Vector flow imaging (VFI) is a new ultrasound technology that provides real time, angle-independent visualization of flow velocities in the heart and great vessels. Thus far, VFI has been used for superficial applications due to the limited penetration depth of available transducer probes; however, this depth in smaller pediatric patients enables adequate aortic views. In this project, VFI was used to study pediatric aortic stenosis (PAS)—a congenital heart defect that results in the narrowing of the aorta and/or aortic valve. This thesis proposes an algorithm that determine the geometry of 3D-printed, custom-designed spring element bands made of thermoplastic elastomer (TPE) for use in a wearable orthotic device to aid in the physical therapy of a human hand exhibiting spasticity after stroke. Each finger of the hand is modeled as a mechanical system consisting of a triple-rod pendulum with nonlinear stiffness at each joint and forces applied at the attachment point of each flexor muscle. We set out in August 2017 to develop an active noise suppressing device that would be usable both in military and civilian activities. Due to constraints in knowledge and time, we decided our best course of action was to divide the project into two equal projects with the hope to be able to combine them into a single project at the end of the allotted time. Based on the principles of cutting edge bone remodeling research, a unique therapeutic exercise device was designed specifically to improve bone quality at the most critical location of the proximal femur prone to fracture: the superior-lateral femoral neck where the fracture first initiates during a fall. The exercise/device is intended to work by inducing enough strain in the bone to stimulate the body’s natural bone remodeling mechanisms to increase bone density in the proximal femur and consequently prevent a fracture from arising if a fall to the side does occur.

The cell membrane is the interface that volumetrically isolates cellular components from the cell's environment. Proteins embedded within and on the membrane have varied biological functions: reception of external biochemical signals, as membrane channels, amplification and regulation of chemical signals through secondary messenger molecules, controlled exocytosis, endocytosis, phagocytosis, organized recruitment and sequestration of cytosolic complex proteins, cell division processes, organization of the cytoskeleton and more. The membrane's bioelectrical role is enabled by the physiologically controlled release and accumulation of electrochemical potential modulating molecules across the membrane through specialized ion channels (e.g., Na⁺, Ca2+, K⁺ channels). …

Monocytes play an important role in the immune system and are responsible for phagocytizing and degrading foreign microorganisms in the body. The isolation of monocytes is important in various immunological applications such as in-vitro culture of dendritic cells. We present a magnetophoretic-based microfluidic chip for rapid isolation of highly purified, untouched monocytes from human blood by a negative selection method. This bioseparation platform integrates several unique features into a microfluidic device, including locally engineered magnetic field gradients and a continuous flow with a buffer switching scheme to improve the performance of the cell separation process. Transcatheter aortic heart valve replacement (TAVR) is a procedure to replace a failing aortic valve and is becoming the new standard of care for patients that are not candidates for open-heart surgery [2]. However, this minimally invasive technique has shown to cause ischemic brain lesions, or “silent infarcts”, in 90% of TAVR patients, which can increase the patient’s risk for stroke by two to four times in future years [3]. Ankle foot orthoses (AFOs) are commonly prescribed to provide stability and foot clearance for patients with weakened or injured musculature. The Dynamic Response AFO (DRAFO) was designed to improve proprioception at heel strike. The design includes a rigid outer shell with a cut out heel and a soft inner lining; it is typically aligned in plantarflexion and may incorporate external heel wedges. The World Health Organization estimates that over 30 million people require some sort of prosthetic technology. However, traditional prosthetic fitting practices take a lot of time and cost a lot of money, making them inaccessible to millions of people around the world. StandUP Worldwide is an interdisciplinary project team devoted to creating low-cost prosthetic technologies for use around the world, especially in resource poor areas. They are currently developing a low cost, below-the-knee prosthetic kit that can be easily deployed in a resource-poor area. Cytokine-based cancer immunotherapies stimulate a host’s immune system to fight cancer. In particular, interleukin-12 (IL-12), a potent pro-inflammatory cytokine, has demonstrated the ability to eliminate tumors in a number of preclinical models. Toxicities associated with the systemic delivery of IL-12 have precluded its use in the clinic. We are developing a novel chitosan-based hydrogel to maintain high local concentrations of cytokines, such as IL-12, in the tumor while minimizing its systemic dissemination. Herein, a robust method to fabricate expanded nanofiber scaffolds with controlled size and thickness using a customized mold during the modified gas-foaming process is reported. The expansion of nanofiber membranes is also simulated using a computational fluid model. Expanded nanofiber scaffolds implanted subcutaneously in rats show cellular infiltration, whereas non-expanded scaffolds only have surface cellular attachment. Compared to unexpanded nanofiber scaffolds, more CD68+ and CD163+ cells are observed within expanded scaffolds at all tested time points post-implantation. Reference point indentation (RPI), a novel form of micro-indentation, quantifies RPI material parameters which correlate with modulus, yield stress, strength, or toughness. Information linking bisphosphonate treatment length with the material properties of osteoporotic trabecular bone is needed to improve patient treatment. The objectives of this study were to: 1) determine if RPI can be used to successfully evaluate human trabecular bone and if so, determine an optimized test method for using RPI on trabecular bone, and 2) use this method to determine if any RPI parameters are related to the duration of bisphosphonate treatment.

