เหตุผลของการทำวิจัย : ผู้ป่วยที่มีอวัยวะเพศกำกวม มักประสบความลำบากในการระบุเพศมีรายงานว่าผู้ป่วยอาจมีบทบาททางเพศ และอัตลักษณ์ทางเพศที่เบี่ยงเบน แต่การศึกษาเรื่องพัฒนาการทางเพศในผู้ป่วยกลุ่มนี้ยังขาดแคลนทั้งในประเทศไทยและต่างประเทศวัตถุประสงค์ : เพื่อศึกษาการระบุเพศ อัตลักษณ์ทางเพศ และบทบาททางเพศในผู้ป่วยอวัยวะเพศกำกวมวิธีการศึกษา : ศึกษาในกลุ่มผู้ป่วยที่มีภาวะอวัยวะเพศกำกวม อายุระหว่าง 3 - 18 ปีจำนวน 30 รายและผู้ปกครอง ที่แผนกผู้ป่วยนอกต่อมไร้ท่อฝ่ายกุมารเวชศาสตร์ โรงพยาบาลจุฬาลงกรณ์ ระหว่างเดือนมกราคมถึงเดือนพฤษภาคม พ.ศ. 2559 โดยใช้การสัมภาษณ์เชิงลึกโดยใช้แนวคำถาม ที่ได้จากการทบทวนวรรณกรรมเกี่ยวกับเรื่อง อัตลักษณ์ทางเพศ บทบาททางเพศ ร่วมกับการรวบรวมข้อมูลจากเวชระเบียนผู้ป่วย เป็นการศึกษาเชิงคุณภาพ และทำการวิเคราะห์แบบโคไลซี่ และcontent analysisผลการศึกษา : พบผู้ป่วยโรค 46XX, disorders of sexual developments (DSD)21 ราย, 46XY, DSD 8 ราย 45X,46XY DSD 1 รายมีผู้ป่วย 9 รายที่เคยได้รับการประเมินทางจิตวิทยา มีผู้ป่วยที่ไม่ได้เข้ารับการรักษาตั้งแต่แรกเกิด 12 ราย มีผู้ป่วยที่ได้รับการระบุเพศแรกเกิดเป็นเพศหญิง20 ราย เพศชาย 10 ราย และมีผู้ป่วยได้รับการระบุเพศใหม่โดยแพทย์5 ราย ผู้ป่วยขอเปลี่ยนเพศเอง 2 ราย พบรูปแบบของบทบาททางเพศในผู้ป่วย 46XX, DSD ด้านทรงผม การแต่งตัว และกลุ่มเพื่อน เป็นแบบหญิง ต่างจากบทบาทด้าน กิจกรรม การเล่นหรือของเล่นที่เป็นแบบชายหรือแบบผสม ในขณะที่บทบาททางเพศของผู้ป่วย 46XY, DSDส่วนใหญ่เป็นแบบชาย พบผู้ป่วยที่มีอัตลักษณ์ทางเพศหญิง 17 ราย,ชาย 9 ราย, ไม่แน่ใจ 1 ราย และยังแยกแยะเพศไม่ได้ 3 ราย โดยผู้ป่วย46XX,DSD ที่มีอัตลักษณ์ทางเพศหญิง 4 ราย แสดงความต้องการจะเป็นอีกเพศหนึ่ง.สรุป : บทบาททางเพศส่วนใหญ่สอดคล้องกับโครโมเซมเพศ ยกเว้นบทบาททางเพศด้านกิจกรรม การเล่นหรือของเล่นในผู้ป่วยอวัยวะเพศกำกวมที่มีโครโมโซมเพศหญิงที่เป็นบทบาทแบบชาย ในขณะที่พบอัตลักษณ์ทางเพศของผู้ป่วยบางรายไม่ตรงกับโครโมโซมเพศ อนึ่งผู้ป่วยจำนวนมากไม่ได้รับการรักษาตั้งแต่แรกเกิด …

