Hyperthermia is an extensively studied cytotoxic agent, with strong radio- and chemosensitizing potential. Recent positive clinical trials combining superficial or deep heating techniques with radiation therapy strongly support a role for hyperthermia as an adjuvant to radiation. Many in vitro and in vivo studies have shown that acute extracellular acidification will compromise fundamental protective cellular responses and enhance tumor response to hyperthermia and chemotherapy.

Breast cancers, like most other tumors, exhibit elevated levels of lactate production that provides a basis for selective acidification. A phase I/II clinical trial is underway to test the hypothesis that hyperglycemia-induced acute acidification will sensitize …

Research on mediation processes have addressed the role of the mediator, but often obliquely. In divorce mediation, the literature has sketched various approaches to the role, which diverge from each other in some significant ways. What is lacking is an integrated picture as to how these approaches to the field differ on key questions in the practice of divorce mediation.

The formation of composite films of double-stranded DNA and cationic lipid molecules (octadecylamine, ODA) and the hybridization of complementary single-stranded DNA molecules in such composite films are demonstrated. The immobilization of DNA is accomplished by simple immersion of a thermally evaporated ODA film in the DNA solution at close to physiological pH. The entrapment of the DNA molecules in the cationic lipid film is dominated by attractive electrostatic interaction between the negatively charged phosphate backbone of the DNA molecules and the protonated amine molecules in the thermally evaporated film and has been quantified using quartz crystal microgravimetry (QCM). BACKGROUND: The survival of recipients of cardiac allografts is limited by rejection, lymphoproliferative disease, and coronary vasculopathy. The purpose of this study in children who had received heart transplants was to evaluate the cardiac allografts for myocardial viral infections and to determine whether the presence of viral genome in the myocardium correlates with rejection, coronary vasculopathy, or graft loss.

METHODS: We enrolled heart-transplant recipients 1 day to 18 years old who were undergoing evaluation for possible rejection and coronary vasculopathy. Endomyocardial-biopsy specimens were evaluated for evidence of rejection with the use of standard criteria and were analyzed for the presence …

AIM: Extraction of a tooth can lead to alveolar ridge resorption which can be minimized by socket preservation. The aim of this study is to analyze vertical and horizontal alveolar ridge dimensions clinically and by CBCT immediately following extraction and 3-4 months following socket preservation. METHODS: The preserved group (P) consisted of 20 patients with1-2 non-molar teeth requiring extraction with socket preservation, while the control group (C) consisted of 5 patients requiring extraction alone. The aim of Schunn, Crowley and Okada’s (1998) study is to address the question of whether the current state of cognitive science, as represented by Cognitive Science and the Cognitive Science Society, “reflects the multidisciplinary ideals of its foundation.” To properly interpret and respond to their results, we need to ask a prior question: What is cognitive science’s multidisciplinary ideal? There are at least two conceptions—a “localist” conception, which seems to be implicit in Schunn, Crowley and Okada’s discussion, and a “holist” conception. I argue that while both have been endorsed by some cognitive scientists, there are reasons for preferring …

The expansion of managed care, which is premised on containing costs while maintaining market share, represents a radical transformation of the U.S. health care system. A central question for medical sociologists is to determine how the changes brought by managed care influence health insurance and health care options. In evaluating the impacts of managed care, obstetrics is a particularly important site of study. Part of this agenda involves gathering baseline data on a specific transformation in obstetrics practice new to the managed care scene: the emergence of health insurance coverage for the option of home birth. Interpersonal Functioning Among Women Reporting A History Of Childhood Sexual Abuse: Empirical Findings And Methodological Issues, David Dilillo

A subset of research exploring the long-term impact of child sexual abuse (CSA) has examined the adult interpersonal functioning of female survivors. The present review discusses empirical findings and critical methodological issues related to this important but often overlooked aspect of adult adjustment. Though characterized by several methodological limitations, this literature, as a whole, suggests that early sexual abuse represents a risk factor for a range of interpersonal dysfunction among female survivors, including problems with intimate partner relations, disturbed sexual functioning, and difficulties in the parental role. Suggested methodological improvements for future research include new approaches to the measurement of …

Infants with neurological disabilities, such as cerebral palsy (CP) and Down syndrome (DS), show severe delays in motor and cognitive development relative to chronological age. Involvement of neural plasticity as a treatment of such neurological disorders is increasingly being seen. Previous studies have found that interventions involving sensory linked motor performance have been critical in facilitating motor improvement. The purpose of this study was to compare self-initiated mobility in children with CP and DS. This policy brief1 is the second in a series of reports focusing on the design of state SCHIP programs as they near full implementation. It examines the extent to which state agencies adopt conventional insurance norms or adhere to special principles of Medicaid coverage design for children in designing separately administered (or freestanding) SCHIP programs. The issue of coverage design is particularly relevant for children with low prevalence conditions and special health care needs. Increasingly, conventional insurance uses standardized coverage norms to limit coverage and treatment. This study assessed the extent to which laboratory methods recommended by the Centers for Disease Control and Prevention were used in tuberculosis testing of patients in California in 1998. While recommended methods were used for most patients, there was room for improvement by hospital and independent non-health maintenance organization laboratories.

