MDS is a group of abnormality in hematologic stem cells characterized by ineffective hematopoiesis and various clinical presentations. Treatment of MDS is costly, and may not be possible in most areas in Thailand. Valproic acid (VPA) is used for the treatment of convulsion with low adverse reaction. It can also act as histone deacetylase inhibitor, and can be administered orally. โรคตับคั่งไขมันและเบาหวานพบร่วมกันได้บ่อย โดยอาจแบ่งเป็น 2 กลุ่มคือ กลุ่มโรคตับคั่งไขมันที่ยังไม่มีโรคเบาหวานตั้งแต่แรกพบว่ามีโอกาสเกิดโรคเบาหวานใหม่ได้มากกว่ากลุ่มคนที่ไม่มีโรคตับคั่งไขมันสูงถึง 3 เท่า ส่วนกลุ่มที่สอง คือ กลุ่มที่มีทั้งโรคตับคั่งไขมันและเบาหวาน กลุ่มนี้จะมีโอกาสเกิดภาวะพังผืดในตับและมีโอกาสเกิดโรคหลอดเลือดหัวใจตีบหรือ coronary artery disease(CAD) ได้มากกว่าผู้ป่วยโรคตับคั่งไขมันที่ไม่มีเบาหวาน อย่างไรก็ตามการรักษายังทำได้โดยมุ่งรักษาภาวะอ้วน ไขมันในเลือดสูง ความดันโลหิตสูง และเบาหวานที่พบร่วมให้ดี รวมทั้งมุ่งเน้นการการยืนยันวินิจฉัยโรคตับคั่งไขมันและประเมินความรุนแรงโรคตับว่ามีพังผืดหรือไม่โดยการประเมินความรุนแรงของการอักเสบของตับในระบบการให้คะแนนจากข้อมูลทางคลินิกหรือ ระบบ scoring system โดยมี 2ระบบที่ใช้กันบ่อยคือ “NAFLD Fibrosis Score” หรือการให้คะแนนคำนวณจากค่าตัวแปร 6 ตัวได้แก่อายุ ดัชนีมวลกาย ค่าการทำงานตับที่ใช้อัตราส่วนเอนไซม์ตับ AST/ALT, ภาวะเบาหวาน จำนวนเกล็ดเลือด และค่าอัลบูมิน และระบบที่สองคือ BARD score ประกอบด้วยค่าอัตราส่วนค่าการทำงานตับ (AST/ALT ratio) โรคเบาหวาน และค่าดัชนีมวลกาย ซึ่งทั้ง 2 ระบบจะช่วยวางแผนการรักษาโดยคัดกลุ่มเสี่ยงต่อการเกิดพังผืดในตับออกมาได้ และแพทย์ต้องแนะนำให้ปรับวิถีชีวิตโดยต้องตั้งเป้าหมายที่ชัดเจนด้วยการควบคุมอาหาร การออกกำลังกายและต้องลงมือทำอย่างจริงจัง และตั้งเป้าหมายลดน้ำหนักประมาณร้อยละ 10 ในช่วงแรก และตั้งต้นใหม่เสมอหากทำไม่ได้ในครั้งแรกส่วนยาแผนปัจจุบันที่ใช้รักษามีไม่มากนัก และควรเลี่ยงการใช้ยาสมุนไพรหรืออาหารเสริมที่ไม่จำเป็นรวมทั้งงดแอลกอฮอล์ด้วย.

