INTRODUCTION: The management of a post-chemotherapy retroperitoneal mass secondary to testicular cancer can present a surgical challenge when involving adjacent organs or major vascular structures. We present the first video of a retroperitoneal lymph node dissection (RPLND) with IVC (inferior vena cava) thrombectomy, caval wall resection resulting from metastatic non-seminomatous germ cell testis (NSGCT) cancer.

METHODS: In this surgical video, we highlight important surgical considerations in the management of a post-chemotherapy retroperitoneal mass with direct IVC wall invasion and level 2 thrombus in such a patient.

Herpes simplex virus type 1 (HSV-1) is a DNA virus that is a ubiquitous pathogen of humans. The hallmark of the HSV-lifecycle is infection of a mucosal surface with spread to sensory neurons where the virus establishes a latent infection with periodic recurrences for the life of the host. The most common course of symptomatic disease with HSV-1 is the typical mucocutaneous lesion that is self-limiting. HSV-1 can also cause acute encephalitis and ocular pathology on reactivation. Nanoparticle research is often performed in vitro with little emphasis on the potential role of cell culture medium. In this study, gold nanoparticle interactions with cell culture medium and two cancer cell lines (human T-cell leukemia Jurkat and human pancreatic carcinoma PANC1) were investigated. Gold nanoparticles of 10, 25, 50, and 100 nm in diameter at fixed mass concentration were tested. Pectus excavatum is the most common congenital chest wall abnormality expressed in children, yet its inheritance is poorly understood. Here we present the first comprehensive assessment of the inheritance of this disorder. After evaluating 48 pedigrees and 56 clinical traits of probands and family members, we find strong evidence of autosomal recessive, genetic control for this disorder. Additionally there is likely more than one pectus disease-associated allele, as well as a relatively large number of disease allele carriers in the human population. Previous studies demonstrated that coplanar PCBs promote inflammation by release of pro-inflammatory cytokines like TNF, MCP-1, and VCAM-1 from endothelial cells as well as adipocytes. Also these PCBs at small doses may contribute to the development of obesity by inducing adipocyte differentiation. Obesity is a known risk factor that promotes cardiovascular disorders like atherosclerosis and AAAs. Evidence shows Ang II, a component of the RAS, leads to the formation of atherosclerosis and AAAs in both normal as well as hyperlipidemic mice. Earlier studies in our laboratory have also shown that coplanar PCB-77 promotes atherosclerotic lesion formation in ApoE-/- mice. The …

Adipocytes express angiotensin II (AngII) receptors; however the direct effects of AngII at the adipocyte remain unclear. Knockout mouse models of renin-angiotensin system components exhibit reduced body weight, reduced adiposity, improved glucose tolerance, and improved blood pressure when fed high fat diets, which may be due to reduced action of AngII through the AT1aR in adipocytes. Additionally, hypercholesterolemic AT1aR deficient mice are protected from AngII-induced increases in atherosclerosis and abdominal aortic aneurysm (AAA) formation. We hypothesized that deficiency of AT1aR in adipocytes would reduce the development of obesity, obesity-induced disorders, and vascular diseases. To test this hypothesis, we created a …

A 69-year-old Caucasian female, with a previous diagnosis of 5q minus myelodysplastic syndrome, presented with conventional renal cell carcinoma (RCC) associated with multiple-epithelioid nonnecrotizing granulomas. Two previous reports of sarcoidosis, primarily involving the lung and skin, have been described in patients with 5q minus myelodysplasia. A cluster of closely linked genes encoding for cytokines such as IL-4, IL-5, and IL-3 are present on chromosome 5q. Hence, in sarcoidosis, cytokine imbalances associated with the deletion of these cytokine genes have been postulated. However, an occurrence of epithelioid granulomas within a carcinoma, in preexisting clonal myelodysplastic syndrome, has not been described. The …

This course is organized around current challenging health issues, such as mandatory immunization, childhood obesity, health insurance, tobacco control, etc. Activities included issues-focused debates, lecture and video presentations, case study discussions, and guest speakers. Students completed fifteen hours of community-based service learning, many in the Lawrence Math-Science Partnership, an outreach program in which undergraduates work on after-school STEM enrichment activities with middle-school students. Several activities complemented the course issues, allowing college students to make connections between course theory and community needs, while engaging middle-school students in important public health concepts. Tomography is the mathematical process of imaging an object via a set of finite slices. In medical imaging, these slices are defined by multiple parallel X-ray beams shot through the object at varying angles. The initial and final intensity of each beam is recorded, and the original image is recreated using this data for multiple slices. I will discuss the central role of the Radon transform and its inversion formula in this recovery process.

It has been suggested that public school teachers are exposed to highly stressful situations which are related to psychological and psychiatric problems. Unfortunately, there is no quantitative data suggesting which factors contribute to improving the mental health of American teachers, nor have there been any prior studies that have directly compared the factors associated with mental health disturbances between teachers and workers in other occupations in the United States. In light of this, I felt it crucial to find the factors associated with psychological/psychiatric problems in public school teachers in the United States using an appropriate reference professional group.

