A Brain-Computer Interface For Closed-Loop Sensory Stimulation During Motor Training In Patients With Tetraplegia, Sarah Helen Thomas 2019 University of Kentucky

A Brain-Computer Interface For Closed-Loop Sensory Stimulation During Motor Training In Patients With Tetraplegia, Sarah Helen Thomas

Theses and Dissertations--Biomedical Engineering

Normal movement execution requires proper coupling of motor and sensory activation. An increasing body of literature supports the idea that incorporation of sensory stimulation into motor rehabilitation practices increases its effectiveness. Paired associative stimulation (PAS) studies, in which afferent and efferent pathways are activated in tandem, have brought attention to the importance of well-timed stimulation rather than non-associative (i.e., open-loop) activation. In patients with tetraplegia resulting from spinal cord injury (SCI), varying degrees of upper limb function may remain and could be harnessed for rehabilitation. Incorporating associative sensory stimulation coupled with self-paced motor training would be a means for supplementing …

Autologous Peripheral Nerve Grafts To The Brain For The Treatment Of Parkinson's Disease, Andrew Welleford 2019 University of Kentucky

Autologous Peripheral Nerve Grafts To The Brain For The Treatment Of Parkinson's Disease, Andrew Welleford

Theses and Dissertations--Neuroscience

Parkinson’s disease (PD) is a disorder of the nervous system that causes problems with movement (motor symptoms) as well as other problems such as mood disorders, cognitive changes, sleep disorders, constipation, pain, and other non-motor symptoms. The severity of PD symptoms worsens over time as the disease progresses, and while there are treatments for the motor and some non-motor symptoms there is no known cure for PD. Thus there is a high demand for therapies to slow the progressive neurodegeneration observed in PD. Two clinical trials at the University of Kentucky College of Medicine (NCT02369003, NCT01833364) are currently underway that …

Pharmacy Faculty Articles and Research

Introduction: Low blood-brain barrier (BBB) penetration and hematopoietic side effects limit the therapeutic development of erythropoietin (EPO) for Alzheimer's disease (AD). A fusion protein of EPO and a chimeric monoclonal antibody targeting the mouse transferrin receptor (cTfRMAb) has been engineered. The latter drives EPO into the brain via receptor-mediated transcytosis across the BBB and increases its peripheral clearance to reduce hematopoietic side effects of EPO. Our previous work shows the protective effects of this BBB-penetrating EPO in AD mice but hematologic effects have not been studied. Herein, we investigate the hematologic safety and therapeutic effects of chronic cTfRMAb-EPO dosing, …

Feature-Guided Deep Radiomics For Glioblastoma Patient Survival Prediction, Zeina A. Shboul, Mahbubul Alam, Lasitha Vidyaratne, Linmin Pei, Mohamed I. Elbakary, Khan M. Iftekharuddin

Electrical & Computer Engineering Faculty Publications

Glioblastoma is recognized as World Health Organization (WHO) grade IV glioma with an aggressive growth pattern. The current clinical practice in diagnosis and prognosis of Glioblastoma using MRI involves multiple steps including manual tumor sizing. Accurate identification and segmentation of multiple abnormal tissues within tumor volume in MRI is essential for precise survival prediction. Manual tumor and abnormal tissue detection and sizing are tedious, and subject to inter-observer variability. Consequently, this work proposes a fully automated MRI-based glioblastoma and abnormal tissue segmentation, and survival prediction framework. The framework includes radiomics feature-guided deep neural network methods for tumor tissue segmentation; followed …

Allogeneic haematopoietic stem cell transplantation currently represents the primary potentially curative treatment for cancers of the blood and bone marrow. While relapse occurs in approximately 30% of patients, few risk-modifying genetic variants have been identified. The present study evaluates the predictive potential of patient genetics on relapse risk in a genome-wide manner. We studied 151 graft recipients with HLA-matched sibling donors by sequencing the whole-exome, active immunoregulatory regions, and the full MHC region. To assess the predictive capability and contributions of SNPs and INDELs, we employed machine learning and a feature selection approach in a cross-validation framework to discover the …

Protecting Patients Who Lack A Voice, Rainer Spiegel 2019 University of Tuebingen (Germany), BG Trauma Hospital Tuebingen

Protecting Patients Who Lack A Voice, Rainer Spiegel

Animal Sentience

Neither human young today nor future human generations nor non-human species have a voice in protecting the biosphere. Treves et al. propose courts and trustees for defending their interests. I describe an analogy with attempts to represent the interests of children and comatose patients in medicine.

