jane Yao
Taiwanese Journal of Orthodontics
Bimaxillary protrusion is defined as convex profile with protrusive upper and lower lips. Therefore the orthodontic treatment goal is to correct the proclination of upper and lower incisors by retracting the incisors into the space usually resulted from four bicuspid extractions. However, with extraordinary development of temporary anchorage device as skeletal anchorage, non-extraction approach could be a treatment option for patients with mild to moderate bimaxillary protrusion. In this case report, we present the treatment of a 28-year-old female patient with skeletal Class II, high mandibular plane angle, and dental Class I bimaxillary protrusion, which combined with gummy smile and …

Adult patients with skeletal Class III malocclusion may have several available treatment options: (1) a combination of orthodontic treatment and orthognathic surgery. (2) camouflaged orthodontic treatment with various extraction selections. In this case report, we use miniscrew anchorage to retract mandibular incisors and control labial tipping of maxillary incisors. Not only improving dental occlusion without extraction of teeth (except for wisdom teeth) but also achieving a balanced profile in only 14 months of treatment time.

Background: Impaction of maxillary central incisors can result in aesthetic problems. The aim of this retrospective study was to examine the periodontal outcomes of impacted maxillary central incisors after surgical exposure and orthodontic extrusion compared to naturally erupted, contralateral incisors (control). Methods: Patients (N=80, 80 central incisors), who had been treated by a surgical-orthodontic approach, underwent a periodontal examination after 6 to 146 months. Periodontal parameters (crown length, keratinized gingival width, gingival scar, and bone loss) of the orthodontically extruded incisors were scored and compared with those of the naturally erupted contralateral incisors (control) in a masked set-up. Results: The …
