Chronic kidney disease (CKD) is directly influenced by the deleterious effects of systemic inflammation and oxidative stress. The vascular endothelium may transiently respond to aerobic exercise and improve post-exercise vascular renal function in moderate stages of CKD. Brachial artery flow-mediated dilation (FMD) is a nitric-oxide-dependent measure of endothelial function that is transiently potentiated by exercise. The purpose of the study was to determine the acute influence of a single bout of high-intensity interval exercise (HIIE) or steady-state moderate-intensity exercise (SSE) on endothelial dysfunction in moderate stages of CKD. Twenty participants (n = 6 men; n = 14 women) completed 30 …

Neurod1 Aav-Based Gene Therapy For Functional Brain Repair After Ischemic Injury Through In Vivo Astrocyte-To-Neuron Conversion, Marlena Ramlatchan
VCU's Medical Journal Club: The Work of Future Health Professionals
This study uses AAV based gene therapy to create an astrocyte- neuron conversion to regain functional abilities damaged by ischemic injuries. NeuroD1 treatment has shown promising results in the treatment of ischemic injuries in rodents and is heading towards clinical trials. Cognitive and motor improvements were shown. The basis of this therapy is to use functional glial cells to promote neurocellular growth, which would restore some function lost as a result of injury.

Journal Articles: Pathology and Microbiology
Unlike the α-helical and β-sheet antimicrobial peptides (AMPs), our knowledge on amino acid-rich AMPs is limited. This article conducts a systematic study of rich AMPs (>25%) from different life kingdoms based on the Antimicrobial Peptide Database (APD) using the program R. Of 3425 peptides, 724 rich AMPs were identified. Rich AMPs are more common in animals and bacteria than in plants. In different animal classes, a unique set of rich AMPs is deployed. While histidine, proline, and arginine-rich AMPs are abundant in mammals, alanine, glycine, and leucine-rich AMPs are common in amphibians. Ten amino acids (Ala, Cys, Gly, His, …

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Delta variant quickly rose to dominance in mid-2021, displacing other variants, including Alpha. Studies using data from the United Kingdom and India estimated that Delta was 40-80% more transmissible than Alpha, allowing Delta to become the globally dominant variant. However, it was unclear if the ostensible difference in relative transmissibility was due mostly to innate properties of Delta's infectiousness or differences in the study populations. To investigate, we formed a partnership with SARS-CoV-2 genomic surveillance programs from all six New England US states. By comparing logistic growth rates, we found that Delta …

Effectively monitoring the spread of SARS-CoV-2 mutants is essential to efforts to counter the ongoing pandemic. Predicting lineage abundance from wastewater, however, is technically challenging. We show that by sequencing SARS-CoV-2 RNA in wastewater and applying algorithms initially used for transcriptome quantification, we can estimate lineage abundance in wastewater samples. We find high variability in signal among individual samples, but the overall trends match those observed from sequencing clinical samples. Thus, while clinical sequencing remains a more sensitive technique for population surveillance, wastewater sequencing can be used to monitor trends in mutant prevalence in situations where clinical sequencing is unavailable.

Orthodontics and Endodontics Theses
In December 2019, the coronavirus disease 2019 (COVID-19) was first identified as an acute infectious disease in Wuhan, China and subsequently led to an ongoing pandemic. At the onset of the pandemic, dental professionals were understood to face the greatest exposure risk to SARS-Cov-2 due to aerosolization of fluids from the oral cavity and respiratory airways . As a result, dental professionals, including academic institutions and their students and residents halted much of their operations to minimize exposure risks and potentially slow the spread of infection to peers and patients alike. Currently, there is little in the literature that describes …

Posttraumatic stress disorder (PTSD) is a debilitating psychiatric disorder which results in deleterious changes to psychological and physical health. Patients with PTSD are especially susceptible to life-threatening co-morbid inflammation-driven pathologies, such as autoimmunity, while also demonstrating increased T-helper 17 (TH17) lymphocyte-driven inflammation. While the exact mechanism of this increased inflammation is unknown, overactivity of the sympathetic nervous system is a hallmark of PTSD. Neurotransmitters of the sympathetic nervous system (i.e., catecholamines) can alter T-lymphocyte function, which we have previously demonstrated to be partially mitochondrial redox-mediated. Furthermore, we have previously elucidated that T-lymphocytes generate their own catecholamines, and strong …

