INTRODUCTION: Natural hazards are elements of the physical environment caused by forces extraneous to human intervention and may be harmful to human beings. Natural hazards, such as weather events, can lead to natural disasters, which are serious societal disruptions that can disrupt dialysis provision, a life-threatening event for dialysis-dependent people. The adverse outcomes associated with missed dialysis sessions are likely exacerbated in island settings, where health care resources and emergency procedures are limited. The effect of natural disasters on dialysis patients living in geographically vulnerable areas such as the Cayman Islands is largely understudied. To inform predisaster interventions, we systematically …

Abington Jefferson Health Papers
Macrocytic anemia is usually associated with vitamin B12 or folate deficiency. However, folate deficiency was rarely reported as a cause of hemolytic anemia. We present a case of a young man with a history of alcohol abuse who initially presented with an acute on chronic abdominal pain and was found to have jaundice and scleral icterus. His liver enzymes were unremarkable, and his abdominal imaging did not reveal any acute pathology. However, he was found to have a severe non-immune hemolytic anemia secondary to folate deficiency.

Journal Articles: Internal Medicine
BACKGROUND: Incidental findings are a well-known complication of imaging studies done for both diagnostic and research purposes. Little is known about the rates and types of incidental findings found on brain MRI in patients with HIV infection, who may be at risk for HIV-Associated Neurocognitive Disorders (HAND).
METHODS: The parent study included 108 adults with HIV infection and 125 demographically-matched uninfected controls who completed MRI and neuropsychological testing. Incidental findings were classified by the study team as vascular, neoplastic, congenital, other neurologic, or non-neurologic. Categorical measures were compared using Pearson chi-square tests; continuous measures were compared using t-tests.
RESULTS: Among …

Department of Family & Community Medicine Presentations and Grand Rounds
E-cigarettes are one of the most difficult and complex public health issues most of us have confronted.
Most anti-tobacco experts and advocates line up on a spectrum from embracing e-cigarettes as a promising way to reduce the harm of tobacco products all the way to being the greatest threat to progress in our 60 year war on big tobacco.

Section of Diabetes, Endocrinology and Metabolism
Introduction: Dapagliflozin is a member of a novel class of drugs (sodium-glucose cotransporter-2 inhibitors) used to treat type 2 diabetes mellitus and licensed in Pakistan in 2017. This retrospective observational study evaluated the effects of dapagliflozin on glycated hemoglobin (HbA1c) concentrations in patients treated at endocrinology clinics in Islamabad, Pakistan. The secondary objectives included assessing the effects of dapagliflozin on weight reduction and blood pressure control and to determining its safety.
Methodology: Patients with type 2 diabetes who were treated with dapagliflozin were identified by screening the electronic medical records at tertiary care hospitals in Islamabad. Data were collected at …

Association Of Hospital Discharge Against Medical Advice With Readmission And In-Hospital Mortality, Sally Y. Tan, Jeremy Y. Feng, Cara Joyce, Jonathan Fisher, Arash Mostaghimi
Biostatistics Collaborative Core: Faculty Publications and Other Works
IMPORTANCE Hospital readmissions contribute to higher expenditures and may sometimes reflect suboptimal patient care. Individuals discharged against medical advice (AMA) are a vulnerable patient population and may have higher risk for readmission.
OBJECTIVES To determine odds of readmission and mortality for patients discharged AMA vs all others, to characterize patient and hospital-level factors associated with readmissions, and to quantify their overall cost burden.
DESIGN, SETTING, AND PARTICIPANTS Nationally representative, all-payer cohort study using the 2014 National Readmissions Database. Eligible index admissions were nonobstetrical/newborn hospitalizations for patients 18 years and older discharged between January 2014 and November 2014. Admissions were excluded …

While it may seem that the efforts of local governments have only a marginal impact on the global issue of climate change, local action can spread to generate large-scale change. Local action can inspire other communities to adopt policies, creating the potential to expand and form regional action on climate change. Furthermore, local policies can influence state and federal policies. This guide will walk you through a five-step framework that was developed from the results of a qualitative research study that explored how three Utah cities, Salt Lake City, Park City, and Moab, successfully adopted 100% renewable electricity resolutions.

Purpose
The optimal strategy for revascularization in below-the-knee (BTK) chronic limb-threatening ischemia (CLTI) remains debatable. High restenosis rates of percutaneous transluminal angioplasty (PTA) often warrants re-intervention or amputations. Novel devices such as drug-eluting (paclitaxel) balloons (DEB), stents, and atherectomy devices- directional (HawkOne, SilverHawk, TurboHawk)™ or orbital (DiamondBack 360) ™ are used infrequently as therapy when BTK. We aim to compare the rate of total re-occlusion between these interventions.
Material & Methods
A retrospective chart review of 97 patients who underwent BTK percutaneous intervention in 2015-2020 at our hospital. Descriptive statistics were used to assess the distribution of variables; continuous variables …

The Efficiency Of Digital Anatomy Teaching Strategies, Jeremy Sandberg, Rachel Calderon, James M. Mckivigan
College of Health & Human Services (TUN) Publications and Research
Background: In medical school, the teaching of anatomy is both time-consuming and complicated. As more schools allot less time to this subject, there is a growing need to restructure anatomy teaching methodologies. This paper examines digital dissection resources and identifies how and when they were implemented in the classroom.
Methods: An online survey tool was sent to osteopathic medical schools throughout the United States to determine the resources and methods being used and to assess how students were performing in corresponding anatomy courses. The anatomy director at each school was given a predetermined set of questions to enable an evaluation …

Quantifying Differences In Pill Swallow Patterns In Adults, Catherine A. Sullivan, Luis F. Riquelme
Annual SHSP Student Research and Scholarship Day
Difficulty swallowing pills has become an increasing complaint among patients visiting swallow evaluation centers across the globe. Deficits are reported in healthy individuals as well as in clinical populations. In the USA, 40% of 679 persons responding to a survey reported difficulty swallowing pills (Business Wire, 2009). In an effort to facilitate pill intake, several modifications have been reported: use of liquid formulations, crushing tablet, opening capsule, whole pill mixed with food or via feeding tube (Cornish, 2005). A survey of 40 nurses revealed the most common modification was to use apple sauce (Riquelme et al., 2009). Current literature on …

BACKGROUND: Diabetes is a risk factor associated with pancreatic ductal adenocarcinoma (PDAC), and new adult-onset diabetes can be an early sign of pancreatic malignancy. Development of blood-based biomarkers to identify diabetic patients who warrant imaging tests for cancer detection may represent a realistic approach to facilitate earlier diagnosis of PDAC in a risk population.
METHODS: A spectral library-based proteomic platform was applied to interrogate biomarker candidates in plasma samples from clinically well-defined diabetic cohorts with and without PDAC. Random forest algorithm was used for prediction model building and receiver operating characteristic (ROC) curve analysis was applied to evaluate the prediction …
