Taiwanese Journal of Orthodontics
This 25-year-old male was characterized with mandibular prognathism and spaced dentition. We applied bilateral sagittal split osteotomy and genioplasty to set back the mandible and the prominent chin. The excess spaces of lower dentition were consolidated and then closed surgically by anterior subapical osteotomy. After treatment, his facial esthetics and occlusion were improved significantly. In the spaced dentition, the space closure by anterior subapical osteotomy after space consolidation may provide a more efficient alternative treatment.

Dental Implant For Orthodontic Anchorage – A Case Report, Szu-Hsien Yu, Hsuan-Fang Wang, Hsun-Cheng Chang, Eddie Hsiang-Hua Lai
Taiwanese Journal of Orthodontics
Three-dimensional planning of a dental-implant site is critical when the dental implants placed in the edentulous area are to be used initially as anchor for tooth movement and later as prosthetic abutments for the fixed restoration. The combined orthodontic and prosthetic implant restorative treatment of an adult patient with missing teeth, diastema of teeth, and malocclusion is described in this article. Plastic cast bases combined with customized position plates allowed the precise transfer of the proposed implant position from the diagnostic arrangement to the original cast to avoid compromising subsequent orthodontic treatment.

Introduction: Tooth eruption is a complex process and may fail to occur due to several reasons. Primary failure of eruption (PFE) is one of the rare eruption disturbances which do not related to any obstruction in the eruptive pathway or any systematic disorders. This type of eruption failure often affects the posterior teeth and results in subsequent posterior open bite. Any orthodontic attempt is ineffective due to lack of inherent eruptive potential of tooth. Case Report: An 8-year-old skeletal class II young boy with posterior open bites was referred to our department for the poor chewing function. Clinical finding showed …

Angel’s Class III malocclusion is common in Oriental people. Orthodontic treatment combined with orthognethic surgery will be necessary to improve facial appearance and correct the malocclusion if skeletal discrepancy exists. Impacted dilacerated tooth is also a tough situation we face during orthodontic treatment. We have to evaluate all kinds of conditions to decide whether we align the tooth or not. This case is about a 19-yearold girl whose chief complain was mandible prognathism and crowded teeth. In the beginning examination, we found the tooth 25 was a impacted dilacerated tooth. In the end of the treatment, by orthodontic treatment combined …

Correction of facial asymmetry is one of the most difficult challenges in orthognathic surgery. Bardinet classified facial asymmetries based on three clinical entities: mandibular lateral deviations, dental asymmetries without skeletal involvement, and skeletal asymmetries. In some cases, combinations of soft tissue deformities and skeletal discrepancies complicate treatment outcome and management. This report presents a case of a 29-year-old male with skeletal Class I malocclusion, high mandibular plane angle, facial asymmetry, and dental Class III malocclusion. Mild left-side down occlusal plane canting and chin deviation to right side was observed. The pre-surgical orthodontic treatment started with full-mouth fixed appliance therapy for …

Application Of Digital Models In Orthodontic And Maxillofacial Surgery – A Literature Review, Yu-Jen Hsu, Kang-Yee Wang, Hung-Cheng Chiu, Emily Yi-Min Liu, Yun-Yun Wu
Taiwanese Journal of Orthodontics
The digital dental models are not subject to physical damage, and require negligible storage space. They can be transferred electronically to specialists easily and can be carried out with virtual setups. A precision 3-dimensional head model can be fabricated by integrating a digital dental model into a maxillofacial 3-D CT image. Using this 3-D image, it is possible to perform dental decompensation, reposition of osteotomized bony structures, and simulate the postoperative results on hard and soft tissue in 3-D on our computer screen. Besides, it also spares the laboratory time with CAD/CAM surgical splint manufactured.

Surgical Orthodontic Correction Of Class Iii Malocclusion With Anterior Openbite - A Case Report, Yi-Jen Liu, Ya-Wen Wang, Wen-Pei Wong, Min-Chih Hung, Chung-Chen Jane Yao, Yi-Jane Chen
Taiwanese Journal of Orthodontics
The etiologies of skeletal Class III malocclusion include excessive growth of mandible, deficient growth of maxilla, and the combination of both. Anterior openbite may be due to skeletal, dental, habitual, or functional problems. Mini-implant can be a reliable anchorage for maxillary molar intrusion, which facilitate anterior openbite closure by counterclockwise rotation of the mandible. However, the mandibular counterclockwise rotation may increase antero-posterior jaw discrepancy in Class III malocclusion. Therefore, surgical orthodontics is often required in the treatment of Class III malocclusion with anterior openbite. In this report, we present the treatment of an adult Class III patient with mandibular prognathism, …

