This case-control study included 215 knee OA patients (mean age 64.32 gif.latex?\pm 7.22 years), diagnosed following the criteria of the American College of Rheumatology, and 218 healthy age-matched volunteers (mean age 64.05 gif.latex?\pm 6.08 years). This research has been approved by the Institution Review Board (IRB) of the Faculty of Medicine, Chulalongkorn University. A total of 5 mL of peripheral blood was drawn from each participant, andtransferred into coded sodium citrate anticoagulant tubes.

Background: Family significantly influences the health of family members in both positive and negative ways.There are limited instruments in Thailand for assessing family state and functioning on adult patients.Objectives: To examine the validity and reliability of the family state and functioning assessment scale (FSFAS)which was designed to assess the extent one perceived family issues and functioning.Methods: The scale was conducted on 1,200 Thai adults: 800 outpatients attending hospitals and 400participants in the community. Abnormal enhancement of cranial nerves (CNs) can be observed in a variety of disease. We present eightcases of cranial nerve (CN) enhancement, including inflammatory process, hematologic malignancy, perineural spreading of extracranial tumor, cerebrospinal fluid seeding of high grade primary brain tumor, demyelination and post radiation change. Based on our case series, the findings cover board differential diagnoses. The use of contrast enhanced 3-dimensional (3D) T1-weighted image (T1WI) magnetic resonance imaging (MRI) and reasonable MRI sequences can increase conspicuity of the finding. Furthermore, incorporating underlying disease, clinical duration, and associated intracranial / extracranial findings may help narrow the probable etiologies.

Background: Sudden unexplained nocturnal death syndrome (SUNDS) or Lai-tai is a sudden death that oftenoccurs in Southeast Asia. Background: Preoperative cognitive dysfunction (PreOCD) has not been well described as a risk factor forpoor outcomes in cardiac surgery. This study was aimed to demonstrate the effects of PreOCD on postoperative outcomes in cardiac surgery.Methods: One hundred adult patients undergoing elective open cardiac surgery at King ChulalongkornMemorial Hospital