Western Journal of Orthopaedics
High-energy proximal tibia fractures present significant challenges for orthopaedic surgeons, with well-recognized risks including malreduction and nonunion. Although double plating through two mini-open parallel incisions is an established technique for selected patients, nonuion can still occur. One potential contributing factor is entrapment of the popliteus muscle within the distal spike of the posteromedial plateau fragment. This report uses illustrative cases to demonstrate how to identify, address, and prevent this complication. Strategies for managing delayed cases that progress to nonunion are also discussed

Fracture fixation procedures typically require precise retraction of soft tissues in order to provide adequate exposure of bone fragments for reduction and fixation. Such procedures require the use of multiple surgical assistants to maintain proper tissue retraction. This manual retraction using traditional instruments, such as Hohmann and Weitlaner retractors, can lead to inefficiencies and poor labor allocation within the operating room (OR). The purpose therefore is to develop and evaluate a three dimensional (3D)-printed Nylon PA12 adapter designed to dock Hohmann retractors onto Weitlaner retractors, enabling hands-free retraction during fracture fixation procedures and potentially improving labor allocation in the OR. …

Background: The purpose of this study is to compare recovery and infection outcomes between teenagers and adults who undergo surgery for traumatic spine injuries and the effect of steroid administration. Steroids are often used in traumatic spine injuries to help recovery, however there is a concern that steroid use could increase the risk of infection due to the suppression of the immune system. We examined the impact of postoperative steroid use on recovery rates and infection rates in both age groups.

Benign Painful Soft Tissue Tumors Of The Digits: A Contemporary Review, Nethmi Withanage, Athikur Rahman, Lawrence Hurst, Samantha Muhlrad

Purpose: Benign painful tumors of the digit are commonly encountered in hand surgery yet frequently pose diagnostic challenges due to overlapping clinical presentations. This review synthesizes current evidence on eight benign soft tissue tumors and cystic lesions that commonly cause digital pain: glomus tumor, giant cell tumor of the tendon sheath (GCTTS), digital mucous cyst, digital ganglion cyst, epidermoid inclusion cyst, pyogenic granuloma, nerve sheath tumors, and angioleiomyoma.
Methods: A comprehensive review of PubMed/MEDLINE was performed, supplemented by hand surgery textbooks and online educational resources. Case series, retrospective cohort studies, and review articles were included.

Law and medicine are often considered to be among the least accessible professions, partly because of the heavy use of non colloquial jargon. This article examines more than 150 instances in which Billy Joel’s lyrics have been used to improve the accessibility of legal and medical scholarship. Legal and medical professionals have used the words of the “Piano Man” to discuss a variety of topics—everything from civil procedure to property rights and the COVID-19 pandemic to colorectal surgery. วัตถุประสงค์ : เพื่อศึกษาผลของโปรแกรมการมีส่วนร่วมในการดูแลเด็กที่มีไข้สูงต่อความพึงพอใจของผู้ดูแล หอผู้ป่วยเด็ก โรงพยาบาลศิครินทร์ กรุงเทพมหานคร
แบบแผนงานวิจัย : การวิจัยแบบกึ่งทดลอง แบบศึกษาสองกลุ่มวัดครั้งเดียว
วิธีดำเนินการวิจัย : กลุ่มตัวอย่างเป็นผู้ดูแลอายุ 20 ปีขึ้นไป ที่รับผิดชอบดูแลผู้ป่วยเด็กที่มีอาการสำคัญในการมาโรงพยาบาลด้วยมีไข้สูง อุณหภูมิกายมากกว่า 38.5 องศาเซลเซียส ซึ่งเป็นอุณหภูมิแรกรับที่รับผู้ป่วยเข้ารับการรักษาตัวในหอผู้ป่วยเด็ก แบ่งเป็นกลุ่มทดลอง และกลุ่มควบคุม กลุ่มละ 24 คน กลุ่มทดลองได้รับโปรแกรมการมีส่วนร่วมในการดูแลเด็กที่มีไข้สูงตามแนวคิดการมีส่วนร่วมของผู้ดูแลร่วมกับการทบทวนวรรณกรรม กลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามข้อมูลทั่วไปของผู้ดูแลและของเด็กที่มีไข้สูง แบบสอบถามความพึงพอใจของผู้ดูแลต่อการพยาบาลเด็กที่มีไข้สูง โดยผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ 3 คน มีค่าดัชนีความตรงตามเนื้อหาเท่ากับ 1 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติทดสอบที
ผลการวิจัย : ค่าเฉลี่ยความพึงพอใจของผู้ดูแลระหว่างกลุ่มที่ได้รับโปรแกรมการมีส่วนร่วมในการดูแลเด็กที่มีไข้สูง สูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 (t = 5.182)
สรุป : โปรแกรมการมีส่วนร่วมในการดูแลเด็กที่มีไข้สูง ส่งผลให้ผู้ดูแลเด็กที่มีไข้สูงเกิดความพึงพอใจสูงขึ้น ผู้บริหารการพยาบาล ควรนำโปรแกรมการมีส่วนร่วมในการดูแลเด็กที่มีไข้สูงนี้ไปอบรมพยาบาลเด็ก เพื่อนำไปสอนญาติหรือผู้ดูแลเด็กที่มีไข้สูงได้อย่างมีประสิทธิภาพ