The overall prevalence of peg-shaped maxillary permanent lateral incisors was 1.8%. The occurrence rates were higher in Mongoloid (3.1%) than in black (1.5%) and white (1.3%) patients. The presence of peg lateral incisors was also a complicated esthetic issue for orthodontist. This case is a skeletal Class III malocclusion 20-years-old male. His problems were mild maxillary skeletal retrognathism combined with mandibular prognathism, anterior crossbite, peg lateral incisors, and upper arch crowding. Treatment involved extraction of two maxillary lateral incisors. Maxillary canine substituted for lateral incisors. Before orthognathic surgery, the extraction spaces were closed for crowding relief of upper arch and …

Orthodontic Treatment Only On The Lower Anterior Teeth For Incisor Intrusion, Cheng-Ting Ho
Taiwanese Journal of Orthodontics
Recently bone screws have been used widely as Temporary Anchorage Devices (TAD'S) in various orthodontic treatments including intrusion of anterior teeth. Here we would like to present an uncommon case report of anterior segmental orthodontic treatment without involving the posterior teeth. A 60 year old male patient reported to our hospital with the complaint of missing maxillary anterior teeth. He wants to restore his function and esthetic with dental implants. However the over eruption of lower anterior teeth interferes with the space needed for upper implants; hence we decide to intrude mandibular incisors. The patient had all metal crown & …

Taiwanese Journal of Orthodontics
Facial asymmetry affects more frequently on the lower face than the upper face. Significant facial asymmetry compromises both occlusal function and facial esthetics. The etiology of facial asymmetry may be skeletal, dental, and soft tissue components of dentofacial structures. Correct diagnosis and appropriate treatment planning are very important in the management of the orthodontic patients with the chief complaint of facial asymmetry. Facial asymmetry in lower face can be clearly detected through the examination on chin deviation and asymmetry of bilateral gonial angle regions. The amount and the direction of deviation usually reveals the dental compensation in the transverse aspect …

Idiopathic condylar resorption (ICR) is a well-documented but poorly understood progressive disease that affects the TMJ. ICR can result in malocclusion, facial disfigurement, TMJ dysfunction, and pain. Some cases occur spontaneously, whereas others appear during orthodontic therapy or as sequelae to orthognathic surgical procedures. It is essential for orthodontists to recognize the onset of ICR in patients as early as possible .This article discusses practical aspects of recognizing and managing ICR in orthodontic patients by a combination of orthodontic and orthognathic surgery treatment.

The effect of oral appliances in treating mild to moderate obstructive sleep apnea syndrome (OSAS) patientis found to be similar to continuous positive airway pressure in a study in 1955. Oral appliances make tongue or mandible forward position to increase airway space. Mandibular advancement devices and tongue retaining devices are oral appliances used in clinic today. Long term effects of oral appliances are discussed in three parts in this article: airway, dentition and temporomadibular joint. Total airway volume increases after using oral appliances. Overbite and overjet are found decreasing. Oral appliances cannot worsen the temporomadibular joint problems.

Bimaxillary protrusion is characterized by protrusive and proclined upper and lower incisors. Since patients with bimaxillary protrusion generally have perfectly good occlusion, orthodontic treatment is often solely sought to correct the protrusive profile and less concern about the airway space change. Typical orthodontic treatment includes extraction of the bimaxillary premolars and anterior tooth retraction, with maximum anchorage. Recently, the relationship between maximum anchorage and tooth displacement is well recognized, retrospective upper airway analysis after retraction of the large incisors remains to be established. This article review previous studies about the pharyngeal airway changes after orthodontic treatment of class I bimaxillary …

Rapid maxillary expansion (RME) is a common method used by orthodontists to increase the transverse dimension of maxillary arch and arch perimeter, by a tooth-borne device to stretch the median palatal suture through the mechanical force. The orthopedic effects produced by the RME device include not only opening the median palatal suture, but also rotating the maxillary alveolar shelves, which has an influence on the adjacent sutures of facial skeleton and the morphology of nasal cavities. This article reviewed the impact of RME on the anatomy of upper airway, respiratory flow resistance, and clinical relation with the nasal airway obstruction …

Growth modification by using removable orthopedic appliances has been commonly used in growing patients to correct maxilla-mandibular jaw discrepancy. Previous studies reported the different effects of various orthopedic appliances on the airway dimension. Low-pull headgear therapy may induce a short-term decrease of the upper airway dimension in Class II patients. Face mask therapy seems to be associated with the increase of upper airway dimension in growing Class III patients. However, mandible protruding appliances can increase the upper airway dimension, especially in the Class II patients with mandible retrusion.
