BACKGROUND: Considerable variability exists in clinical approaches to thoracolumbar fractures. Controversy in evaluation and nomenclature contribute to this confusion, with significant differences found between physicians, between different specialties, and in different geographic regions. A new classification system for thoracolumbar injuries, the Thoracolumbar Injury Severity Score (TLISS), was recently described by Vaccaro. No assessment of regional differences has been described. We report regional variability in use of the TLISS system between United States and non-US surgeons.

Background
Researchers have frequently encountered difficulties in the recruitment and retention of minorities resulting in their under-representation in clinical trials. This report describes the successful strategies of recruitment and retention of African Americans and Latinos in a randomized clinical trial to reduce smoking, depression and intimate partner violence during pregnancy. Socio-demographic characteristics and risk profiles of retained vs. non-retained women and lost to follow-up vs. dropped-out women are presented. In addition, subgroups of pregnant women who are less (more) likely to be retained are identified.

Translational quality control is monitored at several steps, including substrate selection by aminoacyl-tRNA synthetases (aaRSs), and discrimination of aminoacyl-tRNAs by elongation factor Tu (EF-Tu) and the ribosome. Phenylalanyl-tRNA synthetase (PheRS) misactivates Tyr but is able to correct the mistake using a proofreading activity named editing. Previously we found that overproduction of editing-defective PheRS resulted in Tyr incorporation at Phe-encoded positions in vivo , although the misreading efficiency could not be estimated. This raised the question as to whether or not EF-Tu and the ribosome provide further proofreading mechanisms to prevent mistranslation of Phe codons by Tyr. Here we show that, …

We sought to identify the impact of cardiac resynchronization therapy (CRT) on atrial tachyarrhythmia (AT) susceptibility in patients with left ventricular (LV) systolic dysfunction in whom worsening heart failure (HF) resulted in upgrade from conventional dual-chamber pulse generator to cardiac resynchronization therapy-defibrillator (CRT-D).

Background & Aims Heme oxygenase-1 (HO-1) is an antioxidant defense and key cytoprotective enzyme, which is repressed by Bach1. MicroRNA-122 (miR-122) is specifically expressed and highly abundant in human liver and required for replication of hepatitis C virus (HCV) RNA. This study was to assess whether a specific miR-122 antagomir down-regulates HCV protein replication and up-regulates HO-1. Methods We transfected antagomir of miR-122, 2′-O-methyl-mimic miR-122, or non-specific-control antagomir (NSCA) into wild type Huh-7 cells or Huh-7 stably replicating HCV subgenomic core-NS3 (CNS3 replicon cells), or NS3-5B (9–13 replicon cells). The effects of ageing on the cardiovascular system contribute to substantial alterations in cellular morphology and function. The variables regulating these changes are unknown; however, one set of signalling molecules that may be of particular importance in mediating numerous cellular responses, including control of cell growth, differentiation and adaptation, are the proteins associated with the mitogen-activated protein kinase (MAPK) signalling systems. วัตถุประสงค์ เพื่อวิเคราะห์ค่าความถี่สั่นพ้องที่ 1 และ ที่ 2 ของสระ 5 เสียงได้แก่ อะ อิ อุเอะ โอะ และคํานวณ ค่าช่วงกว้างที่สุดระหว่างความถี่สั่นพ้องที่ 1 และระหว่างความถี่สั่นพ้องที่ 2 ในอาสาสมัครชายไทยที่ไม่มีความพิการในช่องปาก วัสดุและวิธีการ บันทึกเสียงสระ 5 เสียงได้แก่ อะ อิ อุ เอะ โอะ ของอาสาสมัครจากคณะทันตแพทยศาสตร์ มหาวิทยาลัยเชียงใหม่ และจุฬาลงกรณ์มหาวิทยาลัย จํานวนทั้งสิ้น 67 คน ด้วยเครื่องบันทึกระบบดิจิทัล วิเคราะห์ เสียงสระทั้ง 5 เสียงของแต่ละคนด้วยโปรแกรมมัลติ-สปช 3700 และด้วยวิธีการเข้ารหัสการทํานายแบบเชิงเส้น เพื่อหาความถี่สั่นพ้องที่ 1 และที่ 2 จากนั้นคํานวณค่าช่วงกว้างที่สุดระหว่างความถี่สั่นพ้องที่ 1 และระหว่าง ความถี่สั่นพ้องที่ 2 ตามลําดับ ผลการศึกษา ค่าความถี่สั่นพ้องที่ 1 และที่ 2 ของสระทั้ง 5 เสียงของอาสาสมัครทั้งสองกลุ่มไม่มีความแตกต่าง กันอย่างมีนัยสําคัญทางสถิติ เมื่อรวมข้อมูลทั้งสองกลุ่มเข้าด้วยกันพบว่าค่าเฉลี่ยความถี่สั่นพ้องที่ 1 และที่ 2 มีค่าสูงที่สุดอยู่ที่เสียง อะ (706.17 เฮิร์ทซ์) และ อิ (2011.16 เฮิร์ทซ์) ตามลําดับ ส่วนค่าต่ําที่สุดของความถี่ สั่นพ้องที่ 1 และที่ 2 อยู่ที่เสียง อุ (282.94 เฮิร์ทซ์) และ โคะ (914.98 เฮิร์ทซ์) ตามลําดับ คํานวณช่วง ความกว้างที่สุดระหว่างความถี่สั่นพ้องที่ 1 และระหว่างความถี่สั่นพ้องที่ 2 ของเสียงสระทั้ง 5 เสียงจะมีค่าเท่ากับ 423.23 เฮิร์ทซ์ และ 1096.18 เฮิร์ทซ์ …