การเปรียบเทียบผลการระงับปวดของยา Ibuprofen และ Mefenamic Acid หลังผ่าตัดฟันคุด, วนิดา เตชาวัฒนวิศาล, วรรณวลี เจริญกุล, วลีรัตน์ ศุกรวรรณ, วัชรียา ทองรัตนาศิริ, วิธูวรรณ แผ้วชนะ, สมชาย เศรษฐศิริสมบัติ

วัตถุประสงค์ เปรียบเทียบผลการระงับปวดหลังผ่าตัดถอนฟันคุดของยาแก้ปวดประเภท NSAIDs 2 ชนิด คือ ibuprofen และ mefenamic acid และวิธีบริหารยาใน 2 ลักษณะ คือ รับประทานเป็นช่วงเวลาอย่างสม่ําเสมอหลัง การผ่าตัด และรับประทานเมื่อปวด วัสดุและวิธีการ เก็บข้อมูลจากแบบสอบถามที่ได้จากการสัมภาษณ์ผู้ป่วยในกลุ่มทดลองจํานวน 40 ราย ที่เข้ารับ การผ่าตัดเพื่อถอนฟันคุด #38 หรือ 148 ชนิดที่ตัวฟันบางส่วนโผล่เหนือเหงือกโดยนิสิตชั้นปีที่ 5 และได้รับยา แก้ปวดในรูปแบบต่าง ๆ กันหลังการผ่าตัด โดยแบ่งเป็น 4 กลุ่ม กลุ่มละ 10 คน นัดผู้ป่วยกลับมาตรวจ 1 สัปดาห์ ภายหลังการผ่าตัดเพื่อตัดไหมและให้ตอบแบบสอบถามอาการปวดภายหลังผ่าตัด และนับจํานวนยาแก้ปวดที่เหลือแล้วนําข้อมูลที่ได้มาทําการเปรียบเทียบด้วยวิธีการทางสถิติ โดยใช้วิธี one-way ANOVA ผลการศึกษา ยา mefenamic acid และยา ibuprofen มีปริมาณยาที่เหลือเฉลี่ยไม่ต่างกันอย่างมีนัยสําคัญ และเมื่อ เปรียบเทียบวิธีบริหารยาที่แตกต่างกันพบว่าวิธีการรับประทานยาทุก 4 ชั่วโมงของยาทั้ง 2 ชนิดจะเหลือปริมาณยาเฉลี่ยต่อคนมากกว่าการรับประทานยาเมื่อปวด สรุป แนะนําให้จ่ายยา ibuprofen โดยวิธีให้รับประทานทุก 6 ชั่วโมง หลังจากการผ่าตัดฟันคุดเนื่องจากยา ibuprofen มีผลไม่พึงประสงค์ของยาน้อยกว่าและมีราคาถูกกว่ายา mefenamic acid แต่ให้ผลในการระงับปวดไม่แตกต่างกันอย่างมีนัยสําคัญ

คอลลาเจนชนิดที่ I เป็นโปรตีนชนิดเส้นใยแบบไม่ละลาย ที่พบมากที่สุดในกระดูก เนื้อฟัน ผิวหนังแท้และ เอ็นกล้ามเนื้อ ในบทความปริทัศน์นี้ จะเน้นถึงกลไกการสร้างและการควบคุมคอลลาเจนชนิดที่ 1 ซึ่งประกอบไปด้วย กระบวนการถอดรหัสพันธุกรรมเพื่อสร้างสายอาร์เอ็นเอนํารหัส การเคลื่อนย้ายสายอาร์เอ็นเอนรหัสจากนิวเคลียสไปสู่ไซโทพลาสซึม, การแปลสัญญาณของนิวคลีโอไทด์บนอาร์เอ็นเอนํารหัสไปเป็นสายคอลลาเจน และ การปรับปรุงสายคอลลาเจนเพื่อให้เหมาะสมกับการทํางาน การมีส่วนเกี่ยวข้องของนิวเคลียร์โปรตีน อาทิเช่น Spl, CAT binding protein (CBF), NF-I และ NP/NMP4 ในการควบคุมการถอดรหัสพันธุกรรม รวมทั้งการศึกษาที่นําไป สู่แนวความคิดที่เสนอว่าเซลล์ของแต่ละเนื้อเยื่อมีการสร้างและควบคุมคอลลาเจนชนิดที่ 1 ที่แตกต่างกัน ดังนั้น ความรู้ที่ได้จึงอาจนํามาประยุกต์ใช้ในการป้องกันการเกิดโรคเช่น ภาวะเกิดพังผืด ภาวะกระดูกพรุน และโรคปริทันต์อักเสบ

