Interrogating Race And Place-Based Inequities In Hiv And Covid-19, Rohan Khazanchi
EMET Projects
Over the last four years, I have developed a research focus examining the intersections of race, place, and health. My M.D. Honors Thesis reflects a snapshot of these efforts. In this collection of brief research reports, I leverage area-based measures to investigate structural inequities in three contexts: the HIV epidemic in our hyperlocal community, the early stages of the COVID-19 pandemic, and clinical trials for novel COVID-19 therapeutics. I apply novel social epidemiologic tools to measure and explore disparate outcomes. And, in reflecting upon my findings, I discuss concrete implications for clinicians, researchers, and policymakers alike.

Intestinal Deletion Of 3-Hydroxy-3-Methylglutaryl-Coenzyme A Reductase Promotes Expansion Of The Resident Stem Cell Compartment, Alexandria M Doerfler, Jun Han, Kelsey E Jarrett, Li Tang, Antrix Jain, Alexander Saltzman, Marco De Giorgi, Marcel Chuecos, Ayrea E Hurley, Ang Li, Pauline Morand, Claudia Ayala, David R Goodlett, Anna Malovannaya, James F Martin, Thomas Q De Aguiar Vallim, Noah Shroyer, William R Lagor

BACKGROUND: The intestine occupies the critical interface between cholesterol absorption and excretion. Surprisingly little is known about the role of de novo cholesterol synthesis in this organ, and its relationship to whole body cholesterol homeostasis. Here, we investigate the physiological importance of this pathway through genetic deletion of the rate-limiting enzyme.
METHODS: Mice lacking 3-hydroxy-3-methylglutaryl-coenzyme A reductase (Hmgcr) in intestinal villus and crypt epithelial cells were generated using a Villin-Cre transgene. Plasma lipids, intestinal morphology, mevalonate pathway metabolites, and gene expression were analyzed.
RESULTS: Mice with intestine-specific loss of Hmgcr were markedly smaller at birth, but gain …

Comparison Of Survival Rates Between Two Cardiopulmonary Resuscitation Protocols And The Association Of Crew Consistency, Stacey L. Parrish, Lori Kupczynski, Pradeep Vanguri
Internet Journal of Allied Health Sciences and Practice
Background: Cardiac arrest is a life-altering event that requires swift, coordinated actions from numerous individuals for patient survival. Although return of spontaneous circulation is the goal of every emergency medical service agency, survival rates continue to be low due to many factors. Many variables are out of direct control of emergency medical service agencies, but some, such as protocol development and personnel training, are well within their power. Purpose: The purpose of this mixed-methods study was to examine the new HP-CPR protocol implemented by a large southeastern urban fire rescue department to see if changes to existing protocol assisted, in …

Allied Health Student Engagement In Telehealth Clinical Placements: A Rapid Review, Kate Bridgman, Shane Erickson, Lisa Furlong, Adam Bird
Internet Journal of Allied Health Sciences and Practice
Purpose: The engagement of allied health students in telehealth consultations whilst on clinical placement has rapidly increased due to the disruption caused by COVID-19. While this is understandable given the circumstances, it has occurred on the largely unfounded assumption that utilising telehealth is pedagogically appropriate to supplement or replace assessed placements. Method: This rapid review used the Cochrane rapid review methodology to synthesise the literature relating to the perspectives of allied health students whilst on clinical placements utilising telehealth. Results: A systematic search and selection process found three studies. In summary, the combined findings of the research suggest that student …

Tuberculous Meningitis Presenting With Stroke In An Immunocompetent Adolescent: A Case Report, Rukhsar S. Osman, Rahim Damji, Zainab Y. Fidaali, Nahida Walli, Mariam Noorani
Faculty of Health Sciences, East Africa
Background:
Tuberculous meningitis (TBM) is a severe infection of the central nervous system that has high mortality. The disease predominantly affects young children and those who are immuncompromised. Strokes have been reported in about one-third of children with tuberculous meningitis and are associated with poor clinical outcomes.
Case report:
A previously healthy 14-year-old girl living in Dar es Salam, Tanzania presented with one month history of weight loss associated with weakness, loss of appetite, apathy; without respiratory symptoms. Anti-TB therapy was started, based on radiological findings of the chest which showed multiple patchy centrilobular nodules with linear branching pattern bilaterally, …

BACKGROUND: Empathic communication skills have a growing presence in graduate medical education to empower trainees in serious illness communication.
OBJECTIVE: Evaluate the impact, feasibility, and acceptability of a shared communication training intervention for residents of different specialties.
DESIGN: A randomized controlled study of standard education v. our empathic communication skills-building intervention: VitalTalk-powered workshop and formative bedside feedback using a validated observable behavioral checklist.
SETTING/SUBJECTS: During the 2018-2019 academic year, our intervention was implemented at a large single-academic medical center in the United States involving 149 internal medicine and general surgery residents.
MEASUREMENTS: Impact outcomes included observable communication skills measured in …
