วัตถุประสงค์ : เพื่อศึกษาการปฏิบัติการพยาบาลเพื่อความปลอดภัยผู้ป่วย และความสัมพันธ์ระหว่างปัจจัยส่วนบุคคล ความรู้เกี่ยวกับความปลอดภัยผู้ป่วย และวัฒนธรรมความปลอดภัยผู้ป่วย กับการปฏิบัติการพยาบาลเพื่อความปลอดภัยผู้ป่วยของพยาบาลวิชาชีพ\n\nแบบแผนงานวิจัย : การวิจัยแบบพรรณนาเชิงหาความสัมพันธ์\n\nวิธีดํ าเนินการวิจัย : กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพทุกคน ที่ปฏิบัติงานมากกว่า 1 ปีขึ้นไป ที่โรงพยาบาลชุมชน 7 แห่ง จํ านวน 259 คน เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามความรู้เกี่ยวกับความปลอดภัยผู้ป่วย วัฒนธรรมความปลอดภัยผู้ป่วย และการปฏิบัติการพยาบาลเพื่อความปลอดภัยผู้ป่วย มีค่าความตรงตามเนื้อหา (Content Validity) เท่ากับ .85, .94 และ .86 ตามลํ าดับ มีค่าความเที่ยงของแบบสอบถาม (reliability) เท่ากับ .61, .87 และ .84 ตามลํ าดับ มีเพียงพยาบาลวิชาชีพจํ านวน 202 คน ตอบกลับแบบสอบถาม คิดเป็นร้อยละ 77.99 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ค่าสัมประสิทธิ์สหสัมพันธ์แบบพอยด์ไบซีเรียลและสัมประสิทธิ์สหสัมพันธ์แบบสเปียร์แมน\n\nผลการวิจัย : กลุ่มตัวอย่างมีคะแนนการปฏิบัติการพยาบาลเพื่อความปลอดภัยผู้ป่วยอยู่ในระดับมากที่สุด (x= 4.25, SD = 9.10) และวัฒนธรรมความปลอดภัยผู้ป่วยเป็นปัจจัยเดียวที่มีความสัมพันธ์กับการปฏิบัติการพยาบาลเพื่อความปลอดภัยผู้ป่วยอย่างมีนัยสํ าคัญทางสถิติที่ระดับ .01 (r = .303, p < .001)\n\nสรุป : ผลการศึกษาในครั้งนี้ ได้แสดงให้เห็นถึงความสํ าคัญของการปรับปรุงวัฒนธรรมความปลอดภัยผู้ป่วยในหน่วยบริการปฐมภูมิ เพื่อที่จะส่งเสริมคุณภาพการดูแลและความปลอดภัยผู้ป่วยที่ดียิ่งขึ้น