The purpose of this study was to determine the effectiveness of an augmentation of traditional articulation intervention on the acquisition of speech sounds. The study examined the effect of Van Riper's traditional articulation intervention (Van Riper & Erickson, 1996) integrated with See the Sound/Visual Phonics (visual phonics) hand cues, in contrast to Van Riper's traditional articulation intervention alone. One child identified with a phonological disorder participated in a single subject multiple baseline across behaviors study. The purpose of this study was to examine differences in executive functions of typically developing, at-risk, and language impaired preschoolers. It also evaluated the relationship between preschool children's language skills and daily functional use of executive function skills as rated by parents.

Subjects included 45 children who attended central Illinois preschool programs. Executive functions were measured with the Behavioral Rating Inventory of Executive Functions -Preschool edition (BRIEF-P), oral language skills were measured with the Peabody Picture Vocabulary Test (PPVT) and the Clinical Evaluation of Language Fundamentals -Preschool Edition (CELF-P), and oral narrative skills were measured using the Renfrew Bus Story. …

There has been a need for the assessment of ground water contamination risk on terrestrial organisms. One terrestrial organism, Eisenia fetida, was chosen as a test species and exposed to three common pharmaceutical and/or pesticide related contaminants: acetaminophen (CAS#103-90-2), caffeine (CAS#58-08-2) and carbendazim (CAS#10605-21-7), respectively. Levels of contamination varied from grams per liter to micrograms per liter (ppb) using distilled water as the solvent. Eisenia fetida was examined through a 28 day range/index toxicity test. A Benchmark Dose (BMD) dichotomous probit analysis, using a 95% confidence interval, was calculated for all three contaminants. All three contaminants were found to have …

The purpose of this study was to examine the validity of the Power Balance Bracelet manufacturer's claims. Twenty male Division I athletes from Eastern Illinois University performed four tests on three different dates. The first session was a control trial and the subsequent sessions were performed with either the Power Balance bracelet or the placebo bracelet. The purpose of this study was to compare a single bout of stretching; either static stretching (SS) or proprioceptive neuromuscular facilitation (PNF) stretching and no stretching (C) on hip range of motion (ROM) after walking on the treadmill for 30 minutes. Introduction to Health Research Methods is required for all students enrolled in a master of public health (M.P.H.) or master of science (M.S.) program in public health at a large public university with a diverse student body, including a high proportion of international students. The M.P.H. and M.S. programs cater to students who work full-time, so each class meets for a 3-hour block one evening each week during a 15-week semester. This course is the third of a three-course research methods sequence; students are expected to have completed a biostatistics and an epidemiology course prior to enrolling in research methods.

The Department of Defense policy Don't Ask, Don't Tell (DADT) ended in September, 2011. The Navy Bloodborne Infection Management Center conducted a post-DADT pilot survey of HIV seroconverters identified when the DADT policy was in effect. Sailors and Marines newly diagnosed as HIV positive from 2005 to 2010 were invited to participate in an online survey. A structured questionnaire elicited risk information about the 3-year period before HIV diagnosis. Respondents reported engaging commonly in same sex sexual activity, having concurrent partners, and poor condom use for anal sex. Human Immunodeficiency Virus (HIV) infection continues at a steady rate among U.S. Sailors and Marines. This study provides the first service-specific description of HIV infection demographics. All Sailors and Marines identified as HIV infected between January 2005 and August 2010 were included. The project compared personnel and epidemiologic data, and tested reposed sera in the Department of Defense Serum Repository. This group comprised 410 Sailors and 86 Marines, predominantly men. HIV infected Marines were more likely to be foreign born than their Navy counterparts, 42% versus 10%, p < 0.001. Approximately half of the patients had deployed including to the wars in Iraq or Afghanistan. Nearly half of each group was infected by the age of 25. Similar to the U.S. epidemic, Black race was over-represented. Unlike national rates, Hispanic Sailors and Marines were not over-represented. Demographics were distinct for those of specific occupational specialties. Certain ship classes carried lower incidences. Clustering of HIV infection risk occurred around deployment. The Navy and Marine Corps have different patterns of HIV infection, which may merit distinct approaches to prevention. The Navy may have unique targets for prevention efforts to include pipeline training and first assignment as well as particular occupational environments.

BACKGROUND: The effects of helminth co-infection on malaria in humans remain uncertain. This study aimed to evaluate the nature of association of intestinal helminths with prevalence and clinical outcomes of Plasmodium infection.

METHODS: A cross-sectional study involving 1,065 malaria suspected febrile patients was conducted at Dore Bafeno Health Center, Southern Ethiopia, from December 2010 to February 2011. Plasmodium and intestinal helminth infections were diagnosed using Giemsa-stained blood films and Kato-Katz technique, respectively. Haemoglobin level was determined using a haemocue machine.

Of 64 canaries with diarrhea in Elazığ Province between May 2002 and May 2006, 18 (28.1%) had Isospora oocysts in their feces. It was determined that the oocysts, examined morphologically before and after sporulating, were Isospora canaria and Isospora sp. Among the infected animals, 8 (44.4%) were concurrently infected with both species. Total numbers of I. canaria oocysts were greater than those of Isospora sp. A total of 2 canaries showing severe clinical signs were necropsied. Liver, spleen, and lung preparations and peripheral blood smears of the necropsied animals were examined for Atoxoplasma, and no development forms were seen. For …