were recruited. Peutz-Jeghers polyps are hamartomas of the small bowel which occur as part of the Peutz-Jeghers syndrome,a rare autosomal dominant condition characterized by gastrointestinal polyps and mucocutaneous melanoticpigmentation. We, hereby, report a case of a young female suffering from this syndrome who presented withhyperpigmentation of the lips and acute intestinal obstruction due to intussusception. An exploratory laparotomy was performed which revealed jejuno-jejunal and jejuno-ileal intussusceptions for which resection and anastomosis was done. There were multiple intraluminal polyps in the jejunum which upon histopathological examination, showed features of Peutz-Jeghers polyp. จุฬาลงกรณ์เวชสาร ฉบับที่ 6 เดือนพฤศจิกายน–ธันวาคม พ.ศ. 2561 ฉบับสุดท้ายของปีนี้ วารสารได้มีการพัฒนาก้าวหน้าดีขึ้นมาก ด้วยเหตุที่ว่า Chulalongkorn Medical Journal ได้รับการตอบรับเข้าไปอยู่ในฐานข้อมูลระดับนานาชาติหลายฐานข้อมูล (journal indexing) เช่น J-Gate, Academic Resource Index, Research Bib, Citefactor, Directory of Research JournalsIndexing (DRJI) Publons และ Google Scholar เป็นต้นดังนั้นในปีหน้า Chulalongkorn Medical Journal จะปรับรูปแบบโดยรับและตีพิมพ์บทความเป็นภาษาอังกฤษเพื่อสอดรับกับนโยบายการนำวิชาการสู่สากลของคณะแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย

เหตุผลของการทำวิจัย : การสูบบุหรี่เป็นปัจจัยเสี่ยงต่อโรคหัวใจและหลอดเลือด โดยทำให้การทำงานของเซลล์เยื่อบุหลอดเลือดผิดปกติและเกิดหลอดเลือดแดงแข็ง แต่ยังไม่มีการศึกษาผลของบุหรี่ต่อการทำงานของเซลล์เยื่อบุหลอดเลือดและการประเมินหลอดเลือดแดงแข็งในอาสาสมัครไทยมาก่อนวัตถุประสงค์ : เพื่อประเมินการทำงานของเซลล์เยื่อบุหลอดเลือดและการประเมินหลอดเลือดแดงแข็งของผู้ที่สูบบุหรี่วิธีการทำวิจัย : อาสาสมัครที่มีสุขภาพดี อายุ 20 - 60 ปี ที่สูบบุหรี่และไม่สูบบุหรี่จำนวน 72 ราย ประเมินการทำงานของเซลล์เยื่อบุหลอดเลือด ด้วยflow mediated dilatation (FMD) และประเมินหลอดเลือดแดงแข็งด้วย cardio-ankle vascular index (CAVI)ผลการศึกษา : พบว่ากลุ่มที่สูบบุหรี่มีการทำงานของเซลล์เยื่อบุหลอดเลือดลดลงเมื่อเทียบกับกลุ่มที่ไม่สูบบุหรี่อย่างมีนัยสำคัญทางสถิติ (P < 0.05) แต่ดัชนีความยืดหยุ่นของหลอดเลือดแดง (CAVI) และดัชนีความแข็งของหลอดเลือด (ankle brachial index; ABI) ไม่แตกต่างกันระหว่างกลุ่มที่ไม่สูบบุหรี่และกลุ่มที่สูบบุหรี่สรุป : การทำงานของเซลล์เยื่อบุหลอดเลือดลดลงในผู้ที่สูบบุหรี่ที่มีการสูบบุหรี่ระดับต่ำ ดังนั้นการประเมินการทำงานของเซลล์เยื่อบุหลอดเลือดอาจใช้เป็นการคัดกรอง ในผู้ที่สูบบุหรี่ก่อนที่จะดำเนินไปสู่โรคหลอดเลือดแดงแข็งตัวรวมถึงโรคหัวใจและหลอดเลือด.