Chronic myeloid leukemia (CML) after chemotherapy for any primary malignancy has been occasionally reported. Herein we reported one definite case of CML who was diagnosed after the treatment of adenocarcinoma of the rectum. A 79-year-old Thai man was diagnosed with moderately differentiated adenocarcinoma of the rectum invading the urinary bladder. And he was treated with rectal resection and lymph nodes excision, followed by 5-fluoro-uracil and leucovorin therapy. He remained in complete remission for 1.5 years until he suddenly passedthe hematochezia for one day and very high white blood cell and platelet counts without weight loss. Each year, one out of three adults (younger than 65 years old) experiences a fall, resulting in fractures, head injuries, and even death. Falling is the leading cause of death from an injury and causes the greatest number of nonfatal injuries and hospital admissions for trauma. Since balance and muscle performance decrease as one ages, it is vitally important to assess these factors as part of a comprehensive strategy to monitor and predict fall risk. Individuals diagnosed with Multiple Sclerosis (MS) often have an increased risk of falling. Additionally, many also have a fear of falling that all can affect balance confidence. Recent advancements in wearable sensor technology have made it possible to assess certain aspects of gait and balance in the clinical setting that were not possible previously. The information collected by sensors may be useful for identifying specific factors that contribute to falls and balance confidence in those with MS which in turn could help enhance treatment approaches.

The Foot Posture Index (FPI) is a screening tool used by clinicians to assess if the foot is pronated, neutral or supinated. It is intended to quantify foot position by assigning positive values to pronated attributes and negative values to supinated attributes. Three hindfoot and 3 forefoot attributes are scored. Although the FPI is valid and reliable, individual component scores may skew the composite score. Therefore, there lies a need to investigate the internal consistency of the FPI.

The rate of game related basketball injuries is 2xs greater in professionals than collegiate players and the most common injuries occur in the lower extremity. Arch mechanics are often cited as a related factor and the Arch Height Index Measurement System (AHIMS) is a reliable and valid system for quantifying mobility. However, normative or comparative values for basketball athletes have not been reported.

This research answers the question: Are physical therapist assistant programs teaching the manual therapy skill of joint mobilizations? This study also gathers information regarding how the joint mobilization techniques are being instructed and assessed within the Physical Therapist Assistant programs. This information could serve as the foundation for future research regarding developing instructional and evaluation strategies for manual therapy skills. The findings of this study may also affect the physical therapist assistants in clinical performance of manual therapy. This study investigates the maternal birth experience through a cross-cultural lens. While the field of medical anthropology has researched birth practices of many cultures, few cross-cultural studies have been performed and no recent studies have suggested a transition in birthing. Ethnographic interviews with women and practitioners in Yucatán, Mexico and with women in Kentucky, United States allowed for a better understanding of the respective birthing environments. Grounded theory was then employed to develop a birth transition theory explaining changes occurring when society transitions from traditional birth practitioners to allopathic birth practitioners. The work presented here was performed in conjunction with an experimental group in order to prove the concept that an alternating magnetic field (AMF) could be used to induce an intramolecular cyclization to effect payload release. The system studied here consisted of a fluorophore tethered to gold-coated iron oxide nanoparticles (Fe3O4 NPs) via an organic linker consisting of 2 key elements: a carbonate (electrophile) and an amine (nucleophile). Many symptoms of concussions can substantially interfere with the cognitive abilities, and skills students use in school. Although students may have similar symptoms, they experience concussions differently. Students returning to the classroom from a concussion often have different needs and abilities.
Research shows that a formal, individualized protocol to treat suspected concussions, both academically and physically, is vital to the successful recovery of each student. Much research is available on concussions for professional athletes, specifically related to recovery and returning to their sport. Objective: To examine how clinicians communicate with parents about influenza vaccination and the effect of these communication behaviors on parental vaccine decision-making.
Study Design: We performed a secondary analysis of data obtained from a cross-sectional observational study in which health supervision visits between pediatric clinicians and English-speaking parents of young children were videotaped. Eligible visits occurred during the 2011-2012 and 2013-2014 influenza seasons, included children ≥ 6 months, and contained an influenza vaccine discussion. Background Patients with leptomeningeal metastasis (LM) have symptoms related to disease within the neuroaxis, cancer sites outside the neuroaxis and treatment toxicities. Furthermore, the meaning ascribed to advanced cancer, such as LM, may impact the perceived severity, frequency and distress of symptoms as well as quality of life (QoL) and depression. Our aim was to explore and describe relationships among meaning of illness (MoI), depression, QoL and symptom burden in LM patients and to explore whether adding spine-related items from the MD Anderson Symptom Inventory-Spine (MDASI-SP) to the MDASI-Brain Tumor (BT) enhanced understanding of symptom burden in this population.