Objective To clone human connective tissue growth factor (hCTGF) from pulpal fibroblasts. Materials and methods Primary human pulpal fibroblasts were isolated from pulpal tissues. The reverse transcriptase-PCR assay was used to amplify the cDNA of hCTGF. The amplified PCR products were ligated into the TOPO R cloning vector and transformed into competent bacteria cells. The putative clones were bidirectionally sequenced to analyze nucleotide sequence and compare with hCTGF cDNA sequence references. Results From RT-PCR reaction, expression of CTGF mRNA was detected in human pulpal fibroblast. วัสดุบูรณะฟันที่ดีควรจะมีสีและคุณสมบัติเหมือนฟันธรรมชาติ มีความเข้ากันได้ดีทางชีวภาพ ทนทาน ต่อการกัดกร่อนและเป็นฉนวน ปัจจุบัน มีการใช้วัสดุบูรณะเซรามิกซึ่งเป็นวัสดุที่มีสีเหมือนฟันธรรมชาติเป็นวัสดุ บูรณะฟันกันมากขึ้น เนื่องจากมีการพัฒนาทางด้านการผลิตร่วมกับการพัฒนาวัสดุระบบยึดติด ทําให้การบูรณะด้วยเซรามิกมีความแข็งแรงและมีคุณสมบัติดียิ่งขึ้น บทความนี้ได้รวบรวมข้อมูลเกี่ยวกับวัสดุบูรณะเซรามิกระบบ ต่าง ๆ ตามการพัฒนา เริ่มตั้งแต่ระบบเซรามิกชนิดผงกับน้ําแบบดั้งเดิม เซรามิกชนิดหล่อ เซรามิกชนิดอัดความดัน เซรามิกชนิดอินฟิลเตรด และเซรามิกชนิดแมคซีนเนเบิล รวมทั้งขั้นตอนการลองชิ้นงาน การเตรียมเพื่อการยึดติดและการยึดติดวัสดุบูรณะเซรามิกกับฟันในคลินิก เพื่อเป็นข้อมูลให้ทันตแพทย์สามารถเลือกใช้งานทางคลินิกได้อย่างเหมาะสม (ว ทันต จุฬาฯ 2550;30:325-36)

ปัจจัยที่มีความสัมพันธ์กับความเครียด ในหน้าที่การงานของทันตแพทย์ไทย, ชาญชัย ให้สงวน

