Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an acquired, autoimmune peripheral neuropathy. It has an insidious disease progression resulting in a debilitating illness. It is a well known neurological disorder. The causative factors are elucidating and it is generally considered idiopathic. However, its’ associations with various systemic disorders is well established albeit under recognized, especially with lupus as evident by few of the case reports/ series published in the recent past. The aim of this report is to highlight this neglected but important aspect of clinical neurology in common practice.

“Eight-and-half” syndrome is “one-and-a-half” syndrome characterized by conjugated horizontal gaze palsy and internuclearopthalmoplegia plus ipsilateral fascicular seventh nerve palsy. We report a case of 50 year old woman who presented with right eight and half syndrome together with contralateral hemiparesis and hemihypesthesia. Non contrast CT scan brain showed hyperdensesignal on right pontine region. MRI brain plain revealed hemorrhagicpontinetegmentuminfarction as well as corticospinal tract and medial meniscus. This spectrum of presentation suggests a possible “Nine syndrome” as a novel neuropthalmicpontine syndrome. The first of its kind ever reported from Pakistan

Tuberculosis of central nervous system is not uncommon in children of developing countries. Introduction:
Bomb blast (BB) injuries outside war zones were a rare phenomenon until recently. With the spreading wave of terrorism across the globe, BB related trauma is resurfacing. Explosions can produce unique patterns of neuromuscular injury. No recent data exists categorizing such injuries.
Methods:
Medical records and electrodiagnostic findings of 20 patients with BB related neuromuscular injuries (NMI) were reviewed retrospectively.
Results:
Most common site of injury was the lower extremities (55%) with majority presenting clinically with foot drop (40%) or weakness (30%). 9/17 patients (45%) had associated shrapnel or penetrating trauma. 6/20 patients had associated fractures. The most …

การตรวจทางห้องปฏิบัติการ ณ จุดดูแลผู้ป่วย (POCT) ทำให้เพิ่มประสิทธิภาพในการดูแลรักษา การตรวจวิเคราะห์นอกห้องปฏิบัติการดังกล่าวยังต้องให้ความสำคัญ และคำนึงถึงระบบคุณภาพที่เน้นความถูกต้อง แม่นยำ ของการตรวจวิเคราะห์ดังกล่าวด้วย เนื่องจากการใช้ POCT ที่ไม่ได้มีระบบคุณภาพรองรับอาจส่งผลต่อความปลอดภัยในการดูแลผู้ป่วยได้ ความผิดพลาดในการวิเคราะห์อาจเกิดได้ทั้งในขั้นตอน pre-analytic, analytic และ post-analytic มาตรฐานในระดับสากลที่นำมาใช้รับรองคุณภาพของ POCT คือ มาตรฐาน ISO 22870:2006 ซึ่งประกอบด้วยข้อกำหนดด้านการบริหารจัดการ 14 ข้อ และข้อกำหนดด้านวิชาการ 8 ข้อ ซึ่งมาตรฐานการดังกล่าวเพื่อให้สามารถตรวจวิเคราะห์ POCT ได้อย่างถูกต้องปลอดภัย เพื่อเพิ่มคุณภาพการบริการผู้ป่วย.

โรคมะเร็งเต้านมเป็นมะเร็งที่พบบ่อยเป็นอันดับหนึ่งในสตรีทั่วโลก การตรวจคัดกรองที่ดีขึ้นสามารถลดอัตราการตายได้ ปัญหาการตรวจคัดกรองมะเร็งเต้านมของประเทศไทยในปัจจุบัน คือแบบประเมินความเสี่ยงต่าง ๆ ถูกสร้างจากฐานข้อมูลประชากรผิวขาว ทำให้ไม่มีประสิทธิภาพกับประชากรเอเชีย และปัญหาเทคโนโลยีภาพถ่ายรังสีเต้านมมีข้อจำกัด ในประชากรไทยการนำ breastMRI มาใช้สามารถลดข้อจำกัดการตรวจคัดกรองในปัจจุบันได้ แต่ breast MRI มีข้อเสียเนื่องจากราคาสูงและเกิด false positive ได้ง่าย ดังนั้นจึงควรมีมาตรการการคัดเลือกผู้ที่มีความจำเป็นที่ดีจึงจะเกิดประโยชน์สูงสุด.

Background: The Healthy People 2020 goal for the public health system is “to ensure that Federal, State, Tribal, and local health agencies have the necessary infrastructure to effectively provide essential public health services.” To address this goal, Missouri established the first statewide, voluntary accreditation program of local health departments (LHDs) and began accrediting the LHDs in 2003. The purpose of this study was to identify organizational, structural, and workforce factors related to accreditation status of LHDs in Missouri.

US state and territorial laws were reviewed to identify Clostridium difficile infection reporting mandates. Twenty states require reporting either under state law or by incorporating federal Centers for Medicare & Medicaid Services' reporting requirements. Although state law mandates are more common, the incorporation of federal reporting requirements has been increasing.