Abstract
Malignant Hyperthermia (MH) is a rare inherited disorder in which exposure to triggering agents such as volatile anesthetics or depolarizing neuromuscular blockers causes a hypermetabolic reaction leading to patient fatality. MH susceptible patients have a genetic mutation on the ryanodine one receptor (RYR1) in skeletal muscle, allowing uninhibited calcium release (Smith, Tranovich, & Ebraheim, 2018). Increases in free calcium result in an uncontrollable sustained muscle contraction through the excitation-contraction coupling of myosin and actin. Sustained muscle contraction rapidly consumes adenosine triphosphate stores and stimulates conversion to lactate producing anaerobic metabolism (Smith et al., 2018). Immediate recognition and treatment of …

Substance use disorder (SUD) affects the anesthesia community disproportionally as compared to the general public (Rupprecht, 2022). It is estimated that SUD will affect 10%-15% of all anesthesia providers at some point in their careers (Rupprecht, 2022). One way that SUD affects the human body is the alteration of microRNAs, which are small segments of RNA that exist to regulate gene expression (Gowen et al., 2020). This poster will discuss how miRNAs change in the SUD-affected individual, anesthesia-specific risk factors, and signs and symptoms to identify when this disorder has affected someone else. Resources for help are available from the …

Cystic fibrosis (CF) is a complex disease caused by an autosomal recessive gene mutation that results in the dysfunction of chloride transport proteins in epithelial tissues. The resultant highly viscous secretions can cause respiratory, gastrointestinal, endocrine, and reproductive complications. Although modern treatment has become more individually targeted and the average life expectancy has increased in recent years, no cure has been discovered. It affects over 80,000 people worldwide, with 30,000 affected individuals living in the United States alone. This poster aims to provide a brief overview of the pathophysiologic processes, treatment therapies, and nursing considerations for Cystic Fibrosis.

To make a safe anesthetic plan, a nurse anesthetist must evaluate a patient's medical history. Anesthesia providers will encounter patients with COPD requiring anesthesia. It is necessary that nurse anesthesia understands the pathophysiology and peri-operative considerations to provide safe care to COPD patients. The poster provided can be a tool to review the pathophysiology and nursing implementations of COPD. It provides information for the nurse anesthetist to apply to the perioperative setting to improve patients’ surgery outcomes. An anesthetist is always striving to provide quality anesthesia care to their patients. COPD patients require extra preparation to prevent complications from arising …

Violence and harassment towards healthcare workers has been an underlying problem throughout history. However, with the rapid and evolving onset of the COVID-19 pandemic, the risks that healthcare workers face has been a focal topic of discussion among administration, staff and the public. What was once considered an occupational hazard is now being raised as a public health issue- an epidemic within a pandemic. This literature review seeks to identify the prevalence and causes of violence and harassment towards healthcare workers. Was healthcare violence as prevalent before COVID-19? Has violence increased because patient volume increased? Additionally, this review will determine …

High-risk neuroblastoma (NB) is an aggressive pediatric tumor which develops from the extracranial sympathetic nervous system and accounts for almost 15% of all childhood cancer-related deaths. Current therapies include high-dose chemotherapy and radiation which have long-term toxic side-effects. Despite these intensive therapies, the overall survival rate of NB is less than 50%. Therefore, developing novel therapeutic approaches by directly targeting the molecular mechanisms that drives NB progression is very important. In the present study, we analyzed genomic datasets of 1235 NB patients and discovered that overexpression of survivin coding gene BIRC5 strongly correlates with poor overall and event-free survival of …

Older adults (≥65 years) are the fastest growing population group. Thus, ensuring nutritional well-being of the ‘over-65s’ to optimise health is critically important. Older adults represent a diverse population – some are fit and healthy, others are frail and many live with chronic conditions. Up to 78% of older Irish adults living independently are overweight or obese. The present paper describes how these issues were accommodated into the development of food-based dietary guidelines for older adults living independently in Ireland. Food-based dietary guidelines previously established for the general adult population served as the basis for developing more specific recommendations appropriate …

"It Will Help Him Wonderfully": Placebo And Meaning Responses In Early Medieval English Medicine, Rebecca Brackmann
Faculty Peer-Reviewed Publications
Many early English medical remedies have been dismissed, even by sympathetic scholars, as “mere placebo,” with the implication that therapeutic benefits to the patient, if any, are thereby insignificant. My article challenges such dismissals of placebo responses in Old English medicine. I draw on current research demonstrating that in certain conditions, a placebo effect, or “meaning response” (to use Daniel Moerman’s term), can have striking success in bringing about healing. I describe contemporary clinical practices shown to enhance patient responses to pharmaceutical treatments both inert and active, and demonstrate that these strategies are also visible in early English medical practice. …