วัตถุประสงค์ ความเครียดในการทํางานมีผลกระทบต่อคุณภาพบริการและความตั้งใจโยกย้ายงานของทันตบุคลากร การศึกษานี้มีวัตถุประสงค์เพื่อประเมินความเครียดในการทํางานของทันตแพทย์ไทยและระบุปัจจัยที่เกี่ยวข้อง วัสดุและวิธีการ แบบแผนการวิจัยเป็นการสํารวจภาคตัดขวาง ดําเนินการสุ่มตัวอย่างแบบเป็นระบบจาก ทันตแพทย์ทั่วประเทศ ใช้แบบสอบถามทางไปรษณีย์รวม 2 รอบ มีทันตแพทย์ส่งแบบสอบถามกลับคืน ซึ่งเป็น ทันตแพทย์ที่ยังปฏิบัติงานอยู่ จํานวน 733 คน ประเมินความเครียดของการทํางานทันตกรรมในรอบปี พ.ศ. 2547 ด้วยดัชนีความเครียดในหน้าที่การงาน ใช้เทคนิควิเคราะห์ความถดถอยเชิงพหุแบบขั้นตอนประเมินปัจจัยที่มี ความสัมพันธ์กับระดับความเครียดในการงาน ผลการศึกษา ทันตแพทย์ร้อยละ 31.7 มีความเครียดในการทํางานทันตกรรมตั้งแต่ระดับปานกลางขึ้นไป ผลการ วิเคราะห์ความถดถอยเชิงพหุ พบว่า ความคิดเห็นเกี่ยวกับปริมาณงานและความตั้งใจโยกย้ายงานมีความสัมพันธ์ เชิงบวกกับระดับความเครียดในทุกแบบจําลอง ขณะที่ความพึงพอใจต่อหน้าที่การงานมีความสัมพันธ์เชิงลบในทุก แบบจําลอง การปฏิบัติงานในภาคเอกชนมีความสัมพันธ์กับทุกมิติย่อยของดัชนีความเครียด แต่มีทิศทางทั้งบวก และลบ ปัจจัยอื่นที่มีความสัมพันธ์กับระดับความเครียด คือ จํานวนชั่วโมงปฏิบัติงาน และรายได้ สรุป การศึกษานี้ยืนยันถึงระดับความเครียดที่มีอยู่สูงในวิชาชีพทันตแพทย์ โดยเฉพาะในภาครัฐ แบบจําลอง ความเครียดสะท้อนให้เห็นถึงอิทธิพลของตัวแปรทั้งระดับปัจเจกบุคคลและระดับองค์กร (ว ทันต จุฬาฯ 2550;302255-68)

วัตถุประสงค์ เพื่อศึกษาฤทธิ์ต้านเชื้อแบคทีเรียของสารสกัดพอลิแซ็กคาไรด์เจลจากเปลือกทุเรียนต่อเชื้อสเตร็ปโต ค็อกคัสมิวแทนส์ และเชื้อแอกกรีเกทแบกเทอร์แอกทิโนไมซีเทมคอมแทนส์ ซึ่งมีส่วนในการก่อโรคฟันผุ และโรค ปริทันต์อักเสบ ตามลําดับ วัสดุและวิธีการ ทําการศึกษาโดยการเลี้ยงเชื้อแบคทีเรียสเตร็ปโตค็อกคัสมิวแทนส์ สายพันธุ์ ATCC 25175 และเชื้อแอกกรีเกทแบกเทอร์แอกทิโนไมซีเทมคอมแทนส์ สายพันธุ์ ATCC 43718 ในอาหารเลี้ยงเชื้อ ชนิดน้ําที่มีสารสกัดพอลิแซ็กคาไรด์เจลจากเปลือกทุเรียน ความเข้มข้นต่าง ๆ (50 100 และ 150 มก./มล.) เป็น เวลา 1 5 10 20 30 และ 60 นาที นําไปเลี้ยงต่อในอาหารเลี้ยงเชื้อชนิดวุ้นเป็นเวลา 48 ชั่วโมง แล้วจึงนับจํานวน เชื้อแบคทีเรียที่มีชีวิต เปรียบเทียบกับคลอร์เฮกซิตีนที่ความเข้มข้นร้อยละ 0.1 และอาหารเลี้ยงเชื้อที่ไม่ใส่สารสกัด ผลการศึกษา ในเวลา 1 นาที สารสกัดพอลิแซ็กคาไรด์จากเปลือกทุเรียนที่ความเข้มข้น 150 มก./มล. และ คลอร์เฮกซิดีนที่ความเข้มข้นร้อยละ 0.1 มีฤทธิ์ฆ่าเชื้อสเตร็ปโตค็อกคัสมิวแทนส์ และเชื้อแอกกรีเกทแบกเทอร์ แอกทิโนไมซีเทมคอมแทนส์ ในขณะที่สารสกัดที่ความเข้มข้น 100 มก./มล. มีฤทธิ์ฆ่าเชื้อสเตร็ปโตค็อกคัสมิวแทนส์และยับยั้งเชื้อแอกกริเกทแบกเทอร์แอกทิโนไมซีเทมคอมแทนส์ ตามลําดับ ส่วนสารสกัดที่ความเข้มข้น 50 มก./มล. มีฤทธิ์ฆ่าเชื้อแบคทีเรียทั้งสองชนิด ในเวลา 60 นาที สรุป ในเวลา 1 นาที สารสกัดพอลิแซ็กคาไรด์เจลจากเปลือกทุเรียนที่ความเข้มข้น 100 และ 150 มก./มล. สามารถฆ่าเชื้อสเตร็ปโตค็อกคัสมิวแทนส์ และเชื้อแอกกรีเกทแบกเทอร์แอกทิโนไมซีเทมคอมแทนส์ ตามลําดับแสดงให้เห็นว่าสารสกัดชนิดนี้อาจพัฒนาเป็นสารออกฤทธิ์หลักในผลิตภัณฑ์ทางทันตกรรมเพื่อควบคุมโรคฟันผุและโรคปริทันต์อักเสบ (ว ทันต จุฬาฯ 2550;30:235-44)