Non-Human Animals Providing Rescue In Medical Emergencies, Rainer Spiegel 2019 University of Tuebingen (Germany), BG Trauma Hospital Tuebingen

Non-Human Animals Providing Rescue In Medical Emergencies, Rainer Spiegel

Animal Sentience

In their target article, Chapman & Huffman challenge the quotation of Sir William Osler that the desire to take medication distinguishes humans from non-human animals. They provide examples of self-medication in non-human animals. Based on these examples, it can be inferred that non-human animals practice at least some form of medicine for symptom control. I would like to extend this view by showing that non-human animals not only provide self-medication, but also rescue others facing emergencies.

Research outputs 2014 to 2021

Background: A significant number of melanoma patients experience recurrence to distant sites, despite having had surgical treatment of the primary lesion, with curative intent. Monitoring of patients for early evidence of disease recurrence would significantly improve management of the disease, allowing timely therapeutic intervention. Circulating tumor DNA (ctDNA) is becoming a well-recognized biomarker for monitoring malignancies and has, in a few studies, been shown to signify disease recurrence earlier than conventional methods.

Methods: We performed a retrospective analysis of plasma ctDNA using droplet digital PCR (ddPCR) in 30 primary melanoma patients with tumors harboring BRAF, NRAS or TERT promoter mutations. …

Optical stretcher is a tool in which two counter-propagating, slightly diverging, and identical laser beams are used to trap and axially stretch microparticles in the path of light. In this work, we utilized the dual-beam optical stretcher setup to trap and stretch human embryonic kidney (HEK) cells and mammalian breast cancer (MBC) cells. Experiments were performed by exposing the HEK cells to counter-propagating laser beams for 30 seconds at powers ranging from 100 mW to 561 mW. It was observed that the percentage of cell deformation increased from 16.7% at 100 mW to 40.5% at 561 mW optical power. The …

Dna Methylation Of The Clusterin Promoter: Associations With Alzheimer’S Disease Risk And Related Phenotypes, Madeline Peretti

Theses: Doctorates and Masters

Background

In 2017 approximately 50 million people worldwide were living with dementia. With Alzheimer’s disease (AD), accounting for 50-70% of dementia cases making this debilitating disease, with no current effective prevention, treatment or cure, a critical healthcare concern. Genome wide association studies (GWAS) have identified a number of risk genes for late onset AD (LOAD); Apolipoprotein E (APOE), a gene involved in the cholesterol/lipid pathway is considered the gene with the greatest risk. The third most associated AD risk gene is Clusterin (CLU), is also involved in the cholesterol/lipid pathway. CLU has been implicated in both …

Research outputs 2014 to 2021

Cutaneous melanoma circulating tumour cells (CTCs) are phenotypically and molecularly heterogeneous. We profiled the gene expression of CTC subpopulations immunomagnetic-captured by targeting either the melanoma-associated marker, MCSP, or the melanoma-initiating marker, ABCB5. Firstly, the expression of a subset of melanoma genes was investigated by RT-PCR in MCSP-enriched and ABCB5-enriched CTCs isolated from a total of 59 blood draws from 39 melanoma cases. Of these, 6 MCSP-and 6 ABCB5-enriched CTC fractions were further analysed using a genome-wide gene expression microarray. The transcriptional programs of both CTC subtypes included cell survival maintenance, cell proliferation, and migration pathways. ABCB5-enriched CTCs were specifically characterised …

On The Efficacy Of Moringa Oleifera As Anticancer Treatment: A Literature Survey, Florentin Smarandache, Victor Christianto 2019 University of New Mexico

On The Efficacy Of Moringa Oleifera As Anticancer Treatment: A Literature Survey, Florentin Smarandache, Victor Christianto

Branch Mathematics and Statistics Faculty and Staff Publications

Medicinal plants are important elements of indigenous medical system that have persisted in developing countries. Many of the botanical chemo-preventions currently used as potent anticancer agents. However, some important anticancer agents are still extracted from plants because they cannot be synthesized chemically on a commercial scale due to their complex structures that often contain several chiral centers. The aim of this study was to test different extracts from the Moringa oleifera leaves. Previous studies have shown potentially antioxidant, antitumor promoter, anticlastogen and anticarcinogen activities both in vitro and in vivo. Emerging evidence indicates that efficacy of Moringa oleifera in cancer …

