BACKGROUND: Pakistan has been hugely struck with massive bomb explosions (car and suicide bombs) resulting in multiple casualties in the past few years. The aim of this study is to present the patterns of skeletal and soft tissue injuries and to review the outcome of the victims who presented to our hospital. METHODS: This is a retrospective chart review from January 2008 to December 2012. The medical record numbers of patients were obtained from the hospital Health Information and Management Sciences (HIMS) as per the ICD-9 coding. Background : Although the results of several studies support the safety of live kidney donation, there is no functional deterioration, some donors have proteinuria and hypertension (HT). Most of the studies were done in Europe and America, in Asia, however, the information about donors living with one kidney is lacking. โรคตับคั่งไขมันเป็นโรคที่พบบ่อยมากขึ้นในปัจจุบัน โดยโรคนี้มีความสัมพันธ์กับโรคอ้วนลงพุงและโรคหัวใจ โรคตับคั่งไขมันสามารถแบ่งได้เป็น 2 กลุ่มย่อยคือ กลุ่มที่มีตับคั่งไขมันอย่างเดียว และกลุ่มที่มีการอักเสบร่วมด้วย ซึ่งในกลุ่มนี้จะอัตราตายที่เพิ่มขึ้นรวมทั้งสามารถพัฒนากลายเป็นโรคตับแข็งและมะเร็งตับได้ สำหรับไมโครอาร์เอ็นเอนั้นมีส่วนสำคัญในการกระบวนการต่าง ๆ ของเซลล์ทั้งในแง่สรีรวิทยาและพยาธิวิทยาเช่น การแบ่งตัว การอักเสบ หรือการตายของเซลล์ ไมโครอาร์เอ็นเอหลายชนิดถูกแสดงว่ามีส่วนเกี่ยวข้องกับพยาธิกำเนิดของโรคตับหลายชนิดโดยเฉพาะโรคตับคั่งไขมันข้อมูลหลายการศึกษาแสดงให้เห็นว่า miR-122 มีบทบาทสำคัญในการควบคุมเมตาบอลิสมของไขมันในตับ การให้สารยับยั้งต่อ miR-122 ถูกคาดหวังว่าจะใช้ในการรักษาเพื่อลดระดับคลอเลสเตอรอลในเลือด แต่ข้อมูลยังคงมีความขัดแย้งกันอยู่ นอกจากนี้ miR-33 ยังเป็นไมโครอาร์เอ็นเอที่ควบคุมสมดุลของคลอเลสเตอรอลและกรดไขมันอีกด้วย ส่วน miR-34a เป็นไมโครอาร์เอ็นเอที่ได้รับความสนใจว่าอาจมีบทบาทสำคัญต่อการอักเสบของตับผ่านทาง miR-34a/SIRT1/p53/ apoptosis pathwayและควบคุมการทำงานของ HMGCoA reductase ซึ่งกลไกเหล่านี้ได้มีการศึกษายืนยันในสัตว์และมนุษย์ ไมโครอาร์เอ็นเอยังมีผลต่อการเกิดพังผืดในตับผ่านทางกลไกต่าง ๆ เช่นการกระตุ้น hepaticstellate cell ในแง่การประยุกต์ใช้ทางคลินิกนั้น ไมโครอาร์เอ็นเอในซีรั่มกำลังถูกศึกษาเพื่อนำมาใช้เป็นเครื่องมือในการประเมินการอักเสบและพังผืดในตับ รวมทั้งในอนาคตอาจมีการนำไมโครอาร์เอ็นเอมาใช้เป็นการรักษาใหม่สำหรับโรคตับคั่งไขมัน.

