The synthesis of proinflammatory leukotrienes implicated in asthma, allergic rhinitis, and atherosclerosis is initiated by the enzyme 5-lipoxygenase (5-LOX). The crystal structure of human Stable-5-LOX revealed a conformation where the catalytic iron was inaccessible to bulk solvent as two aromatic residues on a conserved helix-α2 (Hα2) plugged the substrate access portal. Whether 5-LOX can also adopt a more open conformation has not been resolved. Here, we present a new conformation of 5-LOX where Hα2 adopts an elongated conformation equivalent to that described in other animal lipoxygenase structures. Our observation of the sigmoidal kinetic behavior of 5-LOX, which is indicative of …

Gait alterations after anterior cruciate ligament reconstruction (ACLR) are commonly reported and have been linked to posttraumatic osteoarthritis development. While knee gait alterations have been studied at several time points after ACLR, little is known about how these biomechanical variables change earlier than 6 months after surgery, nor is much known about how they differ over the entire stance phase of gait. The purpose of this study was to examine knee gait biomechanical variables over their entire movement pattern through stance at both 3 and 6 months after ACLR and to study the progression of interlimb asymmetry between the two …

Most artificial intelligence (AI) research and innovations have concentrated in high-income countries, where imaging data, IT infrastructures and clinical expertise are plentiful. However, slower progress has been made in limited-resource environments where medical imaging is needed. For example, in Sub-Saharan Africa the rate of perinatal mortality is very high due to limited access to antenatal screening. In these countries, AI models could be implemented to help clinicians acquire fetal ultrasound planes for diagnosis of fetal abnormalities. So far, deep learning models have been proposed to identify standard fetal planes, but there is no evidence of their ability to generalise in …

COVID-19 dramatically stressed an already overburdened healthcare system. Physicians, susceptible to burnout, bore the brunt of this threat as frontline providers. Limited data are available describing how the pandemic may have affected physician perceptions and beliefs, including experiences of burnout and plans to exit the field. Reddit, a social media website, allows physicians the ability to interact with their peers pseudonymously on a purpose-driven, publicly viewable subreddit (r/medicine). By providing a space for “physicians and other medical professionals” to interact, it is possible to observe unfiltered discourse.
This dissertation explores physician-peer discourse on Reddit prior to and during the COVID-19 …

BACKGROUND AND PURPOSE: The optic nerve is surrounded by the extension of meningeal coverings of the brain. When the pressure in the cerebrospinal fluid increases, it causes a distention of the optic nerve sheath diameter (ONSD), which allows the use of this measurement by ultrasonography (US) as a noninvasive surrogate of elevated intracranial pressure. However, ONSD measurements in the literature have exhibited significant heterogeneity, suggesting a need for consensus on ONSD image acquisition and measurement. We aim to establish a consensus for an ONSD US Quality Criteria Checklist (ONSD US QCC).
METHODS: A scoping systematic review of published ultrasound ONSD …

African populations are the most diverse in the world yet are sorely underrepresented in medical genetics research. Here, we examine the structure of African populations using genetic and comprehensive multi-generational ethnolinguistic data from the Neuropsychiatric Genetics of African Populations-Psychosis study (NeuroGAP-Psychosis) consisting of 900 individuals from Ethiopia, Kenya, South Africa, and Uganda. We find that self-reported language classifications meaningfully tag underlying genetic variation that would be missed with consideration of geography alone, highlighting the importance of culture in shaping genetic diversity. Leveraging our uniquely rich multi-generational ethnolinguistic metadata, we track language transmission through the pedigree, observing the disappearance of several …

Opioids Alter Paw Placement During Walking, Confounding Assessment Of Analgesic Efficacy In A Postsurgical Pain Model In Mice, Victoria E Brings, Maria A Payne, Robert W Gereau Iv
2020-Current year OA Pubs
Introduction: Hind paw-directed assays are commonly used to study the analgesic effects of opioids in mice. However, opioid-induced hyperlocomotion can obscure results of such assays.
Objectives: We aimed to overcome this potential confound by using gait analysis to observe hind paw usage during walking in mice.
Methods: We measured changes in the paw print area after induction of postsurgical pain (using the paw incision model) and treatment with oxycodone.
Results: Paw incision surgery reduced the paw print area of the injured hind paw as mice avoided placing the incised section of the paw on the floor. Surprisingly, oxycodone caused a …

