The Effects Of 465nm Blue Light On Sleepiness & Fatigue, Mark E. Childress
The Effects Of 465nm Blue Light On Sleepiness & Fatigue, Mark E. Childress
DNP Research Projects
Abstract
Background: Exhaustion is now accepted in nursing culture. Staff suffering from sleepiness and fatigue are more likely to commit medical errors and sentinel events. Emergency department nurses have the greatest risk of committing these errors. Currently, there is no relief provided to shift workers beyond a single 30-minute meal break within a 12-hour period. Other researched mitigation strategies have either failed or are unrealistic in this environment.
Local Problem: Emergency Department nurses at Parkridge Medical Center in Chattanooga, TN suffer from chronic sleepiness and fatigue that is exacerbated by shift duration, excessive workload, a stressful environment, and lack of …
Enhancing Hospitalists' Knowledge In Relation To Palliative Care, Herline Raphael
Enhancing Hospitalists' Knowledge In Relation To Palliative Care, Herline Raphael
DNP Research Projects
Abstract
Palliative care was not the specialty of many hospitalists; thus, communication was hindered regarding ELC and advanced directives. Despite healthcare utilization in the United States, inadequate treatment of serious illnesses continues due to the lack of provider education and understanding of ELC (palliative and hospice care). Hospitalists are not formally or informally trained to care for patients that need palliative or hospice care. A quantitative, quasi- experimental and qualitative approach was utilized to determine if hospitalists who received educational training on palliative care communication and collaboration improved knowledge, skills, and collaboration when caring for end-of-life or critically ill patients. …

Improving Nutrition Literacy In Primary Care, Kami D. Dykes
DNP Research Projects
Abstract
By the year 2030, nearly one in every two adults will have obesity (Body Mass Index [BMI] ≥30 kg/m2.), and one in four adults will have severe obesity (BMI ≥35 kg/m2) (Ward et al., 2019). Equally troubling is that obesity poses a threat to the health of individuals, decreases the quality of life, shortens life expectancy, causes and worsens comorbidities, and increases all- cause mortality (Abdelaal et al., 2017). Studies have shown a link between BMI, nutritional knowledge, and dietary practices. Nutrition literacy is a newer term that refers to the complex relationship between nutrition …

Introduction
This novel study sought to observe the general public’s beliefs on the comparison between pharmaceuticals and exercise for the management of chronic disease, as is consistent with the ACSM Exercise is Medicine (EIM) initiative. The EIM initiative seeks to establish physical activity as a standard of care alongside more traditional interventions such as pharmacology. Despite the established benefit of exercise, little is known about the general public's awareness of the pharmacological benefits of structured, prescribed exercise in the treatment of chronic disease.
Objective
Exercise can be an effective intervention for chronic disease and in some cases, as effective as …

Mrt Supportive Housing Evaluation: Enrollment In Supportive Housing Results In Significantly Greater Cross-Sector Cost Savings Than “Treatment As Usual”, Center For Human Services Research, University At Albany
Health and Healthcare Services Reports and Research Briefs
The New York State Medicaid Redesign Team (MRT) was created in 2011 to develop a multi-year reform plan to address unprecedented healthcare cost growth and improve healthcare quality. One innovation tested under this Redesign is the investment in supportive housing, or affordable housing paired with supportive services (e.g., on-site case management, referrals to community-based services). High-cost, high-need Medicaid recipients who were homeless, unstably housed, or living in treatment facilities providing a higher level of care than needed were targeted for enrollment. While expensive, this investment was anticipated to improve quality of life and health outcomes for enrolled clients, thus decreasing …

Free Cholesterol Accelerates Aβ Self-Assembly On Membranes At Physiological Concentration, Mohtadin Hashemi, Siddhartha Banerjee, Yuri L. Lyubchenko
Journal Articles: Pharmaceutical Sciences
The effects of membranes on the early-stage aggregation of amyloid β (Aβ) have come to light as potential mechanisms by which neurotoxic species are formed in Alzheimer's disease. We have shown that direct Aβ-membrane interactions dramatically enhance the Aβ aggregation, allowing for oligomer assembly at physiologically low concentrations of the monomer. Membrane composition is also a crucial factor in this process. Our results showed that apart from phospholipids composition, cholesterol in membranes significantly enhances the aggregation kinetics. It has been reported that free cholesterol is present in plaques. Here we report that free cholesterol, along with its presence inside the …

While overall survival with multiple myeloma (MM) has improved, patients suffer from overwhelming tumor burden, MM-associated comorbidities, and frequent relapses requiring administration of salvage therapies. As a result, this vicious cycle is often characterized by cumulative immunodeficiency stemming from a combination of disease- and treatment-related factors leading to neutropenia, T-cell deficiency, and hypogammaglobulinemia. Infectious etiologies differ based on the duration of MM and treatment-related factors, such as number of previous treatments and cumulative dose of corticosteroids. Herein, we present the case of a patient who was receiving pomalidomide without concomitant corticosteroids for MM and was later found to have cryptococcosis, …