วัตถุประสงค์ : เพื่อพัฒนาและศึกษาผลลัพธ์ของรูปแบบการดูแลผู้ป่วยบาดเจ็บสมองระดับรุนแรงโดยใช้การจัดการรายกรณี \nแบบแผนงานวิจัย : การวิจัยและพัฒนา\n\nวิธีดําเนินการวิจัย : การศึกษาแบ่งเป็น 4 ระยะ ได้แก่ วิเคราะห์สถานการณ์ ออกแบบการพัฒนารูปแบบ การทดลองใช้ และประเมินผล กลุ่มตัวอย่างในการศึกษาครั้งนี้ ประกอบด้วย บุคลากรสหสาขาวิชาชีพผู้ป่วยบาดเจ็บสมองระดับรุนแรงและญาติผู้ป่วย จํา านวน 60 คน โดยใช้วิธีเลือกแบบเจาะจง เครื่องมือที่ใช้ในการเก็บข้อมูลประกอบไปด้วยแบบสอบถามความพึงพอใจของบุคลากรสหสาขาวิชาชีพและแบบสอบถามความพึงพอใจของญาติผู้ป่วย\n\nผลจากการวิจัย :\n \nระยะที่ 1 จากการวิเคราะห์สถานการณ์ พบว่า ระยะเวลารอทําเอกซเรย์คอมพิวเตอร์เฉลี่ย 95 นาทีระยะเวลาที่อยู่รักษาในโรงพยาบาล เฉลี่ย 6 วัน, การปฏิบัติการพยาบาลดูแลผู้ป่วยยังไม่ต่อเนื่อง\n\nระยะที่ 2 มีการจัดทีมพัฒนารูปแบบการดูแลผู้ป่วยบาดเจ็บสมองระดับรุนแรงโดยใช้การจัดการรายกรณีและออกแบบแนวปฏิบัติการพยาบาลซึ่งมีทั้งหมด 9 หมวด ประกอบด้วย การประเมินผู้ป่วยแรกรับ การพยาบาลป้องกันการเกิดแผลกดทับ การพยาบาลบรรเทาความเจ็บปวด การพยาบาลป้องกันภาวะความดันในกะโหลกศีรษะสูง การรักษาอุณหภูมิร่างกาย การป้องกันการพลัดตกเตียง การป้องกันการติดเชื้อระบบทางเดินปัสสาวะ การบันทึกน้ําเข้า-ออก และ การประสานกับแพทย์เมื่อมีภาวะผิดปกติ\n\nระยะที่ 3 เมื่อนํารูปแบบการดูแลไปทดลองใช้กับผู้ป่วย จํานวน 30 คน พบว่า ระยะเวลารอทําเอกซเรย์คอมพิวเตอร์ลดลงจากเดิมโดยเฉลี่ย 58 นาที ระยะเวลาที่อยู่รักษาในโรงพยาบาลเฉลี่ย 5 วัน\n\nระยะที่ 4 การประเมินการพัฒนารูปแบบ พบว่า ระดับความพึงพอใจของบุคลากรสหสาขาวิชาชีพอยู่ที่มากที่สุด (X = 4.87 , SD = 0.34) และระดับความพึงพอใจของญาติอยู่ที่มากที่สุด (X = 4.93, SD = 0.24) \n\nสรุป : การพัฒนารูปแบบการดูแลผู้ป่วยบาดเจ็บสมองระดับรุนแรงโดยใช้การจัดการรายกรณีครั้งนี้ ทําให้เกิดรูปแบบการดูแลที่ชัดเจน เพื่อเป็นแนวทางในการทํางานร่วมกับบุคลากรสหสาขาวิชาชีพอย่างเป็นรูปธรรม\n

Bronchopleural fistula (BPF) with prolonged air leak (PAL) is most often, though not always, a sequela of lung resection. When this complication occurs post-operatively, it is associated with substantial morbidity and mortality. Surgical closure of the defect is considered the definitive approach to controlling the source of the leak, but many patients with this condition are suboptimal operative candidates. Therefore there has been active interest for decades in the development of effective endoscopic management options. Successful use of numerous bronchoscopic techniques has been reported in the literature largely in the form of retrospective series and, at best, small prospective trials. …

Due to the vast geographical distribution and significant economic losses generated, porcine reproductive and respiratory syndrome virus (PRRSV) can be considered the most important swine pathogen of contemporary times. Current control and eradication strategies against PRRSV have difficulty succeeding because of their complex nature and the absence of an effective vaccine. A major obstacle for PRRSV vaccine development is the broad heterogeneity of the virus, both at the genetic and antigenic level, its rapid evolution, and an incomplete knowledge of the immune responses responsible for clearing the virus from the host. Parkinson’s disease (PD) is a progressive and debilitating neurodegenerative disorder that affects over one million people in the United States. Previous studies, carried out in young adult rats, have shown that calcitriol, the active metabolite of vitamin D, can be neuroprotective in 6-hydroxydopamine (6-OHDA) models of PD. However, as PD usually affects older individuals, the ability of calcitriol to promote dopaminergic recovery was examined in lesioned young adult (4 month old), middle-aged (14 month old) and aged (22 month old) rats. Background: Key elements in the clinical practice of prevention, health and wellness are best cultivated in medical professionals during undergraduate medical training. This study explores students' self-assessed stress relative to gender, academic expectations, and level of medical training to guide development of targeted wellness interventions. Methods: In early 2012, undergraduate (M1-M4) students in four Southeastern U.S. allopathic medical schools were surveyed about health-related attitudes and behaviors. Results: A total of 575 students returned completed questionnaires. Students in the preclinical years (M1-M2), especially females, reported significantly higher stress levels. Academic expectations and satisfaction were also significantly implicated. Discussion: These findings highlight …