The overarching purpose of this two-phase study was to gain understanding about and insight into the practice of effective psychodrama supervision as a specialty field. The study explored senior supervisors’ perspectives on their roles as in-class group supervisors in psychodrama graduate school programs in Israel. Grounded theory (GT) methods of data collection and analysis were aimed at learning about participants’ professional approaches, and practice- based experiences. Study 1 involved face-to-face in-depth interviews conducted with open-ended core questions. Yoga improves a variety of impairments in typical pediatric populations: cardiovascular, neuromuscular, musculoskeletal, and pulmonary conditions, balance, strength, and flexibility. Yoga improves mindfulness in adult populations, and emerging evidence shows a similar effect among pediatric populations.

Individuals with dementia are at a high risk of functional decline and falling. The aim of this study was to investigate the possible relationship between cognition and functional performance in older adults with dementia participating in Goodwill Easter Seals adult day service programs. Quality assurance (QA) guidelines are essential to provide uniform execution of clinical trials with uniform quality hyperthermia treatments. This document outlines the requirements for appropriate QA of all current superficial heating equipment including electromagnetic (radiative and capacitive), ultrasound, and infrared heating techniques. Detailed instructions are provided how to characterize and document the performance of these hyperthermia applicators in order to apply reproducible hyperthermia treatments of uniform high quality. Earlier documents used specific absorption rate (SAR) to define and characterize applicator performance. The negative impact of alcohol on an individual’s social, psychological, and physiological health is well known. Despite this knowledge, it remains a prevalent problem in our society. Residents of rural communities encounter a wide range of risk factors including poverty, unemployment, and limited access to healthcare. These factors can influence both the prevalence and treatment for alcohol use disorders. With limited access to medical resources, the Emergency Department (ED) can become the default provider for problems related to alcohol use. In humans, activation of the olivocochlear bundle (OCB) can have a suppression or enhancement effect on cochlear processes. This phenomenon was studied via OCB activation effects on otoacoustic emissions (OAEs). However, it has been suggested that cochlear microphonics (CMs) can provide better, more detailed information regarding OCB function. The laboratory diagnosis of Lyme disease is currently dependent on the detection of IgM and IgG antibodies against Borrelia burgdorferi, the causative agent of the disease. The significance of serum IgA against B. burgdorferi remains unclear. The production of intrathecal IgA has been noted in patients with the late Lyme disease manifestation, neuroborreliosis, but production of antigen-specific IgA during early disease has not been evaluated. Existing theory on competition for hosts between pathogen strains has proposed that immune selection can lead to the maintenance of strain structure consisting of discrete, weakly overlapping antigenic repertoires. This prediction of strain theory has conceptual overlap with fundamental ideas in ecology on niche partitioning and limiting similarity between coexisting species in an ecosystem, which oppose the hypothesis of neutral coexistence. For Plasmodium falciparum, strain theory has been specifically proposed in relation to the major surface antigen of the blood stage, known as PfEMP1 and encoded by the multicopy multigene family known as the var genes. Marijuana is one of the most commonly used recreational drugs in the United States. As marijuana is illegal in the majority of countries, the use of readily available and unregulated synthetic cannabinoids (SCBs) has increased. Little is known about the potential adverse effects of SCBs especially in regards to their nephrotoxicity. Case reports of acute kidney injury (AKI) from acute tubular injury secondary to their use have been reported. However, the exact pathology, mechanism, and extent of renal injury remain unknown. Acute graft-versus-host disease remains a major threat to a successful outcome after allogeneic hematopoietic cell transplantation. While improvements in treatment and supportive care have occurred, it is unknown whether these advances have resulted in improved outcome specifically among those diagnosed with acute graft-versus-host disease. We examined outcome following diagnosis of grade II-IV acute graft-versus-host disease according to time period, and explored effects according to original graft-versus-host disease prophylaxis regimen and maximum overall grade of acute graft-versus-host disease. A 65-year-old woman with recently diagnosed ovarian cancer presented with near syncope, tachypnea, and hypoxia. Transthoracic echocardiography revealed a dilated and hypokinetic right ventricle and a large, mobile mass in the right atrium prolapsing across the tricuspid valve. She was diagnosed with pulmonary embolism in transit and emergent embolectomy was recommended.