ผลกระทบของการร่วมจ่ายในสวัสดิการรักษาพยาบาลข้าราชการต่อการใช้บริการ ทันตกรรมของผู้ป่วยในคลินิกนอกเวลา, ชาญชัย โห้สงวน, นิรมล ดีประเสริฐกุล, บุษกร สายสอน, เบญจวรรณ เลิศปันณะพงษ์

วัตถุประสงค์การวิจัยนี้มีวัตถุประสงค์เพื่อประเมินผลกระทบของการร่วมจ่ายในสวัสดิการรักษาพยาบาลข้าราชการ ต่อพฤติกรรมการใช้บริการทันตกรรม ของผู้ป่วยในคลินิกทันตกรรมนอกเวลาราชการ วัสดุและวิธีการ กลุ่มตัวอย่างคือผู้ป่วยอายุระหว่าง 18-60 ปี ที่มาใช้บริการทันตกรรมในคลินิกบริการทันตกรรม พิเศษ โรงพยาบาลคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ในระหว่างเดือนมกราคม-เดือนกุมภาพันธ์ พ.ศ. 2542 ใช้แบบสอบถามเก็บข้อมูลผู้ป่วยจํานวน 468 ราย แบ่งเป็นกลุ่มข้าราชการ/รัฐวิสาหกิจ 209 ราย และ กลุ่มอาชีพเอกชน 259 ราย ทําการวิเคราะห์เปรียบเทียบพฤติกรรมการใช้บริการทันตกรรมระหว่างผู้ป่วยทั้ง 2 กลุ่ม ผลการศึกษา อัตราการใช้บริการทันตกรรม รวมทั้งจํานวนค่าใช้จ่ายทางทันตกรรมในปี พ.ศ. 2541 เทียบกับปี พ.ศ. 2540 ไม่มีความแตกต่างกันอย่างมีนัยสําคัญ ทั้งในกลุ่มข้าราชการและกลุ่มอาชีพเอกชน แต่อย่างไรก็ตาม ผู้ป่วย กลุ่มข้าราชการใช้บริการทันตกรรมลดลงในประเภทการอุดฟัน (p = 0.045) และการขูดหินน้ําลาย (p = 0.023) และ ยังพบว่ามีความแตกต่างกันอย่างมีนัยสําคัญเกี่ยวกับความคิดเห็นของผู้ป่วยในด้านผลกระทบของการร่วมจ่ายใน สวัสดิการรักษาพยาบาลต่อการใช้บริการทันตกรรม (p < 0.001) สรุป การร่วมจ่ายในสวัสดิการรักษาพยาบาลข้าราชการ อาจมีส่วนส่งผลกระทบต่อพฤติกรรมการใช้บริการทันตกรรม ของผู้ป่วยกลุ่มข้าราชการในคลินิกบริการทันตกรรมพิเศษ

งานทันตกรรมในผู้ป่วยโรคเลือดออกง่ายหยุดยากต้องอาศัยความร่วมมือระหว่างทันตแพทย์และแพทย์ผู้รักษา กลไกการห้ามเลือดที่ผิดปกติเกี่ยวกับหลอดเลือด เกล็ดเลือด และปัจจัยการแข็งตัวของเลือดก่อให้เกิดอาการแสดง ออกของภาวะเลือดออกง่ายที่แตกต่างกัน ดังนั้น การซักประวัติ ตรวจร่างกายและการตรวจทางห้องปฏิบัติการที่ เหมาะสม จะช่วยให้การวินิจฉัยสาเหตุภาวะเลือดออกง่ายหยุดยาก ซึ่งจะนํามาสู่การรักษาที่จําเพาะ รวมทั้งการให้ ส่วนประกอบของเลือดทดแทน การรักษาเฉพาะที่ด้วย dental splint และกาวไฟบริน ช่วยลดภาวะเลือดออกและลด การให้ส่วนประกอบของเลือดในผู้ป่วย อย่างไรก็ตามการไฟบรินชนิดสําเร็จรูปเป็นสินค้าจากต่างประเทศที่มีราคาแพง เมื่อเปรียบเทียบกับกาวไฟบรินที่เตรียมจากไครโอปริซิเตทผงและธร้อมบิน (100 ยูนิต/มล.) ดังนั้น dental splint และกาวไฟบริน เป็นการรักษาเฉพาะที่ในงานทันตกรรม ลดภาวะเลือดออก ลดการรับ ส่วนประกอบของเลือดในผู้ป่วยโรคเลือดออกง่ายหยุดยาก หรืออาจไม่ต้องใช้ส่วนประกอบของเลือดเลย