Background—Disrupted glutamate homeostasis is thought to contribute to cocaine-use disorder, in particular, by enhancing the incentive salience of cocaine stimuli. n-Acetylcysteine might be useful in cocaine-use disorder by normalizing glutamate function. In prior studies, n-acetylcysteine blocked the reinstatement of cocaine seeking in laboratory animals and reduced the salience of cocaine stimuli and delayed relapse in humans.
Methods—The present study determined the ability of maintenance on n-acetylcysteine (0 or 2400 mg/day, counterbalanced) to reduce the incentive salience of cocaine stimuli, as measured by an attentional bias task, and attenuate intranasal cocaine self-administration (0, 30, and 60 mg). Background—Hepatitis C virus (HCV) remains a major contributor to morbidity and mortality worldwide. Since 2009, Kentucky has led the United States in cases of acute HCV, driven largely by injection drug use in rural areas. Improved treatment regimens hold promise of mitigating the impact and transmission of HCV, but numerous barriers obstruct people who inject drugs (PWID) from receiving care, particularly in medically underserved settings.

Background: Most metastatic spinal cord lesions are located either in the intradural, extramedullary, or in the epidural compartments. Intramedullary spinal cord metastasis (ISCM) is a rare central nervous system spread of cancer. The aim of this report was to evaluate ISCM in the published literature.
Methods: A literature review of PubMed from 1960 to 2016 was undertaken for the publications having demographic, clinical, histological, and outcome data.
Results: A total of 59 relevant papers were identified, showing 128 cases of intramedullary metastasis from lung cancer. The incidence of lung cancer as the primary malignancy with intramedullary metastasis was 56%. The …

Background—Among early adolescents in the United States (U.S.), the prevalence of cigarette smoking is at its lowest level in recent decades. Nonetheless, given the risks of smoking in early development, it remains critically important to study both risk factors for smoking and risks from smoking. This longitudinal study with U.S. early adolescents examines smoking initiation and tests a model of reciprocal prediction between ever smoking and the personality trait of urgency (i.e., mood-based impulsivity), a trait that increases risk for multiple forms of dysfunction.

Motives for substance use have garnered considerable attention due to the strong predictive utility of this construct, both in terms of use and problems associated with use. The current study examined the cross-lagged relations between alcohol use and motives, and marijuana use and motives over three yearly assessment periods in a large sample (N = 526, 48% male) of college students. The relations between substance use and motives were assessed at each time point, allowing for the examination of these inter-relations over time. Increased ruminative style of thought has been well documented in borderline personality disorder (BPD); however, less is known about how the content of rumination relates to domains of BPD features. Relationships between forms of rumination and BPD features were examined in an undergraduate sample with a wide range of BPD features. Participants completed self-report measures of rumination and a free-writing task about their repetitive thought. Rumination on specific themes, including anger rumination, depressive brooding, rumination on interpersonal situations, anxious rumination, and stress-reactive rumination were significantly associated with most BPD features after controlling for general rumination. Thiamine (vitamin B1) is an essential nutrient and indispensable for normal growth and development of the organism due to its multilateral participation in key biochemical and physiological processes. Humans must obtain thiamine from their diet since it is synthesized only in bacteria, fungi, and plants. Thiamine deficiency (TD) can result from inadequate intake, increased requirement, excessive deletion, and chronic alcohol consumption. TD affects multiple organ systems, including the cardiovascular, muscular, gastrointestinal, and central and peripheral nervous systems. In the brain, TD causes a cascade of events including mild impairment of oxidative metabolism, neuroinflammation, and neurodegeneration, which are commonly observed in …

Objectives. To determine the post-procedure acceptability of self-collecting a vaginal swab for HPV testing among a highly impoverished and geographically isolated population of medically underserved Black women residing in the Mississippi Delta. Further, to test correlates of reporting that self-collection is preferred over Pap testing. Finally, to determine the prevalence of any of 13 high-risk HPV types among this population and the correlates of testing positive.
Methods. Eighty-eight women were recruited from two churches located in different towns of the Mississippi Delta. After completing a survey, women were provided instructions for self-collecting a cervico-vaginal swab and completing a post-collection survey. …

