Nucleolar organizer regions (NORs) are chromosomal segments in which ribosomal RNA is encoded. The AgNOR technique, which reveals these regions, has rarely been used in diagnosis and preceding the prognosis of pituitary adenoma. The purpose of this study was to evaluate the correlation of hormonal profile with the AgNOR counts, and the correlation of recurrence with the AgNOR counts and to determine the concordance between the evaluations of two different observers. Lung Carbon Dioxide Elimination Corralates With Physiologic Dead Space Volume During Mechanical Ventilatory Support, Murat Sungur, Muhammed Güven

Increased mean airway pressure (Paw) predisposes to increased alveolar dead space volume and, hence, physiologic dead space volume (V{Dphys}. This is the result of overdistending alveoli, converting Zone 2 and Zone 3 units to Zone 1 units. Lung carbon dioxide elimination (LCO_2) is a reflection of pulmonary capillary blood flow. It is hypothesized that as Zone 1 units form or V{Dphys} increases, LCO_2 decreases proportionately and eventually PaCO_2 increases. The purpose of this study is to determine if LCO_2 correlates with V{Dphys} during mechanical ventilation. Aim: To investigate the effects of oral L-glutamine, insulin and laxative on the severity of acute pancreatitis. Materials and Methods: Forty adult male Sprague-Dawley rats were divided into 4 groups. Experimental pancreatitis was induced by ligating the main biliopancreatic duct. All groups were given standard rat pellets and tap water. In addition Group II was given 15 mg/kg/day of L-glutamine via a catheter enterally, Group III was given a 3 ml/day fleet enema via a catheter enterally, and Group IV was given 3 IU/kg/day of NPH insulin via a catheter enterally. This study was conducted in an endemic goitre area to determine thyroid profiles TSH, freeT4 (fT4), T4, fT3 and T3, as well as the degree of the severity of hypothyroidism and the prevalence of the disease. Tonbak is a small village in Kahramanmaraş. The iodine content of the water is < 1 mg/L. A randomly selected group of 121 subjects aged 16-55 were included in the study. The percentage of goitre was 71.1% upon physical examination. Subjects with primary hypothyroidism were 16.5%. Mean scores of fT4 were 8.9±2.6 pmol/l (range: 2.7-12.9) and mean scores of TSH were 18.1±26.1 mU/ml (range: 4.33-100

As the practice of outpatient paediatric surgery advances, the search continues for anaesthesia that provides rapid smooth induction, stable patient haemodynamics, rapid emergence and minimal unpleasant side effects. Therefore, this study was designed to compare the haemodynamic changes, emergence and recovery characteristics of remifentanil-propofol (TIVA) and sevoflurane anaesthesia for adenoidectomy with bilateral myringotomy and insertion of tubes and/or tonsillectomy (ENT surgery) in children. Sublingual Valsartan In Hypertensıve Urgency, Yüksel Gökel, Sai̇me Paydaş, Güven Kuvandik, Nazan Alparslan

The objective of this study was to assess the effect of sublingual valsartan in a group of patients with hypertensive urgency. Forty-one patients with hypertensive urgency and systolic blood pressure >200 mmHg, diastolic blood pressure >100 mmHg were studied in an emergency room. Supine blood pressure readings were taken and the patients were given 80 mg of valsartan sublingually. Blood pressure and heart rate were recorded at 15 min intervals over a 90 min period. Systolic blood pressure decreased from 211.22±14.65 mmHg to 158.17±15.48 mmHg, diastolic blood pressure dropped from 120.12±13.44 mmHg to 93.05±6.01 mmHg. Although senescence is a defining property of euploid mammalian cells, its physiologic basis remains obscure. Previously, cell kinetics properties of normal tissue cells have not been considered in models for senescence. We now provide evidence that senescence is in fact the natural consequence of normal in vivo somatic stem cell kinetics extended in culture. This concept of senescence is based on our discovery that cells engineered to conditionally express the well-recognized tumor suppressor protein and senescence factor, p53, exhibit asymmetric cell kinetics. In September 1997, the Population Council and Lusaka’s University Teaching Hospital (UTH) launched a 15-month study to identify and explore the range of issues relating to the introduction of emergency contraception (EC) within a developing country context. The study allowed clinic-based family planning providers to accumulate enough first-hand experience to be able to identify strategies for overcoming difficulties associated with the introduction or delivery of EC services. One issue on most participants’ minds was the need to expand the delivery of EC services toward young women, especially out-of-school women, who are harder to reach. Living Arrangements Of Older Adults In The Developing World: An Analysis Of Dhs Household Surveys [Arabic], John Bongaarts, Zachary Zimmer

Poverty, Gender, and Youth

Using data from Demographic and Health Surveys, this study examines living arrangements of older adults in 43 developing countries and compares patterns by gender, world regions, and macro-level measures of socioeconomic development. Indicators include household size, headship, relationship to head, and coresidence with spouse, children, and others. Unweighted regional averages and OLS regressions determine whether variations exist. Average household sizes are large, but a substantially higher proportion of elderly adults live alone than do individuals in other age groups. Females are more likely than males to live alone and are less likely to live with a spouse or to head …

The paper explores empirically the relationship between school quality in Egyptian preparatory (middle) schools and the likelihood of school dropout either during preparatory school or before the completion of secondary school. Despite strong empirical evidence for the many positive social and economic returns associated with more years of schooling, there has been little research exploring how the quality of particular schools might influence grade levels attained. The interrelationships between ecological degradation, poverty, and rural out-migration in Ethiopia are examined using data from a Household and Community Survey conducted in 1994-95. The survey, which covered a sample of 2,000 households, collected retrospective data on changes in household composition, including migration of household members, during the period 1984 to 1994. The study hypothesizes that the decision to out-migrate in the impoverished rural areas of northern Ethiopia is influenced by a combination of factors based on individual, household and community characteristics. A multilevel analysis is applied to determine the role of these factors in the decision. The findings show …

This work focused on the regulation of the different isotypes of beta-tubulin during cardiac hypertrophy, with the specific hypothesis that increased levels of the beta I isotype leads to the increase in microtubule density seen during hypertrophy. Partial cDNAs for the three beta-tubulin isotypes expressed in the heart (I, II, and IV) have been cloned and used to show that there is a differential upregulation of the beta I and beta II isotypes during cardiac hypertrophy. Messenger RNA half-life studies of cells isolated from hypertrophied myocardium have shown that this upregulation is due to increased transcription of the genes encoding …

At the core of Kenneth Arrow’s classic 1963 essay on medical uncertainty is a claim that has failed to carry the day among economists. This claim—that physician adherence to an anti-competitive ethic of fidelity to patients and suppression of pecuniary influences on clinical judgment pushes medical markets toward social optimality—has won Arrow near-iconic status among medical ethicists (and many physicians). Yet conventional wisdom among health economists, including several participants in this symposium, holds that this claim is either naïve or outdated. To support the Government of Ghana’s plan to expand community-based distribution (CBD) programs, the Planned Parenthood Association of Ghana (PPAG) and the Population Council conducted a study in 1999 of the CBD programs of 13 nongovernmental agencies. The study also assessed in depth PPAG’s CBD program, which is the country’s largest and oldest. Data sources included interviews with 301 CBD agents, 27 supervisors, and 20 clinicians in rural and urban areas in 16 districts; observations of 51 PPAG agents interacting with 6 clients each; and 15 focus group discussions with community members, former CBD agents, and CBD clients.