เหตุผลของการทำวิจัย : ยาพาราเซตามอลเป็นยาที่ใช้บรรเทาอาการปวดและลดไข้ที่นิยมใช้กันอย่างแพร่หลาย และในช่วง 10 ปีที่ผ่านมาพบว่ามีหลายงานวิจัยที่บ่งชี้ว่าการได้รับยาพาราเซตามอลสามารถส่งผลกระทบต่อระบบต่าง ๆของร่างกายรวมถึงระบบประสาทได้ อย่างไรก็ดีผลกระทบของการใช้ยาของพาราเซตามอลก็ยังไม่สามารถสรุปได้อย่างชัดเจนวัตถุประสงค์ : เพื่อศึกษาผลของการได้รับยาพาราเซตามอลในช่วงระยะเวลาสั้น(0 และ 5 วัน) และในช่วงระยะเวลานาน (15 และ 30 วัน) ต่อการตายของเซลล์ในสมองบริเวณซีรีบรัลคอร์เท็กซ์ของหนูแรทวิธีการทำวิจัย การศึกษานี้ได้ทำการแบ่งหนูทดลองสายพันธุ์วิสต้าออกเป็น 2 กลุ่ม คือกลุ่มควบคุมและกลุ่มที่ได้รับยาพาราเซตามอล โดยในกลุ่มที่ได้รับยาพาราเซตามอลนั้นได้รับยาขนาด 200 มิลลิกรัมต่อกิโลกรัมด้วยวิธีฉีดเข้าช่องท้องเพียงครั้งเดียวสำหรับกลุ่มที่ได้รับยาเป็นเวลา 0 วันและทำการฉีดยาหนึ่งครั้งต่อวันต่อเนื่องกันเป็นเวลา 5, 15 และ 30 วันในกลุ่มที่ได้รับยาพาราเซตามอลต่อเนื่องเป็นเวลา 5, 15 และ 30 วันตามลำดับ ส่วนกลุ่มควบคุมได้รับการฉีด 0.9% normal saline เข้าทางช่องท้องในช่วงระยะเวลาเดียวกับกลุ่มที่ได้รับยาพาราเซตามอลเมื่อครบช่วงเวลาที่กำหนดสัตว์ทดลองทุกกลุ่มถูกทำให้ตายอย่างสงบโดยการฉีดโซเดียมเพนโทบาร์บิทัลที่ความเข้มข้นสูงเข้าทางช่องท้องและเก็บชิ้นเนื้อสดของสมองบริเวณซีรีบรัลคอร์เท็กซ์ เพื่อทำการศึกษาการแสดงออกของเอนไซม์ caspase-3 อีกทั้งทำการเก็บชิ้นเนื้อโดยการคงสภาพไว้ในน้ำยา 4% paraformaldehyde เพื่อนำไปศึกษาด้วยเทคนิค Terminal deoxynucleotidyl transferase (TdT) dUTPNick-End Labeling (TUNEL) assay และอิมมูโนพยาธิวิทยาผลการศึกษา : ผลจากการศึกษาวิจัยในครั้งนี้พบว่าการได้รับยาพาราเซตามอลในช่วงระยะเวลาสั้น (0 และ 5 วัน) ไม่มีผลกระทบต่อเซลล์ประสาท ในขณะที่การได้รับยาพาราเซตามอลเป็นระยะเวลานาน (15 และ 30 วัน)กลับส่งผลให้มีการแสดงออกของเอนไซม์ caspase-3 และมีจำนวนTUNEL-immunoreactive cell เพิ่มสูงขึ้นอย่างมีนัยสำคัญทางสถิติเมื่อเปรียบเทียบกับกลุ่มควบคุมสรุป : จากการศึกษาวิจัยนี้สามารถสรุปได้ว่าการได้รับยาพาราเซตามอลในความเข้มข้น 200 มิลลิกรัมต่อกิโลกรัม ในช่วงระยะเวลาสั้นไม่ได้ส่งผลกระทบต่อการเปลี่ยนแปลงของเซลล์ในสมองบริเวณซีรีบรัลคอร์เท็กซ์แต่การได้รับยาพาราเซตามอลในความเข้มข้นนี้ติดต่อกันเป็นระยะเวลานานกลับเหนี่ยวนำให้มีการเพิ่มการแสดงออกของ caspase-3และมีการตายของเซลล์เพิ่มขึ้นคำสำคัญ : การได้รับยาพาราเซตามอลเป็นระยะเวลานาน, การตายของเซลล์ประสาท, ซีรีบรัลคอร์เท็กซ์, caspase-3.