Objective The aims of this study were to assess the prevalence of tongue lesions and to relate their occurrence to gender, age and health-affected behaviors in a group of Thai dental patients. Materials and methods One thousand and six hundred Thai dental patients, dividing into three age groups were clinically examined for tongue lesions. Information regarding smoking and drinking habits was collected from the interview. วัตถุประสงค์ เพื่อเปรียบเทียบปริมาณและคุณภาพงานของนิสิตปี 6 ในการปฏิบัติงานทันตกรรมสําหรับเด็ก คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย หลังการปรับเปลี่ยนระบบการเรียนการสอนและประเมินผล 1 ปี และ 15 ปี วัสดุและวิธีการ ศึกษาข้อมูลจากใบบันทึกการปฏิบัติงาน ของนิสิตทันตแพทย์ปี 6 ปีการศึกษา 2529 และ 2544 จํานวน 62 คู่ ที่มีเกรดเฉลี่ยสะสมใกล้เคียงกัน บันทึกข้อมูลในแบบบันทึก วิเคราะห์ เปรียบเทียบปริมาณและ คุณภาพงานโดยรวม แยกตามชนิดงานและปีการศึกษาว่ามีความแตกต่างกันหรือไม่ โดยใช้สถิติวิเคราะห์ มันน์วิตนียูและแบบทดสอบค่าทีที่ระดับนัยสําคัญ .05 ผลการศึกษา ปริมาณงานเฉลี่ยของนิสิตปี 6 ปีการศึกษา 2529 เท่ากับ 128.83 เครดิต ในขณะที่ปีการศึกษา 2544 เท่ากับ 144.44 เครดิต พบความแตกต่างอย่างมีนัยสําคัญทางสถิติระหว่างทั้งสองกลุ่ม (p <.05) โดยคะแนน คุณภาพเฉลี่ยปีการศึกษา 2529 เท่ากับ 84.63 และปีการศึกษา 2544 เท่ากับ 81.35 พบว่ามีความแตกต่างอย่าง มีนัยสําคัญทางสถิติ (p <.05) สรุป หลังจากใช้การประเมินระบบใหม่เป็นเวลา 15 ปี ปริมาณงานของนิสิตปีที่ 6 เพิ่มขึ้น ขณะที่คะแนนคุณภาพ ลดลง แต่ยังจัดอยู่ในระดับคุณภาพเดียวกัน ทั้งนี้อาจเนื่องจากวิธีการให้คะแนนคุณภาพที่ปรับเปลี่ยนไป (ว ทันต จุฬาฯ 2550;30:303-12)