Integrating Alexander Technique Into Treatment Of Adolescent Idiopathic Scoliosis To Improve Patient Quality Of Life, Ana Nishioka 2019 Claremont Colleges

Integrating Alexander Technique Into Treatment Of Adolescent Idiopathic Scoliosis To Improve Patient Quality Of Life, Ana Nishioka

Scripps Senior Theses

The following thesis posits training in the Alexander Technique as a possible solution to address the comorbidities of poor body image and reduced feelings of efficacy in patients treated for adolescent idiopathic scoliosis (AIS). Both literature and personal accounts of AIS highlight that current conservative methods of treating AIS offered by the Western medical sphere, including observation, bracing, and physical therapy, do not adequately address, and may actually exacerbate, negative mental health symptoms. Patients are often represented as their external orthopedic diagnosis and receive limited emotional guidance in exploring their internal feelings about their body and its capabilities. Alexander Technique …

Metaproteomic Investigation Of The Vaginal Microbiome In Pregnancy, Zaneera Hassan 2019 Virginia Commonwealth University

Metaproteomic Investigation Of The Vaginal Microbiome In Pregnancy, Zaneera Hassan

Theses and Dissertations

The development of early diagnostics and prevention strategies for preterm birth is an important global health challenge with the potential to impact over 15 million children annually, by improving health outcomes and reducing economic burden. Advances in microbial sequencing technology have opened the door to 16S rRNA gene survey, whole metagenomics, and whole transcriptomics, providing molecular evidence that the composition of the vaginal microbiome affects pregnancy outcomes in women, particularly those of African descent. A current gap in our molecular level understanding of the vaginal microbiome as it relates to healthy pregnancies is the metaproteome which comprises proteins from both …

Clinician-Administered Versus Self-Administered Suboccipital Release On Superficial Backline Function, Colin Wilhite, Kristin A. Paloncy, Karen Rawkowski, Todd Daniel 2019 Mercy Sports Medicine

Clinician-Administered Versus Self-Administered Suboccipital Release On Superficial Backline Function, Colin Wilhite, Kristin A. Paloncy, Karen Rawkowski, Todd Daniel

Clinical Practice in Athletic Training

The effectiveness of trigger point release in the suboccipital region to improve hamstring mobility has been established in the literature, but the research has not clarified whether self-administered soft tissue techniques produce the same improvements to mobility as when the clinician delivers the manual trigger point release. The purpose of the study was to assess whether the same increase in hamstring mobility within the superficial backline function that is achieved with a clinician-administered suboccipital trigger point release can also be obtained through a patient/self-administered method. The study employed a randomized, descriptive laboratory design in which 60 participants reported for a …

Unique Shoulder Arthroscopies: Snapping Scapula Syndrome And Scapular Nerve Entrapment, Erica L. Taylor, Spero Karas

Clinical Practice in Athletic Training

Anatomy Review: The scapula provides the stable base for glenohumeral motion and provides origins and insertions for 17 muscles. The scapulothoracic (ST) joint lacks synovial lining and cartilaginous interface. The anterior scapula glides over the thoracic wall between several bursal and soft tissue planes.1,2 The suprascapular nerve emerges from the superior trunk of the brachial plexus (C5, C6) and travels across the posterior triangle of the the neck of the scapula trough the suprascapular and spinoglenoid notches. The suprascapular nerve innervates half of the rotator cuff musculature, the infraspinatus and supraspinatus. Snapping Scapula Syndrome (SSS) occurs when the ST articulation …

(First paragraph) The development of three-dimensional culture scaffolds represents a revolutionary step forward for in vitro culture systems. Various synthetic and naturally occurring substrates have been developed that support 3D growth of cells. In most fields, including mammary gland biology and tumorigenesis, the two most common substrates used are the basement membrane rich extracellur matrix (ECM) isolated from EngelbrethHolm-Swarm (EHS) mouse sarcomas (e.g. Matrigel) and collagen extracted from rat-tails. The processes of 3D culture in these two substrates has remained unchanged for nearly half a century: cells are either mixed with unpolymerized matrix to disperse them randomly throughout the substrate …