ความสัมพันธ์ระหว่างความหลากหลายทางพันธุกรรมบนยีนNtcp กับการดำเนินโรคจากการติดเชื้อไวรัสตับอักเสบบี, นงณภัสร์ ตูยปาละ, สัญชัย พยุงภร, พิสิฐ ตั้งกิจวานิชย์

เหตุผลของการทำวิจัย : Sodium taurocholate co-transporting polypeptide หรือ NTCPเป็นตัวรับที่จำเพาะต่อไวรัสตับอักเสบบี ซึ่งอยู่บนผิวของเซลล์ตับทำให้ไวรัสตับอักเสบบีสามารถผ่านเข้าไปสู่เซลล์ตับได้ มีงานวิจัยที่ทำการศึกษาในประชากรจีนชาวฮั่นพบว่าความหลากหลายทางพันธุกรรม (single nucleotide polymorphisms, SNPs)ของยีนNTCP มีความสัมพันธ์กับการติดเชื้อไวรัสตับอักเสบบีแบบเรื้อรังอย่างไรก็ตามยังไม่มีการศึกษาความสัมพันธ์ระหว่างความหลากหลายทางพันธุกรรมของยีนนี้กับการดำเนินโรคที่เกิดจากการติดเชื้อไวรัสตับอักเสบบี ในกลุ่มประชากรชาวไทยวัตถุประสงค์ : เพื่อศึกษาความสัมพันธ์ระหว่างความหลากหลายทางพันธุกรรมบนยีนNTCP กับการติดเชื้อไวรัสตับอักเสบบีและการเกิดมะเร็งตับวิธีการทำวิจัย : กลุ่มตัวอย่างที่ศึกษาประกอบด้วยกลุ่มควบคุมที่มีสุขภาพดี ที่ไม่เคยติดเชื้อไวรัสตับอักเสบบีมาก่อน จำนวน 242 ราย, กลุ่มผู้ป่วยที่ติดเชื้อไวรัสตับอักเสบบีที่สามารถหายได้เอง จำนวน 230 ราย และกลุ่มผู้ป่วยที่ติดเชื้อไวรัสตับอักเสบบีแบบเรื้อรัง จำนวน 635 ราย ในจำนวนนี้มีผู้ป่วยที่เป็นมะเร็งตับร่วมด้วย จำนวน 319 ราย การตรวจสอบความหลากหลายทางพันธุกรรมของยีนดังกล่าวทำโดยวิธี Allelicdiscrimination ด้วย TaqMan probe real-time polymerase chainreaction สถิติที่ใช้ ได้แก่ unpaired t-test, analysis of variance(ANOVA) และ Chi-square testผลการศึกษา : ความถี่ของจีโนไทป์ GG, GA และ AA ของยีน NTCP ตำแหน่งrs2296651 ในกลุ่มควบคุมที่มีสุขภาพดีเป็นร้อยละ 74.2, ร้อยละ 22.1และร้อยละ 3.7 ตามลำดับ, กลุ่มที่ติดเชื้อไวรัสตับอักเสบบีที่สามารถหายได้เองเป็นร้อยละ 84.1, ร้อยละ 15.1 และร้อยละ 0.8 ตามลำดับและกลุ่มผู้ป่วยที่ติดเชื้อไวรัสตับอักเสบบีแบบเรื้อรังเป็นร้อยละ 85.0,ร้อยละ 13.4 และร้อยละ 1.6 ตามลำดับ นอกจากนี้พบว่าความถี่ของจีโนไทป์ non-GG (GA และ AA) ลดลงอย่างมีนัยสำคัญทางสถิติในกลุ่มที่ติดเชื้อไวรัสตับอักเสบบีที่สามารถหายได้เอง [odds ratio (OR)0.54; 95 % CI (0.35 - 0.86), P = 0.001] …

ความยืดหยุ่นและแรงสนับสนุนทางสังคมของผู้ติดเชื้อเอชไอวี/เอดส์ ณ ศูนย์ประสานความร่วมมือระหว่างไทยแลนด์-ออสเตรเลีย-เนเธอร์แลนด์ เพื่อการศึกษาวิจัยทางคลินิกด้านโรคเอดส์ ศูนย์วิจัยโรคเอดส์สภากาชาดไทย, นปภัช เมืองมา, เดชา ลลิตอนันต์พงศ์