เหตุผลของการทำวิจัย : ในปัจจุบันจำนวนพ่อแม่เลี้ยงเดี่ยวในไทยมีจำนวนเพิ่มมากขึ้นอย่างไรก็ตาม งานวิจัยเกี่ยวกับพ่อแม่เลี้ยงเดี่ยวยังมีน้อย และยังไม่พบการศึกษาเกี่ยวกับคุณภาพชีวิตของพ่อแม่เลี้ยงเดี่ยววัตถุประสงค์ : เพื่อศึกษาคุณภาพชีวิต และทราบถึงปัจจัยที่ส่งผลต่อคุณภาพชีวิตของพ่อแม่เลี้ยงเดี่ยวที่เป็นสมาชิกชมรมเครือข่ายครอบครัวเลี้ยงเดี่ยวในกรุงเทพมหานครรูปแบบการวิจัย : การศึกษาวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : ชมรมเครือข่ายครอบครัวเลี้ยงเดี่ยวมูลนิธิเครือข่ายครอบครัวตัวอย่างและวิธีการศึกษา : รวบรวมข้อมูลจากพ่อแม่เลี้ยงเดี่ยวในกรุงเทพมหานคร 106 คนโดยใช้ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามคุณภาพชีวิตขององค์การอนามัยโลกชุดย่อ ฉบับภาษาไทย 3)แบบวัดเหตุการณ์ความเครียดในชีวิต 4) แบบคัดกรองภาวะซึมเศร้า5) แบบประเมินคุณภาพชีวิตสมรสในครอบครัวสถิติที่ใช้วิเคราะห์ข้อมูล ได้แก่ Descriptive Statistics, Independent t-test, One WayANOVA, Pearsoncorrelation และ Stepwise Multivariate LinearRegressionผลการศึกษา : พ่อแม่เลี้ยงเดี่ยวมีคุณภาพชีวิตโดยรวม คุณภาพชีวิตด้านร่างกายคุณภาพชีวิตด้านจิตใจ คุณภาพชีวิตด้านสัมพันธภาพทางสังคมและคุณภาพด้านสิ่งแวดล้อมอยู่ในระดับปานกลาง โดยมีค่าร้อยละอยู่ที่ 61.3, 83.0, 37.7, 55.7 และ 54.7 ตามลำดับ โดยปัจจัยที่มีผลต่อคุณภาพชีวิตโดยรวมได้แก่ ปัญหาพฤติกรรมบุตร (p <0.05)เหตุการณ์ความเครียดในชีวิต (p <0.001) คุณภาพชีวิตสมรสกับคู่สมรสปัจจุบัน (p <0.01) และภาวะซึมเศร้า (p <0.001) โดยมีปัจจัยที่สามารถทำนายคุณภาพชีวิตโดยรวมของพ่อแม่เลี้ยงเดี่ยวในจังหวัดกรุงเทพมหานครคือ เหตุการณ์ความเครียดในชีวิต (R2 = 217)สรุป : พ่อแม่เลี้ยงเดี่ยวส่วนใหญ่มีคุณภาพชีวิตอยู่ในระดับปานกลางซึ่งสัมพันธ์กับปัญหาพฤติกรรมบุตรเหตุการณ์ความเครียดในชีวิตคุณภาพชีวิตสมรสกับคู่สมรสปัจจุบันและภาวะซึมเศร้าโดยมีปัจจัยที่สามารถทำนาย คือ เหตุการณ์ความเครียดในชีวิต.

Alzheimer's disease (AD) is the most common form of dementia and its prevalence is increasing. Recent developments in AD management provide improved ways of supporting patients and their caregivers throughout the disease continuum. Managing cardiovascular risk factors, maintaining an active lifestyle (with regular physical, mental and social activity) and following a Mediterranean diet appear to reduce AD risk and may slow cognitive decline. Pharmacologic therapy for AD should be initiated upon diagnosis. All of the currently available cholinesterase inhibitors (ChEIs; donepezil, galantamine, and rivastigmine) are indicated for mild-to-moderate AD. Traumatic brain injury (TBI) is a major cause of death and disability. However, the molecular events contributing to the pathogenesis are not well understood. Mitochondria serve as the powerhouse of cells, respond to cellular demands and stressors, and play an essential role in cell signaling, differentiation, and survival. There is clear evidence of compromised mitochondrial function following TBI; however, the underlying mechanisms and consequences are not clear. MicroRNAs (miRNAs) are small non-coding RNA molecules that regulate gene expression post-transcriptionally, and function as important mediators of neuronal development, synaptic plasticity, and neurodegeneration. Cette étude visait à mieux comprendre les risques et les pratiques en santé et en sécurité du travail, alors que des centaines d’éoliennes de grande puissance s’ajoutent chaque année au Québec. This clinical perspective article describes an inexpensive pedagogical tool intended for use in a laboratory setting as part of a cardiovascular and pulmonary physical therapy course. It uses a dual balance scale schematic/model. As a teaching resource, physical therapy students can use it to visually reinforce their understanding of the complex cause-and-effect interplay between physiological factors that affect ventilatory muscle pump (VMP) performance for sustained breathing or contribute to VMP fatigue/failure.