In the U.S. about half of the HIV-infected individuals are aged 50 and older. In men living with HIV, secondary hypogonadism is common and occurs earlier than in seronegative men, and its prevalence increases with age. While the mechanisms(s) are unknown, the HIV-1 trans-activator of transcription (Tat) protein disrupts neuroendocrine function in mice partly by dysregulating mitochondria and neurosteroidogenesis. We hypothesized that conditional Tat expression in middle-aged male transgenic mice [Tat(+)] would promote age-related comorbidities compared to age-matched controls [Tat(−)]. We expected Tat to alter steroid hormone milieu consistent with behavioral deficits. Middle-aged Tat(+) mice had lower circulating testosterone and …

Herein we report a method for the synthesis of 3,4,5-trisubstituted isoxazoles in water under mild basic conditions at room temperature via a [3 + 2]-cycloaddition of nitrile oxides and 1,3-diketones, β-ketoesters, or β-ketoamides. We optimized the reaction conditions to control the selectivity of the production of isoxazoles and circumvent other competing reactions, such as O-imidoylation or hetero [3 + 2]-cycloaddition. The reaction happens fast in water and completes within 1-2 hours, which provides an environmentally friendly access to 3,4,5-trisubstituted isoxazoles, an important class of structures found in numerous bioactive natural products and pharmaceuticals. Additionally, we optimized the reaction conditions to …

Modulation of the endocannabinoid system (ECS) is of great interest for its therapeutic relevance in several pathophysiological processes. The CB2 subtype is largely localized to immune effectors, including microglia within the central nervous system, where it promotes anti-inflammation. Recently, a rational drug design toward precise modulation of the CB2 active site revealed the novelty of Pyrrolo[2,1-c][1,4]benzodiazepines tricyclic chemotype with a high conformational similarity in comparison to the existing leads. These compounds are structurally unique, confirming their chemo-type novelty. In our continuing search for new chemotypes as selective CB2 regulatory molecules, following SAR approaches, a total of 17 selected (S,E)-11-[2-(arylmethylene)hydrazono]-PBD analogs …

The cannabinoid receptor 2 (CB2 receptor) has attracted considerable interest, mainly due to its potential as a target for therapeutics for treating various diseases that have a neuroinflammatory or neurodegenerative component while avoiding the adverse psychotropic effects that accompany CB1 receptor-based therapies. With the appreciation that CB2-selective ligands show marked functional selectivity, there is a renewed opportunity to explore this promising area of research from both a mechanistic as well as a therapeutic perspective. In this research, we are interested in the discovery of new chemotypes as highly selective CB2 modulators, which may serve as good starting points for further …

Introduction: Untreated ocular infections can damage the unique fine structures of the eye with possible visual impairments and blindness. Ciprofloxacin (CIP) ophthalmic solution is prescribed as first-line therapy in ocular bacterial infections. Natamycin (NT) ophthalmic suspension is one of the progenitors in ocular antifungal therapy. Nanostructured lipid carriers (NLCs) have been widely examined for ocular penetration enhancement and distribution to deeper ocular tissues. The objective of the current study was to prepare NLCs loaded with a combination of CIP and NT (CIP-NT-NLCs) and embed them in an in-situ gelling system (CIP-NT-NLCs-IG). This novel formulation will target the co-delivery of CIP …

Patient falls during hospitalization are a major clinical problem that occurs frequently. It is estimated that between 700,000 and 1,000,000 patients fall in hospitals each year. Between 30% and 50% of patients who fall in hospitals are injured. Consequently, falls with serious injury are among the most frequent hospital-reported sentinel events. Hospitals also suffer from patient falls as the Centers for Medicare and Medicaid Services no longer reimburse for costs related to preventable falls or fall-related injuries. Given the significant negative impact of falls on patients and healthcare organizations, hospitals continue to look for interventions to decrease falls. Research strongly …

Problem: Four million people worldwide die within thirty days of surgery. The EPIC Corporation has created the opportunity to collect and analyze data on the peri-operative care of patients. Initially, implementation of the program has identified barriers to event reporting by anesthesia providers. Barriers identified in the literature and in semi-structured interviews are fear of liability and retribution by patients, employers, and governing bodies. Significance: Despite national efforts to increase the reporting of medical errors, they remain grossly under- reported. The integration of the National Anesthesia Clinical Outcomes Registry (NACOR) events data collection tool into the EHR for anesthesia providers …