In light of the need of tissue palpation for Robotics-assisted Minimally Invasive Surgery (RMIS), this paper presents a microfluidic-based tactile sensor for palpating mice tissues for tumor localization. The core of the sensor is a 3x3 sensing-plate/transducer array built into a single polydimethylsiloxane (PDMS) microstructure, with a transducer spacing of 3.75mmx1.5mm. Mounted on a robot, the sensor is pressed against a tissue region with a pre-defined indentation depth pattern, and consequently the stiffness distribution across the tissue region translates to the deflection distribution of the sensing-plate array and is captured by the transducer array underneath as resistance changes. Studying the development of abdominal aortic aneurysm (AAA) through quantification of its growth kinetics and rupture criteria is important to decrease the risk of this life-threatening event. Uniaxial and biaxial testing of healthy and time-dependent Type-I collagen degraded aortic specimens from pigs was performed. Stress-strain suitable mathematical models describing the behavior of abdominal aortic tissue were utilized to derive specific tissue properties and parameters. Snoring is a sleep disruption that can lead to obstructive sleep apnea (OSA), which interrupts breathing by obstructing the airway. Injecting a protein crosslinker, such as genipin, into the soft palate could decrease the severity of snoring and OSA by stiffening the soft palate. Equine soft palates modeled human palates due to a high incidence of awake snoring and apnea.
The pilot in vivo study treated six horses with two 100 mM injections of the buffered genipin reagent. The efficacy phase horses underwent respiratory audio recordings to document snoring changes using Matlab and ImageJ in the time and frequency domains. …

Subtalar arthrodesis is performed when cartilage in the talo-calcaneal (the ankle) joint is degraded in a patient and there are multiple approaches to this surgery. Using synthetic bone material, fixtures were cast from aluminum, using the process of evaporation pattern casting, and epoxy. This casting was used to grip the calcaneus bone of the foot in order to load it mechanically, simulating different scenarios that would be encountered during walking. Improper positioning of the acetabular cup has been shown to contribute to issues such as high wear rates and dislocations in total hip replacements. The differences in contact mechanics due to varying orientation of the acetabular cup may also significantly affect strain distribution in the femur. A new detection reagent that could possibly augment or replace antibodies research and diagnosis methods are aptamers. Aptamers are ssDNA, RNA or polypeptide constructs that function like active antibodies. Antibodies and aptamers both specifically bind to selected target molecules, and as such they enable the detection or targeting of the presence or absence of a specific antigen.

After reviewing the current literature on ergonomics in laparoscopic surgery it was clear that a comprehensive approach to quantitatively reporting on the biomechanical risk factors associated with surgical hand tools had never been performed. Two areas of research interest in the design and use of lower limb prosthetic devices are how foot performance varies with design and how prosthesis alignment affects gait. The current study addressed both of these areas through the use of the JR3 triaxial transducer mounted at the base of the socket. The transducer is an innovative form of instrumentation that measures forces and moments at the end of the socket. Traditional measurement of gait uses a force plate buried in the floor of a gait laboratory. The study established that the transducer offers a viable means of instrumentation for amputee …

Gas emboli are a clinical issue often encountered in the space exploration, marine construction, and medical fields. Individuals involved with these disciplines will often have asympomatic gas emboli circulating throughout their bodies on a regular basis. A study into the impact of chronic asymptomatic gas emboli on the risk of atherosclerosis in humans is thus presented. This research utilized a custom built cardiovascular flow simulator and accompanying data acquisition system to experimentally determine the influence of gas emboli under varied circumstances on the mean wall shear stress of a tube in-vitro. different mechanical design. Unfortunately the results of ankle replacements are not as good as hip

Two new techniques for quantification of forces generated by an expansion screw apparatus placed between pairs of cylindrical endosseous titanium implants are described, along with preliminary data gained from their use in Macaca rhesus monkeys. Strain gages were bonded to the expansion apparatus and were calibrated either directly by micro load cell, or indirectly by Instron Machine to reflect the applied force.
Immediate loads of up to 6 kilograms have been applied to the implants with no increase in mobility as verified by a Periotest device. No previous studies on the orthodontic uses of implants have evaluated in vivo forces …
