Objective: Quality assurance of large ontological systems such as SNOMED CT is an indispensable part of the terminology management lifecycle. We introduce a hybrid structural-lexical method for scalable and systematic discovery of missing hierarchical relations and concepts in SNOMED CT.
Material and Methods: All non-lattice subgraphs (the structural part) in SNOMED CT are exhaustively extracted using a scalable MapReduce algorithm. Four lexical patterns (the lexical part) are identified among the extracted non-lattice subgraphs. Non-lattice subgraphs exhibiting such lexical patterns are often indicative of missing hierarchical relations or concepts. Background—Efficacious interventions to reduce drug use and its consequences for club drug using populations are not apparent in the literature. We tested interviewer-(CAPI) and self-administered (ACASI) comprehensive health and social risk assessments as distinct interventions compared to waitlist control.
Methods—750 men and women ages 18-39 with multidrug use and heterosexual behavior were randomized in equal proportions to the three conditions. Instrumentation included well-tested measures of drug use, risky sex, mental distress and substance dependence.
Results—The sample was 56% male; mean age=25. Reported risk behaviors and health consequences did not differ by assessment modality. Adjusted HLM analyses showed …

A 59-year-old male developed a proximal stricture of his transplant ureter ten years after a living donor renal transplant. Endoscopic management was unsuccessful, and the patient was temporized with percutaneous nephrostomy tubes for months. Eventually, it became clear he would require surgical revision. Intraoperatively, complete fibrosis of the renal hilum, and intrarenal location of the pelvis precluded the planned pyelovesicostomy. A successful open vesicocalicostomy was performed, anastomosing a bladder flap to a lower pole calix. The patient remains recurrence free after 6 months of follow-up.

Osteoarthritis (OA) affects an estimated 50 million people in the US, and approximately 43% have limitations in daily function due to arthritis pain.3 Individuals with knee osteoarthritis (OA) have heightened sensitization to pain2,5 as well as reduced strength, diminished function and quality of life1 that prompts them to seek care from health care providers. We report the case of a fisherman who was exposed to high concentrations of hydrogen sulfide (H2S) gas from the fish garbage room. The patient survived and was discharged with full recovery from the hospital. H2S is a colourless, foul smelling and highly toxic gas next to carbon monoxide, which causes inhalation death. It is a by-product of various industrial processes particularly involves exposure from agriculture, petrochemical industry and organic matter decomposition from sewage processing. OBJECTIVE:
To explore the demographics of individuals teaching basic science courses in private nursing schools.
METHODS:
The study was conducted in Karachi, and comprised teachers teaching basic science courses in 16 registered private nursing schools. A demographic questionnaire was used to collect data. The study was conducted in the year 2013.
RESULTS:
Teachers holding academic/professional degrees in different science disciplines were involved in teaching science to nurses. In most of the schools, all the basic science courses were taught by one subject specialist science teacher. In this Perspective, HEAL Trafficking, the nation's leading public health anti-trafficking organization maps out a national research agenda to tackle the problem of human trafficking. Given the paucity of research on trafficking, HEAL Trafficking engaged its membership in a consensus development process throughout 2016 to develop its national research agenda. HEAL Trafficking proposes five priorities that public health researchers should focus on in the decade ahead to make meaningful progress on preventing and responding to human trafficking in the Unites States.

BACKGROUND: Diagnostic agreement among pathologists is 84% for ductal carcinoma in situ (DCIS). Studies of interpretive variation according to grade are limited.
METHODS: A national sample of 115 pathologists interpreted 240 breast pathology test set cases in the Breast Pathology Study and their interpretations were compared to expert consensus interpretations. We assessed agreement of pathologists' interpretations with a consensus reference diagnosis of DCIS dichotomised into low- and high-grade lesions. Generalised estimating equations were used in logistic regression models of rates of under- and over-interpretation of DCIS by grade.

Although several lung cancer susceptibility loci have been identified, much of the heritability for lung cancer remains unexplained. Here 14,803 cases and 12,262 controls of European descent were genotyped on the OncoArray and combined with existing data for an aggregated genome-wide association study (GWAS) analysis of lung cancer in 29,266 cases and 56,450 controls. We identified 18 susceptibility loci achieving genome-wide significance, including 10 new loci. The new loci highlight the striking heterogeneity in genetic susceptibility across the histological subtypes of lung cancer, with four loci associated with lung cancer overall and six loci associated with lung adenocarcinoma. Although there are clear guidelines for the treatment of patients with end-organ damage (hypertensive crisis), there is little evidence to guide the evaluation of asymptomatic patients with elevated blood pressure in the ED. A reasonable approach is to attempt to ascertain the baseline blood pressure, determine if there is a high probability of occult end-organ damage, avoid nonbeneficial diagnostic studies, reduce further risk to patients by initiating conservative treatment in the ED, and allow the primary care provider to bring patients to the goal blood pressure. Generally acute treatment is not indicated and may possibly be harmful.