A recent victory with respect to teen sexual behavior is the reduction of the national teen birthrate. In 1991, there were 61.8 births per 1000 females aged 15-19. By 2014, this rate plummeted to 24.2 births per 1000 adolescent females. Averages, however, do not reflect state-level variation. Spontaneous bacterial peritonitis (SBP) is a recognized cause of morbidity and mortality in cirrhotic patients. Enterobacteriaceae have been isolated from the majority of peritonitis cases and the gram negative aerobe Escherichia coli is the most commonly isolated organism. Anaerobic organisms are rarely isolated because of the high oxygen tension in ascetic fluid. We report a patient with a history of alcoholic cirrhosis who developed SBP and concurrent bacteremia with the anaerobe Clostridium tertium. High-level, top-down information such as linguistic knowledge is a salient cortical resource that influences speech perception under most listening conditions. But, are all listeners able to exploit these resources for speech facilitation to the same extent? It was found that children with cochlear implants showed different patterns of benefit from contextual information in speech perception compared with their normal-haring peers. การล่วงละเมิดทางเพศเป็นปรากฏการณ์หนึ่งทางสังคม ซึ่งส่งผลกระทบอย่างกว้างขวางสร้างความหวาดกลัว ตื่นตระหนก และรบกวนการใช้ชีวิตของประชาชนเป็นอย่างมาก การดูแลผู้ถูกล่วงละเมิดทางเพศในอดีตเป็นการดูแลรักษาผู้ป่วยแบบแยกส่วน มุ่งเน้นการรักษาและป้องกันโรคทางกาย ปัจจุบันการรักษาเปลี่ยนรูปแบบไปเป็นการดูแลผู้ป่วยแบบองค์รวมทั้งทางด้านร่างกายจิตใจ สังคม และจิตวิญญาณ อย่างไรก็ตามการเยียวยาด้วยกระบวนการทางกฎหมายยังเป็นสิ่งที่ถูกละเลย เนื่องจากแพทย์จำนวนมากขาดความรู้ความเข้าใจในเรื่องดังกล่าว ทำให้การดูแลรักษาผู้ป่วยในส่วนของแพทย์สิ้นสุดลงโดยปราศจากการติดตามผู้ป่วยไปในชั้นพิจารณาคดี และชั้นการบังคับคดีให้เป็นไปตามคำพิพากษาของศาล ส่วนทางฝั่งของกระบวนการยุติธรรมในอดีตนั้นมุ่งให้ความสำคัญกับการล่วงละเมิดทางเพศของผู้เสียหายที่มีเพศตรงข้ามกับผู้กระทำผิด โดยละเลยการกระทำความรุนแรงทางเพศระหว่างสามีกับภรรยา และผู้เสียหายที่มีเพศเดียวกันกับผู้กระทำผิดทั้งยังมุ่งเน้นการลงโทษผู้กระทำความผิดมากกว่าการสร้างภูมิคุ้มกันทางสังคม และปรับพฤติกรรมของอาชญากร ต่อมาจึงได้มีการพัฒนากฎหมายให้ครอบคลุมถึงผู้เสียหายทุกกลุ่ม รวมทั้งมุ่งเน้นการแก้ไขฟื้นฟูผู้กระทำผิดมากยิ่งขึ้น บทความนี้จึงเป็นการรวบรวมความรู้ระหว่างการแพทย์และกฎหมาย เพื่อให้ผู้อ่านได้เข้าใจถึงวิธีการดูแลผู้ป่วยถูกล่วงละเมิดทางเพศในด้านการตรวจร่างกายการตรวจทางห้องปฏิบัติการ การรักษาแบบองค์รวม และการเยียวยาทางกฎหมาย เพื่อให้บุคลากรทางด้านสาธารณสุขและทางด้านกฎหมายมีความเข้าใจปัญหาการถูกล่วงละเมิดทางเพศอย่างชัดเจนยิ่งขึ้น.

