Background: Although most observational studies identify viral or bacterial pathogens in 50% or less of patients hospitalized with community-acquired pneumonia (CAP), we previously demonstrated that a multi-test bundle (MTB) detected a potential pathogen in 73% of patients. This study compares detection rates for potential pathogens with the MTB versus the Biofire® Pneumonia FilmArray® panel (BPFA) multiplex PCR platform and presents an approach for integrating BPFA results as a foundation for subsequent antibiotic stewardship (AS) activities.

Magnetic resonance imaging (MRI) is a very effective method for identifying any abnormality in the structure and physiology of the spine. However, MRI is time consuming as well as costly. In this work, the authors propose an algorithm which can reduce the time of MRI and thus the cost, with minimal compromise on accuracy. They reconstruct a three-dimensional (3D) image of the spine from a sequence of 2D MRI slices along any one axis with reasonable slice gap. Purpose Specific language impairment (SLI; see also developmental language disorder) and dyslexia are separate, yet frequently co-occurring disorders that confer risks to reading comprehension and academic achievement. Until recently, most studies of one disorder had little consideration of the other, and each disorder was addressed by different practitioners. Jefferson Health has leveraged its acute care telehealth platform to screen, order testing, and manage patients with COVID-19-related concerns.

The most common cause of ascites is liver cirrhosis. Additional causes such as heart failure, cancer, and pancreatitis among others can also precipitate this abnormality. Spontaneous bacterial peritonitis (SBP) is an infection of ascitic fluid that happens without any evidence of an intra-abdominal surgically-treatable cause. Ascites of cardiac origin can also be complicated by SBP. Here we present a case of a 62-year-old male with extensive cardiac history who presented to our service with ongoing dyspnea and orthopnea. He also had significant abdominal distention and pitting edema. The patient was found to have constrictive pericarditis and was admitted for pericardiectomy. …

Reactive oxygen species (ROS) function as critical mediators in a broad range of cellular signaling processes. The mitochondrial electron transport chain is one of the major contributors to ROS formation in most cells. Increasing evidence indicates that the respiratory Complex II (CII) can be the predominant ROS generator under certain conditions. A computational, mechanistic model of electron transfer and ROS formation in CII was developed in the present study to facilitate quantitative analysis of mitochondrial ROS production. Vaginitis is one of the main causes of primary care and gynecological visits in the United States. The most common infectious causes are bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), and trichomoniasis. A physician survey was conducted to measure awareness of vaginitis clinical guidelines and availability of in-office point-of-care (POC) diagnostic tools. Participants were asked to perform a chart review to evaluate diagnostic practices for their symptomatic vaginitis patients. A total of 333 physicians and 984 patient charts were included. Reactive oxygen species have been involved in the pathogenesis of rheumatoid arthritis (RA). Our goal was to determine the effects of selectively scavenging superoxide (O2•−) and hydroxyl radicals with antioxidant nanoparticles, called poly(ethylene glycol)-functionalized hydrophilic carbon clusters (PEG-HCCs), on the pathogenic functions of fibroblast-like synoviocytes (FLS) from patients with rheumatoid arthritis (RA) and on the progression of an animal model of RA. To date, SARS-CoV-2 has infected more than 32 million people, and 30% of cases needs intensive care treatment for severe Covid-19 and related acute respiratory distress syndrome (ARDS). Despite medical efforts, mortality rate is still high, and no specific therapies have been outlined yet. Preliminary evidence from case reports and clinical trials using anti-inflammatory and anti-complement drugs shows a deep and overwhelmed inter-play between adaptive and immune responses triggered by SARS-CoV-2 infection. Therefore, a combinatorial therapeutic strategy targeting both inflammation and immune system might more efficiently stop this inflammatory loop and prevent the development of ARDS.

Gracie Fisher, an artist and composer based in Santa Barbara, California, provided the cover art for the Fall 2020 edition of The Open Journal of Occupational Therapy (OJOT). “The Fruit of Spirit” is a 16” x 20” oil painting on canvas. When a rare neurologic condition left Gracie paralyzed from the neck down, art and music helped to heal her mind and soul. This qualitative pilot study sought to elucidate perspectives regarding student readiness from the viewpoint of occupational therapy academic and fieldwork educators. Data for the study was collected from interviews and a focus group. Background: Occupational therapists are among the first providers to initiate care after a concussion; however, evidence is limited regarding current concussion-related vision disorder practice patterns. A better understanding of these practice patterns is important because of the impact of undetected and untreated vision problems on occupational performance.

Method: A mixed-methods survey was administered online to occupational therapists to explore the assessments and interventions used to address concussion-related vision disorders, occupational therapy’s perceived role, and difficulties encountered when treating these conditions.