เหตุผลของการทำวิจัย : ประเทศไทยมีผู้ติดเชื้อเอชไอวีสะสมทั้งหมด 1,201,839 ราย จากการคาดประมาณ ปี พ.ศ. 2558 และยังมีชีวิตอยู่ประมาน 600,000 รายแม้ว่าหน่วยงานที่เกี่ยวข้องต่างพยายามที่จะลดการเลือกปฏิบัติและการตีตรา แต่ก็มีผู้ติดเชื้อเอชไอวี เพียง 355,123 ราย ที่เข้าถึงการรักษาภายใต้ระบบสุขภาพแห่งชาติ ทั้งนี้ ส่วนหนึ่งอาจเกิดจากผลกระทบของการตีตรา และการเลือกปฏิบัติมีหลายการศึกษา พบว่าการสนับสนุนทางสังคมและความยืดหยุ่น สามารถลดผลกระทบจากการถูกตีตรา และการเลือกปฏิบัติได้ ทั้งยังส่งผลให้เกิดคุณภาพชีวิตที่ดีวัตถุประสงค์ : เพื่อการศึกษาความยืดหยุ่นและแรงสนับสนุนทางสังคมของผู้ติดเชื้อเอชไอวี/เอดส์ ณ ศูนย์ประสานความร่วมมือระหว่างไทยแลนด์-ออสเตรเลีย-เนเธอร์แลนด์ เพื่อการศึกษาวิจัยทางคลินิกด้านโรคเอดส์ศูนย์วิจัยโรคเอดส์ สภากาชาดไทย ณ กรุงเทพมหานคร ประเทศไทยวิธีการทำวิจัย : การศึกษานี้ได้รับการอนุมัติจากคณะกรรมการพิจารณาจริยธรรมการวิจัย จุฬาลงกรณ์มหาวิทยาลัย อาสาสมัครทุกรายได้ เขียนเอกสารให้ความยินยอม ก่อนเข้าร่วมการศึกษานี้ แล้วจึงทำการตอบแบบสอบถามทั้ง 3 ส่วน ประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคล,แบบวัดความยืดหยุ่นฉบับภาษาไทย ซึ่งแปลจาก Wagnlid and Youngและแบบสอบถามแรงสนับสนุนทางสังคม ตั้งแต่เดือนตุลาคม พ.ศ.2559 ถึงเดือน ธันวาคม พ.ศ. 2559 การแปลผลความยืดหยุ่นใช้ระดับคะแนนสูง (147 - 175) ปานกลาง(121 - 146) ต่ำ(<121) และแรงสนับสนุนทางสังคมแปลผลด้วย ระดับคะแนน สูง (≥74), ปานกลาง(47 - 73), ต่ำ (20 - 46) ปัจจัยที่มีความสัมพันธ์กับความยืดหยุ่น และแรงสนับสนุนทางสังคมของผู้ติดเชื้อเอชไอวี/เอดส์ วิเคราะห์ผลโดยไคสแควร์ และสัมประสิทธิ์สหสัมพันธ์เพียร์สัน การวิเคราะห์ทางสถิติดำเนินการโดยใช้ SPSS รุ่น 22.0 และใช้ค่า P <0.05 เพื่อระบุความมีนัยสำคัญทางสถิติผลการศึกษา : จากกลุ่มตัวอย่างผู้ติดเชื้อเอชไอวี จำนวน 130 ราย อายุเฉลี่ย median(IQR) 43 (36 - 49) ปี ส่วนใหญ่เป็นเพศชายร้อยละ 69 และเป็นเพศหญิงร้อยละ 31 ส่วนใหญ่มีสถานภาพโสดร้อยละ 47 และจบการศึกษาระดับปริญญาตรีร้อยละ 43 และประมานร้อยละ 26 มีรายได้อยู่ในช่วง 5,000 - 10,000 บาท กลุ่มตัวอย่างทั้งหมด ได้รับการรักษาด้วยยาต้านไวรัส โดยมีระยะเวลาการรักษา ปริมาณเม็ดเลือดขาวหลังการรักษาเฉลี่ย 11.3 (IQR 5 - 16.6) ปี เกือบ 2 ใน 3 ( ร้อยละ 72)มีปริมานเม็ดเลือดขาว ณ ปัจจุบัน มากกว่า 500 cell/mm3 ค่าเฉลี่ยของเม็ดเลือดขาว CD4 median (IQR) 608.5 (484 - 782) และมีปริมาณไวรัสเอชไอวี ณ ปัจจุบันน้อยกว่า 50 copies/ml ถึงร้อยละ 99ทั้งนี้ร้อยละ 55 มีระดับความยืดหยุ่นระดับสูง (ค่าเฉลี่ย = 142.2,ส่วนเบี่ยงเบนมาตรฐาน = 24.2) ปัจจัยที่มีความสัมพันธ์กับระดับความยืดหยุ่นได้แก่แรงสนับสนุนทางสังคม (สัมประสิทธิ์สหสัมพันธ์เพียร์สัน = 0.25, P <0.01) และร้อยละ 61 มีแรงสนับสนุนทางสังคมในระดับปานกลางค่อนไปทางสูง (ค่าเฉลี่ย = 70.2, ส่วนเบี่ยงเบนมาตรฐาน = 13.3 ) ปัจจัยที่มีความ สัมพันธ์กับแรงสนับสนุนทางสังคมได้แก่อายุ (สัมประสิทธิ์สหสัมพันธ์เพียร์สัน = - 0.348, P <0.01)ระดับการศึกษา (สัมประสิทธิ์สหสัมพันธ์เพียร์สัน = 0.31, P <0.01)รายได้เฉลี่ยต่อเดือน (สัมประสิทธิ์สหสัมพันธ์เพียร์สัน = 0.32, P <0.01)และระยะเวลาในการเข้ารับการรักษา (สัมประสิทธิ์สหสัมพันธ์เพียร์สัน= 0.26, P <0.01)สรุป : จากการศึกษาในกลุ่มตัวอย่างผู้ติดเชื้อเอชไอวีวัยผู้ใหญ่ ที่มีระดับไวรัสต่ำและระดับเม็ดเลือดขาวสูงพบว่าส่วนใหญ่ มีความยืดหยุ่นในระดับสูง และมีแรงสนับสนุนทางสังคมในระดับปานกลางค่อนไปทางสูง ระดับแรงสนับสนุนทางสังคมที่สูงขึ้นมีความสัมพันธ์กับความยืดหยุ่นที่สูงขึ้น ในขณะที่แรงสนับสนุนทางสังคมมีความสัมพันธ์กับอายุที่มากขึ้น การศึกษาระดับสูงและระยะเวลาในการเข้ารับการรักษาที่ยาวนาน

เหตุผลของการทำวิจัย :เวลาในการเคลื่อนไหวในกิจวัตรประจำวันของมนุษย์ เป็นตัวบ่งชี้สำคัญของการมีสมรรถภาพทางกายที่ดี คณะผู้วิจัยจึงสนใจประดิษฐ์โปรแกรมอย่างง่ายสำหรับวัดเวลาในการเคลื่อนไหวในชีวิตประจำวันโดยสามารถวัดได้ทั้งระยะใกล้และไกลโดยใช้ระบบรับสัญญาณแบบไร้สาย โปรแกรมนี้ยังสามารถนำมาใช้ประโยชน์ในการวิจัยทางคลินิกที่เกี่ยวกับการเคลื่อนไหวและการประเมินการเคลื่อนไหวของร่างกายวัตถุประสงค์ : เพื่อทดสอบประสิทธิภาพของโปรแกรมสำหรับวัดเวลาในการเคลื่อนไหวในกิจวัตรประจำวันวิธีการทำวิจัย : ทำการทดสอบความเที่ยงตรง (validity) และความน่าเชื่อถือ (reliability)ของโปรแกรมสำหรับวัดเวลาในการเคลื่อนไหวในกิจวัตรประจำวันในอาสาสมัครที่มีอายุอยู่ระหว่าง 18 - 60 ปี จำนวน 40 รายผลการศึกษา : พบว่าโปรแกรมวัดเวลาในการเคลื่อนไหวที่สร้างขึ้นนี้เมื่อเปรียบเทียบกับโปรแกรมจับเวลามาตรฐานมีความเที่ยงตรงอยู่ในระดับสูงมาก(ICC (3, 1) = 1.00, P <0.001) เมื่อทดสอบความน่าเชื่อถือของโปรแกรมสำหรับวัดเวลาในการเคลื่อนไหวที่สร้างขึ้น พบว่ามีความน่าเชื่อถือในการวัดซ้ำอยู่ในระดับปานกลางถึงสูงมาก ทั้งนี้ขึ้นอยู่กับลักษณะกิจกรรมที่ทดสอบ กิจกรรมที่ใช้เวลาในการทำน้อย ได้แก่การยื่นมือไปแตะเป้าหมายข้างหน้า มีความน่าเชื่อถือในการวัดซ้ำอยู่ในระดับปานกลาง (ICC (3, k) = 0.636, P <0.001) แต่กิจกรรมที่ใช้เวลาทำนานมีความน่าเชื่อถือในการวัดซ้ำสูง ได้แก่ การลุกขึ้นยืน(ICC (3, k) = 0.759, P <0.001) และการเดิน (ICC (3, k) = 0.986,P <0.001)สรุป : โปรแกรมและอุปกรณ์ที่ประดิษฐ์ขึ้นนี้สามารถนำไปใช้วัดเวลาในการเคลื่อนไหวใน 3 กิจกรรม คือ การยื่นมือแตะเป้าหมายข้างหน้าการลุกขึ้นยืน และการเดิน ได้อย่างเที่ยงตรงและน่าเชื่อถือ
