Abstract

OBJECTIVE:

To assess the efficacy of flexible intramedullary Ender nails for the treatment of tibial diaphyseal.

MATERIALS AND METHODS:

This is a retrospective review of patients treated with the Ender Nail for both open and closed tibial shaft fractures. Between January 1989 and December 1994, 43 fractures were treated with these nails. The configuration of each fracture was determined using the Orthopedic Trauma Association classification and the extent of soft tissue damage was assessed using the Gustilo and Anderson's classification. Four patients were excluded from the study due to inadequate follow-up. There were 27 closed and 12 open fractures. …

Alterations in the expression of integrin receptors for extracellular matrix (ECM) proteins are strongly associated with the acquisition of invasive and/or metastatic properties by human cancer cells. Despite this, comparatively little is known of the biochemical mechanisms that regulate the expression of integrin genes in cells. Here we demonstrate that the Ras-activated Raf-MEK-extracellular signal-regulated kinase (ERK) signaling pathway can specifically control the expression of individual integrin subunits in a variety of human and mouse cell lines. การวิจัยเชิงพรรณนา (Descriptive Research) ครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาค่าใช้จ่ายของ ผู้บาดเจ็บจากอุบัติเหตุจราจรทางบกที่เข้ารับบริการโรงพยาบาลมหาราชนครศรีธรรมราช 2) เปรียบเทียบ ค่าใช้จ่ายในประเภทผู้บาดเจ็บที่เป็นคนเดินเท้า คนขับขี่ และคนโดยสาร 3) เปรียบเทียบค่าใช้จ่ายของ การบาดเจ็บที่เกิดจากการใช้ยานยนต์ที่ต่างกัน 4) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่คาดเข็มขัด นิรภัย และไม่คาดเข็มขัดนิรภัย 5) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่สวมหมวกนิรภัยและไม่สวม หมวกนิรภัย 6) เปรียบเทียบค่าใช้จ่ายในกลุ่มผู้บาดเจ็บที่ดื่มแอลกอฮอล์และไม่ดื่มแอลกอฮอล์ 7) เปรียบเทียบ ค่าใช้จ่ายในช่วงเวลาที่ได้รับอุบัติเหตุ กลุ่มตัวอย่าง คือ ผู้บาดเจ็บจากอุบัติเหตุจราจรทางบกที่เข้ารับบริการ รักษาที่โรงพยาบาลมหาราชนครศรีธรรมราช จํานวน 351 ราย โดยการสุ่มตัวอย่างแบบ Cluster Sampling โดยจับสลากผู้บาดเจ็บที่มารับบริการในระยะเวลาหนึ่งเดือน เครื่องมือที่ใช้ในการวิจัยคือ แบบบันทึกการเฝ้าระวังการบาดเจ็บระดับจังหวัด และแบบบันทึกค่ารักษาพยาบาลโรงพยาบาลมหาราช นครศรีธรรมราช วิเคราะห์ข้อมูลโดยหาค่าร้อยละ ค่าเฉลี่ยส่วนเบี่ยงเบนมาตรฐาน ทดสอบความแตกต่าง ของค่าเฉลี่ย โดยการทดสอบค่า ที และการวิเคราะห์ความแปรปรวนทางเดียว \nผลการวิจัยพบว่า ผู้บาดเจ็บที่ไม่นอนโรงพยาบาล จํานวน 251 คน มีค่าใช้จ่ายเฉลี่ย 200 บาทต่อคน ผู้บาดเจ็บที่เป็นคนเดินเท้าจะมีค่าใช้จ่ายมากกว่าผู้ขับขี่และผู้โดยสาร ผู้บาดเจ็บที่การบาดเจ็บเกิดจาก ยานพาหนะอื่น ๆ มีค่าใช้จ่ายสูงกว่าการบาดเจ็บที่เกิดจากรถจักรยานยนต์ ผู้บาดเจ็บที่ดื่มแอลกอฮอล์ มีค่าใช้จ่ายที่สูงกว่า ผู้บาดเจ็บที่ไม่ดื่มแอลกอฮอล์ อย่างมีนัยสำคัญที่ระดับ 0.05 ส่วนค่าใช้จ่ายเฉลี่ย ของผู้บาดเจ็บที่คาดและไม่คาดเข็มขัดนิรภัย, ผู้บาดเจ็บที่สวมและไม่สวมหมวกนิรภัยผู้บาดเจ็บที่เกิดเหตุ ในเวลาที่ต่างกัน ไม่มีความแตกต่างกัน ผู้บาดเจ็บที่นอนโรงพยาบาล จำนวน100 คน มีค่าใช้จ่ายเฉลี่ย 7,196 บาทต่อคน ไม่มีความแตกต่างกัน ในค่าใช้จ่ายเฉลี่ยของผู้บาดเจ็บในกลุ่มของประเภทผู้บาดเจ็บ ประเภทยานยนต์ ผู้บาดเจ็บที่คาดและไม่คาดเข็มขัดนิรภัย, ผู้บาดเจ็บที่สวมและไม่สวมหมวกนิรภัย, ผู้บาดเจ็บที่เกิดเหตุในเวลาที่ต่างกัน

Environmental tobacco smoke exposure has been recognized as an important source of lung cancer morbidity and mortality. Measuring environmental tobacco smoke exposure for risk analysis depends much on the methodologies for determining ETS levels of nicotine and RSP.