The secreted cysteine proteinase SpeB is an important virulence factor of group A streptococci (GAS), whereby SpeB activity varies widely among strains. To establish the degree to which SpeB activity correlates with disease, GAS organisms were recovered from patients with pharyngitis, impetigo, invasive disease or acute rheumatic fever (ARF), and selected for analysis using rigorous sampling criteria; >300 GAS isolates were tested for SpeB activity by casein digestion assays, and each GAS isolate was scored as a SpeB-producer or non-producer. Highly significant statistical differences (p < 0.01) in SpeB production are observed between GAS recovered from patients with ARF (41.5% SpeB-non-producers) compared to pharyngitis (20.5%), invasive disease (16.7%), and impetigo (5.5%). SpeB activity differences between pharyngitis and impetigo isolates are also significant, whereas pharyngitis versus invasive isolates show no significant difference. The disproportionately greater number of SpeB-non-producers among ARF-associated isolates may indicate an altered transcriptional program for many rheumatogenic strains and/or a protective role for SpeB in GAS-triggered autoimmunity.

We measured changes in transcranial Doppler ultrasound (TCD) and near infrared spectroscopy (NIRS) during 70 degrees upright tilt in patients with recurrent vasovagal syncope (VVS, N = 20), postural tachycardia syndrome (POTS, N = 20), and healthy controls (N = 12) aged 15-27 years old. VVS was included if they fainted during testing within 5-15 min of upright tilt. We combined TCD and NIRS to obtain estimates of percent change in the cerebral metabolic rate of oxygen consumption (CMRO2), cerebral blood flow velocity (CBFv), and oxygen extraction fraction (OEF). Pulmonary ventilation (V̇I) in awake and sleeping goats does not change when antagonists to several excitatory G protein-coupled receptors are dialyzed unilaterally into the ventral respiratory column (VRC). Concomitant changes in excitatory neuromodulators in the effluent mock cerebral spinal fluid (mCSF) suggest neuromodulatory compensation. Herein, we studied neuromodulatory compensation during dialysis of agonists to inhibitory G protein-coupled or ionotropic receptors into the VRC. Microtubules were implanted into the VRC of goats for dialysis of mCSF mixed with agonists to either μ-opioid (DAMGO) or GABAA (muscimol) receptors. Single nucleotide polymorphisms (SNPs) in the human angiotensinogen (hAGT) gene may modulate its transcription and affect the regulation of blood pressure via activation of the renin-angiotensin aldosterone system (RAAS). In this regard, we have identified polymorphisms in the 2.5 Kb promoter of the hAGT gene that form two haplotype (Hap) blocks: -6A/G (-1670A/G, -1562C/T, -1561T/C) and -217A/G (-532T/C, -793A/G, -1074T/C & -1178G/A). hAGT gene with Hap -6A/-217A (Hap I) is associated with increased blood pressure whereas, Hap -6G/-217G (Hap II) is associated with normal blood pressure in human subjects.