เหตุผลของการวิจัย : การใช้เวลาที่มีคุณภาพในครอบครัว เป็นการใช้ช่วงเวลาระหว่างสมาชิกในครอบครัวที่ก่อให้เกิดผลดี มีความสำคัญต่อสัมพันธภาพในครอบครัว อย่างไรก็ตาม ยังไม่พบการศึกษาเชิงปริมาณเกี่ยวกับการใช้เวลาที่มีคุณภาพของครอบครัวในประเทศไทยวัตถุประสงค์ : เพื่อศึกษาการใช้เวลาที่มีคุณภาพในครอบครัว และค้นหาปัจจัยที่เกี่ยวข้องกับการใช้เวลาที่มีคุณภาพในครอบครัวของนักเรียนวัยรุ่นวิธีการทำวิจัย : การวิจัยนี้จัดทำขึ้นระหว่างเดือนพฤศจิกายน ถึง ธันวาคม พ.ศ. 2557โดยรวบรวมข้อมูลจากนักเรียนในระดับมัธยมศึกษาตอนต้นจำนวน348 ราย ผู้เข้าร่วมการวิจัยตอบแบบสอบถาม ซึ่งประกอบด้วยแบบสอบถามข้อมูลส่วนบุคคลและแบบวัดการใช้เวลาคุณภาพในครอบครัว ใช้สถิติเชิงพรรณนาเพื่อบรรยายข้อมูลส่วนบุคคล ใช้สถิติไคสแควร์ และ Crude Odd Ratio (95%CI) ที่ระดับนัยสำคัญทางสถิติที่ 0.05 เพื่อศึกษาความสัมพันธ์ระหว่างแต่ละตัวแปรกับการใช้เวลาที่มีคุณภาพในครอบครัวจากนั้นใช้การวิเคราะห์การถดถอยเชิงพหุเพื่อวิเคราะห์ปัจจัยทำนายการใช้เวลาที่มีคุณภาพในครอบครัวในระดับต่ำผลการศึกษา : นักเรียนทั้งหมด 348 ราย พบว่ามีการใช้เวลาที่มีคุณภาพในครอบครัวระดับต่ำ กลาง สูง จำนวน 59 ราย (ร้อยละ 16.9) 170 ราย(ร้อยละ 48.9) และ 119 ราย (ร้อยละ 34.2) ตามลำดับ เมื่อวิเคราะห์ด้วยการวิเคราะห์การถดถอยเชิงพหุ พบว่าตัวแปรที่มีความทำนายความเสี่ยงการใช้เวลาที่มีคุณภาพในระดับต่ำ ได้แก่ ระดับผลการเรียนเฉลี่ยที่ต่ำ และสถานภาพสมรสระหว่างบิดากับมารดาเป็นแบบหย่าร้างหรือแยกกันอยู่สรุป : จากการวิจัย พบว่านักเรียนชั้นมัธยมศึกษาตอนต้นในโรงเรียนสังกัดกรุงเทพมหานครส่วนใหญ่มีคะแนนการใช้เวลาที่มีคุณภาพในครอบครัวอยู่ในระดับปานกลางและสูง เนื่องจากนักเรียนที่มีผู้ปกครองหย่าร้างหรือแยกกันอยู่ มีการใช้เวลาที่มีคุณภาพในครอบครัวระดับต่ำ จึงควรส่งเสริมให้ผู้ปกครองที่เลี้ยงบุตรคนเดียวได้เรียนรู้และจัดให้มีช่วงเวลาที่มีคุณภาพในครอบครัวให้มากขึ้นการสนับสนุนทางสังคมเป็นสิ่งจำเป็นเพื่อช่วยเหลือผู้ปกครองเลี้ยงเดี่ยวให้สามารถจัดสรรเวลาที่มีคุณภาพสำหรับบุตรได้.

