INTRODUCTION: The search for effective therapeutic interventions for sickle cell disease (SCD) has been an ongoing endeavor for over 50 years. During this period, only hydroxyurea (HU), which received US FDA approval in February 1998, was identified as an effective therapeutic agent in preventing or ameliorating the frequency of vaso-occlusive crises, acute chest syndrome and the need for blood transfusion. Approximately 25% of patients with sickle cell anemia (SCA), however, do not respond to HU and some patients experiencing serious side effects of this chemotherapeutic agent. Background

Data on the influence of obesity on atherosclerosis in Hispanics are inconsistent, possibly related to varying cardiometabolic risk among obese individuals. We aimed to determine the association of obesity and cardiometabolic risk with subclinical atherosclerosis in Mexican‐Americans.

Methods and Results

Participants (n=503) were drawn from the Cameron County Hispanic Cohort. Metabolic health was defined as <2 of the following: blood pressure ≥130/85; triglyceride ≥150 mg/dL; high‐density lipoprotein cholesterol <40 mg/dL (men) or <50 mg/dL (women); fasting glucose ≥100 mg/dL; homeostasis model assessment of insulin resistance value >5.13; or high‐sensitivity C‐reactive protein >3 mg/L. Carotid intima media thickness (cIMT) was measured. In vitro antibacterial activity of Bacillus polyfermenticus (BP) against tilapia streptococcal pathogen was evaluated using the agar dilution method and co-incubation method. Both in vitro tests confirmed the inhibitory effect of B. polyfermenticus on the growth of Streptococcus agalactiae. The suppression of S. agalactiae was observed in the test system containing B. polyfermenticus and the inhibition was more evident in the system with high density of B. polyfermenticus. Subsequently, feeding administration of BP was employed in tilapia broodstocks (12 months old) and fry (37.33 ± 1.37 mg body weight) to examine the effect of BP on health performance. In Vitro Modulatory Effects Of Quercetin On Bovine Neutrophil Effector Functions, Phongsakorn Chuammitri, Chiradet Amphaiphan, Panupong Nojit

The Thai Journal of Veterinary Medicine

Bovine neutrophils perform numerous effector functions to overcome microbial invasions by utilizing innately-equipped intracellular and extracellular killing mechanisms. In this study, we explored the modulatory effects of quercetin hydrate (QH), an herbal flavonoid compound, on bovine neutrophil functional activities. Isolated neutrophils were incubated with various concentrations of QH (0-100 mM). Neutrophil viability results showed no signs of cytotoxicity (p = 0.33). In other assays, neutrophils were stimulated with a pre-defined concentration of QH (50 mM). Results showed promising effects of QH on enhancing intracellular ROS generation (p < 0.0001) compared to control. Phagocytic activity of fluorescently labeled E.coli was also augmented by the effects of this compound (p = 0.05). However, intracellular killing of live E.coli quantified by MTT assay demonstrated a slight decrease in activity (p = 0.304). Additionally, extracellular killing driven by NET-DNA release was remarkably higher in QH-stimulated cells (p = 0.0004). Supplementation of QH to LPS stimulated neutrophils resulted in significantly decreased pro-inflammatory cytokines, IL-1b (p < 0.0001) and TNF-a gene expression (p = 0.004). Taken together, quercetin could exert its noteworthy actions, both at cellular and molecular levels, at a certain physiological concentration.

An acute toxicopathological study of silver nanoparticles (AgNPs) was conducted in concentrations of 0, 1, 10 and 100 ppm in adult tilapia (Oreochromis niloticus) at days 0, 1, 2, 3 and 4. Mortality, clinical sign, histopathology and immunohistochemistry were observed and evaluated. Calculated LC50 at 24 hr was 53 ppm. The major clinical sign was respiratory distress. Histopathologic lesions were found mainly in gill, kidney, spleen and liver. Severity of the lesions depended on the concentration and exposure time. Superoxide dismutase (SOD) positive immunostaning was found in gill and renal tubular epithelium. Metallothionein (MT) positive staining was observed in renal …

Studies on nonelite distance runners suggest that men are more likely than women to slow their pace in a marathon.

Purpose: This study determined the reliability of the sex difference in pacing across many marathons and after adjusting women's performances by 12% to address men's greater maximal oxygen uptake and also incorporating information on racing experience.

Methods: Data were acquired from 14 US marathons in 2011 and encompassed 91,929 performances. For 2929 runners, we obtained experience data from a race-aggregating Web site. We operationalized pace maintenance as the percentage change in pace observed in the second half of the marathon …

Introduction: Stroke-related changes in maximal dynamic hip flexor muscle fatigability may be more relevant functionally than isometric hip flexor fatigability. Methods: Ten chronic stroke survivors performed 5 sets of 30 hip flexion maximal dynamic voluntary contractions (MDVC). A maximal isometric voluntary contraction (MIVC) was performed before and after completion of the dynamic contractions. Both the paretic and nonparetic legs were tested. Results: Reduction in hip flexion MDVC torque in the paretic leg (44.7%) was larger than the nonparetic leg (31.7%). Background: Detailed cost analysis is an important tool for review of health policy and reforms. We provide an estimate of cost of service and its detailed breakup on out-door patient visits (OPV) to basic health units (BHU) in Pakistan.