วัตถุประสงค์ เพื่อประเมินความพึงพอใจต่อระบบการตรวจ และส่งต่อผู้ป่วยนอก ณ จุดเดียวในผู้ป่วย อาจารย์ นิสิต และเจ้าหน้าที่ที่เกี่ยวข้องของคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย วัสดุและวิธีการ ให้อาจารย์ นิสิตชั้นปีที่ 5 และ 6 เจ้าหน้าที่ที่เกี่ยวข้องกับระบบการตรวจและส่งต่อผู้ป่วยนอก ทั้งหมด รวมถึงผู้ป่วยที่ต้องการรับการรักษากับนิสิตทันตแพทย์จํานวน 393 คนตอบแบบสอบถามปลายเปิด และ ปลายปิด รวมทั้งสัมภาษณ์ผู้ป่วย 50 คนโดยใช้แบบสอบถามสําหรับผู้ป่วยชุดเดียวกัน ในช่วงระหว่างเดือน ตุลาคม พ.ศ. 2548 ถึง เดือนกุมภาพันธ์ พ.ศ. 2549 ผลการศึกษา พบว่าผู้ป่วยส่วนใหญ่จะแนะนําผู้อื่นมารักษากับทางคณะฯ (ร้อยละ 97) โดยส่วนใหญ่พอใจ ที่ได้รับการตรวจหลายครั้งจากอาจารย์ผู้เชี่ยวชาญในแต่ละสาขา (ร้อยละ 93) และพอใจกับการได้รับการถ่ายภาพ รังสีพานอรามิกโดยไม่เสียค่าใช้จ่าย (ร้อยละ 94) สําหรับนิสิตคิดว่าระบบการตรวจ และส่งต่อผู้ป่วยนอก ณ จุดเดียวดีกว่าระบบเดิม (ร้อยละ 59) อย่างไรก็ตามส่วนใหญ่เกิดความสับสนกับข้อสรุปของอาจารย์ผู้ควบคุม จากภาควิชาต่าง ๆ ในการวางแผนการรักษา (ร้อยละ 64.5) ส่วนอาจารย์ และเจ้าหน้าที่ที่เกี่ยวข้องส่วนใหญ่ พึงพอใจในระบบเดิม โดยพบว่าการรับภาระที่หนักขึ้นและบรรยากาศในการทํางานของอาจารย์ที่เกี่ยวข้อง มีความสัมพันธ์กับความเครียดที่สูงขึ้น อาจารย์ส่วนใหญ่ (ร้อยละ 57) ที่เกี่ยวข้องกับระบบโดยตรงเห็นว่า ควรมีการปรับปรุงระบบการตรวจและส่งต่อผู้ป่วยนอก ณ จุดเดียว สรุป ภายใต้ข้อจํากัดของงานวิจัย ผู้ป่วยและนิสิตส่วนใหญ่พอใจกับระบบการตรวจ และส่งต่อผู้ป่วยนอก ณ จุดเดียว แต่อาจารย์และเจ้าหน้าที่ที่เกี่ยวข้องพอใจกับระบบเดิม (2 ทันต จุฬาฯ 2550;30:313-24)

SHPS‐1/SIRPα1 is a transmembrane glycoprotein that belongs to the immunoglobulin (Ig) super family. In the present study, we show that SHPS‐1 strongly associates with Concanavalin A (Con A), a plant lectin obtained from jack beans. Further studies with SHPS‐1 mutants reveal that the extracellular domain of SHPS‐1 containing the Ig sequence is responsible for its association with Con A. Con A treatment induces cross‐linking and multimerization of the SHPS‐1 protein in the plasma membrane, accompanied by its tyrosine phosphorylation and recruitment of SHP‐2. Lead-contaminated candies from Latin America are beginning to gain attention in the public media1,2 and in the medical literature.3–5 These candies come from a number of sources and are manufactured outside Food and Drug Administration regulatory control. In 2005, we sampled 50 imported Latin American candies sold in Southern Nevada. A total of 20 (40%) tested positive with an average lead content of 1.46  0.27 mg/kg in the candies’ wrappers and straws, based on standard Graphite Furnace Atomic Absorption Spectrophotometry methodology. There are relatively few health promotion programs for people with disabilities who live in rural areas. An exception is Living Well with a Disability, a health promotion program for people with disabilities developed by researchers at the RTC: Rural (Ravesloot & Seekins et al.,1994). The Living Well program was originally designed to be delivered in-person by peer-support staff of Centers for Independent Living (CILs) to groups of participants with disabilities. For many rural people with disabilities, however, the distances and travel difficulties inherent in their environment make onsite group programs impractical or inaccessible.