เหตุผลของการทำวิจัย : การปรับตัว เป็นการแก้ปัญหาเพื่อให้ชีวิตมีความสุข หากแก้ปัญหาหรือปรับตัวไม่ได้ อาจส่งผลให้เกิดปัญหาต่าง ๆ ในการเรียน การทำงานตลอดจนปัญหาการปรับตัวในเรื่องอื่น ๆ การศึกษาการปรับตัวของนักศึกษาแพทย์สามารถนำไปใช้เป็นแนวทางในการวางแผนป้องกันและแก้ไขปัญหา ตลอดจนให้คำแนะนำแก่นักศึกษาแพทย์ได้อย่างถูกต้องวัตถุประสงค์ : เพื่อศึกษาการปรับตัวและปัจจัยที่เกี่ยวข้องกับการปรับตัวของนักศึกษาแพทย์ชั้นปีที่ 1 คณะแพทยศาสตร์ศิริราชพยาบาลมหาวิทยาลัยมหิดลวิธีการทำวิจัย : โดยเก็บรวบรวมข้อมูลจากนักศึกษาแพทย์ชั้นปีที่ 1 คณะแพทยศาสตร์ศิริราชพยาบาล มหาวิทยาลัยมหิดล จำนวน 284 ราย เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) แบบสอบถามข้อมูลพื้นฐาน 2) แบบประเมินความฉลาดทางอารมณ์ของกรมสุขภาพจิต 3) แบบสำรวจปัญหาการปรับตัว ใช้สถิติเชิงพรรณนาในการอธิบายลักษณะทั่วไปของประชากร ได้แก่ ค่าสัดส่วนร้อยละ ค่าเฉลี่ยเลขคณิต และส่วนเบี่ยงเบนมาตรฐาน และใช้สถิติเชิงอนุมานในการทดสอบความสัมพันธ์ของปัญหาการปรับตัวกับปัจจัยส่วนบุคคลและความฉลาดทางอารมณ์โดยใช้สถิติ Pearson’s correlation coefficient, Independentsamples t-test, One way analysis of variance และ multipleregression analysisผลการศึกษา : กลุ่มตัวอย่างเป็นเพศชาย 150 ราย (ร้อยละ 52.8) และเป็นเพศหญิง134 ราย (ร้อยละ 47.2) มีอายุเฉลี่ย 19 ปี ส่วนใหญ่มีปัญหาการปรับตัวโดยรวมอยู่ในระดับต่ำ (ค่าเฉลี่ย 1.93 จากคะแนนเต็ม3) มีคะแนนความฉลาดทางอารมณ์ในด้านรวม อยู่ในเกณฑ์ปกติโดยปัจจัยที่มีความสัมพันธ์และทำนายปัญหาการปรับตัว คือความฉลาดทางอารมณ์ ซึ่งสามารถทำนายได้ร้อยละ 36.7สรุป : นักศึกษาแพทย์ชั้นปีที่ 1 คณะแพทยศาสตร์ศิริราชพยาบาลมหาวิทยาลัยมหิดล ส่วนใหญ่มีปัญหาในการปรับตัวโดยรวมอยู่ในระดับต่ำ มีความฉลาดทางอารมณ์อยู่ในเกณฑ์ปกติ และความฉลาดทางอารมณ์มีความสัมพันธ์ และเป็นปัจจัยทำนายปัญหาการปรับตัว.

Irreversible electroporation (IRE) as a non-thermal tumor ablation technology has been studied for the treatment of pancreatic carcinoma and has shown a significant survival benefit. We discovered that moderate heating (MH) at 43°C for 1-2 minutes significantly enhanced ex vivo IRE tumor ablation of Pan02 cells by 5.67-fold at 750 V/cm and by 1.67-fold at 1500 V/cm. This amount of heating alone did not cause cell death. An integrated IRE system with controllable laser heating and tumor impedance monitoring was developed to treat mouse ectopic pancreatic cancer. Following traumatic brain injury (TBI), treatment with rapamycin suppresses mammalian (mechanistic) target of rapamycin (mTOR) activity and specific components of hippocampal synaptic reorganization associated with altered cortical excitability and seizure susceptibility. Reemergence of seizures after cessation of rapamycin treatment suggests, however, an incomplete suppression of epileptogenesis. Hilar inhibitory interneurons regulate dentate granule cell (DGC) activity, and de novo synaptic input from both DGCs and CA3 pyramidal cells after TBI increases their excitability but effects of rapamycin treatment on the injury-induced plasticity of interneurons is only partially described.
