Exploring The Interaction Between Cranial Nerves V And Vii In Chronic Eyelid Myokymia: A Case Report, Karina Miramontes, Adam Walsh, Tomas Tafoya

Eyelid myokymia involves involuntary contractions of the orbicularis oculi muscle and is commonly associated with stress, fatigue, and craniocervical factors. Emerging evidence suggests that altered trigeminocervical interactions and craniomandibular muscle tension may contribute to symptom persistence; however, the condition remains underrecognized in physical therapy. This report describes a 65-year-old woman with chronic benign eyelid myokymia and cervical pain who completed 12 physical therapy sessions that included suboccipital functional dry needling, temporomandibular soft-tissue mobilization (STM), and cervical stabilization exercises. A reduction in eyelid myokymia was first observed at the fifth visit, coinciding with the initiation of temporomandibular STM. These findings suggest …

Gemella morbillorum is a gram-positive coccus found in the gastrointestinal tract, and is an uncommon cause of human infections, including prosthetic joint infections (PJI). Reported risk factors include immunosuppression, recent gastrointestinal procedures, and poor dental hygiene. This report describes a case of knee PJI caused by G. morbillorum, likely due to hematogenous seeding following a colonoscopy three months earlier. The patient is a 70-year-old man with well-controlled diabetes mellitus who had undergone total knee arthroplasty three years prior. He presented with three weeks of left knee swelling. The pisotriquetral joint is the smallest of the four joints comprising the wrist and is a source of ulnar-sided wrist pain. This condyloid joint connects the pisiform to the triquetrum and is supported by several surrounding ligaments. A variety of pathologies may affect the pisotriquetral joint, including the presence of loose bodies. Because of its small size and the inherent difficulty of visualization, the pisotriquetral joint is often overlooked as an independent joint. Purpose: This study investigated how medical students – as future physicians – perceive the relevance of Mintzberg’s ten managerial roles to physician leadership, and whether these perceptions change throughout medical education.

Method: In 2021, all first‑year (n = 116/116) and 98% of final‑year (n = 107/109) students at the University of Oulu, Finland completed a web‑based survey rating Mintzberg’s ten managerial roles in fulfilling physician leaders’ leadership work on a 5‑point Likert scale. Item reliability and internal consistency were examined with Cronbach’s α. Background: Heavily calcified or destroyed mitral annulus present significant technical challenges for conventional valve replacement, often requiring alternative surgical strategies. Aim: To evaluate the safety, efficacy, operative outcomes, and the competency of valved conduit in patients with heavily calcified or destroyed mitral valve annulus. This case report describes a patient who was presented with severe incisional pain following an uncomplicated total knee arthroplasty. There was suspicion for an incisional neuroma; however, excisional biopsy revealed an extradigital glomus tumor. The patient had immediate symptom resolution post-operatively. Clinicians should maintain a suspicion for the possibility of extra-digital glomus tumors at sites of prior extremity surgery, causing exquisite tenderness and pain.

Aim: This study investigates the mechanical performance of obturators fabricated from PEKKTON, PEEK, and traditional Cobalet-chromium materials using finite element analysis. Methods : One three dimensional finite element model was created for this study. Objective: To investigate the impact of serum vitamin D levels on marginal bone loss (MBL) in patients restored with unsplinted-mandibular-implant overdentures using LOCATOR attachments. Materials and Methods: Thirty participants rehabilitated with three-implant-mandibular overdentures were included. Based on serum vitamin D at the time of prosthetic loading, participants were categorized into two equal groups: a test group with deficient vitamin D (< 20 ng/mL) and a control group with sufficient levels (≥30 ng/mL). Objective: This study aimed to establish cephalometric norms for the Tanzanian population, assess sexual dimorphism in cephalometric measurements, and compare these findings with established Caucasian and West African norms to enhance orthodontic diagnosis and treatment planning. Materials and Methods: A crosssectional study was conducted on 309 subjects (142 males, 167 females) at Muhimbili University Dental Clinic. Cephalometric radiographs were digitally traced using Quick Ceph Studio. Reliability was assessed using the inter-class correlation coefficient. The Kolmogorov-Smirnov test checked normality, while one-sample t-tests and Student’s ttests (p < 0.05) were used for comparisons. Data were analyzed with RStudio. Results: Tanzanians showed significantly smaller craniofacial measurements than Caucasians. Males had larger skeletal dimensions than females. Compared to West Africans, Tanzanians had generally higher values, with a few exceptions. Conclusion: Tanzanians display distinct craniofacial characteristics, differing from both Caucasians and West Africans. These findings support the need for population-specific cephalometric norms to guide accurate orthodontic diagnosis and treatment.