Measurement invariance across time and reporter is rarely reported in the literature for measures of youth disclosure, even though it is often necessary to establish at least strong invariance before proceeding to further analyses such as comparing means across time or reporter. Measurement invariance was examined across time (ages 11, 14, and 17) and across reporter (youth report of disclosure to mother, youth report of disclosure to father, mother report of youth disclosure, father report of youth disclosure) with a sample of 348 youth and their parents. BACKGROUND: Melanoma is the most common malignancy encountered during pregnancy. Conflicting data have led to ongoing confusion regarding pregnancy-associated melanoma (PAM) in the media and among the public. The objective of this study was to better characterize both the clinical presentation of PAM and its prognostic implications.
STUDY DESIGN: Female patients of reproductive age, with stage 0 to IV cutaneous melanoma, were identified from our prospectively maintained database. The international Testicular Cancer Consortium (TECAC) combined five published genome-wide association studies of testicular germ cell tumor (TGCT; 3,558 cases and 13,970 controls) to identify new susceptibility loci. We conducted a fixed-effects meta-analysis, including, to our knowledge, the first analysis of the X chromosome. This poster summarizes a variety of methods to further characterize the gut-brain axis by tracing motor and sensory nerves between the gut and brain and identifying cell bodies in the sensory ganglia. Determines the meth method to trace the sensory and motor nerves was through fluorescence and the best method to identify sensory neuron cell bodies was the Nissi stain.

Alcohol Use And Drinking Motives Among Sanctioned And Non-Sanctioned Students, Diana M. Doumas

This study examined differences in the relationship of drinking motives to drinking behavior among sanctioned and non-sanctioned first year students (N = 298). Results of hierarchical regression analyses indicated for both sanctioned and non-sanctioned students, alcohol use was predicted by social and enhancement motives and alcohol-related consequences were predicted by social, enhancement, and coping motives. Additionally, high levels of conformity motives predicted alcohol use and alcohol-related consequences for sanctioned students only. Counseling implications are discussed.

During ischemic stroke, neurons and glia are subjected to damage during the acute and neuroinflammatory phases of injury. Production of reactive oxygen species (ROS) from calcium dysregulation in neural cells and the invasion of activated immune cells are responsible for stroke-induced neurodegeneration. Scientists have failed thus far to identify antioxidant-based drugs that can enhance neural cell survival and improve recovery after stroke. However, several groups have demonstrated success in protecting against stroke by increasing expression of antioxidant enzymes in neural cells. Genes and pathways that allow cells to cope with oncogene-induced stress represent selective cancer therapeutic targets that remain largely undiscovered. In this study, we identify a RhoJ signaling pathway that is a selective therapeutic target for BRAF mutant cells. RhoJ deletion in BRAF mutant melanocytes modulates the expression of the pro-apoptotic protein BAD as well as genes involved in cellular metabolism, impairing nevus formation, cellular transformation, and metastasis. Short-term treatment of nascent melanoma tumors with PAK inhibitors that block RhoJ signaling halts the growth of BRAF mutant melanoma tumors in vivo and induces apoptosis in melanoma cells in vitro via …

Sepsis is defined as “life-threatening organ dysfunction caused by a dysregulated host response to infection” (Singer et al, 2016). Despite being a lesser-known evil when compared to myocardial infarction and cerebrovascular accident, sepsis occurs in more than 230,000 patients in the United States annually and is the cause of more than 40,000 deaths per year (Seymour & Rosengart, 2015). Although the prevalence of sepsis in the hospital setting is common, the presentation varies making sepsis difficult to diagnose. Background: Resistant hypertension is a well-recognized clinical challenge yet there are no reported data on its prevalence in Pakistan. These patients are subjected to a higher risk of developing hypertensive complications. The objective of our study was to evaluate the prevalence and determinants of resistant hypertension in an Asian cohort of hypertensive patients.
Methods: This cross-sectional study was carried out among hypertensive patients visiting a tertiary care hospital in Karachi from September-December 2015. Military aircraft routinely operate in harsh, corrosive environments that necessitate aggressive maintenance methods and products to prevent deterioration. Three primary tasks associated with corrosion control of these aircraft are abrasive blasting, sanding, and primer paint application. The most frequently used primer paints for these applications contain chromium VI (Cr VI) in the form of barium chromate and strontium chromate. Due to aircraft size and the nature of these tasks, control of Cr VI exposures is often difficult.

Background: Since the District Health Director (DHD) leads the district health organization, plans must be put in place to ensure that we retain current directors and anticipate the future when the DHD retires from the position.
Methods: Best practices in the areas of recruiting, retention, and succession planning are used to present resolutions to the problems.
Results: By following the steps identified in the best practices, there is a reasonable chance that the district will be prepared for any change in leadership.