วัตถุประสงค์: เพื่อเปรียบเทียบความสะอาดของลำไส้ใหญ่ และความพึงพอใจการบริการพยาบาล ผู้ที่เตรียมตัวก่อนส่องกล้องลำไส้ใหญ่ครั้งแรกก่อน-หลังเข้าร่วมโปรแกรมส่งเสริมการจัดการตนเองโดยการให้ข้อมูลอย่างมีแบบแผน
รูปแบบการวิจัย: การวิจัยแบบกึ่งทดลอง (Quasi-experimental research design) แบบ 2 กลุ่ม
วิธีการดำเนินการวิจัย: การวิจัยนี้เก็บข้อมูลในระหว่างวันที่ 1 กุมภาพันธ์ ถึง 30 เมษายน 2567 ในผู้ที่เตรียมตัวก่อนส่องกล้องลำไส้ใหญ่ครั้งแรก ณ หอผู้ป่วยนอก แผนกโรคทางเดินอาหารและตับ โรงพยาบาลเอกชน ที่อายุตั้งแต่ 20 ปีขึ้นไป ทั้งเพศชายและเพศหญิง จำนวน 50 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 25 คน โดยจับคู่กลุ่มตัวอย่างให้มีคุณสมบัติใกล้เคียงกันด้วย อายุและโรค กลุ่มทดลองได้รับโปรแกรมส่งเสริมการจัดการตนเองโดยการให้ข้อมูลอย่างมีแบบแผน ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล 3 ชุด ได้แก่ 1) ข้อมูลทั่วไปส่วนบุคคล 2) แบบประเมินสะอาดของลำไส้ 3) แบบประเมินความพึงพอใจบริการพยาบาลของผู้ป่วยนอก สัมประสิทธิ์แอลฟาของครอนบาคได้ 0.8 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และ Mann Whitney U test โดยกำหนดระดับนัยสำคัญทางสถิติที่ .05
ผลการวิจัย: หลังสิ้นสุดโปรแกรม กลุ่มทดลองมีความสะอาดของลำไส้ใหญ่ดีกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และค่าเฉลี่ยคะแนนความพึงพอใจการบริการพยาบาลดีกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05
สรุป: โปรแกรมส่งเสริมการจัดการตนเองโดยการให้ข้อมูลอย่างมีแบบแผน ตามแนวคิดทฤษฎีการจัดการตนเอง Creer (2000) ทำให้ผู้ที่เตรียมตัวก่อนส่องกล้องลำไส้ใหญ่ครั้งแรกมีความรู้ และสามารถในการจัดการตนเองได้ เพื่อความสะอาดของลำไส้ใหญ่และความพึงพอใจการบริการพยาบาลได้

Background and Objective:Menstrual irregularities are a common gynecological complaint, often linked to thyroid dysfunction, which disrupts the hypothalamic–pituitary–ovarian axis. This study sought to assess prevalence of thyroid dysfunction among reproductive-aged women with menstrual irregularities and to evaluate the specific clinical and hormonal correlations between different menstrual patterns and Thyroid Status. Methods:A cross-sectional study was conducted on 120 women presenting with menstrual irregularities. Gunshot wounds (GSWs) with nerve injuries in the 21st century differ significantly from those reported in the 20th century. The vast majority are now low-velocity civilian injuries, in contrast to high-velocity military injuries previously reported. The rate of nerve injury in civilian GSWs is approximately 5%, compared with the 30% reported in military cohorts. However, the most common pattern of nerve injury in the current civilian population is neurotmesis and partial neurotmesis, in contrast to the majority axonotmesis of the military cohorts. Peripheral nerve injuries require reconstruction when direct, tension-free repair is not possible. A range of reconstructive strategies are used to restore continuity and promote axonal regeneration, including graft-based reconstruction, nerve transfers, conduit-based repair, supercharged end-to-side augmentation, and emerging regenerative therapies. Autologous interfascicular nerve grafts and vascularized grafts remain established techniques, whereas processed nerve allografts provide a donor-sparing alternative. Nerve transfers are increasingly used for proximal injuries with long regeneration distances, and conduits are an option for gaps shorter than 5 mm. Background: Clinical rotation scores have been reported to predict resident performance. However, the reporting of clerkship grades varies widely across institutions. The authors explored the correlation of an institution’s reputation and the percentage of students receiving honors. Methods: During the review of applicants for orthopaedic residency, the percentage of students receiving the highest grade in the surgery and internal medicine clerkships was recorded for each institution.