Mental illness becomes one of the main problems that most pilots do not usually address. It is not because pilots do not have the courage or are open enough to talk with someone, but because the Federal Aviation Administration (FAA) forces them to hide from mental depression. Most of the time, the pilots are not willing to declare such illnesses as they fear losing their job; simultaneously, the Federal Aviation Agencies across the world require pilots to be in peat health, including their mental condition, to operate the aircraft. While it can be said that the passengers’ and crews’ safety …

Introduction: Medical providers may not be familiar with the genitourinary and sexual symptoms of transgender and non-binary (TGNB) individuals. This lack of familiarity may hinder a provider's ability to address these issues as patients may hesitate to report symptoms due to fear of stigma, misgendering, and being treated disrespectfully.
Aim: To describe the array of genitourinary and sexual symptoms in transfeminine individuals.
Methods: Upon institutional review board approval, researchers used semi-structured interviews with 25 transfeminine individuals assigned male at birth to explore urinary and sexual symptoms on a sample of convenience. Participants were recruited and interviews were conducted until saturation …

Nursing faculty are challenged to integrate immunization content in prelicensure nursing curricula. Historically, most immunization content has been delivered in pediatrics courses, with less emphasis on other populations across the lifespan. Skills related to vaccine administration may be prioritized over the most current immunization science, such as pathophysiology, immunology, and epidemiology. As the most trusted profession rated by the public (Saad, 2020), nurses are ideally suited to address vaccine hesitancy and promote vaccination in the communities they serve. Nurses apply active listening, problem solving, and communication skills with patients and their families, contributing to a person's confidence in their decision …

The Relationship Between Emergency Physicians’ Creative Thinking Preference And Their Risk Of Burnout: An Opportunity To Make A Difference, Naghma S. Khan
Creativity and Change Leadership Graduate Student Master's Projects
The FourSight theory contends that individuals exhibit a preference for the mental operations involved in creative problem solving. The four fundamental mindset preferences measured by FourSight are Clarifiers, Ideators, Developers and Implementers. Individuals can exhibit a peak preference for one, two, or three of these mindsets, or they can show an even distribution of energy across all four creative-thinking preferences. Robust and creative solutions to complex and recurrent problems are only possible if an individual or a team of individuals consciously move through the four stages of problem solving. Creative problem-solving can be taught. The ability to be creative at …

Introduction: Apical root resorption is one of the most common adverse effects of orthodontic treatment. Forces used during orthodontic tooth movement can cause permanent loss of apical surfaces of teeth and although most reported cases of root resorption are asymptomatic, the process is irreversible and often unpredictable. Thus, it is of clinical interest to investigate which patients may be at greatest risk.
Purpose: This study utilized Cone Beam Computed Tomography (CBCT) to evaluate whether rapid maxillary expansion (RME) in Hispanic patients increased the severity of orthodontically induced apical root resorption.
Materials and Methods: Pre-treatment (T1) and post-treatment …

In order to better understand the contributing factors to low visibility and an overall lack of representation of Coptic Orthodox individuals within the field of mental health, a needs assessment was conducted. Previous research studies have focused on the low visibility of the Coptic Orthodox population within the field of mental health but have focused primarily on stigma, without determining other needs and contributing factors. This needs assessment utilized the Delphi method of data collection in order to conceptualize the needs of the community from the perspectives of experts within the group. An extensive literature review was completed in order …

The present literature review aims at mending the research to practice gap for the treatment of Post-Traumatic Stress Disorder (PTSD). Biological mechanisms of the stress response system are explored as they pertain to trauma and its subsequent effects on the brain and body. In addition, American Psychological Association (APA) recommended evidence-based treatments for PTSD are reviewed as well as more recent emerging somatic therapies. There is a critical need for further development in the area of both topdown and bottom-up interventions. Biological working mechanisms remain unknown for a majority of treatments and physiological measures are scarcely used in treatment outcome …