The process of acquiring language requires children to learn grammatical categories and apply these categories to new words. Researchers have proposed various explanations of this process in the form of algorithms and computational modeling. Recently, adaptation and selection models have been tested and applied as a possible explanation to the process of acquiring grammatical categories. These studies have proven promising, however, the external validity of this approach has not been examined by grammatically coding samples outside the training corpus. It is understood that children learn the use of grammatical categories in their native language, and previous models have only been partially successful in describing this acquisition. The present study uses an adaptation selection algorithm to continue the work in addressing this question. The input for the computer model is child-directed speech towards three children, ages ranging from 1;1 to 5;1 during the course of sampling. The output of the model consists of the input words labeled with a grammatical category. Introduction: Identifying the need for strengthening of the immune system, the investigator has developed new Human Immunodeficiency Virus (HIV) nosode and evaluated its effect on HIV positive individuals through a clinical trial. Methods: Standardized and the scientific method of HIV nosode preparation has been described and documented. Thirty-seven HIV-infected persons were registered for the trial, and ten participants were dropped out from the study, so the effect of HIV nosode 30C and 50C, was concluded on 27 participants under the trial. A vestibular schwannoma, often called an acoustic neuroma/schwannoma, is a benign primary intracranial tumor of the myelin-forming cells of the vestibulo-cochlear nerve (8 th cranial nerve). This tumor arises from the Schwann cells responsible for the myelin sheath that helps keep peripheral nerves insulated. [1] Approximately, 3000 cases are diagnosed each year in the United States with a prevalence of about 1 in 100,000 worldwide. It comprises 5-10% of all intracranial neoplasms in adults. BACKGROUND: Patients with free pulmonary regurgitation or mixed pulmonary stenosis and regurgitation and severely dilated right ventricles (RV) show little improvement in ventricular function after pulmonary valve replacement when assessed by traditional echocardiographic markers. We evaluated changes in right and left ventricular (LV) function using speckle tracking echocardiography in patients after SAPIEN transcatheter pulmonary valve (TPV) placement.
METHODS: Echocardiograms were evaluated at baseline, discharge, 1 and 6 months after TPV placement in 24 patients from 4 centers. Researchers designing clinical trials often specify usual care received by participants as the control condition expecting that all participants receive usual care regardless of group assignment. We describe the assessment of usual care within the 16 studies in MACH 14, a multi-site collaboration on adherence to antiretroviral therapy. Only five of the studies in MACH 14 assessed usual care. Assessment protocols varied as did the timing and frequency of assessments. All usual care assessments addressed patient education focused on HIV, HIV medications, and medication adherence. Malaria is a leading cause of pediatric mortality, and Uganda has among the highest incidences in the world. Increased morbidity and mortality are associated with delays to care. This qualitative study sought to characterize barriers to prompt allopathic care for children hospitalized with severe malaria in the endemic region of southwestern Uganda. Minimally structured, qualitative interviews were conducted with guardians of children admitted to a regional hospital with severe malaria. Using an inductive and content analytic approach, transcripts were analyzed to identify and define categories that explain delayed care. OBJECTIVE:
To assess how the frequency, nature and outcome of road traffic crashes differ during the fasting month of Ramadan.
METHODS:
The retrospective study was conducted in Karachi and comprised data from the Road Traffic Injury Surveillance Project which entailed information on all road traffic injury victims presenting to Emergency Departments in the city between September 2006 and September 2011. The pharmacognostic profile of crude drug has a key role in standardization for quality, purity and drug identification. The present study deals with pharmacognostic evaluation of aerial part of Chamaecyparis lawsoniana (Murr.) Parl. a drug used in homoeopathic system of medicine for diverse clinical uses such as terrible pain in stomach, tumors, keloid, warts and lipoma of thigh. The study includes collection, identification, macroscopy, microscopy and organoleptic characteristics of aerial part of Chamaecyparis lawsoniana. Objectives: The objective of present preliminary study was to assess the anti-inflammatory, analgesic and anti-arthritic effect of some homoeopathic mother tinctures viz. Ricinus communis (RCMT), Rauwolfia serpentina (RSMT), Bellis perennis (BPMT), Curcuma longa (CLMT), Terminalia arjuna (TAMT) and Tribulus terresteris (TTMT). Materials and Methods: Paw oedema was induced by administration of 0.1ml 1% carrageenan in normal saline into right hind paw. Degree of inflammation was evaluated according to paw swelling. Children with childhood apraxia of speech (CAS) have been hypothesized to continuously monitor their speech through auditory feedback to minimize speech errors. We used an auditory masking paradigm to determine the effect of attenuating auditory feedback on speech in 30 children: 9 with CAS, 10 with speech delay, and 11 with typical development. The masking only affected the speech of children with CAS as measured by voice onset time and vowel space area. These findings provide preliminary support for greater reliance on auditory feedback among children with CAS.

The eye generally is considered to be an immune-privileged organ, but this notion is being increasingly challenged as ocular antigens can be expressed in the generative lymphoid organs, resulting in attainment of self-tolerance. What triggers a break in this tolerant state is a fundamental question in autoimmunity research. The general belief is that exposure to environmental microbes can break self-tolerance in genetically susceptible individuals, leading to the induction of autoimmune responses. The molecular mimicry hypothesis has been proposed as one major mechanistic, pathway through which microbes, by generating crossreactive immune responses, can induce ocular damage of the kind that might …