Buprenorphine is a unique pharmaceutical in the management of chronic pain and opioid use disorder (OUD). Buprenorphine is a semisynthetic partial opioid agonist at the mu opioid receptor and an antagonist of the kappa opioid. Buprenorphine Maintenance Therapy (BMT) is utilized for the long-term treatment of patients with OUD. The attraction to this methadone alternative is increased safety profile, more convenient patient access to the drug, as well as increase of ease for the provider. A significant proportion of patients present with locally advanced prostate cancer with inherent higher risk of local recurrence and systemic relapse after initial treatment. Recent literature favors combination of radiation therapy and extended hormonal therapy for this subset of patients. The effectiveness of Radical prostatectomy (RP) alone for locally advanced prostate cancer is controversial and is a focus of debate. However, it can decrease the tumor burden and allows the accurate and precise pathological staging with the need for subsequent treatment. In some parts of western Africa, Ebola treatment centers (ETCs) have reached capacity. Unless capacity is rapidly scaled up, the chance to avoid a generalized Ebola epidemic will soon diminish. The World Health Organization and partners are considering additional Ebola patient care options, including community care centers (CCCs), small, lightly staffed units that could be used to isolate patients outside the home and get them into care sooner than otherwise possible. Using a transmission model, we evaluated the benefits and risks of introducing CCCs into Sierra Leone’s Western Area, where most ETCs are at capacity. We found that use of …

The Acute Care for the Elderly (ACE) Tracker is an innovative clinical decision support tool generated from the electronic health record (EHR). The EHR delirium marker is derived from variables noted on the ACE Tracker: nursing documentation in the EHR, use of physical restraints, and pharmacological treatment for delirium (use of quetiapine, haloperidol, olanzapine, or risperidone). The combining of these data elements is consistent with a previous study. เหตุผลของการทำวิจัย : ในปัจจุบันมีจำนวนมารดาวัยรุ่น (อายุระหว่าง 14 - 18 ปี) เพิ่มมากขึ้นซึ่งส่วนมากมักเป็นการตั้งครรภ์ที่ไม่พร้อม ซึ่งอาจส่งผลต่อการเกิดปัญหาทางอารมณ์ในมารดาวัยนี้ ที่ผ่านมามีการศึกษาเกี่ยวกับปัญหาทางอารมณ์ โดยเฉพาะภาวะซึมเศร้าในมารดาหลังคลอดที่เป็นผู้ใหญ่จำนวนมาก แต่ยังไม่ค่อยมีการศึกษาในมารดาวัยรุ่น ผู้วิจัยจึงสนใจที่จะศึกษาภาวะซึมเศร้าในมารดากลุ่มนี้วัตถุประสงค์ : เพื่อศึกษาความชุกและปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในมารดาวัยรุ่นช่วง 1 - 2 สัปดาห์ และ 4 - 6 สัปดาห์หลังคลอดรูปแบบการวิจัย : การศึกษานี้เป็นการศึกษาเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : โรงพยาบาลสิรินธร สำนักการแพทย์ กรุงเทพมหานครตัวอย่างและวิธีการศึกษา : เก็บข้อมูลจากมารดาวัยรุ่นหลังคลอด อายุอยู่ในช่วง 14 - 18 ปี ที่มาคลอดบุตรและนอนพักรักษาในโรงพยาบาลสิรินธร ในช่วงที่ทำการเก็บข้อมูล (ธ.ค. 2556 – มี.ค. 2557) จำนวน 147 ราย โดยใช้แบบสอบถาม1) ข้อมูลส่วนบุคคล ข้อมูลทางกายภาพ ข้อมูลทางจิตสังคม2) แบบวัดภาวะซึมเศร้าหลังคลอด Edinburgh Postnatal DepressionScale (EPDS) ฉบับภาษาไทย (วัด 2 ครั้ง) 3) แบบสอบถามแรงสนับสนุนทางสังคม 4) แบบสอบเหตุการณ์ความเครียดในชีวิต5) แบบสอบถามการปรับตัวบทบาทการเป็นมารดา การวิเคราะห์ข้อมูลใช้ Univariate analysis เพื่อหาปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าหลังคลอดในผู้ป่วยกลุ่มดังกล่าวผลการศึกษา : พบว่ากลุ่มตัวอย่างมีภาวะซึมเศร้า ในช่วง 1 - 2 สัปดาห์ หลังคลอดจำนวน 44 ราย (คิดเป็นร้อยละ 29.9) และมีภาวะซึมเศร้า ในช่วง4 - 6 สัปดาห์ หลังคลอดจำนวน 31 ราย (คิดเป็นร้อยละ 25.5) โดยปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในช่วง 1 - 2 สัปดาห์หลังคลอดได้แก่อายุของคู่สมรส อาการปัสสาวะบ่อย …