บทความวิชาการนี้เป็นการสังเคราะห์องค์ความรู้เกี่ยวกับสมรรถนะของพยาบาลเฉพาะทางการบำบัดทดแทนไตและแนวทางการพัฒนาคุณภาพการดูแลผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายโดยทบทวนวรรณกรรมและเอกสารทางวิชาการที่เกี่ยวข้องทั้งในและต่างประเทศรวมถึงแนวปฏิบัติทางคลินิกและมาตรฐานวิชาชีพพยาบาลเพื่อนำเสนอกรอบสมรรถนะหลักที่จำเป็นสำหรับพยาบาลเฉพาะทางในการให้การดูแลผู้ป่วยที่ได้รับการบำบัดทดแทนไตทั้ง3รูปแบบได้แก่การฟอกเลือดด้วยเครื่องไตเทียม การล้างไตทางช่องท้อง และการปลูกถ่ายไต  ผลลัพธ์ที่ได้จากการศึกษาพบว่าสมรรถนะหลักของพยาบาลเฉพาะทางการบำบัดทดแทนไตประกอบด้วย 9 หัวข้อสำคัญ ได้แก่ 1)สมรรถนะด้านการปฏิบัติการพยาบาลทางคลินิก 2)สมรรถนะด้านเทคโนโลยีและทักษะการใช้เครื่องมือฟอกไต 3)สมรรถนะด้านการสอนและการให้คำปรึกษา 4)สมรรถนะด้านจริยธรรมและความเป็นมืออาชีพ 5)สมรรถนะด้านการใช้เทคโนโลยีสาร 6)สมรรถนะด้านการสื่อสารและการทำงานเป็นทีม 7)สมรรถนะด้านการป้องกันและควบคุมการติดเชื้อ 8)สมรรถนะด้านคุณภาพและงานวิจัย 9) สมรรถนะด้านผู้นำและบริหารจัดการการพัฒนาคุณภาพการดูแลสามารถดำเนินการผ่าน4แนวทางหลักคือการพัฒนาศักยภาพบุคลากรอย่างต่อเนื่องผ่านการฝึกอบรมและการพัฒนาทางคลินิกการจัดทำและใช้แนวปฏิบัติทางคลินิกที่เป็นมาตรฐานการพัฒนาระบบติดตามประเมินผลและตัวชี้วัดคุณภาพและการส่งเสริมการทำงานเป็นทีมสหสาขาวิชาชีพ ซึ่งส่งผลให้ผู้ป่วยได้รับการดูแลที่มีคุณภาพ ลดภาวะแทรกซ้อน และมีคุณภาพชีวิตที่ดีขึ้น

ในยุคแห่งการเปลี่ยนผ่านสู่เทคโนโลยีดิจิทัล ปัญญาประดิษฐ์ (Artificial Intelligence: AI) มิได้เป็นเพียงเครื่องมือที่ช่วยวิเคราะห์ข้อมูลเท่านั้น แต่กำลังพลิกโฉมบทบาทของวิชาชีพพยาบาลอย่างรอบด้าน บทความนี้มีวัตถุประสงค์เพื่อวิเคราะห์ผลกระทบของปัญญาประดิษฐ์ต่อบทบาทของพยาบาลระดับปฏิบัติการขึ้นไปผ่านกรอบแนวคิด 3 มิติ ได้แก่ มิติด้านการปฏิบัติงาน มิติด้านความสัมพันธ์และจริยธรรม และมิติด้าน อัตลักษณ์ทางวิชาชีพ  ผลการวิเคราะห์พบว่า มิติด้านการปฏิบัติงาน ปัญญาประดิษฐ์ปรับบทบาทพยาบาลจากผู้ปฏิบัติตามกระบวนการสู่ผู้วิเคราะห์และกำกับการใช้ข้อมูลเพื่อสนับสนุนการตัดสินใจทางคลินิกมิติด้านความสัมพันธ์และจริยธรรมแม้ปัญญาประดิษฐ์จะเพิ่มความแม่นยำในการประเมินและเฝ้าระวังภาวะสุขภาพแต่ยังมีข้อจำกัดในด้านความฉลาดทางอารมณ์ ซึ่งไม่สามารถทดแทนความเข้าอกเข้าใจ การสัมผัสและเอาใจใส่ต่อผู้ป่วยได้ สำหรับมิติด้านอัตลักษณ์ทางวิชาชีพ การประยุกต์ใช้ปัญญาประดิษฐ์นำไปสู่การปรับขอบเขตความเชี่ยวชาญของพยาบาลโดยจำเป็นต้องพัฒนาสมรรถนะด้านดิจิทัลและการคิดเชิงวิพากษ์ควบคู่กับการดูแลที่ยึดผู้ป่วยเป็นศูนย์กลาง บทความนี้เสนอแนวคิดว่า พยาบาลควรเป็นผู้ใช้ปัญญาประดิษฐ์อย่างมีวิจารณญาณ โดยยึดหลักแนวคิดการบูรณาการระหว่างศักยภาพมนุษย์และปัญญาประดิษฐ์เพื่อยกระดับบทบาทของพยาบาลให้ สามารถคิด วิเคราะห์ และสร้างนวัตกรรมการดูแลที่ผสมผสานระหว่างเทคโนโลยีและหัวใจของความเป็นมนุษย์ได้อย่างสมดุล