The author decided to measure the linear multistage dose-response curve relating the level of ETS exposure to the lung cancer incidence. The research was conducted using human time-activity patterns and microenvironmental ETS measurements through a self-designed questionnaire. A cohort of New Haven Mormons was selected for a low-dose group and the subjects from the NHAPS study were utilized for the high-dose group. …

National surveys to determine trends in contraceptive use in the United States have occurred every 6 to 10 years since 1955.1,2,3 Since that time, there has been a rapid decline in the use of natural methods of family planning; a sharp rise and slow decline in the use of oral contraceptives and, in the past ten years, a major reliance on surgical sterilization for contraception. Information from these national surveys has provided useful information for contraceptive providers, policy makers, and religious groups. Morphine Enhances The Permeability Across Vascular Endothelial Cell Barriers, Jenine Anday

Using three types of in vitro endothelial cell barrier models, we investigated the direct effects and mechanisms of morphine on vascular permeability. Our current studies illustrate LPS induces permeability across these VEC barriers, and is significantly enhanced when co-treated with morphine, displayed using the [ 14 CJ-inulin paracellular marker. Incubation with morphine alone induces permeability in a concentration­ depeodent manner, and is not blocked by the addition of naloxone. Morphine enhances the detrimental effects of LPS on cell viability and alone also decreases endothelial cell viability, concentration-dependently, which is also not affected by naloxone, as demonstrated by the trypan blue …

Nutrition implications associated with nutritional care of the high-risk infant and obstetric population are emphasized. Statistical information was gathered concerning the population growth of multiple births over the past twenty years. Literature to identify both maternal and neonatal risk factors associated with multiple gestation pregnancies was also reviewed. Studies conducted over the past twenty years have shown that there has been an increase (25%-50%) in the amount of twin births throughout the world. The purpose of the present study was to compare feature retention patterns between children developing speech typically (TD) and children with phonological impairment (PI) and to discuss these findings in terms of characteristics, severity, and implication for the identification of developmental apraxia of speech (DAS). A second purpose was to determine if a relationship exists between phonological knowledge and feature retention.

Diabetes mellitus is a serious problem that affects 15.7 million individuals in the United States. The complications of this disease are catastrophic and can lead to blindness, kidney disease, lower limb amputations, nerve damage, increased risk of heart disease, stroke, and death. Among Native Americans, diabetes has reached epidemic proportions. A variety of psychosocial variables has demonstrated relationships to diabetic outcome. Past research has shown a relationship between psychological variables and glucose control. The current study of Native Americans shows a similar pattern using the psychosocial instruments to measure the constructs of depression, anger, hostility, social support, and perceived stress. …

Chronic back pain is a serious problem in the U.S. for which about 10% of back pain sufferers will undergo elective surgery. Unfortunately, back surgery is not successful in alleviating back pain in a substantial number of surgery patients. Various psychological and psychosocial variables have a demonstrated relationship to back surgery outcome. The most widely used personality test used to predict back surgery outcome is the Minnesota Multiphasic Personality Inventory (MMPI). The objectives of this study were to describe ways in which doctors make suboptimal diagnostic and treatment decisions, and to discuss possible means of alleviating those biases, using a review of past studies from the psychological and medical decision-making literatures. A number of biases can affect the ways in which doctors gather and use evidence in making diagnoses. Biases also exist in how doctors make treatment decisions once a defi nitive diagnosis has been made. These biases are not peculiar to the medical domain but, rather, are manifestations of suboptimal reasoning to which people are susceptible in general. None the …

This site visit looked at programs in the greater Philadelphia area to move welfare recipients into employment and support services aimed at keeping them employed. Four years after implementation of the Personal Responsibility and Work Opportunity Reconciliation Act, welfare caseloads were much reduced, but challenges remained. Pennsylvania officials described their plans to assist beneficiaries who would reach their five-year lifetime cash assistance limit beginning in 2002. Site visitors met with state and county assistance office staff, employers, consumer advocates, and employment and training contractors. A 30% decrease in osmolarity stimulated ³H-taurine, ³H-GABA and glutamate (followed as ³H-D-aspartate) efflux from rat hippocampal slices. ³H-taurine efflux was activated rapidly but inactivated slowly. It was decreased markedly by 100 μM 5-nitro-(3-phenylpropylamino) benzoic acid (NPPB) and 600 μM niflumic acid and inhibited strongly by tyrphostins AG18, AG879 and AG112 (25–100 μM), suggesting a tyrosine kinase-mediated mechanism. Hyposmolarity activated the mitogen-activated protein kinases (MAPK) extracellular-signal-related kinase-1/2 (ERK1/ERK2) and p38, but blockade of this reaction did not affect ³H-taurine efflux. Hyposmosis also activated phosphatidyl-inositide 3-kinase (PI3K) and its prevention by wortmannin (100 nM) essentially abolished …

In the summer of 1996, we screened 18,931 calves in 128 beef herds located in five US states for persistent bovine viral diarrhea virus (BVDV) infection. Of these, 76 herds were randomly selected from the client database of collaborating veterinary practices, and 52 herds were suspected by the collaborating veterinarians to have BVDV infection based on history or clinical signs. Serum was obtained from each calf in the cooperating herds prior to 4 months of age and tested for the presence of BVDV by microtiter virus isolation.