Objective : To assess the efficacy of aromatherapy as an intervention for agitated behaviors in patients with dementia at the Dementia Clinic, King Chulalongkorn Memorial Hospital. The authors report a case of Ehler-Danlos syndrome with gastric volvulus undergoinga successful surgical treatment. The diagnosis was made with imaging studies. Preoperativeresuscitation and nutritional improvement were carried out. Surgical treatment for gastricvolvulus in this patient included gastrojejunostomy and anterior gastropexy. The prevalence of metabolic syndrome is increasing parallel to obesity epidemics. The syndrome includes obesity, insulin resistance, hypertension, and dyslipidemia. Obstructive sleep apnea (OSA) is a disorder characterized by repetitive upper airway obstructions during sleep. OSA is known to increase the risk of metabolic syndrome up to 9 times. Symptoms of OSA include snoring, excessive daytime sleepiness, morning headache, nocturia, nocturnal choking, and witnessed apnea. Hepatorenal syndrome (HRS) is characterized by severe renal arterial vasoconstriction without other underlying pathology. As for the worsening renal function, and it can be reversible after undergoing standard treatment. HRS develops in patients with decompensated cirrhosis who have marked circulatory dysfunction due to splanchnic arterial vasodilatation resulting in deterioration of the cardiac function as well as the renal function. Background : The study of dentine hypersensitivity and predisposing factors may help decrease dentine hypersensitivity and support treatments in patients. Objectives : To investigate possible factors of dentine hypersensitivity among Thai patients visiting King Chulalongkorn Memorial Hospital. Design : A cross-sectional descriptive study. Setting : King Chulalongkorn Memorial Hospital, Bangkok, Thailand. Materials and Methods : Sixty-eight patients were recruited and evaluated to confirm that they were suffering from dentine hypersensitivity due to gingival recession or cervical erosion. การวัดความดันลูกตา เป็นสิ่งที่จำเป็นในการตรวจตาทั่วไป ตามมาตรฐานการวัดความลูกดันตาจะใช้เครื่องมือ Goldman applanation tonometer ซึ่งเป็นการวัดความดันลูกตาโดยใช้เครื่องมือกดลงบนกระจกตาจนทำให้ผิวของกระจกตาราบในระดับหนึ่งแล้ววัดค่าความดันลูกตาในหน่วยมิลลิเมตรปรอท ข้อจำกัดของวิธีนี้คือ ผู้ป่วยที่มีปัญหาที่ผิวกระจกตา หรือผู้ป่วยที่ได้รับอุบัติเหตุต่อดวงตานอกจากนี้ ยังมีความจำกัดด้านทรัพยากรบุคคลที่มีความชำนาญด้านการตรวจ อีกทั้งตัวอุปกรณ์เองและสถานที่ จึงมีความจำเป็นที่จะต้องประมาณค่าความดันตาด้วยการคลำด้วยนิ้วมือในบางครั้งซึ่งวิธีนี้เป็นวิธีที่ง่าย สะดวก และเกิดอันตรายน้อย เนื่องจากวิธีการประมาณค่าความดันลูกตาด้วยนิ้วมือนั้น ยังมีความไม่แน่นอนในการคลำ และยังไม่ทราบถึงค่าความละเอียดที่บอกความสามารถในการคลำด้วยนิ้วมือ การทดลองที่จะหาค่าความละเอียดของการประมาณค่าความดันลูกตาด้วยนิ้วมือในผู้ป่วยจริงนั้นอาจทำให้เกิดปัจจัยรบกวนต่าง ๆ ดังนั้นคณะผู้วิจัยจึงเลือกที่จะทดลองในดวงตาจำลองก่อน จึงเป็นที่มาของการผลิตอุปกรณ์แบบจำลองเพื่อการประเมินค่าความดันลูกตาชนิดนี้

Background : Radiofrequency inferior turbinate reduction has been proven to reduce the volume of the inferior turbinate. The aim of lateral outfracture of the inferior turbinate is to displace the position of the inferior turbinate laterally which makes the nasal cavity wider. Problem/background : Hookworm infection is one of the major neglected tropical infectious diseases causing both overt and cryptic morbidity among ~600 million people living in tropical areas. As conventional hookworm control measures by improving sanitation and mass anthelminthic drug administration are not sustainable, development of hookworm vaccines is mandatory. Background : Azithromycin is a broad spectrum azalide antibiotic which was suggested to be first-line treatment in children for some respiratory tract infections, urethritis and mycobacterium avium complex infection with AIDs complications. The drug is well absorbed and has long half-life which supports a short time administration at higher dose regimens.