Method: Six BHUs were randomly selected from each of the five districts in Khyber Pukhtonkhawa (KPK) and two agencies in Federally Administered Tribal Areas (FATA) of Pakistan for this study. BACKGROUND: The impact of surgical conditions on global health, particularly on vulnerable populations, is gaining recognition. However, only 3.5% of the 234.2 million cases per year of major surgery are performed in countries where the world's poorest third reside, such as the Democratic Republic of the Congo (DRC).

Sjögren's Syndrome (SS) is an autoimmune disease that causes extreme dryness, or sicca,of the eyes and mouth, as well as other potential drying of the throat and intestines. Speech, voice, and swallowing problems are common in individuals with SS. Therefore, this study examined the possible changes in acoustic characteristics of monophthongs (/i, æ, α, u, ʌ/) in eight females with SS following laryngeal hydration treatments. Intricate predatory strategies are widespread in the salticid subfamily Spartaeinae. The hypothesis we consider here is that the spartaeine species that are proficient at solving prey-capture problems are also proficient at solving novel problems. We used nine species from this subfamily in our experiments. Eight of these species (two Brettus, one Cocalus, three Cyrba, two Portia) are known for specialized invasion of other spiders’ webs and for actively choosing other spiders as preferred prey (‘araneophagy’). BACKGROUND:

Mycobacterium tuberculosis (MTB) PE_PGRS genes belong to the PE multigene family. Although the function of PE_PGRS genes is unknown, it is hypothesized that the PE_PGRS genes may be associated with antigenic variability in MTB.

MATERIAL AND METHODS:

Whole genome sequencing analysis was performed on (n=37) extensively drug-resistant (XDR) MTB strains from Pakistan, which included Lineage 1 (East African Indian, n=2); Other lineage 1 (n=3); Lineage 3 (Central Asian, n=24); Other lineage 3 (n=4); Lineage 4 (X3, n=1) and T group (n=3) MTB strains.

Research suggests that relationship education produces positive outcomes with some groups. However, whether populations who are socially and economically more at-risk of relationship problems and relationship dissolution are gaining from relationship education remains an open question. Objective: To determine the difference in Blood Pressure (BP) readings taken before, during and after the clinic encounter.

Study Design: Descriptive study.

Place and Duration of Study: Cardiology Clinic, The Aga Khan University Hospital, Karachi, from January to August 2013.

Methodology: Hypertensive and normotensive participants aged ≥ 18 years were recruited. Pre-clinic BP was measured by a nurse and in-clinic BP by a physician. After 15 minutes, two post-clinic BP readings were taken at 1 minute interval. All readings were taken using Omron HEM7221-E.

OBJECTIVES: To evaluate the effect of low dose Albumin i.e. 4 grams per litre of ascitic fluid after large volume paracentesis (LVP) for the prevention of paracentesis induced circulatory dysfunction (PICD) related renal impairment in cirrhosis.

METHODS: Case records of all patients with cirrhosis who underwent LVP from January 12(th), 2011 till December 29(th), 2013 were reviewed. Patients were excluded if they had spontaneous bacterial peritonitis, creatinine >1.5 mg/dl, hepatoma or if volume of ascitic fluid removed was <5 litres. Data including age, gender, cause of cirrhosis, CTP score and volume of ascitic fluid drained were noted. In addition serum creatinine and serum sodium at baseline and one week post paracentesis were recorded.

Both approval and disapproval of one's social network have been shown to predict relationship outcomes. Additional research has shown that attachment can buffer the negative effects of various factors (e.g., depression) on relationships. This thesis researches the effects of disapproval of friends and family and attachment on relationship outcomes. More specifically this study looks at the potential moderating effects of couple-specific attachment behaviors on the relationship between social network disapproval and relationship quality. OBJECTIVE: To describe the use of antimicrobial stewardship policies and to investigate factors associated with implementation in a national sample of acute care hospitals.

DESIGN: Cross-sectional survey.

PARTICIPANTS: Infection Control Directors from acute care hospitals participating in the National Healthcare Safety Network (NHSN).

METHODS: An online survey was conducted in the Fall of 2011. A subset of hospitals also provided access to their 2011 NHSN annual survey data.

Raman spectroscopy is fast becoming a valuable analytical tool in a number of biomedical scenarios, most notably disease diagnostics. Importantly, the technique has also shown increasing promise in the assessment of drug interactions on a cellular and subcellular level, particularly when coupled with multivariate statistical analysis. We present an in-depth case study of GVK Emergency Management and Research Institute, an Indian public–private partnership (PPP), which successfully brought emergency medical response to remote and urban settings. Drawing insights from the case, we investigate how the organization established itself through institutional entrepreneurship using a process conceptualized as opportunity framing, entrenchment, and propagation. The case and context highlight the need for innovation in organizational design and governance modes to create a new opportunity that connects state actors, private healthcare providers, and the public at large.