Background: This research offers two exploratory frameworks for medical regimen compliance and medical immediacy. The first classifies compliance awareness, compliance mitigation, and financial limitation for those patients that exhibit nonadherence with a medical regimen. The second classifies medical immediacy and characterizes avoidable utilization.
Methods: Representative sampling of adult patients presenting at an emergency department (62,000/ppy) triaged as low acuity; emergency department physician assessment of noncompliance with medical regimen for those patients with a complaint related to a chronic condition; and emergency department physician assessment of medical immediacy and avoidable utilization.

The outcomes of children with congenital hemolytic anemia (CHA) undergoing total splenectomy (TS) or partial splenectomy (PS) remain unclear. In this study, we collected data from 100 children with CHA who underwent TS or PS from 2005 to 2013 at 16 sites in the Splenectomy in Congenital Hemolytic Anemia (SICHA) consortium using a patient registry. We analyzed demographics and baseline clinical status, operative details, and outcomes at 4, 24, and 52 weeks after surgery. Although magnesium (Mg) is a unique biodegradable metal which possesses mechanical property similar to that of the natural bone and can be an attractive material to be used as orthopedic implants, its quick corrosion rate restricts its actual clinical applications. Background: One of the effects of diabetes mellitus (DM), peripheral neuropathy, affects feet sensation and can increase the chance of falling. The purpose of the study was to investigate the effect of 8 weeks of Tai Chi (TC) training combined with mental imagery (MI) on improving balance in people with diabetes and an age matched control group. Methods: Seventeen healthy subjects and 12 diabetic sedentary subjects ranging from 50-80 years of age were recruited. BACKGROUND: Whole body (WBV) vibration and plyometrics are common training techniques which increase strength, blood flow, force and power. The effects these techniques have on sedentary population is unknown. It is our aim to assess the effectiveness of WBV and plyometrics on sedentary population. SUBJECTS: Three groups of nine sedentary subjects were assigned to either the control group (C), jumping only group (J), or jumping with vibration group (JV). Background: Despite investments in infrastructure and evidence for high acceptability, few mHealth interventions have been implemented in sub-Saharan Africa.
Objective: We sought to (1) identify predictors of uptake of an mHealth application for a low-literacy population of people living with HIV (PLWH) in rural Uganda and (2) evaluate the efficacy of various short message service (SMS) text message formats to optimize the balance between confidentiality and accessibility.

Background: A number of developing countries have contracted out public health facilities to the Non-Government Organizations (NGOs) in order to improve service utilization. However, there is a paucity of in-depth qualitative information on barriers to access services as a result of contracting from service users’ perspective.
